고압맥동 평류자극이 가토 상처치유에 미치는 영향 (Effect of High Voltage Pulsed Galvanic Current on Wound Healing in Rabbits)

  • This study was performed to assess the efficacy of high voltage pulsed galvanic current for the healing of wounds in rabbits. Skin wounds were created laterally on the flank of 12 domestic rabbits($3{\times}3cm$). The wounds of each group were treated with an intensity of 170 V at a frequency of 70 pulses per second, which was applied for 30 minutes a day for 10 days. The experimental groups were randomly assigned to either EXP I (n=3), EXP II(n=3), EXP III(n=3) or control(n=3). Each group was stimulated under the following conditions : 1) EXP I (Negative polarity), 2) EXP II (Change in polarity, negative electrode stimulation during the first 3 days and then positive electrode stimulation from 4 to 10 days), 3) EXP III(Positive polarity), 4) control(No stimulation). An active electrode was placed over the wound and a dispersive electrode on the buttock. The rate of wound closure was compared with the original wound size, evaluated by a tracing film in each measurement period. Finally, on the wound in each group, skin tissue was excised for histological evaluation after treatment for 10 days. The results obtained are as follows : 1) It was found that the control group did not show a complete remodeling of epitherial layer and had a chronic inflammatory response. Judging from the irregularity of intercellular space and the loose alignment of connective tissue, these findings show that wound healing was delayed. 2) EXP I showed a significant bactericidal effect, but a moderate response of vasodilation. The rate of wound closure was slower when compared with EXP II, III. 3) EXP II showed a complete remodeling of epitherial layer and a positive repair of connective tissue. Its rate of wound closure was best when compared with the others. 4) EXP III had a slower rate of wound closure than EXP II, but judging from the greater proliferation of collagen fibers and the dense alignment of connective tissue, this positive electrode was very effective in the formation of neo - connective tissue.

Marjolin 궤양으로 발생한 편평 상피암의 치료결과 (Outcomes of Treatment for Squamous Cell Carcinoma Originating as a Marjolin's Ulcer)

  • 목적: Marjolin 궤양으로 발생한 편평 상피암의 치료결과에 대해 원발성 편평 상피암과 비교 분석하고자 한다. 대상 및 방법: Marjolin 궤양에 의한 편평 상피암으로 치료받았던 14예를 대상으로 하였으며, 같은 기간 치료받았던 원발성 편평 상피암 20예를 대조군으로 하였다. 평균 연령은 61.2세였으며, 남자 24예였고, 여자가 10예였다. 두 군간의 발생 부위, 조직학적 분류, 병기, 치료 방법, 전이, 재발, 생존율에 대해 비교 분석하였다. 결과: 평균 추시 기간은 54.8개월(12-168개월)이었다. 국소 재발은 6예에서 발생하였고, 5예는 Marjolin 궤양 군에서, 나머지 1예는 원발성 편평 상피암 군에서 발생하였다. 최초 진단 후 국소 재발까지의 평균 기간은 9개월(2-20개월)이었다. 전이는 총 6예에서 발생하였는데 이들 중 2예(14.3%)는 Marjolin 궤양 군에서, 나머지 4예(20.0%)는 원발성 편평 상피암 군에서 발생하였다. 전이 또는 국소 재발은 총 10예에서 발생하였는데 이들 중 6예는 Marjolin 궤양 군에서, 나머지 4예는 원발성 편평 상피암 군에서 발생하였다. 5년 무병 생존률은 Marjolin 궤양 군에서는 64.3%였고, 원발성 군에서는 95%였다. 결론: Marjolin 궤양에 동반된 편평 상피암은 적극적인 치료에도 불구하고 높은 재발율 및 사망률을 보이므로, 치료 결과를 향상시키는 새로운 치료법에 대한 연구가 요구된다.

