Systematic Review on Epstein-Barr Virus (EBV) DNA in Diagnosis of Nasopharyngeal Carcinoma in Asian Populations

  • Objective: To conduct a meta-analysis to investigate the value of EBV DNA in diagnosis of nasopharyngeal cancer (NPC) in Asian populations, and provide important evidence for screening. Methods: Prospective or respective case-control or cohort studies regarding the detection role of EBV DNA for NPC were included in our study. We conducted a comprehensive literature search in PubMed, EMBASE, and the Chinese Biomedical Database (CBM database between January 1980 and March 2012. Results: A total of 18 studies with 1492 NPC cases and 2641 health controls were included. Almost of the included studies were conducted in China, and only one other conducted in Thailand. The overall results demonstrated that the pooled sensitivity, specificity, positive likelihood (+LR) and negative likelihood (-LR) were 0.73 (0.71-0.75), 0.89 (0.88-0.90), 8.84 (5.65-13.84) and 0.19(0.11-0.32), respectively. The overall EBV DNA detection showed the largest area of 0.932 under the summary receiver operator curve (SROC). The accuracy of detection by plasma for NPC (0.86) was higher than in serum (0.81), with largest areas under the SROC of 0.97 and 0.91, respectively. Conclusion: Our results demonstrated the EBV DNA detection in plasma or serum has high sensitivity and specificity in diagnosis of NPC, especially in Chinese populations with a high risk of cancer.

Are p53 Antibodies a Diagnostic Indicator for Patients with Oral Squamous Cell Carcinoma? Systematic Review and Meta-Analysis

  • Background: P53 has been reported to be involved with tumorigenesis and has also been implicated as a significant biomarker in oral squamous cell carcinoma(OSCC). However, the diagnostic value of p53 antibodies remains controversial; hence, we comprehensively and quantitatively assessed the potential in the present systematic review. Materials and Methods: A comprehensive search was performed using PubMed and Embase, up to October 31, 2014, without language restriction. Studies were assessed for quality using QUADAS (quality assessment of studies of diagnostic accuracy). The positive likelihood ratio (PLR) and negative likelihood ratio (NLR) were pooled separately and compared with overall accuracy measures using diagnostic odds ratios (DORs) and symmetric summary receiver operating characteristic (SROC) curves. Results: Of 150 studies initially identified, 7 eligible regarding serum p53 antibodies met the inclusion criteria. Some 85.7% (6/7) were of relatively high quality (QUADAS $score{\geq}7$). The summary estimates for quantitative analysis of serum p53 antibody in the diagnosis of squamous cell carcinoma were: PLR 2.06 [95% confidence interval (CI) : 1.35-3.15], NLR 0.85 (95%CI: 0.80-0.90) and DOR 2.47 (95%CI: 1.49-4.12). Conclusions: This meta-analysis suggests that the use of s-p53-antibodies has potential diagnostic value with relatively high sensitivity and specificity for OSCC particularly with serum specimens for discrimination of OSCCs from healthy controls. However, its discrimination power is not perfect because of low sensitivity.

IPMN-LEARN: A linear support vector machine learning model for predicting low-grade intraductal papillary mucinous neoplasms

  • Backgrounds/Aims: We aimed to build a machine learning tool to help predict low-grade intraductal papillary mucinous neoplasms (IPMNs) in order to avoid unnecessary surgical resection. IPMNs are precursors to pancreatic cancer. Surgical resection remains the only recognized treatment for IPMNs yet carries some risks of morbidity and potential mortality. Existing clinical guidelines are imperfect in distinguishing low-risk cysts from high-risk cysts that warrant resection. Methods: We built a linear support vector machine (SVM) learning model using a prospectively maintained surgical database of patients with resected IPMNs. Input variables included 18 demographic, clinical, and imaging characteristics. The outcome variable was the presence of low-grade or high-grade IPMN based on post-operative pathology results. Data were divided into a training/validation set and a testing set at a ratio of 4:1. Receiver operating characteristics analysis was used to assess classification performance. Results: A total of 575 patients with resected IPMNs were identified. Of them, 53.4% had low-grade disease on final pathology. After classifier training and testing, a linear SVM-based model (IPMN-LEARN) was applied on the validation set. It achieved an accuracy of 77.4%, with a positive predictive value of 83%, a specificity of 72%, and a sensitivity of 83% in predicting low-grade disease in patients with IPMN. The model predicted low-grade lesions with an area under the curve of 0.82. Conclusions: A linear SVM learning model can identify low-grade IPMNs with good sensitivity and specificity. It may be used as a complement to existing guidelines to identify patients who could avoid unnecessary surgical resection.

