New series of 6-(arylthio)uracils, 6-(4-substituted-1-piperazinyl)uracils, 2,4,5-trioxo-1H,3H-benzothiopyrano[2,3-d]pyrimidine and 5-aryl-2,4-dioxo-1H,3H-pyrimido[5,4-f]benzo[1,4]thiazepines have been prepared and screened for their in vitro activity against herpes simplex-1 virus (HSV-1) and human immunodeficiency virus-1 (HIV-1). The in vitro cytotoxic activity was also evaluated. The results of biological testing revealed that compound 5b showed marginal activity against HSV-1, while compounds 5b and 5f exhibited marginal activity against HIV-1. The rest of the tested compounds were found devoid of antiviral activity against both HSV-1 and HIV-1.

The araC prodrugs (1~5) carrying a special acyl group at 5'-Ο-or $N^4$-position were evaluated for in vitro antiviral activity against various human viruses. When tested against HSV-1 and HSV-2 cultured in the verso cells, the prodrugs exhibited slightly higher $ED_{50}$ values compared with one of the parent araC but showed more increased $CC_{50}$ values in all cases. Consequently the overall selectivity indexes of prodrugs were higher than that of arab. The prodrugs, except compound 5, exhibited very potent activity similar to that of araC ($ED_{50}$ about $0.12{\mu}g/mι$) when evaluated against another human DNA virus, cytomegarovirus. However, theses araC prodrugs were completely inactive against RNA viruses i.e. poliovirus and coxackie B3 virus at the concentration of 4250{\mu}g/mι.$

Trigeminal neuralgia(tic douloureux) is a common clinical syndrome which is characterized by a painful facial condition. The clinician must be able to identify a patient with tic douloureux because this most severe pain syndrome can almost always be controlled. Gangliolysis is the most recent development in the long history of destructive procedures for tic douloureux. H$\ddot{a}$kason pioneered a technique of placing a needle for injecting glycerol into the trigeminal cistern which he found safe and effective for pain control. In August at 1991, I injected pure sterile glycerol three times into the cistern of trigeminal ganglion using the H$\ddot{a}$rtel approach to relieve a patient from pain. The results were as follows; 1) The response of pain relief to a glycerol gangliolysis was excellent. 2) As a complication, there were mild sensory deficit, transient headache and herpes simplex around the mouth angle.

The two distinctive clinical features of varicella-zoster virus (VZV) are varicella (chickenpox) by primary infection and zoster (singles) by the reactivation of latent infection. In addition to the two typical clinical symptoms mentioned above, diverse clinical manifestations have been reported as a result of VZV reactivation, including chronic radicular pain without rash, visual loss, facial palsy, dysphagia, sore throat, odynophagia, otalgia, hearing loss, dizziness, headache, hemiplegia, etc. Most of these symptoms are derived from neuropathy and vasculopathy of affected nerves and arteries. Diagnosis of VZV disease can be difficult if there is no appearance of a skin rash during development of atypical symptoms. In addition to natural infection, vaccination and anti-viral agent treatment have influenced the changes of epidemics and clinical presentations of varicella and zoster. In this article, diverse clinical manifestations caused by VZV reactivation, particular without skin rash, are reviewed.

Postherpetic neuralgia is the most frequent complication of herpes zoster. Treatment of this neuropathic pain syndrome is difficult and often disappointing. Although postherpetic neuralgia is generally a self-limited condition, it can last indefinitely. Continuous epidural blockade for patients with acute zoster can shorten the duration of treatment. However, continuous epidural block has some complications such as infection, dural puncture, and total spinal and nerve damages. We report a case of myoclonus during continuous epidural block with ropivacaine, morphine, and ketamine in an acute zoster patient.

근래에 이르러 예전의 좀 감정적인 용도로서의 성병(venereal disease)이라는 용어보다는 "성적으로" 전염되는 질환(sexually transmitted diseases; STDs)이라는 용어를 사용한다. STDs는 전염성 질환의 일단으로 그 주요한 전염경로가 성행위에 의한 것이다. 예전 용어 그 자체의 함축된 의미를 극소화시킬 필요성은 그렇다하더라도, 임질(gonorrhoea), 매독(syphilis), 비특이성(non-specific) 혹은 비임균성(non-gonococcal) 요도염 (urethritis), 연성하감(chancroid), 림포그래뉼로마 베네레움(lymphogranuloma venereum), 그래뉼로마 잉규나레(granuloma inguinale), 크라미디아 질환(chlamydial infection), 음부 혜르페스(genital herpes), 음부 사마귀(genital warts), 캔디다증(condidiasis), 트리코모나스(trichomoniasis), 마이코프라스마(mycoplasma))을 포함한 성병 (venereal disease)의 범위를 확대해야 할 필요가 있다. B형 간염(hepatitis B), B형 연쇄상구균(B-streptococcus), 사이토메가로바이러스(cytomegalovirus)도 역시 성적 전염이 가능하다. 전염이 가능하다.

