The Effects of Paced Breathing in Specific Respiration Rate on Heart Rate Variability (특정 호흡수에서 행한 통제호흡이 심박변이도에 미치는 영향)

  • Objectives We observed what effects of Paced Breathing(PB) in specific respiration rate have on heart rate variability (HRV) according to Sasang Constitution.Methods HRV of 72 healthy participants in sitting position was recorded while they carried out usual breathing, 0.2Hz, 0.1Hz, and 0.05Hz PB each 5 minutes in consecutive order. Five minute of relaxation was permitted between each breathing. Finally, HRV indices were statictically analyzed of 32 participants (SOEUM: 11, SOYANG: 10, TAEEUM: 11) after data out of accord with respiration rate or outliers were excluded.Results and Conclusions According to respiration rates, there was no statistical significance of HRV among Sasang Constitution. Regardless of Sasang Constituion, 0.2Hz PB increased mean heart rate and decreased natural logarithmic low frequency(lnLF) oscillation of HRV without the change of natural logarithmic high frequency(lnHF), while 0.1Hz PB increased lnLF and standard deviation of N-N interval(SDNN), and slightly decreased lnHF without the change of mean heart rate. 0.05 Hz PB also showed the same effect as 0.1Hz PB, but was impracticable.

Network Analysis Using the Established Database (K-herb Network) on Herbal Medicines Used in Clinical Research on Heart Failure (심부전의 한약 임상연구에 활용된 한약재에 대한 기구축 DB(K-HERB NETWORK)를 활용한 네트워크 분석)

  • Objectives: Heart failure is a chronic disease with increasing prevalence rates despite advancements in medical technology. Korean medicine utilizes herbal prescriptions to treat heart failure, but little is known about the specific herbal medicines comprising the network of herbal prescriptions for heart failure. This study proposes a novel methodology that can efficiently develop prescriptions and facilitate experimental research on heart failure by utilizing existing databases. Methods: Herbal medicine prescriptions for heart failure were identified through a PubMed search and compiled into a Google Sheet database. NetMiner 4 was used for network analysis, and the individual networks were classified according to the herbal medicine classification system to identify trends. K-HERB NETWORK was utilized to derive related prescriptions. Results: Network analysis of heart failure prescriptions and herbal medicines using NetMiner 4 produced 16 individual networks. Uhwangcheongsim-won (牛黃淸心元), Gamiondam-tang (加味溫膽湯), Bangpungtongseong-san (防風通聖散), and Bunsimgi-eum (分心氣飮) were identified as prescriptions with high similarity in the entire network. A total of 16 individual networks utilized K-HERB NETWORK to present prescriptions that were most similar to existing prescriptions. The results provide 1) an indication of existing prescriptions with potential for use to treat heart failure and 2) a basis for developing new prescriptions for heart failure treatment. Conclusion: The proposed methodology presents an efficient approach to developing new heart failure prescriptions and facilitating experimental research. This study highlights the potential of network pharmacology methodology and its possible applications in other diseases. Further studies on network pharmacology methodology are recommended.

Effect of Highly Concentrated Oxygen Administration on Addition Task Performance and Physiological Signals (고농도 산소가 덧셈과제 수행능력과 생리신호에 미치는 영향)

  • This study investigated the effect of 40% oxygen administration on the addition task performance in three levels of difficulties and physiological signals. Ten male and female college students were selected as the subjects for this study. The experiment consisted of two runs: one was a addition task, with normal air (21% oxygen) administered and the other was with hyperoxic air (40% oxygen) administered. The experimental sequence in each run consisted of Rest1 (3 min), Task1 (1 min, one digit addition task), Task2 (1 min, two digit addition task), Task3 (1 min, three digit addition task), and Rest2 (4 min). Blood oxygen saturation and heart rate were measured throughout the five phases. The accuracy rates of the addition task were enhanced with 40% oxygen administration compared to 21% oxygen. Difference in the accuracy rates grew higher with the rise of difficulty. When 40% concentration oxygen is supplied, blood oxygen saturation increased and heart rate was decreased comparing to 21%. This study showed that the supply of high concentration oxygen increases blood oxygen saturation, which in turns accelerates brain activation resulting from cognitive process and enhances arithmetic abilities. Particularly when difficulty is high, demand for oxygen increases and, as a result, the effect of high concentration oxygen becomes more significant.

