A STUDY ON THE HEMOGRAM OF PULP IN CONJUNCTION WITH PULP DISEASE (치수병변(齒髓病變)에 따르는 국소혈액상(局所血液像)에 관(關)한 연구(硏究))

  • The purpose of this study was to determine the close relation between clinical symptom and histopathologic finding in pulp. 20 samples of pulp tissue were collected from badly decayed teeth with considerable pulp vitality. Hemogram was also made with the first drop of bleedieg after total pulp extirpation (10cases) and vital pulpotomy(10cases). Histologic specimen was made routinely with extirpated pulp and stained by H&E and P.A.S. Stains. The results under microsopic examination were as follows; 1. Hemogram obtained from dental pulp with acute inflammation revealed increase of polymorphonuclear leukocyte in number, more lymphocytes and monocytes. 2. Hemogram from pulp with chronic inflammation shown the number of lymphocyte and monocyte was remarkably increased. 3. Histologi specimen obtained from teeth with severe clinical symptome showed polymorphonuclear leukocyte infiltration and the hemogram also showed polymorphonuclear leukocytes. 4. Specimen from teeth with moderate symptom showed lymphocyte infiltration and vessel dilatation underneath decayed area. Erythrocytes were massively accumulated inside of the vessel wall.

Clear Cell Ependymoma Occurring in the Cauda Equina

  • The authors present a rare case of clear cell ependymoma that developed in the cauda equina. A 54-year-old man was admitted to hospital with intermittent lower back pain. A neurological examination conducted on admission revealed no sensory or motor disturbance. Deep tendon reflexes in both lower extremities were normal. Magnetic resonance images demonstrated a 1.0 cm-sized intradural mass at the filum terminale. Gross total resection was performed via total laminectomy of L1 and L2. The tumor was confirmed to be clear cell ependymoma by histopathologic examination. His symptom was relieved after surgery.

  • Intraneural ganglia in the upper extremity are rare, and the involvement of the digital nerve of hand has not been reported. The following case report demonstrates a 57-year-old woman with a symptomatic nodular mass on the thenar area of the left hand. Magnetic resonance images showed a lobulated, homogeneous mass of high signal intensity on T2-weighted images and low signal intensity with peripheral enhancement on T1-weighted images. Excisional biopsy and histopathologic examination revealed an intraneural ganglion of the digital nerve of the thumb. Her symptom disappeared immediately after the surgery, and has remained free of abnormal sensation and parestheia for the 3-year follow-up period.

Unilaterally Injected Vocal Fold : Clinical Aspects in 6 Patients (편측 성대발적 6례에 대한 임상적 고찰)

  • Among the patients presented with the chief complaint of voice change, the one with unilaterally injected vocal fold was occasionally found at the out patient clinic. The most patients had rarely improved in symptom with conservative treatment under the impression of chronic laryngitis. Some papers were reported that the injected vocal fold(s) is due to laryngeal tuberculosis hick is sometimes associated with pulmonary tuberculosis. We investigated six patients presented unilaterally injected vocal fold with videolaryngoscope and performed histopathologic confirm with laryngeal biopsy respectively. There were low cases of laryngeal tuberculosis, one case of squamous cell carcinoma and one case of chronic larngitis. Proper management was done according to the tissue diagnosis. Pretretment and posttreatment videolaryngoscopic findings as well as some literature review were done.

  • Pleomorphic adenoma is a benign salivary gland tumor with histologic diversity. The majority of these tumor occurs in the parotid gland. The authors experienced the patients, who complained the tumor-like soft tissue mass on the palatal area. After careful analysis of clinical, radiological and histopathological findings, we diagnosed it as pleomorphic adenoma in the palatal area, and obtained characteristic features were as follows: 1. Main clinical symptom was a painless, slow growing, soft tissue mass with normal intact overlying mucosa on the palatal area. 2. In the radiographic examminations, well encapsulated homogeneous soft tissue mass was shown in the lesion site, and cortical thinning on the palate was also observed. 3. In histopathologic examminations, proliferated cellular components in the hyaline stroma were observed as double layered duct-like structure and densely solid sheet appearance.

