Super-resolution Convolutional Neural Network를 이용한 전산화단층상의 화질 평가 (Image Quality Evaluation in Computed Tomography Using Super-resolution Convolutional Neural Network)

  • 고화질의 전산화단층촬영상을 통해 정확한 병변 검출과 진단을 할 수 있다. 이와 같은 장점 때문에 전산화단층촬영 시 방사선량을 줄이면서 영상 화질을 개선하기 위해 많은 연구가 수행되었다. 최근 전산화단층촬영상 화질을 향상시키기 위한 딥러닝 기반 기술이 개발되었고, 기존의 기술에 비해 우수한 성능을 보이고 있다. 본 연구에서는 전산화단층촬영상의 공간분해능을 향상시키기 위해 초고해상도 합성곱 신경망 모델을 사용하였으며, 초고해상도 합성곱 신경망 모델의 성능을 결정하는 초 매개 변수 변화에 따른 영상 화질을 평가하여 초고해상도 합성곱 신경망 모델에 대한 초 매개 변수의 효과를 검증하였다. Profile, 구조적 유사성 지수, 최대신호 대 잡음비 및 반치폭을 측정하여 초 매개 변수 변화에 따른 초고해상도 합성곱 신경망 모델의 성능을 평가하였다. 연구결과, 초고해상도 합성곱 신경망 모델의 성능은 epoch와 training set이 증가함에 따라 향상되었으며, 전산화단층촬영상 화질을 향상시키기 위해 learning rate 최적화가 필요하다는 사실을 확인하였다. 따라서 최적의 초 매개 변수와 함께 구현된 초고해상도 합성곱 신경망 모델은 전산화단층촬영상의 품질을 향상시킬 수 있다.

  • With the increasing resolution of modern CT scanners, analysis of the larger numbers of images acquired in a lung screening exam or diagnostic study is necessary, which also needs high accuracy and reproducibility. Recent developments in the computerized analysis of medical images are expected to aid radiologists and other healthcare professional in various diagnostic tasks of medical image interpretation. This article is to provide a brief overview of some of computer-aided diagnosis schemes in chest CT.

  • Multi-detector row CT (MDCT) provides faster speed, longer coverage in conjunction with thin slices, improved spatial resolution, and ability to produce high quality muliplanar and three-dimensional (3D) images. MDCT has revolutionized the non-invasive evaluation of the central airways. Simultaneous display of axial, multiplanar, and 3D images raises precision and accuracy of the radiologic diagnosis of central airway disease. This article introduces central airway imaging with MDCT emphasizing on the emerging role of multiplanar and 3D reconstruction.

CBCT에서 관찰되는 trifid mandibular canal (Trifid mandibular canal in Cone-Beam CT : A case report)

  • Trifid mandibular canal (TMC) is one of the anatomical variation of mandibular canal with clinical importance. An extra mandibular canal may explain inadequate anesthesis and be damaged causing paresthesia or bleeding during mandibular surgery. CBCT with high-level spatial resolution is an useful tool for the detection of mandibular canal and its variation. The aim of this report is to present a case of trifid mandibular canal with CBCT images and to give information on this anatomical variation of mandibular canal.

Diagnostic Approaches for Idiopathic Pulmonary Fibrosis

  • Idiopathic pulmonary fibrosis (IPF) is a chronic, progressive, fibrosing interstitial pneumonia with a very poor prognosis. Accurate diagnosis of IPF is essential for good outcomes but remains a major medical challenge due to variability in clinical presentation and the shortcomings of existing diagnostic tests. Medical history collection is the first and most important step in the IPF diagnosis process; the clinical probability of IPF is high if the suspected patient is 60 years or older, male, and has a history of cigarette smoking. Systemic assessment for connective tissue disease is essential in the initial evaluation of patients with suspected IPF to identify potential causes of interstitial lung disease (ILD). Radiologic examination using high-resolution computed tomography plays a pivotal role in the evaluation of patients with ILD, and prone and expiratory computed tomography images can be considered. If additional tests such as surgical lung biopsy or transbronchial lung cryobiopsy are needed, transbronchial lung cryobiopsy should be considered as an alternative to surgical lung biopsy in medical centers with experience performing this procedure. Diagnosis through multidisciplinary discussion (MDD) is strongly recommended as MDD has become the cornerstone for diagnosis of IPF, and the scope of MDD has expanded to monitoring of disease progression and suggestion of appropriate treatment options.

  • As an advancement of medical imaging modalities and analyzing software with multi-function, active researches to acquire high contrast and high resolution image being done. In recently, development of medical imaging modalities like as Computed Tomography (CT) and Magnetic Resonance Imaging (MRI) is aiming to display anatomical structure more accuracy and faster. Thus, one of the important areas in CT today is the use of CT scanner for the quantitative evaluation of 3-D reconstruction images from 2-D tomographic images. In CT system, the effective slice thickness and the quality of 3-D reconstructed image will be influenced by imaging acquisition parameters (e.g. pitch and scan mode). In diagnosis and surgical planning, the accurate distance measurements of 3-D anatomical structures play an important role and the accuracy of distance measurements will depend on the acquisition parameters such as slice thickness, pitch, and scan mode. The skull phantom was scanned with SDCT for various acquisition parameters and acquisition slice thicknesses were 3 and 5 mm, and reconstruction intervals were 1, 2, and 3 mm to each pitch. 3-D visualizations and distance measurements were performed with PC based 3-D rendering and analyzing software. Results showed that the image quality and the measurement accuracy of 3-D SDCT images are independent to the reconstruction intervals and pitches.

