일부 농촌지역 주민의 High Sensitivity C-reactive Protein(hsCRP)과 고혈압의 관련성 (The Association of High Sensitivity C-reactive Protein(hsCRP) with Hypertension in Some Rural Residents)

  • Objectives : This study was performed to assess the association between high sensitivity C-Reactive Protein (hsCRP) and hypertension. Methods : We evaluated the relationship between hsCRP with hypertension and other cardiovascular risk factors, using a cross-sectional survey of 202 people over the age of 50, living in a rural area. A logistic regression analysis was used to study the association between hsCRP and hypertension. The hsCRP levels were divided in quartiles, and the odds ratios (OR), with 95% confidence intervals (95% CI), calculated, using the lowest quartile as a reference. Results : The subjects consisted of 37.1% men and 62.9% women, with a mean (SD) hsCRP level of $1.9({\pm}3.0mg/{\ell})$ . The overall prevalence of hypertension was 61.4%. The prevalence of hypertension according to the hsCRP quartile was not statistically significant. After adjustment for confounding variables, the prevalence of hypertension according to the subjects in the 2nd, 3rd and 4th hsCRP quartiles were 1.418 (95% CI=0.554-3.628), 1.124 (95% CI=0.392-3.214) and 0.892 (95% CI=0.312-2.547) times higher, respectively, compared to those in the 1st quartile. Conclusions : The results showed that the level of hsCRP was not a risk factor for hypertension among adults aged over 50 years, living in a rural area. A further study should be performed to find the association between hsCRP and hypertension.

비알코올성 지방간 소견을 보이는 성인에 대한 간 기능 및 hs-CRP 혈액 검사 항목 평가 (Evaluation of Liver Function and Blood Exam including hs-CRP in Adults with Nonalcoholic Fatty Liver Findings)

  • 지방간 진단을 위한 검사로, 최근 초음파 검사와 혈액검사를 동시에 병행하여 시행하고 있다. 특히 혈액검사 중 hs-CRP의 경우, 심혈관 질환 뿐 아닌, 인체의 다양한 부위에 대한 염증 수치를 나타내는 지표로 사용되고 있다. 따라서 본 연구를 통해 비알콜성 지방간 정도에 따른 대사증후군 구성 요소와 간 기능 및 hs-CRP수치 등의 연관성을 분석해 지방간 진단 임상 지표로 활용하고자 연구를 진행하였다. 2021년 3월~2021년 8월 한국 건강관리 협회 광주 전남지부에서 복부 초음파 검사를 실시하여 비알콜 지방간이 관찰된 환자 중 대사증후군 구성요소와 간 기능 및 hs-CRP 검사를 모두 실시한 만 20세 이상, 남녀 총 1,139명의 피검사 수치 데이터를 대상으로 하였다. 남녀 전체를 대상으로 분석한 경우 경도 지방간 환자의 간 검사 수치가 AST 30 U/L, ALT 32.1 U/L hs-CRP 0.14 mg/dL로 중등도 지방간 환자의 혈액 검사 수치 AST 38 U/L, ALT 47.6 U/L, 54.9 IU/L, hs-CRP 0.22 mg/dL보다 낮았으며 통계적으로 유의했다(P<0.001). hs-CRP 검사의 경우 경도 지방간 환자보다 중등도 지방간 환자에서 통계적으로 유의할 만큼 높은 수치 차이를 나타낸 것으로 보아 hs-CRP 검사가 지방간 예방과 관리를 위한 임상적 데이터로도 활용될 수 있을 것으로 사료된다.

한국 성인의 치주질환과 고감도 C-반응단백질 농도의 상관관계 (Relationship between periodontal disease and level of high-sensitivity C reactive protein in Korean adults)

  • Objectives: The purpose of this study was to comprehensively examine the relationship between periodontal disease and high-sensitivity C reactive protein (hs-CRP) level. Methods: This study was conducted using the data from the sixth Korea National Health and Nutrition Examination Survey. SPSS 18.0 for Windows was used for statistical analysis. The data were collected from 4,576 subjects aged ≥19 years. To analyze the association between periodontal disease and hs-CRP level, chi-squared test and logistic regression analysis were used. Results: Consequent to correcting all the disturbance variables, the moderate risk of hs-CRP was 1.39 times higher in patients with periodontal disease than in those without (OR=1.39; 95% CI:1.14-1.69), whereas the high risk of hs-CRP was 1.10 times but there was no statistical significance (OR=1.10; 95% CI:0.79-1.53). Conclusions: Periodontal disease contributes to raising the risk of systemic inflammation and hs-CRP from low to moderate. Periodontal disease is associated with an early rise in hs-CRP.

