Body Mass Index and Waist Circumference was Risk Factors for Increased Fasting Blood Sugar, and Hemoglobin A1c in Korean Adults without Diabetes Mellitus

  • Purpose: This study confirmed the effects of body mass index (BMI) and waist circumference (WC) on fasting blood glucose (FBS) and hemoglobin A1c (HbA1c) in adults without diabetes mellitus (DM). Methods: The subjects were 4,659 adults (≥ 20 years), and data were extracted from the 6th National Health and Nutrition Survey 2015 (KNHANES VI-3). The subject's data were analyzed using a complex sample t-test, χ2-test, and multiple logistic regression analysis (SPSS 24.0). Results: The incidence of the high FBS group was increased 1.60 times in males with a WC ≥ 90 cm and 1.78 times in females with a WC ≥ 85 cm. The incidence of the high HbA1c group was increased 1.54 times in those who were overweight and 2.22 times in those who were obese. The incidence of the high HbA1c group was increased 1.99 times in males with a WC ≥ 90 cm and 1.87 times in females with a WC ≥ 85 cm. Conclusion: This study presented evidence that interventions for BMI and WC should be included in DM prevention programs. It also suggested that these findings can be utilized for early detection of DM.

Evaluation of Hemoglobin A1c Levels in Endometrial Cancer Patients: a Retrospective Study in Turkey

  • Background: Hemoglobin A1c(HgA1c) is a marker of poor gylcemic control and elevation HgA1c is associated with increased risk of many cancers. We aimed to determine the HgA1c levels in endometrial cancer cases and any relationship with stage and grade of disease. Materials and Methods: A retrospective data review was performed between June 2011 and October 2012 at a tertiary referral center in Turkey. The study included 35 surgically staged endometrial cancer patients and 40 healthy controls. Preoperative HgA1c levels drawn within 3 months before surgery were compared. Also the relationships between HgA1c levels and stage, grade and hystologic type of cancer cases were evaluated. Results: The mean HgA1c levels were statistically significantly higher at $6.19{\pm}1.44$ in endometrial cancer cases than the $5.61{\pm}0.58$ in controls (p=0.027). With endometrial cancer cases, the mean HgA1c level was found to be $6.62{\pm}1.40$ for stage I and $6.88{\pm}1.15$ for stages II-IV (p=0.07). The figures were $6.74{\pm}1.65$ for endometrioid and $6.63{\pm}1.41$ for non-endometrioid type tumors (p=0.56). Mean HgA1c levels of $6.72{\pm}1.14$ for grade 1 and $6.62{\pm}1.42$ for grade 2-3 were observed (p=0.57). Conclusions: HgA1c levels in endometrial cancer patients were statistically higher than healthy controls. However, HgA1c did not show any significant correlation with stage, grade and histologic type in endometrial cancer cases.

A Study on Self Management, Hemoglobin A1c (HbA1c), and Perceived Health Status for the Type II Diabetes Patients (제2형 당뇨병 환자의 자가관리, 당화혈색소 및 주관적 건강상태)

  • Purpose: This study is conducted to examine the relationship among self management, HbA1c, and perceived health status of type II diabetes patients in community health department. Methods: Data were collected from 93 type II diabetes patients who are registered at five community health center using comprehensive survey, modified self care behavior, blood glucose and HbA1c were assessed by nurses. The data was analyzed by the SPSS (14.0) computer program, and it included descriptive statistics, t-test, ANOVA, and pearson's correlation coefficient. Results: There was significant relationship between perceived health status and HbA1c (r=-.252, p=.015). There was significant relationship between self management and HbA1c (r=-.279, p=.007). Conclusion: In order to control blood glucose in the type II diabetes patients, it is necessary to develop supportive self management programs by considering perceived health status.

