The Analysis of Korean Medicine Treatment for Anal Disease and Skin Disorder around Anal (항문질환 및 항문주위 피부과질환의 한의학적 치료에 대한 국내 임상 연구 경향)

  • Objectives : The purpose of this study is to analyze the trend of Korean medical treatments on Anal diseases and skin disorder around anal. Methods : Based on KCI, OASIS, NDSL, KISS with the keyword 'Anal', 'Hemorrhoid', 'Ani', 'Anal fistula', 'incontinence', 'anal prolapse', 'anal fissure'. published in 2000-2018.11.10., total 9 studies have been found and analyzed. Results : Among 9 studies, 15 cases were selected. Acupuncture, herb medicine, phamacopuncture, moxibustion, fumigation therapy and habit correction were used as treatment for Anal disease. All of research which selected have reported that the Korean medical treatment was effective. Conclusions : According to the results, Korean medical treatments can be effective treatment for anal diseases. Based of this result, studies which is multifocal will be needed steadily.

  • Diffuse cavernous hemangioma of the rectosigmoid is a rare lesion usually presented in children and young adult, often with life threatening hemorrhage. The clinical diagnosis is difficult and often delayed because of lack of awareness of the clinical features. We report a case of diffuse cavernous hemangioma of the rectosigmoid in a 12 year old boy who had undergone suture ligation under the impression of hemorrhoid at his age 3. Sphincter saving coloanal pull through procedure were applied as the surgical treatment. Sclerotherapy was needed with recurrent rectal bleeding 6 months after the pull through operation. The patient is well at 12 months follow-up.

  • Korean folk medicine 'Koaeng I Bab' has been used to cure hemorrhoid, boil and dermatopathy and as an antidote. The botanical origin of the crude drug has not been studied pharmacognostically. To clarify the botanical origin of 'Koaeng I Bab', the morphological and anatomical characteristics of the leaves of Oxalis species growing in Korea, i.e. O. acetosella, O. acetosella var. purpurascens, O. corniculata, O. obtriangulata were studied. As the result, the botanical origin of 'Koaeng I Bab' has been proved to be O. corniculata and O. acetosella var. purpurascens.

Lumbar Sympathectomy in the Management of Rectal Tenesmoid Pain (항문 이급후중의 치료에 있어 요부 교감 신경절 차단)

  • Rectal tenesmus is a persistent, painful and ineffectual sensation of straining at stool or opening of the bowels. Lumbar sympathectomy was performed in patient whose main complaint was rectal tenesmoid pain resulting from hemorrhoid operation, and in whom analgesic or psychotropic drugs had failed in controlling the symptom. After chemical lumbar sympathectomy, patient was free from the rectal tenesmoid pain. It is concluded that lumbar sympathectomy is a safe and effective treatment for rectal tenesmus.

A study on the use of fish as food treatment in Singnyochanyo (『식료찬요(食療纂要)』에서 어류(魚類)의 식치적(食治的) 활용에 관한 연구)

  • Objectives : The researcher figured out the kinds of fish that were used as a treatment for various diseases, and based on the result, studied on how to enhance healthy life and facilitate disease treatment with fish-based treatment. Methods : The researcher first extracted prescriptions based on fish from Singnyochanyo and later, analyzed the extracts by main efficacies, ingredients, recipes and intake periods. Results & Conclusions : According to study, Singnyochanyo used snakeheads twice, eels eleven times, crucian carps eight times, gray mullets once, carps twelve times and yellow croakers three times. When it comes to crucian carps, Singnyochanyo utilized them for asthma-related symptoms, stomach-related diseases, diarrhea, dysentery and hemorrhoid. Donguibogam, however, used crucian carps for more specific purposes - dysentery and stomach-related diseases. In Singnyochanyo, gray mullets and yellow croakers were used to treat stomach-related diseases only one to three times. Carps were applied to treat diseases twelve times in total, five of them for pregnancy-related diseases. For pregnant women, it was recommended to make a soup of carps with side ingredients.

  • Klippel-Trenaunay syndrome (KTS) is a rare disorder characterized by a triad of abnormal bone and soft tissue growth, the presence of a port-wine stain, and venous malformations. Gastrointestinal (GI) manifestations of KTS are relatively common and generally do not cause significant problems. However, persistence can lead to chronic GI blood loss or even massive bleeding in rare cases. The majority of the severe GI manifestations associated with KTS present as vascular malformations around the GI tract and exposed vessels can lead to serious bleeding into the GI tract. Herein, we report a case of a 16-year-old boy with severe iron deficiency anemia who was previously misdiagnosed as hemorrhoid due to small amount of chronic bleeding. The actual cause of chronic GI bleeding was from an uncommon GI manifestation of KTS as rectal polyposis.

Ischiorectal Block with Bupivacaine for Post Hemorrhoidectomy Pain

  • Background: Hemorrhoid is one of the most common surgical diseases occurring in the anorectal region. In this study, we evaluated the effect of ischiorectal fossa block on alleviating post hemorrhoidectomy pain. Methods: In this study, 90 patients suffering from hemorrhoids were evaluated. They were randomly divided into 3 groups. The first group had no block, the second group an ischiorectal block with placebo (normal saline), and the third group a preemptive ischiorectal block with bupivacaine. Postoperative variables such as pain intensity, pethidine consumption, nausea, and vomiting were compared between the groups. Results: The postoperative pain score in group 1 was $8.5{\pm}1.3$ and $8.1{\pm}0.9$ (P=NS) in group 2. The post operative analgesic demand was $3.1{\pm}1.5$ and $3.3{\pm}1.8$ hours in groups 1 and 2, respectively (P=NS). The post operative pain score and analgesic demand were $4.2{\pm}2.1$ and $9.3{\pm}2.7$ hours, respectively, in group 3 (P < 0.0001). Conclusions: Preemptive ischiorectal block reduces the posthemorrhoidectomy pain and opioid demand.