Factors associated with long head of the biceps tendon tear severity and predictive insights for grade II tears in rotator cuff surgery

  • Background: In rotator cuff repair, the long head of the biceps tendon (LHB) is commonly used as graft material. However, factors influencing LHB tear severity are poorly understood, and predicting grade II LHB tears is challenging. This study aimed to identify these factors preoperatively. Methods: The demographics, medical parameters, and pain severity of 750 patients who underwent arthroscopic surgery from January 2010 to February 2021 were evaluated to determine the factors associated with LHB tear severity and grade II tears. Both overall and large-to-massive rotator cuff tear (RCT) cohorts underwent ordinal and binary logistic regression analyses. Predictive accuracy for grade II LHB tears was determined using the area under the receiver operating characteristic curve (AUC). Results: In the overall cohort, high-sensitivity C-reactive protein (hs-CRP) >1 mg/L (P<0.001), subscapularis tear (P<0.001), hypothyroidism (P=0.031), and the tangent sign (P=0.003) were significantly associated with LHB tear severity, and hs-CRP>1 mg/L, subscapularis tear, and Patte retraction degree were significantly associated with grade II LHB tears (P<0.001). In the large-to-massive RCT cohort, hs-CRP>1 mg/L, hypertension, and age ≥50 years (P<0.05) were significantly associated with LHB tear severity, and hs-CRP>1 mg/L (P<0.001) and hypertension (P=0.026) were significantly associated with grade II LHB tears. In both cohorts, hs-CRP >1 mg/L demonstrated good predictive accuracy for grade II LHB tears (AUCs: 0.72 and 0.70). Conclusions: Serum hs-CRP >1 mg/L is associated with LHB tear severity and serves as a reliable predictor of grade II LHB tears, facilitating preoperative assessment of the LHB as potential graft material in arthroscopic rotator cuff repair. serves as a reliable predictor of grade II LHB tears, facilitating preoperative assessment of the LHB as potential graft material in arthroscopic rotator cuff repair. 연결발화에서 마비말화자의 음질 특성 (Voice Quality of Dysarthric Speakers in Connected Speech)

  • This study investigated the perceptual and cepstral/spectral characteristics of phonation and their relationships in dysarthria in connected speech. Twenty-two participants were divided into two groups; the eleven dysarthric speakers were paired with matching age and gender healthy control participants. A perceptual evaluation was performed by three speech pathologists using the GRBAS scale to measure the cepstrual/spectral characteristics of phonation between the two groups' connected speech. Correlations showed dysarthric speakers scored significantly worse (with a higher rating) with severities in G (overall dysphonia grade), B (breathiness), and S (strain), while the smoothed prominence of the cepstral peak (CPPs) was significantly lower. The CPPs were significantly correlated with the perceptual ratings, including G, B, and S. The utility of CPPs is supported by its high relationship with perceptually rated dysphonia severity in dysarthric speakers. The receiver operating characteristic (ROC) analysis showed that the threshold of 5.08 dB for the CPPs achieved a good classification for dysarthria, with 63.6% sensitivity and the perfect specificity (100%). Those results indicate the CPPs reliably distinguished between healthy controls and dysarthric speakers. However, the CPP frequency (CPP F0) and low-high spectral ratio (L/H ratio) were not significantly different between the two groups.

