As the elderly population are rapidly growing, the elderly who needs special care and nursing homes for them are also growing. However, many nursing homes were planned without considering the hierarchial space organization such as division of housing unit and nursing unit based on the cognitive and behavioral characteristics of the elderly or accessibility and supportiveness of the staff. This study was to examine the hierarchial characteristics in space organization of nursing homes by using Space Syntax. The subjects were 8 nursing homes in Busan. First, spaces in nursing homes were divided into five areas based on the literature review. Then the hierarchy of space organization for the access layer and layers of living floors was grasped by using G-graph. The access layer has a tree-type hierarchical structure and high visuality. Also, average depth of the space was 5. Then the full integration was analyzed. Mobile area, such as elevator, hallways, or living rooms has a high accessibility and the middle corridor type has commonly been characterized. The nursing stations were analyzed through the control degree and it was low in three nursing homes such as B, C, and G. The low clarity was showed in B, D, and F. The low clarity means that it is relatively difficult to recognize the whole space organization of nursing home. Even though the hierarchy of their living floors was tree-type structure with a high visuality, they has a type of grouping spaces around hallways and spreading, or a ring type.
Purpose: The accessibility of medical facilities for cancer patients affects both their comfort and survival. Patients in rural areas have a higher socioeconomic burden and are more vulnerable to emergency situations than urban dwellers. This study examined the feasibility and effectiveness of a cancer care model integrating a regional cancer center (RCC) and public health center (PHC). Methods: This study analyzed the construction of a safety care network for cancer patients that integrated an RCC and PHC. Two public health institutions (an RCC in Gyeongnam and a PHC in Geochang County) collaborated on the development of the community care model. The study lasted 13 months beginning in February 2019 to February 2020. Results: The RCC developed the protocol for evaluating and measuring 27 cancer-related symptoms, conducted education for PHC nurses, and administered case counseling. The staff at the PHC registered, evaluated, and routinely monitored patients through home visits. A smartphone application and regular video conferences were incorporated to facilitate mutual communication. In total, 177 patients (mean age: 70.9 years; men: 59%) were enrolled from February 2019 to February 2020. Patients' greatest unmet need was the presence of a nearby cancer treatment hospital (83%). In total, 28 (33%) and 44 (52%) participants answered that the care model was very helpful or helpful, respectively. Conclusion: We confirmed that a combined RCC-PHC program for cancer patients in rural areas is feasible and can bring satisfaction to patients as a safety care network. This program could mitigate health inequalities caused by accessibility issues.
휠체어를 사용하는 장애인은 주거 환경에서 접근성이 부족하여 일상생활활동에 어려움을 겪는다. 다양한 스마트 기술을 이용한 스마트홈은 휠체어 사용 장애인의 일상생활활동에 있어 접근성을 높여줄 수 있다. 본 연구에서는 휠체어를 사용하는 장애인을 위해 스마트홈을 구축하였고, 구축된 스마트홈에 대한 사용성평가를 실시하였다. 사용성평가는 시나리오기반사용경험평가, 도구적 일상생활활동평가, 그리고 심층설문방법을 사용하였다. 지역사회 거주 휠체어 사용 장애인 3명을 대상으로 스마트홈 사용성평가를 실시한 결과, 기존 거주지에 비해 구축된 스마트홈의 사용성이 더 높은 것으로 나타났다. 하지만 심층설문 결과, 스마트홈 내부의 부분적인 개선 요구사항 또한 도출되었다. 본 연구를 통해 스마트홈 구축 시 휠체어 사용 장애인의 주거환경 구축에 고려해야 하는 다양한 요소들을 확인할 수 있었다.
Purpose: The purpose of this study was to review systematically the effects of home based pulmonary rehabilitation in chronic obstructive pulmonary disease (COPD) patients using qualitative synthesis. Methods: Studies designed randomized controlled trials were identified to extract data and to assess the quality of studies in 8 domestic and 3 foreign search engines, and hand search reference reviews. Results: A total of 9 studies met the inclusion criteria. Overall, the methodological quality of the studies ranged from average to poor. Home based pulmonary rehabilitation consisted of exercise, education, and psychosocial interventions. Several exercises such as aerobic, strength, respiratory muscle training, and flexibility were used in home based pulmonary rehabilitation for COPD patients. Upper muscle exercise and walking were the most frequently used exercises. The most common topics for education were 'knowledge of the illness' and 'drug management'. Walking distance test (WDT) and quality of life (QoL) were the most frequently measured as the dependent variables to evaluate rehabilitation outcomes, followed by pulmonary function test (PFT) and dyspnea. WDT was the most effective outcome, followed by dyspnea, QoL and PFT. Conclusion: This study informed about the details of scientific and effective home based rehabilitation programs in COPD patients for future researchers and clinicians. These findings can help expand the recognition and accessibility of home based rehabilitation in COPD patients.
