대상포진 질환에 대한 이해와 물리치료적 접근 (The Comprehension of Herpes Zoster and The Approach of Physical Therapy)

  • Purpose : Herpes zoster is a common dermatologic disorder and is caused by reactivation of varicella zoster virus lying dormant in the ganglion of the dorsal root Methods : The aim of this study is to elucidate the clinical characteristics of herpes zoster and it's nature of pain, and is to review the method of physical therapy for pain control. Results : Herpes zoster is characterized by segmental rash, pain, and sensory symptoms, For most patients skin healing and pain resolution occur within 3-4 weeks, However, pain can continue after the rash has healed. Pain and paresthesia often the eruption of herpes zoster and vary from itching to stabbing. The preeruptive pain may simulate other diseases and may lead to misdiagnosis and misdirected interventions. Motor symptomatology is less well known and is most often related to central nervous system disease, although true lower motor neuron application is also thought to exist Subclinical motor involvement is relatively more common than clinical motor weakness and is easily detected by using electromyography. Higher incidences of herpes zoster were observed in female and in the elderly. Conclusion : The nature of pain associated with herpes zoster varied from a superficial itching to server stabbing or bursting, and paresthesia occurred most frequently. Therefore, the study of herpes zoster will be more research and comprehend, and the approach of physical therapy should be need positively.

  • Herpes zoster는 varicella zoster 바이러스에 의해 발생하는 급성 바이러스성 감염질환으로 삼차신경의 분지들을 침범할 수 있다. 치성동통과 유사한 증상을 보이는 43세 남자 환자가 내원하였으며 임상 및 방사선 검사 후에도 명확한 원인을 발견할 수 없었다. 피부과의사에 의해 삼차신경의 세 번째 분지에 발생한 herpes zoster로 진단되었고 항바이러스 치료를 시행하였다. 2개월 뒤, 환자의 피부 병소 및 동통은 대부분 사라졌으나 반흔조직은 다소 남아있었다. 진단과정 중 임상가는 구강안면부 동통이 herpes zoster와 연관될 수 있다는 가능성을 숙지하여야 할 것이다.

대상포진에 대한 한방 단독 치료 치험 2례 (Two Cases Report of Herpes Zoster Patients Improved by Korean Medicine Treatment alone)

  • Objectives : The purpose of this study is to report a 67-year-old male patient with Herpes zoster(caseI) and a 38-year-old female patient with Herpes zoster(caseII), whose symptoms were relieved after Korean medicine treatment alone. Methods : Both patients took Yeonryeonggobondan and Pyungwee-san daily. Everytime they came to the hospital, they received acupuncture treatment, Hwidam's Su-Gi therapy and External Gigong Therapy. Acupuncture was applied by gall bladder seunggyeok(膽勝格, GB-), spleen jeonggyeok(脾正格, SP+), large intestine seunggyeok(大腸勝格, LI-) of Saam's Acupuncture Method. Hwidam's Su-Gi therapy was applied on neck. External Gigong Therapy was applied on skin lesion area. the patients' symptoms were evaluated with photographs and VAS. Results : The results were as follows: 1. In the case of a 67-year-old male patient, it took 14 days to recover the skin lesions and to reduce the pain after treatment started. And he visited 13 times during that period. 2. In the case of a 38-year-old female patient, it took 23 days to recover the skin lesions to reduce and the pain after treatment started. And she visited 10 times during that period. 3. Intensive treatment early in treating herpes zoster helped to shorten the treatment period. 4. Taking Yeonryeonggobondan and Pyungwee-san and receiving acupuncture treatment(SP+) can help to improve immunity and recover skin lesions in herpes zoster diagnosed with spleen deficiency with dampness encumbrance(脾虛濕困) and blood stasis due to qi stagnation (氣滯血瘀). 5. The combination of acupuncture treatment(GB-) and External Gigong Therapy was effective in controlling pain. 6. External Gigong Therapy is considered to be effective for the recovery of the skin as well as the pain of the herpes zoster. Conclusions : Korean medicine treatment alone has a great effect on the above two patients with herpes zoster. I hope the active research about Korean medicine treatment will be done not only for herpes zoster but also for various intractable pain diseases.

  • 저자들은 건강했던 12세 남아가 발열, 연하통, 삼킴 곤란 증세를 호소하여 시행한 내시경 상 식도의 궤양과 수포성 병변이 보이고 헤르페스 I형 항체 양성반응으로 진단된 헤르페스 식도염을 항바이러스제와 proton pump inhibitor 등 보존적 치료로 치유시킨 1예를 경험하였기에 문헌고찰과 함께 보고하는 바이다.

  • Herpes Zoster is an acute viral infection characterized by the appearance of vesicles that occur on the skin and mucous membrane along the pathway of an involved sensory nerve. Although Herpes zoster is usually as benign viral infection, complications may occur especially when cranial nerves are involved. There are few reports of bony and dental complications by Herpes Zoster infection, all of which were isolated in a single quadrant. These include devitalized teeth, abnormal development of permanent teeth, internal resoption and spontaneous exfoliation of teeth with osteonecrosis of the alveolar bone. No agreement has been reached concerning the pathogenesis of osteonecrosis and tooth exfoliation associated with herpes zoster infection. We recently experienced series of maxillary osteonecrosis and spontaneous teeth exfoliation in patients with Herpes Zoster infection and present two cases with review of literature.