전산화단층촬영장비 테이블의 에어 매트리스 적용에 따른 화질평가 (Image Quality Evaluation according to the Application of Air Mattress on Computed Tomography Equipment Table)

  • 전산화단층촬영장비 테이블로 인한 화질 저하의 경감을 위하여 개발한 에어 매트리스(Air mattress)의 유용성을 평가하고자 하였다. 5, 10 cm 두께의 에어 매트리스(Air mattress)를 개발하여 적용 유무와 두께에 따른 AAPM 팬텀을 스캔한 영상을 통하여 화질을 측정하였으며 일원배치 분산분석으로 통계적 유의성을 확인하였다. AAPM 팬텀을 표준 방식으로 스캔한 영상과 비교했을 때 에어 매트리스를 적용한 영상은 차이가 나타나지 않았으나, 에어 매트리스를 적용하지 않았을 때의 영상은 CT Number와 균일도가 낮아지고 노이즈가 높게 나타났으며 공간분해능이 낮아지는 것으로 나타났다. 개발된 에어 매트리스는 진단 영상에 아무런 해가 없을 뿐만 아니라, 화질을 높이는 데에 매우 효과적이며 최신, 고가 장비에 적용되는 기술을 이용하지 않고도 기존의 장비에 단순히 에어 매트리스를 적용함으로써 화질을 증가시킬 수 있다.

Duplicated Internal Auditory Canal: High-Resolution CT and MRI Findings

  • Objective: To summarize the high-resolution computed tomography (HRCT) and magnetic resonance imaging (HRMRI) features of duplicated internal auditory canals (DIACs). Materials and Methods: Ear HRCT data of 64813 patients with sensorineural hearing loss (SNHL), obtained between August 2009 and November 2017, were reviewed. Among these patients, 12 (13 ears) were found to have DIACs, 9 of whom underwent HRMRI. Their images were evaluated by two otoradiologists. Results: The rate of occurrence of DIAC among SNHL patients was 0.019% (12/64813). The internal auditory canals of 13 ears were divided into double canals by complete (n = 6) and incomplete (n = 7) bony septa, with varied orientations ranging from horizontal to approximately vertical. All of the anterosuperior canals extended into the facial nerve (FN) canal, except for 1, which also extended to the vestibule. The posteroinferior canals ended in the cochlea and vestibule, except for 2, which also connected to the FN canals. Magnetic resonance images revealed that 77.8% (7/9) and 22.2% (2/9) of vestibulocochlear nerves (VCNs) were aplastic and hypoplastic, respectively. Furthermore, 88.9% (8/9) of FNs were normal, except for 1, which was hypoplastic. All of the affected ears also had other ear anomalies: a narrow, bony cochlear nerve canal was the most common other anomaly, accounting for 92.3% (12/13). Malformations of other systems were not found. Conclusion: Double-canal appearance is a characteristic finding of DIAC on HRCT, and it is usually accompanied by other ear anomalies. The VCN usually appears aplastic, with a normal FN, on HRMRI.

Three-dimensional microstructure of human alveolar trabecular bone: a micro-computed tomography study

  • Purpose: The microstructural characteristics of trabecular bone were identified using micro-computed tomography (micro-CT), in order to develop a potential strategy for implant surface improvement to facilitate osseointegration. Methods: Alveolar bone specimens from the cadavers of 30 humans were scanned by high-resolution micro-CT and reconstructed. Volumes of interest chosen within the jaw were classified according to Hounsfield units into 4 bone quality categories. Several structural parameters were measured and statistically analyzed. Results: Alveolar bone specimens with D1 bone quality had significantly higher values for all structural parameters than the other bone quality categories, except for trabecular thickness (Tb.Th). The percentage of bone volume, trabecular separation (Tb.Sp), and trabecular number (Tb.N) varied significantly among bone quality categories. Tb.Sp varied markedly across the bone quality categories (D1: $0.59{\pm}0.22mm$, D4: $1.20{\pm}0.48mm$), whereas Tb.Th had similar values (D1: $0.30{\pm}0.08mm$, D4: $0.22{\pm}0.05mm$). Conclusions: Bone quality depended on Tb.Sp and number-that is, endosteal space architecture-rather than bone surface and Tb.Th. Regardless of bone quality, Tb.Th showed little variation. These factors should be taken into account when developing individualized implant surface topographies.

Micro-computed tomography evaluation of general trends in aligner thickness and gap width after thermoforming procedures involving six commercial clear aligners: An in vitro study

  • Objective: To assess the effects of thermoforming on aligner thickness and gap width in six aligner systems with the same nominal thickness. Methods: Six passive upper aligners of different brands were adapted to a single printed cast. Each sample was evaluated with high-resolution micro-computed tomography. To investigate aligner thickness and gap width, two-dimensional (2D) analysis was conducted assessing the effects of the following variables: tooth type (central incisor, canine, and first molar), 2D reference points, and aligner type. Data were analyzed and compared using analysis of variance and Tukey's post-hoc tests (p < 0.05). Results: Tooth type, dental region, and aligner type affected both the gap width and aligner thickness. The aligner thickness remained moderately stable across the arch only in the F22. Conclusions: All thermoformed samples displayed smaller aligner thickness and gap width at anterior teeth and both gingival and coronal centers than at posterior teeth and occlusal surfaces.