사상체질(四象體質)과 심혈관질환 예측인자인 High-sensitivity C-reactive protein의 상관성에 관한 연구 (Impact of the Sasang Constitutional Type on the High-Sensitivity C-Reactive Protein, a Predictor of Cardiovascular Disease)

  • 1. Objectives The purpose of this study was to investigate the high-sensitivity C-reactive protein (hsCRP) related characteristics found in each Sasang Constitutional type. 2. Methods This cross-sectional epidemiological study was conducted using data from 2,842 men and women. After typing each participant into one of the four Sasang constitutional types, they were analyzed on various cardiovascular disease-related variables. 3. Results 1) The predictors of cardiovascular disease (metabolic syndrome, high risk FRS group, high risk hsCRP group) was found to show the highest prevalence in the Taeeum type. 2) In the group with metabolic syndrome, the mean hsCRP concentration in the Taeeum and Soyang types were higher than in the Soeum type. In the group without metabolic syndrome, the mean hsCRP concentration in the Taeeum type were higher than in the Soyang and Soeum types. 3) In the FRS low risk group, the mean hsCRP concentration were higher in order of Taeeum type > Soyang type > Soeum type. In the FRS high risk group, the mean hsCRP concentrations were not significantly different among the Sasang constitutional types. 4. Conclusions The results of this study suggest that the Taeeum constitutional type is a risk factor for high hsCRP and cardiovascular disease. These findings suggest that the prevention of cardiovascular disease is more important in the Taeeum type compared to other constitutional types. Moreover, preventive measures are warranted even in the Taeeum-type persons with low clinical cardiovascular risk.

Investigation of High-Sensitivity C-reactive Protein and Erythrocyte Sedimentation Rate in Low Back Pain Patients

  • Background: Chronic low back pain can be a manifestation of lumbar degenerative disease, herniation of intervertebral discs, arthritis, or lumbar stenosis. When nerve roots are compromised, low back pain, with or without lower extremity involvement, may occur. Local inflammatory processes play an important role in patients with acute lumbosciatic pain. The purpose of this study was to assess the value of erythrocyte sedimentation rate (ESR) and high sensitivity C-reactive protein (hsCRP) measurements in patients with chronic low back pain or radiculopathy. Methods: ESR and hsCRP were measured in 273 blood samples from male and female subjects with low back pain and/or radiculopathy due to herniated lumbar disc, spinal stenosis, facet syndrome, and other diseases. The hsCRP and ESR were measured prior to lumbar epidural steroid injection. Results: The mean ESR was 18.8 mm/h and mean hsCRP was 1.1 mg/L. ESR had a correlation with age. Conclusions: A significant systemic inflammatory reaction did not appear to arise in patients with chronic low back pain.

Association between obesity and high-sensitivity C-reactive protein in Korean adults without cardiovascular disease

  • Purpose: This study aimed to investigate the association between obesity and high-sensitivity C-reactive protein (hs-CRP) levels in Korean adults without cardiovascular disease (CVD). Methods: The subjects were 3,634 adults, and data were extracted from the seventh Korea National Health and Nutrition Examination Survey (KNHANES VII-3). A complex sampling design analysis was applied to reflect the stratified and clustered weights. The data were analyzed using the complex sample Rao-Scott chi-square test and multiple logistic regression analysis (in SPSS for Windows version 26.0). Obesity, according to body mass index (BMI), was defined as obesity (BMI = 25-29.9 kg/m2), high obesity (BMI = 30-34.9 kg/m2), and super-high obesity (BMI ≥ 35 kg/m2), and abdominal obesity (AO) was defined as a waist circumference (WC) ≥ 90 cm in males and WC ≥ 85 cm in females. Results: The odds ratios for moderate CVD risk (hs-CRP; 1-3 mg/dL) were 2.21, 4.16, and 7.13 in the obesity, high obesity, and super-high obesity groups, respectively, compared to the normal BMI group. The odds ratio for moderate CVD risk was 2.18 in males with AO and 1.88 in females with AO. The odds ratios for high CVD risk (hs-CRP > 3 mg/L) were 4.40 and 17.55 in the high obesity and super-high obesity groups, respectively, compared to the normal BMI group. Conclusion: This study provides evidence that early detection and prevention programs for CVD should include obesity-related interventions aiming to modulate hs-CRP.

High-Sensitivity C-Reactive Protein and Risks of All-Cause and Cause-Specific Mortality in a Japanese Population

  • Background: High-sensitivity C-reactive protein (hsCRP) levels are lower in Japanese compared with Western subjects. Since it is uncertain whether hsCRP is a potent predictor of mortality at low CRP concentrations, the present study examined associations with all-cause and cause-specific mortality in a large population of Japanese. Materials and Methods: Subjects were 4,737 men and 6,343 women aged 49-76 years participating in the baseline survey of an ongoing cohort study of lifestyle-related diseases between February 2004 and July 2006. Hazard ratios for all-cause and cause-specific mortality associated with hsCRP levels were estimated using Cox proportional hazards regression. Results: A total of 436 all-cause deaths occurred during a median follow-up of 8 years. The main cause of death was cancer. In men, hsCRP levels were positively associated with the risk of all-cause mortality as well as deaths from cancer and cardiovascular disease (CVD). All-cause mortality hazards for the 2nd (0.34-0.84 mg/L) and the 3rd (${\geq}0.85mg/L$) tertiles of hsCRP were 1.27 (95% confidence interval [CI], 0.93-1.73) and 1.75 (1.30-2.37), respectively (p for trend=0.001). In women, increased risk of all-cause and cause-specific mortality associated with elevated hsCRP levels was observed, but the associations were not statistically significant. Conclusions: HsCRP may be an independent predictor of all-cause, cancer and CVD mortality in apparently healthy Japanese men, but not women. The differential effect of hsCRP in predicting mortality risk by sex warrants further investigation.

