제2형 당뇨병 환자의 자가간호행위, 임파워먼트 및 사회적 지지가 당화혈색소에 미치는 영향 (The Influence of Self-care Behaviors, Empowerment and Social Support on Glycosylated Hemoglobin in Patients with Type 2 Diabetes)

  • Purpose: The purpose of the study was to analyze the effects of self-care behavior, empowerment, and social support on glycosylated hemoglobin in patients with type 2 diabetes. Methods: The data were collected during the period of July 1 to July 31, 2016. In total, 172 participants were recruited from outpatients who had been diagnosed with type 2 diabetes at a health care center, a health promotion center at National Health Insurance Corporation, and a tertiary hospital. Statistical data were analyzed with SPSS 20.0 using frequency analysis, t-test, ANOVA, $Scheff{\acute{e}}$ test, Pearson's correlation coefficients, and hierarchical regression analysis. Results: The study results showed that self-care behavior (${\beta}=-.34$, p<.001), empowerment (${\beta}=-.34$, p<.001), and social support (${\beta}=-.20$, p=.018) were found to be influential factors affecting glycosylated hemoglobin, with an overall descriptive power of 69%. Conclusion: Self-care behavior, empowerment, and social support are considered to be important factors in blood glucose management for the patients with type 2 diabetes. Therefore, self-caring blood glucose programs and internal synchronizing education through social support and empowerment need to be improved.

  • Purpose: The purpose of this study was to investigate the depression trends by gender in type2 diabetic patients. Method: One hundred Participants were recruited from the endocrinology outpatient department of a tertiary care hospital in an urban city. Depression was measured using the Beck Depression Inventory. Glycosylated hemoglobin was determined by a high-performance liquid chromatography technique. Fasting plasma glucose and 2 hour post-prandial blood glucose were analyzed by the glucose oxidase method. Results: The Depression score was significantly higher in diabetic women (l0.4) than in male diabetic patients (7.8). In male diabetic patients, depression was positively correlated with glycosylated hemoglobin (r=0.35, p=0.006). However, there was no correlation between depression and glycosylated hemoglobin in female diabetic patients. Conclusion: These findings indicate that depression score was significantly higher in female diabetic patients and the depression was positively correlated with glycosylated hemoglobin in male diabetic patients.

  • Purpose: The purpose of this study was to evaluate the effect of web-based diabetic education on glycosylated hemoglobin(HbAlc) and self-care in people with type 2 diabetes. Method: Twenty-five patients were requested to input the blood glucose level weekly at http://www.biodang.com by cellular phone or wired Internet for one year. The researcher sent optimal recommendations weekly to each patient using the short message service for cellular phones and wired Internet. Results: Patients had a significant mean decrease in HbAlc, from 8.0% at pre-test to 6.9%, after three months and rested at 6.8% until 12 months. Patients had a significant mean increase in physical exercise adherence, from 3.3 days at pre-test to 4.1 days per week, after three months. Patients had a significant mean increase in foot care adherence, from 4.8 days at pre-test to 6.9 days per week, after one year. Conclusion: These findings indicate that Internet education could be used to improve HbAlc, physical exercise, and foot care adherence in type 2 diabetic patients.

당뇨환자를 대상으로 한 영양상담의 효과측정 (Measurement of Nutrition Counseling Effects for Diabetes Mellitus Patients)

  • The purpose of this study was to measure the counseling effects for diabetes mellitus patients. To measure the effects of the nutrition counseling, 40(13 men and 27 women) outpatients with diabetes mellitus were selected and they were randomly assigned to either the counseling group or non counseling group. Twenty-one counseling group patients received nutrition counseling weekly for 6 weeks and 19 patients served as non counseling group(control group) did not received counseling over same study period. The results of this study can be summerized as follows : 1) The food attitude score of the counseling group significantly increased from 61.9$\pm$15.9 prior to counseling to 87.0$\pm$7.8 after counseling (p<0.001). 2) Fasting blood glucose level and postgrandial blood glucose level also showed significant difference between two groups(p<0.01). In the counseling group, the fasting blood glucose level significantly decreased from 163.5$\pm$48.6mg/dl to 142.3$\pm$40.6mg/dl(p<0.01), and the postgrandial blood glucose level significantly decreased from 281.3$\pm$105.1mg/dl(p<0.001). 3) There was a significant difference of glycosylated hemoglobin between the two groups(p<0.005), and the glycosylated hemoglobin level of counseling group significantly decreased from 11.2$\pm$2.9% to 9.7$\pm$3.6%(p<0.005). 4) There was a significant correlation between the food attitude score difference and the glycosylated hemoglobin level difference in counseling group(p<0.05).