Xiaoke(消渴) had been defined symptoms as polyuria with polydysia in Han and Dang Dynasty(漢唐時代), diagnosed variable disease as to the region of typical symptoms in Jin and Yuan Dynasty(金元時代) and classified into Upper-jiao(上消), Middle-jiao(中消), Lower-jiao(下消) by typical symptoms in Ming and Qing Dynasty(明淸時代). 2. The complications of Xiaoke(消渴) consist of carbuncle and phlegmon(癰疽), hemorrhoid and diarrhea(痔疾), an eye disease(眼病) such as nyctalopia(雀目) and cataract(內障) without Zhongman and Guzhang(中滿鼓脹) resulted from wrong medical treatmentis considered the pathology of Huore(火熱) 3. The pathology of Xiaoke(消渴) has been taken an instance of Shen-zao(腎燥), Five zang-organs's ganzao(五臟乾燥), Zao-re(燥熱), Shi-huo(實火) and Xu-huo(虛火), and since Jin and Yuan Dynasty(金元時代) generally cosidered Zao-re(燥熱). 4 The complications of Xiaoke(消渴) are explained the pathology of Huore(火熱), in the an opinion that the the pathology of Huore(火熱) changed into Zao-re(燥熱), and then Zao(燥), the pathology of Xiaoke(消渴) is similar to Huo(火), the pathology of Jungpung(中風).

A Case Report of A Mixed Hemorrhoid Patient Treated by Herbal medicine Treatment (한약 치료로 개선된 혼합 치핵 환자에 관한 증례 보고)

  • Objectives: The purpose of this study is to report the clinical course of a patient with mixed hemorrhoids treated with Korean medicine alone. Methods: A patient visited for an mixed hemorrhoids was treated with Korean herbal medicine. The effects of the treatment were evaluated via photography of hemorrhoids and symptoms of pain, bleeding, prolapse, itching, discharge and Total Symptom Score(TSS). Results: After treatment, in photography before and after, the size of hemorrhoids was reduced and lost. Scores of Pain (4→0), bleeding (3→0), prolapse (3→2), itching (0→0), discharge (1→0) and TSS (11.5->3) were observed. Conclusion: In light of the reduced size of hemorrhoids and scores of symptoms, herbal medicine might be a possible treatment method on mixed hemorrhoids.

A Literatural Study on the Evidence of Using Thermotherapy and Cryotherapy of Cutaneous and Muscle Meridian in Korean Medical Physiotherapy (한방이학요법 중 경피경근온냉요법에 대한 문헌적 고찰)

  • Objectives : The purpose of this study is to establish literatural evidence about thermotherapy and cryotherapy for Korean medicine through literatural review. Methods : Applicable paragraphs which were related to the thermotherapy and cryotherapy of cutaneous and muscle meridian were phrased from in "Yibujicheng(醫部集成) and "Dongyibaojian(東醫寶鑑)" where were archiving of Oriental or Korean medicine literatures. Searched paragraphs were analysed for establishing historical and theoretical bases of thermotherapy and cryotherapy in Korean medicine. Results : Thermotherapy of cutaneous and muscle meridian(經皮經筋溫熱療法) such as hot pack, warm water therapy, paraffin bath, ultrasound is originated from yu(慰) warm water(溫水) hot water(熱水). Matching indications are various pain conditions(caused by coldness(寒), hard-work(僗若), extravasated blood(瘀血), inflammatory skin disease, frostbite and several internal diseases. It also treats gynecological diseases and facial palsy. Diathermic therapy on acupuncture points(穴位照射溫熱療法) such as infra-red, microwave, shortwave is originated from huolu(火爐), wenlu(溫爐), xianglu(香爐), lamp light(燈火). Its objective is to improve the effects of herb medicine by aiding sweating or to treat the residual symptoms of fever disease or to care skin disease and pain from bone fracture, contusion. Cryotherapy of cutaneous and muscle meridian(經皮經筋溫寒冷療法) such as ice pack, ice spray, iced whirpool, cool water bath is originated from lengfu(冷敷), lengtie(冷貼), lengshiyu(冷石熨). Matching indications are contusions, animal bite injury, corn(肉刺) and (淋病), eye disease, nasal bleeding, hemorrhoid, inflammatory skin disease and chicken pox. Conclusions : Thermotherapy and cryotherapy of cutaneous and muscle meridian(經皮經筋溫冷療法) are the treatments which were used in Korean medicine from the ancient Korean medicine. As scientific equipments were originated from yu(慰), huolu(火爐), wenlu(溫爐), xianglu(香爐), lamp light(燈火). lengfu(冷敷), lengtie(冷貼), lengshiyu(冷石熨). It can be said that these are elements of Korean medicine. More rigorous studies are needed to establish clinical evidence about not only thermotherapy and cryotherapy but also the other physiotherapy of Korean medicine.