Surgical Treatment of Primary Spinal Tumors in the Conus Medullaris

  • Objective : The objective of this study was to evaluate the characteristics and surgical outcome of the conus medullaris tumors. Methods : We retrospectively reviewed 26 patients who underwent surgery for conus medullaris tumor from August 1986 to July 2007. We analyzed clinical manifestation, preoperative MRI findings, extent of surgical resection, histopathologic type, adjuvant therapy, and outcomes. Results : There were ependymoma (13), hemangioblastoma (3), lipoma (3), astrocytoma (3), primitive neuroectodermal tumor (PNET) (2), mature teratoma (1), and capillary hemangioma (1) on histopathologic type. Leg pain was the most common symptom and was seen in 80.8% of patients. Pain or sensory change in the saddle area was seen in 50% of patients and 2 patients had severe pain in the perineum and genitalia. Gross total or complete tumor resection was obtained in 80.8% of patients. On surgical outcome. modified JOA score worsened in 26.9% of patients, improved in 34.6%, and remained stable in 38.5%. The mean VAS score was improved from 5.4 to 1.8 among 21 patients who had lower back pain and leg pain. Conclusion : The surgical outcome of conus medullaris tumor mainly depends on preoperative neurological condition and pathological type. The surgical treatment of conus medullaris tumor needs understanding the anatomical and functional characteristics of conus meudllaris tumor for better outcome.

Glomus Tumors: Symptom Variations and Magnetic Resonance Imaging for Diagnosis

  • Background The typical clinical symptoms of glomus tumors are pain, tenderness, and sensitivity to temperature change, and the presence of these clinical findings is helpful in diagnosis. However, the tumors often pose diagnostic difficulty because of variations in presentation and the nonspecific symptoms of glomus tumors. To the best of our knowledge, few studies have reported on the usefulness of magnetic resonance imaging (MRI) in diagnosing glomus tumors in patients with unspecific symptoms. Methods The inclusion criteria of this study were: having undergone surgery for subungual glomus tumor of the hand, histopathologic confirmation of glomus tumor, and having undergone preoperative MRI. Twenty-one patients were enrolled. The characteristics of the tumors and the presenting symptoms including pain, tenderness, and sensitivity to temperature change were retrospectively reviewed. Results Five out of 21 patients (23%) did not show the typical glomus tumor symptom triad because they did not complain of pain provoked by coldness. Nevertheless, preoperative MRI showed well-defined small soft-tissue lesions on T1- and T2-weighted images, which are typical findings of glomus tumors. The tumors were completely resected and confirmed as glomus tumor histopathologically. Conclusions Early occult lesions of glomus tumor in the hand may not be revealed by physical examination because of their barely detectable symptoms. Moreover, subungual lesions may be particularly difficult to evaluate on physical examination. Our cases showed that MRI offers excellent diagnostic information in clinically undiagnosed or misdiagnosed patients. Preoperative MRI can accurately define the character and extent of glomus tumor, even though it is impalpable and invisible.

Clinical and ultrasonographic studies for the diagnosis of ethylene glycol intoxication in dogs (개의 Ethylene glycol 중독 진단을 위한 임상 및 초음파학적 연구)