This research aimed to assess nursing homes for the elderly with geriatric diseases in Japan, and to prepare the knowledge basis to develop nursing home for the elderly with geriatric diseases in Korea. For this, researcher visited 5 facilities in Tokyo from October 3 to October 9 in 2002, collected data by observations, interviews, and visual materials by taking photographs. Visual materials were analyzed according to the environmental assessment matrix consisted of supportiveness, flexibility, efficiency in perception, accessibility, safety, amenity, and social Interaction that were assorted and were given a name by these researchers in the process of this research. Among the characteristics of environmental design of the analysis cases, 992 items picked out from 722 visual materials were used in the analysis. The data were analyzed using the frequency and percentage with SPSS 11 program. The analysis results were the fellowing. The characteristics of recent nursing home design in Japan tended to focus on supportiveness, amenity, and efficiency in perception but to leave much desired in flexibility and safety. The purpose of this study is to analyze physical characteristics, spatial composition, and using behavior of lounges in elderly skilled nursing facilities as spaces for activity programs and also to evaluate the spaces with the framework based on the indices for performing activity programs in elderly facilities. The results of the study were as follows: First, the lounge of an independent type ensured the privacy to perform programs, and so activities were managed more systematically. On the other hand, an expanded corridor type made the elderly feel difficult to pay attention to activities due to co-use of lounges and corridors. In a lounge of a hall type, the elderly had easy access to the place, but it also had weak home-like atmosphere because the space was used as a lobby entrance. Second, the facilities with western types of tables in whole lounges showed big changes in spatial composition, such as moving all the tables for any activities. It resulted in more preparation time and created obstacles in passages and space use. Third, in the evaluation of lounges based on the framework, most of lounges had accessibility of good quality, but they needed to improve home-like atmosphere and flexibility. To create home-like atmosphere, various spatial compositions and classifications in the lounge should be tried. Moreover, enough space and easy movable furniture can be considered for flexible spatial compositions.
This study analyzes the need for appropriate response education in schools for national health and sustainable development education in connection with particulate matter, which has recently emerged as a serious environmental and social problem. The survey was conducted on 1,200 teenagers(middle and high school students) in North Jeolla Province. The results of the study are as follows. First, the survey results were generally low in terms of environmental attitude and response(actuality) as well as the level of knowledge of particulate matter. Second, particulate matter-related education is rarely conducted at school sites; therefore, it is also necessary to develop an environmental education programs that can be a significant educational experience along with a specific educational orientation that is required to ensure the achievement of sustainable residential environment education. Finally, we hope that this study will help raise awareness on sustainable residential environment education amid difficulties in accessibility. Issues that need further study include: the absence of education materials, environmental education related to particulate matter, and guidance on how to maintain healthy living for individuals that help increase interest in a sustainable living environment for future generations.
The principal measure of housing demand is income and the preferences expressed by households through their respective indifference curves. In this context, housing essentially becomes a derived demand, i.e., the household consumes land and a location (or distance-in time and money costs), according to its relative preferences for space, accessibility, and all other nonhousing goods. This paper attempts to deal with both aspects of housing (apartment) demand and household mobility in the Seoul Metropolitan Areas. Housing services will be measured using hedonic regression technique. From observations on the market prices of dwelling units and on the underlying characteristics of housing, one can estimte the relationships between the two empirically. In predicting the probability of the future moves into new towns in the Seoul Metropolitan areas, the best predictors of the future moves into new best predictors are found to be the degree of satisfaction not only with the current residence as a whole, but with some of the major amenities, accessibility and child education. The reasons for moving into new towns are diverse depending on the households' current situation; the most frequently cited is "improvement of housing conditions," followed by "improvement of living environment," "asset improvement" and "home ownership". It appears that people move houses because of a dissatisfaction with their current housing status, relative their income or needs, or a desire to improve their housing and neighborhood amenities, or both. Eater Seal, the child care experts, observes that children with disability significantly benefit from receiving appropriated care along side their peers. Typically developing children serve as role models from which children with disabilities learn age-appropriated communications and social behavior. Therefore the children with disability can actively participate in well-designed child care settings. But unfortunately in Seoul, Korea, only 2% of children with disability are taken care of in child care centers. And even the disabled children at the child care centers are mostly mentally or emotionally disabled because the building, programs and services of the centers are restrictive to the children with severe physical disabilities. This study reviewed Korean domestic physical environment of child care centers for children with disabilities. Questionnaire survey was conducted to 103 centers located in Seoul by mail. Our survey revealed that they need more areas for nursing room and special rooms and the accessibility to buildings has to be improved. Also, there are no elevators in all buildings surveyed. Accessibility to toilet is noted as significant problem, especially wet floors in toilets. In a nursing room, a space for psychological rest and special education is needed for children with mental disability. Technical specifications on how to make buildings and facilities accessible for the children with disability should be developed. The goal of this study is to provide basic information to develop domestic design guidelines to ensure that the child care centers are safe, convenient, and usable for everyone possible.
The purpose of this study is to investigate the influence of family-friendly life environment factors including local community's living environment satisfaction (safety, convenience, comfort, accessibility, exchange of neighbor satisfaction), family support facility satisfaction, family support program satisfaction and nurture and education environment satisfaction, and local community's living information accessibility on the effect of multiple roles of married men and women with children. Data were collected from 380 married men and women with children located Seoul, GyeongGi-Do, ChungCheongBuk-Do, and KyungSangNam-Do. The results show that, first of all, related to general trend of role satisfaction and role strain, the subjects have higher experience of role satisfaction than role strain. Secondly, the variables of social demography, such as monthly income, non-regular full-time working system, white collar occupation, number of children and level of education, and living environment variables, such as satisfaction of living environment stability and satisfaction of neighbor exchange in living environment were significantly related to role strain. Thirdly, social demography variables, such as average monthly earnings, gender (male), non-regular part-time working system, satisfaction of stability in living environment, and satisfaction of convenience in living environment were significantly related to role strain.
