Anticonvulsant hypersensitivity syndrome is a rare but fatal complication. It manifests as fever, skin rash, lymphadenopathy, and hepatitis. Phenytoin, phenobarbital, and carbamazepine are the most frequently involved drugs. We here report a case of phenytoin-induced anticonvulsant hypersensitivity syndrome. A 37-year-old woman presented with fever and generalized skin rash, 3 weeks following commencement of phenytoin 400mg daily for treatment of seizure after superficial temporal artery-middle cerebral artery(STA-MCA) anastomosis for moyamoya disease. Her temperature was $39.3^{\circ}C$ and her face was edematous. Laboratory findings showed elevated hepatic enzymes and eosinophilia. Blood and urine culture were all negative. Initially, prednisolone was commenced at 30 mg daily. But fever and skin rash did not improved and hepatic function was more aggravated. After increasing dose of steroid(methylprednisolone 125mg/day), fever and skin rash disappeared and hepatic enzymes returned to normal range.

The segmentation of vessel including portal vein, hepatic vein and artery, from Computed Tomography (CT) images plays an important role in the therapeutic strategies for hepatic diseases. Representing segmented vessels in three dimensional spaces is extremely useful for doctors to plan liver surgery. In this paper, proposed method is focused on smoothing technique of segmented 3D liver vessels, which derived from 3D region growing approach. A pixel expand algorithm has been developed first to avoid vessel lose and disconnection cased by the next smoothing technique. And then a binary volume filtering technique has been implemented and applied to make the segmented binary vessel volume qualitatively smoother. This strategy uses an iterative relaxation process to extract isosurfaces from binary volumes while retaining anatomical structure and important features in the volume. Hard and irregular place in volume image has been eliminated as shown in the result part, which also demonstrated that proposed method is a suitable smoothing solution for post processing of fine vessel segmentation.

The aim of this report is showing the case that we could give exact navigation of perigastric vessels for gastrectomy with 3D CTA. A 74-year-old male patient visited hospital with gastric cancer. Early gastric cancer, type IIb was found at stomach antrum great curvature side. Before surgery, he underwent 3D CT angiography. 3D volume rendering images and MIP images were made by post processing. He had replaced Lt. hepatic artery arising from Lt. gastric artery. Surgeon could get patient's specific vascular anatomy before surgery including surgically relevant anatomical distance and direction and could finish gastrectomy within 4 hours and just 53ml blood loss.
Unknown age, spayed female mixed dog was presented with a severe lethargy. Radiography, ultrasonography, and blood screen test were performed to make a diagnosis. There were no specific radiographic findings. On the ultrasonography, small amount of ascites was found around gallbladder and hepatic vein was dilated approximately 6.1 mm. Blood screen test revealed a severe anemia. Result of heartworm ELISA kit was positive. At necropsy, parasites were in the left atrium, right ventricles, aorta, pulmonary artery, right renal artery.

Alagille syndrome is a autosomal dominant disorder characterized by intrahepatic bile duct paucity and resultant chronic cholestasis in combination with cardiac(mainly peripheral pulmonary stenosis), skeletal, ocular, and facial abnormalities. In addition to the pulmonary stenosis, in large series, anecdotal reports of vascular lesions have concerned the renal artery, aorta, hepatic artery, carotid artery, celiac artery or subclavian artery. Theses diffuse vascular abnormalities, which appear to be a feature of Alagille syndrome, suggest Notch signaling pathway defects affect angiogenesis. The associations of Alagille syndrome with moyamoya disease, the chronic cerebrovascular occlusive disease, were reported and suggested as additional evidence of vasculopathy of Alagille syndrome. We report another 25 month-old Alagille syndrome girl who presented with acute left hemiparesis and was diagnosed with moyamoya disease through the cerebral angiographic study.
Contrast-enhanced CT has an important role in assessing liver lesions, the optimal protocol to get most effective result is not clear. The mein goal when deciding injention protocol is to optimize lesion detectability with rapid scanning when lesion to liver contrast is maximum. For this purpose, we developed a physiological model of the contrast medium enhancement based on the compartment modeling and pharmacokinetics. Blood supply to liver is achieved in two paths. This dual supply characteristic distinguishes the CT enhancement of liver from that of the other organs. The first path is by hepatic artery and to second, by portal vein. However, it is assumed that only gepatic artery can supply blood to hepatocellular carcinoma(HCC) compartment, thus, the difference of contrast enhancement is resulted between normal liver tissue and hepatic tumor. By solving differential equations for each compartment simultaneously using the computer program Matlab, CT contrast-enhancement curves were simulated. The simulated enhancement curves for aortic, hepatic, portal vein, and HCC compartments were compared with the mean enhancement curves from 24 patients exposed to the same protocols as the simulation. These enhancement curves showed a good agreement. Furthermore, we simulated lesion-to-liver curves for various injection protocols, and the effects were analyzed. The variables to be considered in the injection protocol were injection rate, dose, and concentration of contrast material. These data may help to optimize scanning protocols for better diagnosis.