고속 데이터 전송을 위한 광모뎀 개발에 관한 연구 (A Study on the optical MODEM Development for high Speed Data Transmission)

  • 기존 RF변조복조기의 한계를 극복하고, 고속 장거리 전송이 가능한 광변복조기를 설계 제작하였다. 인터페이스는 기존시스템과의 호환성을 위해 EIA RS-232C와 CCITT V.24를 채택하였고 디지털 코딩 방식으로는 Biphase코딩방식을 사용하였다. 또한 시스템의 자체 진단을 위한 Loopback Test기능을 부가하였다. 광 송수신기의 단파장대의 LD-APD를 사용하여 $10^-9$BER에서 -30dBm의 수신 감도를 갖도록 설계하였다. 구성된 시스템은 동기식의 경우 1200bps~57.6Kbps, 비동기식의 경우 DC~200Kbps 까지 임의의 속도로 데이타 전송이 가능하였다.

자동 변환 임피던스 매칭 네트워크를 갖는 CMOS FM 수신기 프론트엔드 구현 (Implementation of a CMOS FM RX front-end with an automatic tunable input matching network)

  • 본 논문에서 2개의 다른 특성을 갖는 FM 안테나들을 사용할 수 있도록 자동 변환 매칭 네트워크를 갖는 CMOS FM 수신기 프론트엔드 구조를 제안하였고 이를 65nm CMOS 공정을 이용하여 설계하였다. 제안된 FM 수신기는 높은 주파수 선택 특성을 갖는 임베디드 안테나를 사용 시 FM 전체 주파수 밴드에서 일정한 수신감도를 유지하기 위해서 저 잡음 증폭기의 입력 매칭 회로의 공진 주파수를 채널 주파수에 따라 가변이 가능하도록 구현하였다. 구현된 FM 프론트엔드의 시뮬레이션 결과는 약 38dB 전압이득, 2.5dB 이하의 잡음 지수 특성, -15.5dBm의 IIP3 선형성 특성을 보이고 1.8V 전원에 3.5mA 전류를 소모한다.

서울대학교 전파천문대 부근의 1.4GHz 대역 전파 환경 (1.4GHz-BAND RADIO INTERFERENCES AT SEOUL RADIO ASTRONOMICAL OBSERVATORY)

  • We have carried out measurements of 1.2-1.6GHz radio interferences around Seoul Radio Astronomy Observatory located in the campus of Seoul National University. We received interference signals using a pyramidal horn antenna and measured its power using a spectrum analyzer with 1MHz resolution after $\~60dB$ amplification. In order to check the spatial characteristics, we made observations at every $30^{\circ}$ in azimuth at elevation of $30^{\circ}\;and\;60^{\circ}$. Also, in order to check the temporal characteristics, we repeated the all-sky observations five times at every six hours. The results may be summarized as follows: (1) There are strong $({\geq}-20dBm)$ interferences between 1.2 and 1.4GHz. Particularly strong interferences are observed at 1.271 and 1.281GHz, which have maximum powers of -0.34dBm and -0.56dBm, respectively. (2) The characteristics of the interferences do not depend strongly on directions, although the interferences are in general weak at high elevation and in east-west direction. (3) The interferences appear for a very short $(\leq0.01s)$ period of time, so that the average power is much smaller than the maximum power. Strong interferences with large $(\leq-49.0dBm)$ average power have been observed at 1.271, 1.281, 1.339, and 1.576GHz. At these frequencies, the interferences appear repeatedly with a period of $\leq0.1s$ By analyzing the observed power, we find that, for the strongest 1.271GHz interference, the average intensity is $-171dBW/m^2/Hz$ and that the maximum intensity is $-122dBW/m^2/Hz$. If this interference is delivered to the detector without any shielding, then its power would be much greater than the rms noise of a typical line spectrum. Therefore, it is important to shield all the parts of receiver carefully from radio interferences. Also, without appropriate shielding, the sensitivity of a receiver could be limited by the interference.