조기 위암의 근치적 절제술 후 재발예측인자 (Risk Factors for Recurrence after Curative Surgery for Early Gastric Cancer)

  • Purpose: Even with excellent surgical outcome, recurrence of early gastric cancer (EGC) after a curative resection is not declining because the incidence of EGC is increasing. The aim of this study was to propose an appropriate treatment strategy by assessing the risk factors for recurrence of curatively resected early gastric cancer. Materials and Methods: Of 3662 patients who had undergone gastric resections for gastric cancer from 1987 to 1996, the cases of 1050 curatively resected EGC patients were reviewed retrospectively. Among those 1050 patients, 50 patients ($4.8\%$) were diagnosed as having recurrent cancer, which was confirmed by clinico-radiological examination or re-operation. The risk factors that determined the recurrence patterns were investigated by using univariate and multivariate analyses. Results: The mean time to recurrence was 30.9 months, and hematogenous recurrence was the most frequent type ($32.0\%$). Among the 50 recurred patients, peritoneal recurrence showed the shortest mean time to recurrence ($18.5\pm17.7$months). Between the recurred and the non-recurred patients, there was no statistically significant difference with respect to age, sex, operation type, tumor size, tumor location, gross appearance, or histological differentiation. However, depth of invasion (submucosal invasion) and nodal involvement were significantly different (P<0.001) between the two groups. Using logistic regression analyses, nodal involvement was the only significant risk factor for recurrence in early gastric cancer (P<0.001). The median survival after the recurrence had been diagnosed was 4 months. Conclusion: Although the prognosis for EGC patients is excellent and recurrence of EGC after a curative resection is rare, the time to recurrence and the patterns of recurrence in EGC patients were diverse and unpredictable, and the result after recurrence is dismal. Considering the impact of lymph node metastasis on recurrence of EGC, a systematic lymphadenectomy, rather than limited surgery, should be performed if lymph node involvement is confirmed pre- or intraoperatively. Also if the postoperative pathologic findings reveal lymph node involvement, adjuvant chemotherapy is recommended.

D-Galactosamine으로 유발된 간손상에 대한 민들레 열수추출물의 예방효과 (Hepatoprotective Activity of Dandelion (Taraxacum officinale) Water Extract against D-Galactosamine-Induced Hepatitis in Rats)

  • 본 연구에서는 실험동물에 민들레 열수추출물 식이를 급여한 후, GalN으로 간손상을 유발함으로써 그 예방효과를 혈액중의 생화학적 변화 및 간조직의 효소적인 변동을 통해서 규명하고자 하였다. GalN의 투여로 현저히 증가하였던 AST, ALT의 활성은 민들레 열수추출물 투여로 억제되었으나 군간의 차이는 보이지 않았고 ALP의 활성과 TBARS 함량은 3%의 추출물을 급여한 군에서 유의적인 감소를 보였다. GalN의 투여로 현격하게 높아졌던 혈중 $TNF-{\alpha}$의 농도 또한 감소하는 경향을 확인하였으나 유의적인 차이는 보이지 않았다. GalN의 투여로 억제되었던 catalase, GSH-reductase, Mn-SOD의 활성은 민들레 추출물 투여로 유의적인 회복이 관찰되었으나 GSH-px의 활성은 그 경향만을 확인할 수 있었다. 조직 검경을 통해 민들레 열수추출물의 간염 예방효과를 확인한 결과 GalN으로 인해 유발된 광범위한 간세포의 괴사와 변성, 지방변성 등이 민들레 열수추출물식이로 다소 감소하는 것을 관찰할 수 있었다. 이상의 결과로 민들레 열수추출물은 AST, ALT와 ALP의 활성 및 산화적 스트레스를 감소시키고 활성산소 해독계에 관여하는 효소의 활성을 증가시킴으로써 GalN으로 인한 간 손상을 예방하는 것으로 사료된다.