Purpose : Hepatic arterial perfusion scintigraphy with Tc-99m-macroggregated albumin (HAPS) study was carried out in 16 patients with hepatocellular carcinoma(HCC) and in six patients without liver tumor to evaluate HAPS findings of hepatocellular carcinoma and usefullness of HAPS. Materials and Methods : HAPS with planar and SPECT study were performed in 22 patients after conventional hepatic or celiac arteriography. For HAPS study, 4-5 mCi of MAA mixed with 2ml of saline was injected into proper hepatic artery or its distal branches at the rate of approximately 1ml/sec. We analysed 21 HCCs over 2cm in diameter(average diameter; 6.4cm) and 17 of 21 HCCs were over 4cm in diameter(Table 1). CT, sonography and angiography were performed within two week in all 16 patients and liver scan was performed in 12 patients. Results : Three different pattern of tumor perfusion were observed in 16 patients with HCC (Table 2). 1) diffuse increased perfusion in 16 of 21(76%)(Fig. 1) 2) increased peripheral perfusion in 4 of 21(19%) (Fig. 2) 3) diffuse decreased perfusion in 1 of 21 (5%) Arteriovenous shunt indicated by lung uptake of MAA were observed in 9 of 16(56%)(Fig. 4). In contrast, angiography demonstrates arteriovenous shunt in 2 of 16(13%). There was no accumulation of radioactivity on RBC-blood pool scan in all six patients with HCC examined (Fig. 1). Conclusion : HAPS is useful study in evaluation of perfusion pattern or vascularity of HCC and in detection of arteriovenous shunt.
To elucidate the mechanism of splanchnic hyperemia associated with chronic portal hypertension, we have investigated the alteration in visceral reflexes in conjuction with circulatory hemodynamics in portal ligated portal hypertension in cats. When capsaicin, bradykinin and vasopressin were injected via splenic artery of sham cat, respectively, they caused not only reflex excitation of systemic arterial pressure, but also elevation of splenic venous pressure with unchanged heart rates. Simultaneously, they evoked the sympathetic efferent excitation of liver (spleno-hepatic reflex) as well as of spleen (spleno-splenic reflex). Similarly, capsaicin upon pledging on the liver surface evoked a significant increase in the pressor reflex with hepatic nerve excitation (hepato-hepatic reflex). After portal ligation, the splenic venous pressure was gradually elevated in association with decrease in systemic arterial pressure. However, the excitation of pressor reflex was enhanced on the and day, thereafter, being returned to the control, and the reflexly induced spleno-splenic, spleno-hepatic and hepato-hepatic sympathetic excitations were significantly diminished on the 8th day following portal vein ligation. In conclusion, it is suggested that sympathetic reflexes to spleen and liver are specifically intervened by the same central pathways and furthermore, the diminution of these viscero-visceral reflex excitations after portal ligation may be related to the intestinal hyperemia.
Curcumin, the yellow pigment in turmeric, curry and mustard, has anti-inflammatory and anti-carcinogenic activities. In this study; we investigated the hypocholesterolemic and anti-atherogenic effect of curcumin in rabbits. Hypercholesterolemia was induced by feeding high cholesterol diet to male rabbits for 30 days, and the animals were then fed high cholesterol diet containing 0.1% (w/w) or 0.5% (w/w) curcumin for additional 30 days. Supplementation of 0.l% curcumin tended to lower serum total cholesterol and low density lipoprotein (LDL)-cholesterol levels and inhibit serum lipid peroxidation. In the 0.5% curcumin-supplemented group, serum total cholesterol was significantly lowered by 11.7%, LDL-cholesterol by 12.8% and lipid peroxidation by 47.9% compared to the control group. Hepatic cholesterol and triglyceride contents were also significantly lowered by 50.6% and 37.4%, respectively compared to the control group. Lipid staining of the arteries isolated from the curcumin-treated rabbits showed that curcumin significantly decreased formation of fatty streaks and atheromatous plaques on the intima of the arteries. These results demonstrated that curcumin lowered serum cholesterol concentration, hepatic cholesterol and triglyceride contents, and accumulation of cholesterol in the artery These cholesterol lowering effects of curcumin, together with its anti-oxidative and anti-inflammatory activities, may play some important roles in preventing atherosclerosis.