  • The effectiveness, the safety and the usefulness of HPS-A(An Jung San with Chyag Yag Kam Cho Tang), herbal digestive, was examined on 32 patients complained digestive symptoms. This medicine was administered 1 package per 1 time 3 times per 1 day, during 2 weeks. From the examination of the improvemnt and the safety mainly by subjective symptoms, the usefulness was gained. The results were as follows ; 1. In the improvement rate of susbjective symptoms, high rate symptoms was gastric distension, anorexia, heart burn, gastric discomport, dyspepsia, acrid eructation, indigestion, nausea. 2. The improvment over moderate effective was 85.7% after 1 week 96.4% after 2 weeks. The 4 cases breaked medicine during experimental period were all over moderate offectiveness. 3. There weren't any cases complaind particular problem about safety except for 1 case complained mild pushing up of heat. There weren't also any side effect of medicine on common blood count, blood chemistry and urinalysis. 4. In the dicision of usefulness, 19cases(59.4%) were usefulnesss, 12cases(37.5%) were moderate usefulness, 1 case was uselessness(3.1%). So the cases over moderate usefulness were 31 and rate was 96.9%. 5. The usefulness rates according to physical features were all over 95.5% in sturdiness, middle feature and infirmity. So the rates were no relations with the features. 6. The usefulness rates over moderate usefulness according to total seriousness were all over 90% in high, middle, and mild cases. So the rates were no relations with the seriousness. Totally HPS-A was a medicine that was highly effective, safe and useful to patients complained various digestive symptomes.

Reoperations for valvular heart disease: report of 29 cases (심장판막 재수술: 29례 보)

  • It has been over 20 years since successful operations of Cardiac valves at the Department of Thoracic and Cardiovascular surgery, college of medicine, Yonsei University. About six hundreds of patients with severely symptomatic valvular heart disease have had valve operations with complete loss or sharp decrease in their cardiac symptoms since 1956. As the number of cardiac patient increases, reoperation on valves assumes greater importance. To define the group of patients undergoing reoperations on valves and the factors influencing their survival, we have reviewed our experiences of the reoperation on valves at the Yonsei University, Severance Hospital. This is a report of 29 cases which was undergone secondary or more surgery for valvular heart disease from 1966 to 1983. The primary operations includes 159 cases of open heart surgery from 1966 to 1975 and 476 cases from 1976 to march, 1983. The secondary operations are classified into groups of secondary valvuloplasty or valvotomy [8 cases], prosthetic valve replacement following valvuloplasty or valvotomy [14 cases] and prosthetic valve rereplacement [2 case] for such as calcification, degeneration and perforation of the cusps and paravalvular leakage, of the bioprosthetic valves. The leading indication for reoperation of mitral valve was restenosis or stenoinsufficiency, The indications of aortic valve replacement was active bacterial endocarditis, medically uncontrollable prosthetic endocarditis or paravalvular leakage. Overall death rate of the reoperation was 17.4% [5 death among the 29 patients] and the leading causes of death were myocardial failure, arrhythmia, cerebral embolism, acute renal failure due to low output syndrome. And it was followed by sepsis associated with active prosthetic endocarditis. The death rate of reoperation was 4.3% in the elective cases except urgent cases and the death rate of overall cardiac valve except reoperation cases was 4.1% in the last two years. Although the general mortality of reoperation was high, both mortality rates were comparable except emergency cases due to urgent preoperative patient’s condition.

A Study of Noncontact Heartbeat and Respiration Detection Using the Doppler Radar (도플러 레이더를 이용한 비접촉 방식의 심박 및 호흡 검출에 관한 연구)

  • In this paper, a 2.4 GHz doppler radar system consisting of a doppler radar sensor and a baseband module were designed to detect heart beat and respiration signal without direct skin contact. The doppler radar system emits RF signal of 2.4 GHz toward human chest, and then detects phase modulation of the reflected signal so as to investigate cardiopulmonary activities. The heartbeat and respiration signals acquired from I/Q channels of the doppler radar system are applied to the pre-processing circuit, the amplification circuit, and the offset circuit of the baseband module. The designed system was tested on mouse, rabbit and mankind, which have different range of heart rates and respiration signals, to evaluate detection accuracy of the system. ECG acquisition system and respiration transducer were used to generate the reference signal. In our experiments, a performance of detection were found to be high in the case that the subject stays still. In this paper, we confirmed that non-contact heart beat and respiration detection using the doppler radar has the possibility and limitation according to distance, cardiopulmonary activities, range of heart rates and respiration.

