의료종사자의 주사침 자상 관련 특성과 지식 및 표준주의 이행도 (Characteristics and Knowledge of Needlestick Injuries and Compliance with Standard Precautions in Healthcare Workers)

  • Purpose: This study was conducted to examine the characteristics and knowledge of needlestick injuries, and compliance with standard precautions, in healthcare workers. Method: The participants were 185 healthcare workers working at university hospitals. Data were collected in May 2016 and analyzed using descriptive statistics, t-test, ANOVA, and Pearson correlation coefficients using SPSS 18.0 program. Results: It was found that 45.4% of the subjects had experienced needlestick injuries. The most common rate of getting injured was 1 or 2 times, and the most common reason for the occurrence of needlestick injuires was carelessness(66.6%). The mean scores for knowledge of needlestick injuries and compliance with standard precautions were above average. Significant correlations were found between knowledge of needlestick injuries and compliance with standard precautions in health workers, and Characteristics were related to the significant differences seen in both. Conclusion: These findings indicate that standard precautions and guidelines for the use of sharp instruments should be emphasized in order to prevent needlestick injuries in healthcare workers.

생활시설 종사자가 경험한 남성노인의 성적욕구 표출에 관한 연구 (A Study on Sexual Desire Outburst of the Male Elderly Residents Experienced by the Workers in Living Facilities)

  • This study has been performed to explore verbal and behavioral expression of sexual desires among male elderly residents who have been living in long tenn care facilities. There are three topics covered in this study; first, in what situations and how seriously do care workers encounter expression of sexual desires of the elderly residents? Second, what kind of negative consequences do they believe those sexual behaviors will lead to? Third, how can we implement defensive measures against the sexual behaviors? In this study, twenty three care workers working full time in five retirement and care facilities were asked about their experience and perception of the above study agendas, and answers of the qualitative interview were drawn as follow; first, the elderly residents apparently show a variety of sexual harassment and provoking behaviors such as sticking to specific women, physically touching and attacking, and induce obscenely activities against female residents, care workers, and volunteer visitors. Second, their sexual behaviors are often influenced by their isolated and abandoned emotionality as well as living situation in rural areas. Third, their sexual behaviors often critically affect care work plan and facility managements by severely discouraging female care givers and community supporters. Therefore in this study, suggestions and defensive measures were made as follows: first, education and counselling programs toward female workers and volunteers need to be developed, and the programs should cover psychological and behavioral mechanism of sexuality in later life. Second, self control plans need to be empowered toward the elderly residents; in the plans, the elderly residents shall be encouraged to evaluate primary cause and proper solutions of sexual behaviors of their peering residents. Third, combination of healthy housing and care facilities for frail elderly need to be integrated in a neighboring location, so that when residents and workers encounter extreme episodes of sexuality of healthy residents in a housing facility, the problematic sexual residents are partially transferred into a neighboring care facility and thereafter other residents and cafe givers are relieved from stressful contacts with the extremely sexual residents.

노인요양시설 요양보호사의 구강관리 및 노인구강위생관리에 따른 구강보건지식 수준 (Oral health knowledge level on oral care and elder oral hygiene care of the nursing home staff in long-term care facilities)

  • Objectives : This research was conducted to investigate geriatric care workers' oral care, and the oral care conditions of residents of elder care facilities in relation to workers' knowledge of oral care. Methods : A survey was conducted from November 2011 to February 2012 with geriatric-care workers in elder care facilities located in Busan and other areas of Gyeongsangnam-do. Results : With regard to their oral self-care practices, 52.5% of the geriatric workers "brushed their teeth from top to bottom" and 58.1% brushed their teeth 3 times a day. Further, 52.0% did not use any oral care products, while 36.5% used Mouthrinese. The workers who brushed their teeth in their own ways or from top to bottom had significantly higher oral care knowledge scores than those who brushed from left to right (p<.05). Concerning oral hygiene care for residents, 72.6% of the geriatric workers recommended that elders visit dentists for oral care, 96.6% helped elders oral wash their teeth every day, and 54.7% used oral care products for the elders. The workers who did not recommend visits to dental clinics scored slightly higher than those who did (p<.05). Additionally, those who did not help elders oral wash their teeth scored significantly higher than those who did (p<.05). Except for oral wash to recuperated old man and recommendation of visiting the dentist, relation between oral care and knowledge about oral health and oral hygiene care is plus mutual relation. Conclusions : To manage the accuracy and quality of oral care work performed by geriatric care workers in elder care facilities, enhancing standard training and education on oral care is necessary. Providing systematic, regular education on such knowledge and practices would also be essential.