Influence of Heating, Cooling and Acidity on the Permeability of the Rabbit Erythrocyte Membrane (고온, 한냉 및 산도가 토끼 적혈구막 투과성에 미치는 영향)

  • Outward movement of hemoglobin and $K^+$ ion across rabbit erythrocyte membrane after heating, cooling and in acid medium was studied. One milliliter of rabbit blood was centrifuged and packed red cells were obtained. Packed red cells were resuspended by addition of 4 ml of 0.9% NaCl solution and were subjected to heating $(57^{\circ}C\;for\;5\;minutes)$ or cooling $(-4^{\circ}C{\sim}-8^{\circ}C\;of\;-10^{\circ}C{\sim}-11^{\circ}C\;for\;10\;minutes) $. For acid medium experiment packed ref cells were resuspended by addition of 4 ml of acid medium of PH 4.5 consisting of 0.01% glacial acetic acid in 0.85% NaCl solution and kept standing for 10 minutes. All red cell suspensions were centrifuged again and packed red cells were separated. This packed red cells were again suspended in 4 ml of NaCl solution of 0.8%, 0.7%, 0.6%, and 0.5% concentration respectively and kept standing for 20 minutes. The concentration of hemoglobin and $K^+$ in the supernatant of the above red cell suspensions were measured and the following results were obtained. 1. Outward movement of hemoglobin and $K^+$ was greatest in red cells subjected to heating. The movement paralled to the osmolal concentration gradient between extra- and intra-cellular phase of red cells. 2. In acid medium the outflux of hemoglobin and $K^+$ increased as compared to the control. 3. In red cells subjected to the cold of $-10^{\circ}C{\sim}-11^{\circ}C$ the outflux of hemoglobin and $K^+$ increased. Whereas in the environment of $-4^{\circ}C{\sim}-8^{\circ}C$ there was no change in the outflux of $K^+$. The he-moglobin outflux showed rather a decreased as compared to tile control.

A Case Report of Patient with Type 2 Diabetes Mellitus Treated with Galgeun-tang (갈근탕을 투여하여 호전을 보인 제 2형 당뇨병 증례 1례)

  • Objectives: The aim of this study was to explore and describe how fasting blood sugar (FBS), postprandial 2 h Glucose (PP2h), and glycated hemoglobin (HbA1c) of a patient diagnosed with type 2 diabetes mellitus can be reduced by treatment with Galgeun-tang herbal medicine. Methods: The patient was administered herbal medicine to reduce serum glucose levels. The prescribed herbal medicines included Galgeun-tang and Galgeun-tang-gami. Results: The therapeutic outcomes were control of blood sugar and glycated hemoglobin (HbA1c) levels and decreased insulin administration. Conclusion: The herbal medicine, Galgeun-tang, appears to be a valid treatment for type 2 diabetes mellitus. Serum glucose (FBS/PP2hrs) and HbA1c were well controlled and insulin administration was decreased. Galgeun-tang was effective in controlling the daily glucose levels in a patient with type 2 diabetes mellitus.

Relationship Between HbA1c and Estimated Average Glucose by Hemoglobin Concentration of Diabetic Patients

  • Glycated hemoglobin ($HbA_{1c}$) is a most preferably used baseline of diabetes, implicating average blood glucose levels over a 2-3 month period of time. Recently the American Diabetes Association has recommended the $HbA_{1c}$ assay as one of the criteria for diabetes. Although some studies provide data with "estimated average glucose", by converting the $HbA_{1c}$ results from simple linear regression, the results are not applicable to whole diabetes. We compared the relationship between $HbA_{1c}$ and estimated average glucose by anemia degree of diabetic patients in Korea. The data from the 2008~2009 Korean National Health and Nutrition Examination Survey were used. Analysis was done for 1,257 diabetes subjects with $HbA_{1c}$ results. The distribution of subjects was 34.1% in 60's, 25.9% in 70's, 21,6% in 50's, showing that there was more than 80% in over 50's. To take a close look of the differences depending on the anemic degree, we applied WHO criteria (hemoglobin<13.0 in men and hemoglobin<12.0 in women) and divided anemia degree. The regression equation for A1c and estimated average glucose was $eAG_{mg/dL}=24.3{\times}A1c-32.0$ ($R^2=0.54$, p<0.001) in all subjects, $eAG_{mg/dL}=33.1{\times}A1c-96.1$ ($R^2=0.52$, p<0.001) in slight anemia ($11.0{\leq}$Hb<13.0 in men, $10.0{\leq}$Hb<12.0 in women), and $eAG_{mg/dL}=13.5{\times}A1c+34.9$ ($R^2=0.12$, p =0.075) in moderate anemia (Hb<11.0 in men, Hb<10.0 in women). The regression was not significant in moderate anemia. The relationship between HbA1c and eAG was lower correlation than ADAG study, and eAG showed lower value in all ranges among $HbA_{1c}$ 5~13%. Such as a korea where, there are many diabetic patients among the old aged and higher prevalence rate of anemia, we should be extra careful when we reflect eAG using $HbA_{1c}$ and need to establish criteria which can be applicable to koreans.