Knowledge of molecular mechanisms governing malignant transformation brings new opportunities for therapeutic intervention against cancer using novel approaches. One of them is gene therapy based on the transfer of genetic material to an organism with the aim of correcting a disease. The application of gene therapy to the cancer treatment has led to the development of new experimental approaches such as suicidal gene therapy, inhibition of oncogenes and restoration of tumor-suppressor genes. Suicidal gene therapy is based on the expression in tumor cells of a gene encoding an enzyme that converts a prodrug into a toxic product. Representative suicidal genes are Herpes simplex virus type 1 thymidine kinase (HSV1-tk) and cytosine deaminase (CD). Especially, physicians and scientists of nuclear medicine field take an interest In suicidal gene therapy because they can monitor the location and magnitude, and duration of expression of HSV1-tk and CD by PET scanner.

Segmental zoster paresis (SZP) is a clinically rare complication of herpes zoster. But it has not been reported that acute cerebral motor cortical infarction coincidentally occurred in SZP. A 86-year-old woman was admitted due to pain, tingling sensation, and weakness of left arm. She had an acute onset of pain and tingling sensation in left arm at first day, shoulder weakness at second day, and multifocal vesicles at fourth day. Deep tendon reflexes of left arm were decreased than right. Electromyography showed an axonal polyneuropathy at superior trunk level of left brachial plexus. Median and ulnar sensory evoked potential tests were normal. Brain MRI showed a high signal in right primary motor cortex on diffusion weighted image. We report a case of acute cerebral motor cortical infarction coincidentally occurred in SZP.

바이러스 치료제 개발을 위하여 합성된 핵산유도체 11개에 대한 in vitro 항바이러스 약효검색을 수행하였다. 검색대상 바이러스로서 외피보유 DNA 바이러스인 human herpesvirus에 속하는 herpes simplex virus type 1과 type 2에 대해서는 Vero 세포체계에서 3일 후 CPE 저해정도를 MTT 검색법으로 cytomegalovirus에 대해서는 HEL 세포체계에서 7일 후 Giemsa 염색법으로 약효를 측정하였다. 외피비보유 RNA 바이러스인 picornavirus에 속하는 poliovirus type 1과 type 3과 coxsackie B virus type 3에 대한 약효를 HeLa 세포체계에서 2일 후 CPE 저해정도를 MTT 검색법으로 측정하였다. 아울러 selectivity index를 구하기 위하여 Vero와 HeLa 세포에 대한 약물자체의 독성인 cytocidal effect를 MIT 검색법으로 측정하였다. 그 결과 항 herpesvirus 약효는 어떤 물질에서도 발견되지 않았으나 한 물질이 poliovirus type 1과 3에 대하여 selectivity index 10정도 (CC$_{60}$ 38 ug/ml, EC$_{50}$ 1-4 ug/ml)를 나타내었고 자세한 기작은 좀 더 조사할 필요가 있다.

Macrtophages play an important role in defense against virus infection by intrinsic resistance and by extrinsic resistance. Since interferon-induced enzymes which are 2'-5' oligoadenylate synthetase and p1/eIF-2 protein kinase have been shown to be involved in the inhibition of viral replication, I examined the mechanism by which poly I:C, an interferon inducer, exerts its antiviral effects in inflammatory macrophages infected with herpes simplex virus type 1 (HSV-1). The data presented here demonstrate that poly I:C-induced antiviral activity is partially due to the activation of 2'-5' pligoadenylate synthetase. The activation of 2'-5' oligoadenlate A synthetase by poly I:C is also at least mediated via the production of interferon-.betha.. Taken together, these data indicate that interferon-.betha. produced in response to poly I:C acts in an autocrine manner to activate the 2'-5' oligoadenylate synthetase and to induce resistance to HSV-1.
