마미총에 발생한 해면상 혈관종 - 증례보고 - (Cavernous Hemangioma of the Cauda Equina - A Case Report -)

  • Spinal cavernous hemangiomas of the cauda equina are extremely rare vascular malformations. We report a case of intradural extramedullary cavernous hemangioma of the cauda equina with it's clinical, radiologic and surgical findings. This is the twelveth case of cavernous hemangioma of the cauda equina in the literature. The pertinent literatures are reviewed.

Thoracic Extradural Cavernous Hemangioma Mimicking a Dumbbell-Shaped Tumor

  • Dumbbell-shaped spinal extradural cavernous hemangioma is rare. The differential diagnosis of dumbbell-shaped spinal tumors based on magnetic resonance imaging includes schwannoma and lymphoma. Here, we report a dumbbell-shaped spinal extradural cavernous hemangioma with intrathoracic growth on T2-3 in a 64-year-old man complaining of right side infrascapular area back pain with no neurologic deficit. The cavernous hemangioma was resected through combined video-assisted thoracoscopy and laminectomy without a fusion procedure. The patient had tolerable operative wound pain with no neurologic deficit after surgery. Based on magnetic resonance imaging findings and a review of the literature, we discuss cavernous hemangioma among the differential diagnosis of paravertebral dumbbell-shaped spinal tumors and the importance of complete resection.

  • Purpose: Hemangioma of the breast is an infrequent finding and usually encountered incidentally when checking for other disease. Most of hemangiomas of the breast are asymptomatic, not palpable perilobular type. Cavernous hemangioma of the breast is rare and only a few reports about this type of lesion are present. No example has been reported about reconstruction of the breast after resection of large cavernous hemangioma. Methods: We report here a case of immediate breast reconstruction using a Becker implant after subcutaneous mastectomy for a large cavernous hemangioma involving almost entire breast. Results: Symmetry is well maintained after 3 years without deformity or recurrence. Conclusion: The clinical prognosis of breast cavernous hemangioma is good after total excision and reconstruction.

  • Cavernous hemangiomas were first reported in 1929 by Globus and Doshay, and are defined as benign vascular structures developed between the neural tissues occurring in the central nervous system, consisting of a dilated vascular bed. Cavernous hemangiomas comprise nearly 5-12% of all spinal vascular malformations; however, existence in the epidural space without bone involvement is rare. Only 4% of all cavernous hemangiomas (0.22/1.000.000) are purely epidural cavernous hemangiomas. In this case report, we removed a hemorrhagic thoracic mass presenting with progressive neurological deficits in a 55-year-old male patient. We found this case to be appropriate for presentation due to the rare occurrence of this type of cavernous hemangioma.

Development of De Novo Cavernous Hemangioma after Radiosurgery for Cavernous Hemangioma

  • We report a rare case of cavernous hemangioma (CH) which developed in adjacent location to a preexisting CH after gamma knife radiosurgery (GKRS). A 36-year-old woman underwent GKRS for a CH in the left lentiform nucleus. Three-and-half years after radiosurgery, MRI revealed a new CH in the left caudate nucleus. Surgical excision of the new lesion was performed. The pathological examination confirmed the diagnosis of CH. In radiosurgery for CH, it should be noted that a new CH may develop, which is likely to result from the interaction between radiation and predisposing factors of the patient.

  • Cavernous hemangioma of the gallbladder is an extremely rare benign tumor. The tumor has only a few cases being reported in literature. However, to the best of our knowledge, no reports focusing on the MRI findings of cavernous hemangioma of the gallbladder have been published. This study reports a case of gallbladder hemangioma with pathologic and radiologic reviews, including MRI findings.

  • Hemangioma is one of the most common congenital tumors in the region of the face and neck. Although histologically benign, these facial masses are clinically malignant for their deforming and inexorable growth, especially in so-called 'cavernous hemangioma'. Carvenous hemangioma is the most common primary tumor occurring in the adult orbit. This tumor has symptoms that characteristically develop over several years with slowly progressive proptosis, eyeball deviation, hyperopia, diplopia and optic nerve compression. Today, hemangiomas are being treated by various methods; steroids, electrocoagulation, injection of sclerosing agent, cryotherapy, radiation therapy, laser therapy, and surgical treatment, etc. In principle, surgical approaches to the orbit must provide maximum safety and optimal visualization. We have experienced a case of large cavernous hemangioma in the orbit inferolaterally. The surgical treatment of tumor was achieved by the bicoronal approach combined with inferomedial and inferolateral orbitotomy. This surgical approach allows better visualization of the tumor and greater protection of essential anatomic structures. We obtained satisfactory results in terms of aesthetic and functional consideration. We present our case with a brief review of the literature related to orbital cavernous hemangioma.

A Case of Mediastinal Cavernous Hemangioma

  • We report a case of cavernous hemangioma of the anterior mediastinum that was incidentally detected by chest radiography taken at a routine health check-up. A mass lesion was seen in the anterior mediastinum on computed tomography and magnetic resonance imaging. Direct surgical removal was performed for diagnosis and treatment through right thoracotomy. Histopathology confirmed the mass as a cavernous haemangioma. Post-operative course was uneventful.

전종격동에 발생한 거대 해면상 혈관종 - 1례 보고 - (Cavernous Hemangioma in the Anterior Mediastinum - A case report -)

  • 종격동에 발생하는 해면상 혈관종(cavernous hemangioma)은 매우 드문 양성종양이다. 환자는 3년 8개월 된 여아로서 내원 2개월 전 폐렴으로 입원하여 우연히 종격동 종양을 발견하였다. 단순 흉부 촬영상 좌측 전폐야에 걸쳐있는 비정상적인 음영이 있었으며 추가검사 후 수술적 절제를 시행하였다. 절제된 종괴는 약 15 $\times$ 10 cm크기의 해면상 혈관종으로 진단되었으며 수술 후 8일째 퇴원하여 이상소견 없이 외래 관찰중이다. 전 종격동에서 발생하여 좌측 전폐를 압박하고 있는 거대 해면상 혈관종을 수술 치험 하였기에 이를 보고하는바이다.

  • The cavernous hemangioma often occur on the skin and mucosal surfaces of the body, but are also found in many viscera, particularly the liver, spleen, pancreas and occasionally in the brain. But it is rarely encountered in the mediastinum, but when found occur predominantly in the anterior mediastinum. It can occur in any age and have no characteristic symptoms or roentgenographic findings including angiocardiography. Inspite of its histologic benignancy, it may be locally invasive and can result in rib erosion or adjacent structural compression. Usually, surgical exploration is not only the sole means of assuring a diagnosis and the only treatment. Recently, we experienced one case of cavernous hemangioma in the anterior mediastinum, which was removed surgically, being proved to be cavernous hemangioma on histologic examination. Related literatures were reviewed.