전암성 폐병변 및 편평상피세포폐암 조직에서 CBP(cAMP-responsive Ele-ment Binding Protein) 전사 공동 활성인자의 면역조직화학적 발현양상의 비교 (Comparison of Immunohistochemical Expression of CBP(cAMP-responsive Element Binding Protein) Transcriptional Co-activator between Premalignant Lesions and Squamous Cell Carcinomas in the Lungs)

  • 폐암의 발생은 여러 많은 유전자의 변화가 축적되어 나타나는 일련의 과정에 의한다. 세포 내 전사 조절 인자의 하나인 CBP는 폐를 포함한 인체 내 여러 조직에서 상피세포의 분화 및 증식에 중요한 역할을 담당하며, 유전자들에서 전사조절인자로서 세포의 성장에 관여하며 발암 과정에서도 중요할 것으로 기대된다. 이에 아직까지 폐암에서 CBP에 대한 연구가 확정된 바가 없어, 폐의 전암성 병변(상피 화생 20예, 이형성증 40예) 및 편평상피세포폐암 60예를 대상으로 하여 CBP의 발현정도를 면역화학적 방법으로 비교 분석하여 다음과 같은 결과를 얻었다. 1) 화생성 병변(7예; 35%)에 비해 이형성 병변(26례; 65%)이나 편평세포암종(42례; 70%)에서 CBP의 발현이 유의하게 높았다(p<0.05). 2) 이형성 병변의 경우, 경도의 이형성 병변(20예 중 10예; 50%)보다 고도의 이형성 병변(20예 중 16예; 80%)에서 높은 CBP의 발현율을 보였다(p<0.01). 3) 편평세포암의 분화도별로 살펴보았을 때, 고분화암에서 95%(20예 중 19예), 중등도 분화암에서 85%(20예 중 17예), 저분화 암에서는 30%(20예 중 6예)의 발현율을 보였다(p<0.05). 이상과 같은 결과를 볼 때, CBP는 폐 조직에서 정상 기관지 상피 세포가 전암성 병변으로 변하거나 전암성 병변이 암으로 진행하는 과정에서 중요한 역할을 하는 것으로 보이며, 세포가 암으로의 발전할 수 있는 잠재성을 가늠하는 표지자가 될 수 있을 것으로 보인다.

후두실 탈출증 1례 (A case of prolapse of the laryngeal ventricle)

  • 후두실의 탈출은 Bryce 및 Cranston에 의하면 후두실 점막이 염증성 비후 또는 부종으로 인해 후두강내에 탈출된 것이라고 했다. 환자는 53세의 여자로서 1977년 3월 11일 호흡곤란과 사성의 주소를 갖고 본원에 입원하였다. 입원당시 후두소견은 가성대 하부에 표면이 평활하고 둥근 연홍색의 종물이 양측으로부터 돌출되어 기도를 거의 폐쇄하고 있었으며, 종물에는 궤양은 없었으며 주위 조직에 침윤된 소견은 볼 수 없었다. 경부에는 임파선비대를 촉진할 수 없었다. 호흡곤란이 심하여 기관절개술을 실시한 후에 직달후두경하에 조직검사를 하였으며, 검사결과는 만성염증으로 판정되었다. 1977년 4월 2일 전신마취하에 후두절개술을 시행하여 후두강을 관찰한 결과, 후두실의 앞에서 뒤까지 전역에 걸쳐 기시된 표면이 평활한 판상의 연홍색 종물이 양쪽에서 상대적으로 후두강내로 자라나와 중앙부에서는 거의 서로 맞닿아 있었다. 제거된 종물의 조직검사 결과도 술전과 같이 염증성비후로 나타났다. 수술후 경과는 양호하여 현재 호흡곤란은 완전히 소실되었고 음성은 경도의 사성을 호소할 뿐이고 후두강내에는 종물의 재발 증후는 없었다. 본증은 임상증상, 수술시의 후두강 소견 및 조직소견으로 보아 "후두실 탈출증"이라고 진단되었다. 이 후두실 탈출증은 문헌상 희소한 것으로서 일증례를 문헌적 고찰과 함께 보고하는 바이다.