Production of methionine γ- lyase in recombinant Citrobacter freundii bearing the hemoglobin gene

  • The production of antileukemic enzyme methionine ${\gamma}$-lyase (MGL) in distinctly related bacteria, Citrobacter freundii and in their recombinants expressing the Vitresocilla hemoglobin (VHb) has been studied. This study concerns the potential of Citrobacter freundii expressing the Vitreoscilla hemoglobin gene (vgb) for the methionine ${\gamma}$- liyase production. Methionine ${\gamma}$- liyase production by Citrobacter freundii and its $vgb^-$ and $vgb^+$ bearing recombinant strain was studied in shake-flasks under 200 rpm agitation, culture medium and $30^{\circ}C$ in a time-course manner. The $vgb^+$ and especially the carbon type had a dramatic effect on methionine ${\gamma}$- liyase production. The $vgb^+$ strain of C. freundii had about 2-fold and 3.1-fold higher levels of MGL than the host and $vgb^-$ strain, respectively.

Analysis of glycosylated hemoglobin (HbA1c) level on maxillofacial fascial space infection in diabetic patients

  • Objectives: This study was performed to evaluate the impact of glycosylated hemoglobin (HbA1c) level on characteristics and prognosis of maxillofacial fascial infection in diabetic patients. Materials and Methods: We reviewed the medical records of 72 patients (35 patients with HbA1c lower than 7.0% and 37 patients with HbA1c higher than 7.0%) diagnosed with maxillofacial fascial space infection and hospitalized for treatment at the Department of Oral and Maxillofacial Surgery in Dankook University Hospital (Cheonan, Korea) from January 2005 to February 2014. We compared demographics, parameters of glucoregulation (HbA1c), laboratory parameters of inflammation (white blood cell [WBC], C-reactive protein [CRP] count), type and number of involved spaces, type and number of antibiotics, period of hospitalization, number of surgical operations, need for tracheostomy, complications, computed tomography (CT), and microorganisms between the two groups. Results: Compared with the well-controlled diabetes mellitus (DM) group (HbA1c <7.0%), patients in the poorly-controlled (HbA1c ${\geq}7.0%$) DM group had the following characteristics: longer hospitalization periods, higher values of laboratory parameters of inflammation (WBC, CRP count) at the time of admission, higher number of antibiotics prescribed, more frequent complications, frequent deep neck space involvement, and distinctive main causative microorganisms. As the HbA1c level increases, hospitalization periods and incidence of complications increase gradually. Conclusion: This retrospective study suggests that regulation of DM significantly impacts maxillofacial fascial infection. Poorly controlled DM with high HbA1c level negatively influences the prognosis of infection.

A Spetrophotometric Cyanmethemoglobin Method for the Estimation of Serum Hemoglobin Levels (혈청(血情)해모구로빈 측정(測定)에 관(關)한 연구(硏究))

  • Hemoglobinemia is found in hemolytic anemia, paroxysmal nocturnal hemolobinuria and paroxysmal hemoglobinuria. Recently increasing use of extracorporeal circulation in cardiac surgery has stimulated the development of more precise methods for rapidly evaluating the formation and clearance of extracoporeal hemoglobin. A spectrophotometric method of analysis seemed to offer the best solution to the requirements. The method presented is on the basis of hemoglobin derivatives converted to cyanmethemoglobin, and the absorbance is measured in a spectrophotometer at 540 and 680 nm. The blank value ismeasured at 680 nm while the absorbance at 540 nm measures the hemoglobin derivatives. This method estimates all the hemoglobin derivatives such as oxyhemoglobin, carboxyhemoglobin, methemoglobin, sulfhemoglobin and hemialbumin. The method was tested in recovery experiments, which is given table 1. A good degree of correlation was obtained in a comparision with the method described by Crosby and Furth for non-hemolyzed serum. The spectrophotometric technique described offers many advantages in speed and simplicity over the chemical procedure.