Epidemiological Study on the Incidence of Herpes Zoster in Nearby Cheonan

  • Background: Herpes Zoster is a disease that occurs after the virus is reactivated due to infection of the varicella virus in childhood. Risk factors are advanced age, malignant neoplasm, organ transplantation, immunosuppressive agents taking are known. The purpose of this study was to investigate the relationship between the seasonal effect and other risk factors on the incidence of herpes zoster. Methods: The medical records of 1,105 patients admitted to the outpatient diagnosed with herpes zoster were retrospectively examined. The patients' sex, age, dermatome, onset, underlying disease, residential areas were collected. Results: The incidence of women outnumbered men and increased for those above the age of 50. The number of occurrences of herpes zoster patients was higher in the spring and summer than in winter. Unlike men, women had the most frequent outbreaks in March. The most common occurrence of dermatome is in the thoracic region. The number of occurrence was similar on the left as the right. Conclusions: In this study, herpes zoster occurs more often in women than in men and more frequently occurs in women in the spring and summer.

  • Herpes zoster oticus is one of complication of varicella zoster virus (VZV) reactivation in the geniculate ganglion of the facial nerve, which is the most common presentation of herpes zoster in the head and neck region. However, VZV infection of the larynx has rarely been described in the literature compared with Herpes zoster oticus. Moreover, zoster laryngopharyngitis simultaneously occurred with recurred Herpes zoster oticus which has no newly developing motor dysfunction has not been reported yet. Therefore, these diseases are difficult to diagnose due to its rareness. However, distinctive appearances such as unilateral herpetic mucosal eruptions and vesicles are useful and essential in making a quick and accurate diagnosis. Thus, we report a characteristic case of zoster laryngopharyngitis simultaneously occurred with recurred Herpes zoster oticus not accompanied by any newly developing motor palsy.

蜂藥針療法을 병행한 한방치료로 호전된 대상포진 및 대상포진후 신경통환자 2례 (Two Cases of Herpes Zoster and Post-herpetic Neuralgia Improved by Oriental Medicine with Bee Venom Acupuncture)

  • After administration of Oriental medicine with Bee venom acupuncture to two cases of Herpes zoster and Post-herpetic neuralgia. We can observe remarkable efficacy on relief from pain. This results suggest that Oriental medicine with Bee venom acupuncture can be one of a good remedy for Herpes zoster and Post-herpetic neuralgia.

이성대상포진으로 인한 안면신경마비환자 11례에 대한 임상적 고찰 (Clinical Studies on 11 Cases of Facial Nerve Palsy in Herpes Zoster Oticus)

  • Background: Herpes Zoster Oticus, which can be caused from infection by Varicella-zoster virus, is known to be expected poor prognosis, since it has higher possibility of complete injury of facial nerve. Conservative therapy of the Western medicine is effective on the treatment for the herpes zoster and pain. However, its effectiveness on the facial verve palsy has not been proven. Objective : This study was performed to evaluate the treatment of oriental medicine and an analyze prognosis of the facial nerve palsy in Herpes Zoster Oticus. Methods : This retrospctive study was conducted to observe the clinical manifestation of Herpes Zoster Oticus, and the result of electrical diagnostic study on the patients with the facial nerve palsy in Herpes Zoster Oticus who were diagnosed and treated at the Department of Acupuncture & Moxibustion in Kyung Hee Oriental Medical Hospital. Results : 1. According to the six clinical types of Herpes Zoster Oticus, the patients in this study can be grouped as 7 cases of the type II, 2 cases of the type V, and one case of the type IV and the type VI. 2. The shorter the period from onset to the first visit to a hospital, the prognosis of facial nerve palsy in Herpes Zoster Oticus was the better. 3. According to the follow up study, the H-B grades of all cases were improved. 4. According to the follow up study, the result of electrical diagnostic study was improved. Conclusions : We found in our study that the treatment of oriental medicine, such as acupuncture, herbal medicine and aroma therapy, helps the recovery of facial nerve palsy in Herpes Zoster Oticus. The further clinical study about the more cases of facial nerve palsy in Herpes Zoster Oticus and comparative study between the group of oriental medicine and that of western medicine are needed.

외상 후 반흔에 생긴 안면부 대상포진의 경험 (Clinical Experience of Herpes Zoster Developing within Recent Surgical Scar Area)

  • Purpose: Herpes zoster is a common dermatologic disease characterized by unilateral pain and vesicular lesions over the unilateral sensory dermatomes being caused by the reactivation of Varicella zoster virus and its incidence seems to be increasing recently. In case of involving the ganglion of the fifth cranial nerve(trigeminal nerve), it can descend down the affected nerve into skin, then producing an eruption in the dermatome. Among the patients, about 40 - 50% had associated conditions such as diabetes mellitus, hypertension, pulmonary tuberculosis, liver diseases, peptic ulcer, hypothyroidism, pharyngitis but rare facial trauma. Methods: Retrospective study was done for 3 cases of Herpes zoster from May 2000 to May 2007, which had been treated with acyclovir and steroid. Results: The clinical course was uneventful. Follow-up length was about 3 months. After treatment, the patients became stable and there was no complications. Conclusion: Herpes zoster was commonly associated with systemic disorders and the treatment duration was prolonged in associated diseases. But herpes zoster occurring specifically at the site of previously traumatized facial skin has not yet been reported. We experienced the treatment of herpes zoster developing within recent operative facial scar and three cases are presented with the review of literatures. Finally, facial trauma might be a risk factor for herpes zoster in traumatized patients.