Analysis of glycosylated hemoglobin (HbA1c) level on maxillofacial fascial space infection in diabetic patients

  • Objectives: This study was performed to evaluate the impact of glycosylated hemoglobin (HbA1c) level on characteristics and prognosis of maxillofacial fascial infection in diabetic patients. Materials and Methods: We reviewed the medical records of 72 patients (35 patients with HbA1c lower than 7.0% and 37 patients with HbA1c higher than 7.0%) diagnosed with maxillofacial fascial space infection and hospitalized for treatment at the Department of Oral and Maxillofacial Surgery in Dankook University Hospital (Cheonan, Korea) from January 2005 to February 2014. We compared demographics, parameters of glucoregulation (HbA1c), laboratory parameters of inflammation (white blood cell [WBC], C-reactive protein [CRP] count), type and number of involved spaces, type and number of antibiotics, period of hospitalization, number of surgical operations, need for tracheostomy, complications, computed tomography (CT), and microorganisms between the two groups. Results: Compared with the well-controlled diabetes mellitus (DM) group (HbA1c <7.0%), patients in the poorly-controlled (HbA1c ${\geq}7.0%$) DM group had the following characteristics: longer hospitalization periods, higher values of laboratory parameters of inflammation (WBC, CRP count) at the time of admission, higher number of antibiotics prescribed, more frequent complications, frequent deep neck space involvement, and distinctive main causative microorganisms. As the HbA1c level increases, hospitalization periods and incidence of complications increase gradually. Conclusion: This retrospective study suggests that regulation of DM significantly impacts maxillofacial fascial infection. Poorly controlled DM with high HbA1c level negatively influences the prognosis of infection.

Aerobic and Graduated Treadmill Exercise Decreases Blood Glucose Levels, Lipid Levels and Oxidative Stress in an Animal Model of Type 1 Diabetes Mellitus

  • Purpose: Exercise has been shown to be a simple and economical therapeutic modality that may be considered as an effective aid for diabetic mellitus. For example, exercise training increases insulin sensitivity in type 2 diabetes. But we found no reported of how exercise affect type 1 diabetes. This study investigated the impact of aerobic and graduated treadmill exercise regimens on body weight, glucose and insulin concentrations, lipid profiles, and oxidative stress indicators in rats with streptozotocin (STZ) induced diabetes. Glycosylated hemoglobin ($HbA_{1c}$) was determined as an indicator of glucose control during exercise. Methods: In our study, a total of 40 rats were used. Three groups of 10 rats each were given STZ to induce diabetes. The remaining 10 rats became the normal group. After 28 days we determined biochemical parameters such as glucose, glycosylated hemoglobin ($HbA_{1c}$), insulin concentration, serum total cholesterol (TC), triglycerides (TG), and high-density lipoprotein (HDL). Superoxide dismutase (SOD) and catalase activities were also measured. Results: Concentrations of blood glucose and $HbA_{1c}$ in the moderated exercise groups were significantly decreased after 28 days compared with the control group (p<0.05). There was a significant reduction in serum TC and TG in the experimental groups. The activity of SOD increased significantly by 17.70% and 48.25% respectively. Conclusion: These results indicate that physical training and exercise training affects body weight, fasting blood glucose, $HbA_{1c}$, insulin, lipid profiles, and antioxidant status in rats with streptozotocin-induced diabetes. We suggest that graduated treadmill exercise may have therapeutic, preventative, and protective effects against diabetes mellitusby improving glycemic control, oxidant defenses, and lipid metabolism.