  • 본 연구의 목적은 2008년 7월에 실시된 노인 장기요양 보험제도 실시에 따라서 보건 복지 전문직 종사자의 수요모형을 구축하고 수요를 예측함으로써 인프라구축이 필요한 교육기관의 학과개설과 증원에 대비하고자 함이다. 2009년 5월 현재 노인 장기요양 보험 시행 1주년 주요 통계 현황을 바탕으로 2010부터 2030년까지 장래 노인인구의 노인 장기요양 인정자 추계와 이에 따른 시설 요양기관, 재가 서비스기관, 가족급여등의 요양보험 이용자로 인한 보건 복지 전문직 종사자의 수요를 측정하였다. 사회복지사는 2020년 16,624명 2030년 24,688명으로 전망되었다. 간호사는 2020년 11,287명, 2030년에는 16,764명으로 수요증가가 가속화될 전망이다. 요양보호사의 경우 2009년에 필요한 인원은 44,824명이며 8월 31일 현재 1,078개의 교육기관에서 1년간 약 50만명 이상을 양성함으로써 소요인원의 10배 이상으로 과다 양성되었다.

방문간호센터 경영효율성 개선 사례 분석: 경영 컨설팅 적용을 중심으로 (Case Analysis of Visiting Nursing Center for Improving Efficiencies: Based on Business Management Consulting)

  • Purpose: This study aimed to present the management strategies necessary to improve the operational efficiency of visiting nurse centers and evaluate their effectiveness. Methods: The subjects of this study were visiting nurse centers registered as long-term care centers. Based on value chain analysis, cost information analysis, and data envelope analysis, the study was carried out according to the Magerison's management consulting procedure, for six months. This procedure comprised eight sub-steps of approach and application. Results: The following management strategies were agreed upon: establishment of a cooperative network with other visiting care centers, creation of high satisfaction of external customers by providing practical training to care workers, and making rehabilitation and exercise services as the core nursing activities to be focused on. Conclusion: The management consulting process and analysis method applied in this study can referred to as a useful methodological framework for revitalizing visiting nursing centers in the future.

노인장기요양보험 재가서비스의 문제점과 개선방안 (A Study on Problems and Improvement of Home-help Services of Long-term Care Insurance)

  • 본 연구는 제도 시행 초기인 2008년 10월 현재 여러 부분에서 동시다발적으로 발생하는 노인장기요양보험 재가서비스의 다양한 문제점과 원인을 파악하여 분석한 후, 그에 대한 개선방안을 마련하는 데 목적이 있다. 연구방법은 크게 선행 연구된 2차 자료를 분석하는 문헌조사와 관련 전문가를 대상으로 하는 서베이 조사방식을 통해 진행되었다. 자료의 분석은 길버트와 스펙트의 정책분석틀에 근거하여 대상자체계, 급여(서비스)체계, 재정체계, 전달체계의 영역으로 구분하여 분석하였다. 본 연구를 통해 도출된 노인장기요양보험 재가서비스의 문제점은 첫째, 대상자체계에서는 가입대상자와 서비스 대상자의 불일치, 수요자 추계의 문제점 및 등급판정 관련 문제 등이 있으며, 둘째, 서비스 체계에서는 서비스 인력의 전문성 결여, 방문요양기관 인력기준 완화로 인한 서비스 질 저하, 서비스 이용시간 제한의 불합리성 및 방문요양 수가의 등급별 균등지급의 문제가 있는 것으로 나타났다. 셋째, 재정체계에서는 본인부담금과 비급여항목으로 인한 이용자 부담과중, 기존운영비 지원 기관에 대한 지원중단으로 인한 서비스의 사각지대 발생 및 구조조정 및 파트타임 증가로 서비스 질 저하 문제가 나타났으며, 마지막으로 전달체계에서는 서비스의 질 관리체계 미비, 재가장기요양기관 남설로 인한 과다 경쟁, 영리적 운영으로 인한 서비스 공공성과 질저하, 대상자 모집의 어려움으로 인한 운영난 및 요양보호사 교육기관 난립으로 인한 과다배출과 부실교육 등의 문제가 있는 것으로 조사되었다. 이상의 결과에 근거하여 다음과 같이 제언하였다. 첫째, 장기적으로 가입자와 서비스 대상자를 일치시킬 필요가 있다. 둘째, 공단직원의 전문성 향상 및 인력충원이 필요하며, 셋째, 등급외자에 대한 효율적 서비스 연계, 넷째, 체계적인 요양보호사 관리시스템 구축 및 서비스 표준지침서 개발, 다섯째, 서비스 내용과 절차에 관한 일부 법규의 수정이 필요하며, 여섯째, 본인부담금의 일부 조정도 필요하다. 일곱째, 노인복지법에 의한 재가시설에 운영비를 지원해야 하며, 여덟째, 관리감독 기관의 모니터링 시스템 강화 및 서비스 제공 기관과 서비스 인력에 대한 평가 체계 마련, 그리고 마지막으로 기관설치 기준의 강화가 필요하다.