Clinical Outcomes of Non-carbapenem Treatment for Urinary Tract Infections Caused by Extended-spectrum β-lactamase-producing Escherichia coli

  • Purpose: The purpose of this study was to investigate the clinical outcomes of non-carbapenem treatment for urinary tract infections (UTIs) caused by extended-spectrum β-lactamase (ESBL)-producing Escherichia coli (E. coli) in young children. Methods: We retrospectively reviewed the medical records of children under 2 years of age who were diagnosed and treated for UTIs caused by ESBL-producing E. coli from September 2014 to March 2020. Results: Forty-three children under 2 years of age were treated with non-carbapenem antimicrobials for UTIs caused by ESBL-producing E. coli without bloodstream infections. The overall clinical and microbiological success rates for empirical antimicrobial treatment were 90.7% and 97.7%. Three of the patients (7.0%) experienced a relapse of UTI within a month. An in vitro susceptibility test showed that two patients were sensitive and one was resistant to the antimicrobial treatments. Furthermore, there were no significant differences in the time to defervescence, clinical success, microbiological success, and relapse rate between the susceptible (n=13) and non-susceptible groups (n=30). Conclusion: In this study, the overall relapse rate of patients treated with non-carbapenem antimicrobials was 7.0%. The patients showed high success rates in the clinical and microbiological responses to the non-carbapenems regardless of the results of the in vitro antimicrobial susceptibility test. These results provide evidence that non-carbapenems may be viable alternative treatments for UTIs caused by ESBL-producing E. coli.

Influence of fat levels and types of dietary fiber on lipid metabolism of rats (식이중 지방 수준과 Fiber 종류가 흰쥐의 지방대사에 미치는 영향)

  • The influence of fat level and dietary fiber on serum and liver lipid concentration of the rat was investigated. Groups of rats were fed respectively 3%, 5%, 20% fat containing and free fiber diet until 5th week. From 5th week to 9th week high fat diet groups of rates were devided high fat diet group, high fat pectin supplemented diet group and high fat celulose supplemented diet group. Pectin and cellulose at 10% was supplemented to high fat diets. After rats were fed for 9 weeks, all rats were sacrificed to collect the liver and blood samples by heart puncture. Serum and liver lipids were determined on all rats and compared the histochemical lipid staining method with the biochemical analysis of serum and liver lipids. A high fat diet caused increased level of the total lipids, total cholesterol and free cholesterol contents in the liver and the serum. Rats fed pectin reduced body weight, werum lipid and liver lipid but cellulose had no effect to reduce serum and liver lipid concentration. These results indicate that dietary fat level led to changes in the lipid metabolism of rats and that pectin was more effective in lowering serum and liver lipid than cellulose.

The Effect of Fentazine-induced Sedation in Farmed Elk (Cervus canadensis) (사육 엘크의 Fentazine 진정효과)

  • This study was performed to assess clinical signs, sedative effect and clinicschemical profile of a mixture of fentanyl-azapemne-xylazine(Fentazine$^{(R)}$) in formed elk Twelve male elk(Cervus canadensis) were immobilized with Fentazine, and blood samples were taken of femoral venous blood. Samples were analyzed in the conditions of 10- and 30-minute after administration of the drug. Heart rates, respiratory rates, and body temperatures were in normal ranges during Fentazine anesthesia. After iqiection of Fentazine, most of elk were recumbency and did not respond to needle prick In young adult(3.5.4.5 years old) elk a high dose(>3.0 ml/head) of Fentazine does not result in more sedation, but it does prolong the duration of sedative effect. Fentazine induced sufficient analgesia far velvet antler removal and hoof trimming in elk Salivation, urination, intermittent apnea and mild bloat were observed in elk Globulin, alanine aminotransferase, aspartate aminotransferase, blood urea nitrogen, calcium, magnesium and phosphorus values were in normal ranges for at least 30 minutes after Fentazine administration. Total protein, albumins cortisol and prothrombin values were slightly increased during sedation(p<0.05). It was concluded that Fentazine is effective analgesic drug being useful for velvet antler removal and hoof trimming.

Analysis on Psychological Self-regulation and Arousal Variation of Actors on Performance (공연 시 배우의 각성변화와 심리적 자기조절 분석)

  • The purpose of this study was to analyze self-regulation about changing heart rate and perceived arousal variation when actors start their performance. Object of this study were 5 actors who belong to the theater selected using the purposive sampling method. This study used a measuring instrument of heart rate to measure arousal status, used behavior observation paper to observe and record self-regulation behavior and executed personal consultation after ending performance. There was graph mad by variation transition which calculated average and standard deviation about variation of heart rate each time of performance. The results of this study were as follows. First, there were high variation of awake level that actor's heart rates were rising before starting performance and the most high level of arousal was from 20minutes to 5minutes before starting performance. Second, there were self-regulation behaviors appeared such as deep breathing, breath controling, self talking, talking with other actors, concentrating an script, going to toilet, smoking, checking closes before starting performance. Third, when performance start, actors used psychological self-regulational method such as relaxation, self concentration, confidence reinforcement, coping with state or accommodation for controling raised arousal status.