Positive Interaction Between CG, CC Genotypes of Cryptochrome Circadian Clocks 1, and Energy-Adjusted Dietary Inflammatory Index on High Sensitivity C-Reactive Protein Level in Women With Central Obesity

  • Creating a complex balance between dietary composition, circadian rhythm, and the hemostasis control of energy is important for managing diseases. Therefore, we aimed to determine the interaction between cryptochrome circadian clocks 1 polymorphism and energy-adjusted dietary inflammatory index (E-DII) on high-sensitivity C-reactive protein in women with central obesity. This cross-sectional study recruited 220 Iranian women aged 18-45 with central obesity. The 147-item semi-quantitative food frequency questionnaire was used to assess the dietary intakes, and the E-DII score was calculated. Anthropometric and biochemical measurements were determined. By polymerase chain response-restricted length polymorphism method, cryptochrome circadian clocks 1 polymorphism was assigned. Participants were categorized into three groups based on the E-DII score, then categorized according to cryptochrome circadian clocks 1 genotypes. The mean and standard deviation of age, BMI, and high-sensitivity C-reactive protein (hs-CRP) were 35.61 ± 9.57 years, 30.97 ± 4.16 kg/m2, and 4.82 ± 5.16 mg/dL, respectively. The interaction of the CG genotype and E-DII score had a significant association with higher hs-CRP level compared to GG genotype as the reference group (β, 1.19; 95% CI, 0.11-2.27; p value, 0.03). There was a marginally significant association between the interaction of the CC genotype and the E-DII score with higher hs-CRP level compared to the GG genotype as the reference group (β, 0.85; 95% CI, -0.15 to 1.86; p value, 0.05). There is probably positive interaction between CG, CC genotypes of cryptochrome circadian clocks 1, and E-DII score on the high-sensitivity C-reactive protein level in women with central obesity.

폐경 전후 여성의 요추 골밀도와 심혈관 위험인자와의 연관성 (The Relationship Between Lumbar Spine Bone Mineral Density and Cardiovascular Risk Factors in Premenopausal and Postmenopausal Women)

  • 최근 연구들은 골다공증과 동맥경화증의 병인 간의 연관성을 제시한다. 이 연구에서는 건강한 중년 여성에서 high-sensitivity C-reactive protein(hs-CRP), 지질 농도 등을 포함한 심혈관 위험인자들과 요추 골밀도의 상관관계를 분석하였다. 총 300명의 성인여성을 대상으로 하여 신체계측을 시행하였고, 공복혈당, 지질, hs-CRP를 포함한 심혈관질환 위험인자들을 측정하였다. 동맥경화지수는 총콜레스테롤 농도를 고밀도지단백 콜레스테롤 농도로 나누어서 계산하였다. Dual X-ray abosorptiometry를 이용하여 요추 골밀도를 측정하였다. 연구결과 연령, 수축기 및 이완기 혈압, 총 콜레스테롤, 고밀도지단백 콜레스테롤은 요추 골밀도와 음의 상관관계를 보였다. 통계적으로는 연령과 체질량지수, 고밀도지단백 콜레스테롤이 요추 골밀도와 의미 있는 상관관계를 보였다. 요추 골밀도에 영향을 미치는 심혈관 인자들은 연령과 고밀도지단백 콜레스테롤이 요추 골밀도($R^2=0.272$)의 독립적 위험인자로 나타났다.

조현병 및 조현정동장애 입원환자에서 혈청 염증표지자와 정신병적 증상과의 관련성 (Relationship between Serum Inflammatory Marker and Psychotic Symptoms in Inpatients with Schizophrenia or Schizoaffective Disorder)

  • Objectives Despite the growing research interest in the role of immunological markers in schizophrenia, a few studies, with conflicting results, have focused on the association between high sensitivity C-reactive protein (hs-CRP) levels and clinical characteristics in schizophrenia. The aim of the present study was to examine the association of serum hs-CRP with psychopathology in schizophrenia. Methods Fifty-five inpatients with schizophrenia or schizoaffective disorder were enrolled. Serum levels of hs-CRP were measured, and each patient was assessed with the Korean version of the Positive and Negative Syndrome Scale (PANSS). Results In correlation analysis of hs-CRP with PANSS subscales, positive subscale score has significant positive correlation (r = 0.271, p = 0.046). In independent t-test analysis, subjects with hs-CRP > 0.3 mg/dL (elevated CRP group, n = 43) had significantly higher PANSS positive subscale score (t = -3.273, df = 24.107, p = 0.003) than those with hs-CRP ${\leq}$ 0.3 mg/dL (normal CRP group, n = 12). Conclusions Elevated serum levels of high sensitivity C-reactive protein in schizophrenia are associated with the severity of psychotic symptoms.