Public Reporting on the Quality Ratings of Nursing Homes in the Republic of Korea

  • Background: Quality ratings could provide vital information to help people in choosing a nursing home. Purpose: This study investigated factors aligned with quality ratings of nursing homes. Methods: We employed a cross-sectional descriptive design to assess publicly available data on 1,354 nursing homes with 30 or more beds in the Republic of Korea. After excluding 289 nursing homes with no reported quality-evaluation ratings, we analyzed the 2015 data of 1,065 nursing homes. To prevent multicollinearity among independent variables, we carefully selected the final set of variables based on clinical and theoretical meaningfulness to direct nursing care. Quality, the ordinal outcome, was scored from 1 to 5 with a higher score indicating higher quality of the organization. We constructed a multivariate ordered logistic regression model. Results: Higher quality ratings of nursing homes was significantly related to the number of unoccupied beds (OR=0.99, p=.024), registered nurses (RNs) (OR=1.30, p=.003), qualified care workers (OR=1.03, p=.011), cognitive-improvement programs (OR=1.05, p=.024), and other programs for residents' activities (OR=1.09, p<.001). Conclusion: The number of RNs had the strongest influence on the publicly reported quality rating, while the rating of qualified care workers demonstrated little effect and that of nursing assistants had no effect. The number of RNs could be used as a crucial indicator for high-quality homes; more resident-engaging programs also demonstrated better quality of nursing home care.

요양보호사 교육생을 위한 기본심폐소생술 교육프로그램 개발 및 효과 (Development and effects of basic cardiopulmonary resuscitation education program for trainee care workers)

  • Purpose: This study set out to use a one-group pretest-posttest experimental design to develop a highly motivating CPR program for help care workers who lack CPR education, and to evaluate its effects on their knowledge about program application, attitude, and self-efficacy. Methods: For this purpose, 140 questionnaires were administered to subjects who underwent training at an educational institution for care workers in S city and G province. The program was developed from April, 25 to May, 15 in 2016, and the evaluation period was from May to July, 15. The collected data were analyzed through SPSS version 23, and a paired t-test was conducted. Results: The program spanned 240 minutes, including 80 minutes on theoretical education and 160 minutes on practical education. Knowledge of CPR in the post-test proved to be higher than that in the pre-test, from 6.4 to 9.2 (t=-15.72, p<.001),; the score on attitude increased from 38.4 to 41.7 (t=-5.89, p<.001),; and self-efficacy increased from 41.0 to 66.7 (t=-21.83, p<.001). Conclusion: The basic CPR education program emphasizing motivation was effective for the care worker trainees and could be used for other curriculum by expanding the scope of the subject.

일 가정간호 교육기관 교육생의 임상수행능력 평가에 관한 연구 (A Study for the Evaluation of Students on a Practical Performance of the Home Health Care)

  • This study was conducted to investigate the evaluation of students on a practical performance of the home health care and to provide data for practical education on the home care nurse institution. Research was performed from March to December. 2002. 40 Respondents participated in the evaluation four times. The data were analysed by using the SPSS/PC + version 10. 0. The results of this study were as followings: 1. The average age of the subjects was 32. Majority of the subjects were interested in home health care. Majority of the subjects were staff nurse(57.5%) and workers at general hospitals(50%). 2. According to the result of the comparison of practical performances. performance evaluations of 20 items were higher when they admitted special practice than when they admitted any other practice. There was an positive increase in the evaluation of health assessment. nursing documentation. explanation. coping with emergency status. coordination and self assertion. In addition to health assessment. CPR. oxygenation. education & consultation. developmental assessment of child. nursing documentation. mastering nursing practice and consideration of client were significantly different. 3. The subjects were useful for practical circumference where they are working at. From these research findings. the following suggestions have been drawn: it is needed to manage practical curriculum variously and to include not only clinical aspect but also organic or administrational aspect and to reflect on students' needs and to choose to be reasonable items and to develop instruments for evaluation capability of home care nurse.

기혼 취업여성의 자녀돌봄 및 가사서비스 이용실태와 요구도 (A Study on Working Mothers' Demand for Child Care and Housekeeping Home Services)

  • The purpose of this study is to analyze working mothers' demand for child care and housekeeping home services, and to investigate the present supply condition of home services in Korea. Data collection was implemented to examine the service demand of working mothers(n=700) in four areas. The data were analyzed with SPSS. The results were as follows: 1) For home-based child-care dispatch services social enterprises are superior to other private service suppliers in terms of the treatment of service workers, wages, and quality control. As for child care service contents, the provision of meals, and casual safeguards were mostly expected. The most important qualification for care providers was found to be in faithfulness, and the certification related with child care was required. An affordable wage range was perceived as ₩3,000~5,000 per hour, and part time services. For the time range, services which commenced before 9 am and terminated after between 5~6 pm were preferred. 2) For housekeeping dispatch services, standard services, including basic cleaning, preparation of meals and side dishes, special cleaning, and washing, were in demand. The required qualifications of housekeeping service providers were similar to those of child care service providers. Possessing certification was not considered as an important quality; however, personality and career type were important factors. An acceptable wage range was ₩3,000~5,000 per hour and part-time services were preferred. In terms of the service time range, services which commenced between 8~9 am and terminated between 5~7 pm were preferred.