  • Purpose: This study was to identify the effect of the experience of diabetes education on knowledge, self-care behavior and glycosylated hemoglobin (HbA1C). Further, this study was held to examine about patient's preferred methods of education and re-education frequency. Methods: 166 type 2 diabetes patients from two hospitals in Incheon participated in this study. Data were analyzed by using descriptive analysis, t-test, ANOVA, Scheffe's test and multiple regression analysis. Results: 72.3% patients needed re-education and the average interval of re-education was 8.53 months. Patients preferred education methods were lectures, practical training, and studying from pamphlet. Depending on the frequency of diabetes education, there were significant differences in the level of diabetes knowledge (F=10.88, p<.001) and self-care behaviors (F=4.59, p=.012), but there was not significant difference with HbA1C (F=1.53, p=.220). As to how much the diabetes education helped managing diabetes, there was a significant difference in the level of self-care behaviors (t=2.01, p=.049), but there were not significant differences in level of knowledge (t=1.10, p=.275) and HbA1C (t=-.33, p=.746). The experience of diabetes education was a significant factor which influenced patient's knowledge (t=3.93, p<.001) and self-care behaviors (t=2.21, p<.001). But HbA1C was not influenced by the experience of diabetes education (t=-1.68, p=.096). Conclusion: It is necessary to provide diabetes education with appropriate interval and methods and subjects that reflect the needs of patient through the study results.

제2형 당뇨병 환자의 질병관련 지식 (A Survey of Knowledge of Diabetes in Patients with Type 2 Diabetes)

  • Purpose: The purpose of this study was to investigate the knowledge of diabetes and insulin use in patients with type 2 diabetes. Method: Participants (139) were recruited from the endocrinology outpatient department of a tertiary care hospital in an urban city. Knowledge of diabetes knowledge was measured by a brief diabetes knowledge test which had two components: a 14-item general test and a 9-item insulin-use subscale. Result: The average scores for knowledge of diabetes and insulin were 62.4 and 41.8 respectively. Scores were lower for items on the definition of glycosylated hemoglobin, diabetic diet, signs of ketoacidosis, and insulin reaction compared to other items. Knowledge scores were significantly lower for older participants, those who had lower levels of education, those who did not have a spouse, those who did not have a job, those who were not being treated with insulin, and those who tested their blood glucose less than 3 times a day. Conclusion: Definition of glycosylated hemoglobin, diabetic diet, signs of ketoacidosis, and insulin reaction should be included in education for patients with type 2 diabetes. Special education programs should be developed for elderly people, those with low levels of education those without a spouse, or without a job, or who are not being treated with insulin.

제2형 당뇨병 환자의 하부요로증상 관련요인 (Factors Associated with Lower Urinary Tract Symptoms in Patients with Type 2 Diabetes Mellitus)

  • Purpose: This study was performed to identify lower urinary tract symptoms (LUTS), and to evaluate the factors affecting LUTS in patients with type 2 diabetes mellitus (T2DM). Methods: The cross sectional study was used with a structured questionnaire to collect data through interviews with 181 T2DM patients and their clinical data from a university hospital diabetes clinic from October 2010 to April 2012. LUTS were measured using the International Prostate Symptom Score (IPSS), depression using the Center for Epidemiologic Studies Depression Scale (CES-D), and glycosylated hemoglobin (HbA1c) from the clinical data. Results: Of all patients with T2DM, the mean IPSS of LUTS was $9.34{\pm}6.86$. Concerning the reported severity of LUTS, 53.6% of the subjects were in the moderate and severe group. In each symptom score of LUTS (range 0-5), nocturia was the highest 2.04, weak stream 1.62, and frequency 1.45. LUTS was significantly predicted by HbA1c and depression, and 14.3% of the variance in LUTS was explained. Conclusion: HbA1c and depression were found to be very important factors associated with LUTS in T2DM patients.