간섬유화 동물에서 D-페니실라민의 항섬유화 효과 검색 (The Antifibrotic Effects of D-penicillamine in Liver Fibrosis Animal)

  • One of therapeutics in liver disease (morbus wilson) is D-penicillamin (D-pen: D-3-mercapto-valin). Especially the cross-linking of collagen molecules could be inhibited by D-pe n in extracellular space. In this study we investigated the antifibrotic effects of D-pen in rats that were induced the liver fibrosis by bile duct ligation and scission (BDL/S). Rats were treated for 4 weeks with D-pen after BDL/S operation or sham operation. The balance between fibrogenesis-marker (PNIIIP) and the fibrolysis-maker (PNIVP) were observed in sera by RIA (radioimmunoassay), and the parameter of collagen deposition in liver tissue (hydroxyproline: HYP) was measured by colorimetry. The weight of liver in BDL/S operated group was increased significantly in compared with sham operation group (15.2g${\pm}$1.1, vs 11.9g${\pm}$3.9: p<0.005, p<0.05). The rats group treated by D-pen showed the lower level of PNIIIP (6.7ng/ml${\pm}$1.5, vs 9.5ng/ml${\pm}$2.8) and the higher value of PIVCP (14.0ng/ml${\pm}$1.9, vs 7.9ng/ml${\pm}$1.5) in sera that compared to untreated rats. The content of HYP was decreased by 141% in BDL/S with D-pen treated group than that of it in BDL/S group. No correlation was revealed between collagen parameters in sera and HYP in liver tissue of BDL/S operated and D-pen treated rats. The group treated with D-pen showed the lower value of clinical biochemistry parameters (GOT: glutamate oxalacetate transaminase, Total-Bilirubin) in compared with only BDL/S operated rats, but the value of GPT (glutamate pyruvate transaminase) and Alkaline phosphatase in two BDL/S groups was nearly same. In the histological finding, we observed mild bile duct proliferation, weak inflammation and fibrosis in BDL/S with D-pen treated group, but BDL/S operated group showed the formation of septum (island of hepatocytes), massive bile duct proliferation. This result represents that the BDL/S operation induces liver fibrosis (cirrhosis) in 4 weeks, and D-pen inhibits the synthesis of collagen weakly and stimulates the degradation of collagen in the extracellular space. We conclude that the monitoring of PNIIIP, PIVCP in sera is useful parameter for screening of antifibrotic effect, and D-pen delay the liver fibrosis.

급성 위염 유발 마우스 동물 모델에서 구기자(枸杞子) 에탄올 추출물의 위점막 손상 보호 효과 (Protective Effects of a Lycium chinense Ethanol Extract through Anti-oxidative Stress on Acute gastric lesion mice)

  • Objectives : Gastric lesions affect many people around the world and their development are results of the imbalance between destructive and protective factors in the gastric mucosa. Lycium chinense has been widely used as a traditional Korean medicine, it was recently reported that they have potent anti-inflammatory effects in chronic hepatitis models. Therefore, this study aimed to investigate the anti-inflammatory activity of Lycium chinense extract (LCE) on HCl-Ethanol induced gastric lesion mice.Methods : The ICR mice were divided randomly into five groups of six animals each. Group A was normal mice, and group B was treated orally with 0.5 ml 150 mM HCl-60% Ethanol. Mice in group C and D were pre-treatment of LCE (100 mg/kg and 200 mg/kg bodyweight, p.o before HCl/ethanol treatment) and group E was orally administered sucralfate (10 mg/kg).Results : 150mM HCl/60% ethanol-induced gastric mucosal injury mice were ameliorated mucosal damage upon histological evaluation by treatment of LCE. Pre-treatment of LCE attenuated reactive oxidative species (ROS) and produces peroxynitrite (ONOO-) in stomach tissues. As results of stomach protein analyses, LCE effectively reduce inflammatory-related factors such as cyclooxygenase-2 (COX-2), tumor necrosis factor alpha (TNF-α), inducible nitric oxide synthase (iNOS) and interleukin-6 (IL-6) in gastric lesion mice. In addition, nuclear factor kappa B (NF-κB) and inhibitor of phosphorylation of nuclear factor kappa B (p-IκB) were down-regulated in LCE-administrated gastric lesion mice.Conclusions : Our discovery supports that the therapeutic activity of LCE ameliorate the development of gastric lesion via suppressing the oxidative stress and gastric partial inflammation induced by 150 mM HCl/60% ethanol.