  • This study was performed to evaluate the ultrasonographic findings of ethylene glycol intoxication. Ten healthy mongrel dogs which was administered with ethylene glycol, were evaluated in terms of clinical findings, hematological findings, blood chemistry, and ultrasonographic and histopathological findings of kidney. The results obtained through these experiment could be summarized as follows : 1. Typical clinical symptoms such as vomiting, initial apprehension, depression, thirst, dehydration, tremor, anorexia, hematuria, anuria, weakness, weight loss, flaccid paralysis, tachypnea, coma, and death, were revealed after administration of ethylene glycol. 2. Special symptom of bloody diarrhea was occurred by administration of ethylene glycol. 3. After administration of ethylene glycol, PCV was decreased continuously(p<0.01), and total leukocyte count was increased gradually, revealed the highest value at day 5 and thereafter decreased. 4. Remarkable changes of ultrasonographic findings such as high echo intensity of renal parenchyma and emergence of halo in corticomedullary junction, were revealed after administration of ethylene glycol. Early(hour 8) enlargement and late(day 3) enlargement were observed in kidney(p<0.01). Especially, late enlargement was observed concurrently with the elevation of BUN and creatinine values. 5. Calcium oxalate crystals, metabolites of ethylene glycol, were observed in histopathologic findings of kidney. Also, degeneration and necrotic exfoliation of epithelial cells were recognized in addtion to swelling of renal tubules.

Surgical Treatment of Esophageal Cancer (식도종양의 외과적 치료)

  • Fourty nine patients out of 127 esophageal cancer were managed surgically from January 1986 to December 1991, at the Department of Thoracic and Cardiovascular Surgery, Jeonbuk National University Hospital. Most frequent preoperative symptom was dysphagia and its mean duration was 3.1 months. In histopathologic examination, squamous cell carcinomas were 44 cases [89.8% , and adenocarcinomas 5 [10.2% . The tumor location were the upper esophagus in 6.1%, middle esophagus in 57.2%, lower and cardiac portion of stomach in 36.7%. Involved and metastatic organs, which were detected perioperatively, were celiac lymph nodes in 6 cases, aorta 2, stomach 2, pericardium 2, cervical lymph node 1. The esophagus was resected radically, and the procedures for esophageal replacement were performed with esophagogastrostomy in 45 cases, esophagocologastrostomy 3, and esophagojejunostomy 1. Postoperative complications occurred in 16 cases [hospital morbidity = 32.6% ,anastomotic leak 3, anastomotic stricture 2, respiratory insufficiency 2, hemoperitoneum 1, chylothorax 1, intussusception 1, empyema 1, non-A,non-B hepatitis 1, and mediastinitis 1. Hospital deaths were experienced 3 cases [ hospital mortality = 6.1% . The 6 month, one, two, and five year actuarial survival rates were 85.7%, 71.4%, 57.1%, and 27.9%, respectively. One year survival rates of stages were 100% in stage I, 90.9% in stage IIa, 63.6% in stage IIb, 25.0% in stage III, and 7.2% in stage IV.

Kikuchi's Disease: Clinical Characteristics and Overview (괴사성 림프절염의 임상적 고찰)

  • Background and Objective: Kikuchi's disease(KD) is an idiopathic, self-limited lymphadenopathy that was described as a distinctive type of necrotizing lymphadenitis affecting primarily cervical lymph nodes of young adults independently by Kikuchi and Fujimoto et al at first in 1972. The purpose of this study is a knowledge about clinicopathologic findings, many laboratory tests and differentiation of KD from other lymphadenitis due to lymphoma, systemic lupus erythematosus(SLE) and many viral disease. Materials and Methods: Thirty-four case of KD collected at Chonnam University Hospital in Kwang-Ju from 1992 through 2000 were evaluated with retrospective chart review. Results: The patients were consisted of 11 men and 23 women. All patients had tender or nontender cervical mass and fever was the most common associated symptom. The others was pain, weight loss, chills, cold sweating and headache et al. Multiple bilateral involvement of cervical lymphnodes was 25 cases(74%) and solitary involvement was 9 cases(26%). In laboratory tests, leukopenia was 12 cases(75%), elevated ESR 5 cases (34%) and elevated LDH 11 cases(69%). Conclusion: KD is necessary to differentiate from lymphoma and SLE, because of the different of therapeutic modality and prognosis. The diagnosis is established on the basis of histopathologic studies with excisional biopsy of lymph node.