ROC 분석을 이용한 골격성 III급 부정교합의 수평계측방법간 비교연구 (COMPARATIVE STUDY ON THE HORIZOTAL MEASUREMENTS OF SKELETAL CLASS III MALOCCLUSION USING THE ROC ANALYSIS)

  • 본 연구는 III급 부정 교합을 판별하는데 있어, 수평 부조화의 진단에 이용되는 여러 진단 항목들의 진단학적 효율과 타당성을 ROC analysis로 비교하는데 그 목적이 있다. ROC(Receiver Operating Characteristic) analysis는 연속적으로 변하는 cut-off value에서의 sensitivity와 1-specificity에 의해 그려지는 곡선으로서 진단 방법의 타당성을 결정하고, 여러 진단 방법들을 비교하는 분석법으로 알려져 있다. 부정교합자 496명을 대상으로 측모 두부 X-선 계측사진과 진단모형을 이용하여, 진단모형 계측을 통해 부정교합군을 분류하였으며, 이중 III급 부정 교합자는 245명이었다. 측모 두부 X-선계측사진에서 16개의 계측항목을 선정하였으며, 이 계측항목들과 III급 부정교합의 관계를 알아보고자 각도 계측항목에서는 $1^{\circ}$ 간격으로, 선계측항목에서는 1mm의 간격으로 sensitivity와 specificity를 구해 ROC curve를 그렸다. 그리고, 이 계측항목들의 직접적인 비교를 위해 ROC curve 아래의 면적을 계산해냈다. 결과는 다음과 같다. 1. III급 부정교합을 판별하는데 있어, "Wits" appraisal이 다른 계측 항목에 비해 더 나은 진단 효율을 보였다. 2. AB plane angle, ANB angle, App-Bpp distance, AF-BF distance, APDI, N perpendicular to A 와 Pog to N perpendicular의 차이, maxillomandibular differential도 높은 진단 가치를 보였다. 3. 하악골의 위치를 평가하는 계측항목은 중정도의 진단 효율을 보였다. 4. 상악골에 대한 계측항목은 III급 부정교합의 판별에 대한 진단 가치가 낮았다.

준지도학습 방법을 이용한 흉부 X선 사진에서 척추측만증의 진단 (Diagnosis of Scoliosis Using Chest Radiographs with a Semi-Supervised Generative Adversarial Network)

  • 목적 흉부 X선 사진에서 척추측만증을 조기진단 할 수 있는 딥러닝 기반의 스크리닝 소프트웨어를 준지도학습(semi-supervised generative adversarial network; 이하 GAN) 방법을 이용하여 개발하고자 하였다. 대상과 방법 두 곳의 상급종합병원에서 촬영된 흉부 X선 사진에서 척추측만증을 조기진단할 수 있는 스크리닝 소프트웨어를 개발하기 위하여 GAN 방법이 이용되었다. GAN의 훈련과정에서 경증에서 중증의 척추측만증을 보이는 흉부 X선 사진들을 사용하였으며 upstream task에서 척추측만증의 특징을 학습하고, downstream task에서 정상과 척추측만증을 분류하도록 훈련하였다. 결과 수신자 조작 특성 곡선의 곡선하면적(area under the receiver operating characteristic curve), 음성예측도, 양성예측도, 민감도 및 특이도는 각각 0.856, 0.950, 0.579, 0.985, 0.285이었다. 결론 우리가 GAN 방법을 이용하여 개발한 딥러닝 기반의 스크리닝 소프트웨어는 청소년의 흉부 X선에서 척추측만증을 진단하는데 있어서 높은 음성예측도와 민감도를 보였다. 이 소프트웨어가 건강검진을 목적으로 촬영한 청소년의 흉부 X선 사진에 진단 스크리닝 도구로써 이용된다면 영상의학과 의사의 부담을 덜어주며, 척추측만증의 조기진단에 기여할 것으로 생각된다.