난알부민으로 알레르기 비염이 유발된 마우스에서 형개의 개선효과 (Effects of Schizonepeta Spica water extrect on the OVA-induced BALB/c mice model of allergic rhinitis)

  • Objectives : The aim of this study was to evaluate the effect of Schizonepeta Spica water extract (SS) on the OVA-induced BLAB/c mice allergic rhinitis model.Methods : Thirty two BALB/c mice were equally assigned to 4 groups; the sham group, the control group, the cetirizine hydrochloride (Cet) treatment group, and the SS treatment group. Sham group was sensitized and challenged with saline, and the other groups were sensitized and challenged with OVA. The dosage of SS was 7.6 mg /kg·day, and Cet was 10 mg/kg·day. Nasal rubbing and sneezing were measured by the behavior observation. The concentrations of IL-1β, IL-10, TNF-α and MIP-2 in the sera of allergic rhinitis model were measured by mouse cytokine/chemokine magnetic bead panel kits. Total IgE and OVA-specific IgE were measured by ELISA method. Epithelial thickness and eosinophil infiltration of nasal septum was investigated by histological examination.Results : The clinical symptoms that increased in control group were significantly reduced in SS-treated group. Serum total IgE and OVA-specific IgE in the SS-treated group were significantly reduced compared to the control group. The concentration of IL-1β, IL-10, TNF-α and MIP-2 in SS-treated group showed a significant reduction compared to the control group. The infiltration of eosinophil into nasal tissues of SS-treated group decreased markedly compared to control group, and thickness of nasal septum in nasal mucosa showed a significant reduction compared to control group.Conclusions : According to the above result, it is suggested that SS may inhibit the early and late phase of allergic rhinitis reaction.

민꽃게 추출물의 역류성 식도염 동물모델에서 유효성 평가 (Effect on rat model of reflux esophagitis treated with Charybdis japonica extract)

  • Objectives : In this study, we investigated the protective effects of Charybdis japonica (C. japonica) water extract on the acute reflux esophagitis in rat models. Methods : Twenty rats were divided into four groups for examination: normal control group (n=6), the reflux esophagitis group (n=6), reflux esophagitis treated with positive control group (ranitidine 40 mg/kg, n=6), reflux esophagitis treated with C. japonica group (100 mg/kg, n=6). All rats fasted for 18 hr and then were induced with reflux esophagitis by a pylorus and forestomach ligation operation. After 4 hr, the rats were sacrificed. The proinflammatory cytokine and proteins expression measured by western bolt assay, and the histopathological analysis of the esophageal mucosa measured by hematoxylin and eosin staining. Results : C. japonica administration significantly was protecting esophageal mucosal damage upon histological analysis of reflux esophagitis in rats. The C. japonica treatment confirmed the protection of the reduction of claudin-5, an evaluation index of the damage of tight junctions in the reflux esophagitis. C. japonica was also found to inhibit the expression of proteins such as COX-2 and TNF-α in the rat esophagus. C. japonica markedly attenuated the activation of NF-κB and phosphorylation of IκBα at the same time. Conclusion : These results indicated that C. japonica suppressed the development of esophagitis through the modulation of inflammation by regulating NF-κB activation. Based on these findings, we concluded that C. japonica can prevent reflux esophagitis.
