  • Purpose: The purpose of this study was to describe the lived experience of patients with heart transplantation in Korea. Methods: Individual indepth interviews and a focus group interview were used to collect the data from nine patients who had heart transplantations in 2015. All interviews were audio-taped and verbatim transcripts were made for the analysis. Data were analyzed using Colaizzi's phenomenological method. Results: Among the nine participants, eight were men. Mean age was 57.30 years. Six theme clusters emerged from the analysis. 'Joy of rebirth obtained by good luck' describes the pleasure and expectation of new life after narrow survival. 'Suffering from adverse drug effects' illustrates various psychosocial difficulties, such as low self-esteem, helplessness, alienation, and burnout, arising from the side effects of medications. 'Body and mind of being bewildered' illustrates disintegrated health and haunting fear of death. 'Alienation disconnected with society' describes isolated feeling of existence due to misunderstandings from society. 'Suffering overcome with gratitude and responsibility' includes overcoming experience through various social supports and suitable jobs. Finally, 'acceptance of suffering accompanied with new heart' illustrate changed perspective of life itself. Conclusion: The findings in this study provide deep understanding and insights of the lived experience of heart related illness for these patients and should help in the development of tailored-interventions for patients with heart transplantation.

  • The purpose of this study is to identify the influences of psychological variables and fashion-related psychological variables on purchasing fashion items on the Internet. Boredom proneness and public self-consciousness were selected as psychological variables, and dressing style was selected as a fashion-related psychological variable. It was hypothesized that boredom proneness and public self-consciousness not only influence the purchasing frequency of fashion items on the Internet directly, but also indirectly through dressing style. Data were gathered by surveying university students in Seoul using convenience sampling. Two hundred and eighty-six questionnaires were used in the statistical analysis. SPSS was used for exploratory factor analysis, and AMOS was used for hypothesized relationship testing. The factor analysis of boredom proneness revealed five dimensions, "helplessness," "affective response," "lack of internal stimulation," "lack of external stimulation," and "perception of time." The factor analysis of public self-consciousness revealed two dimensions, "appearance-consciousness" and "style-consciousness," and the factor analysis of dressing style revealed one dimension. The overall fit of the hypothesized model suggests that the model fits the data well. The hypothesized relationship test proved that boredom proneness and public self-consciousness influence the purchasing frequency of fashion items on the Internet indirectly through dressing style. The results implicate effective strategies for Internet shopping malls and suggestions for future study.

Method: The study sample was 100 living at home and institutions. Data were collected by interview from Oct. to Dec. in 2003. The instruments were helplessness scale developed by Jung (1998), sense of belonging scale SOBI-A and nutritional initial screening. Results: The mean score of powerlessness was $2.71{\pm}0.30$, sense of belonging was $2.87{\pm}0.19$ and nutritional status was $4.42{\pm}3.34$. There was a statistically significant differences in powerlessness according to age (F=3.185, p=0.027), health status (F=6.945, P=0.002), religion (F=5.941, P=0.001), current spouse (t=-0.384, p=0.026), in sense of belonging according to age (F=4.40, P=0.006), length of education (F=10.64, P=<.0001) and in nutritional status according to age (F=3.34, P=0.022), health status (F=2.86, P=0.027). There was a statistically significant correlation between powerlessness and nutritional status (r=0.219, p=0.029). Conclusions: Nurses are able to decrease powerlessness or increase sense of belonging and nutritional status of the elderly by developing the health promotion program, improving perceived health status and empowering social interaction of the elderly specifically living at institutions.

  • Purpose: This was a methodological research to develop an instrument to assess the emotional response of family members of physically restrained patients. Methods: A primary instrument with 68 questions was developed based on literature review and semi-structured interviews with family members. A group of experts revised individual questions and removed 4 irrelevant questions. This secondary instrument, then, was tested with 199 family members of physically restrained patients in intensive care units of a university hospital. The validity and reliability of the instrument were tested by factor analysis. Results: After item analysis, 3 questions with a correlation coefficient under .30 were discarded and the questions with a factor loading under .45 on Varimax Rotation were also removed. After factor analysis on the final 37 questions, 7 factors were identified; avoidance, shock, helplessness, grudge, depression, anxiousness, and acceptance. The total variance explained was 55.63%. The reliability of this instrument was 0.93 of Cronbach's alpha. Conclusion: This instrument was statistically reliable and valid to measure family's emotional response to physical restraints of the patients. This instrument can be useful in assessing the effects of nursing interventions for family members of restrained patients.

Methods: Data were collected by 2 focus group interviews with 9 ICU nurses. The interview were recorded and transcribed, and data were analyzed using qualitative content analysis. Results: The results of data analysis, five themes were drawn: 'Confusion about for the attempted suicidal patient care', 'Helplessness for dying with attempted suicidal patients', 'Guilty for insufficient care', 'Ambivalence for the attempted suicidal patients', 'Recognition of need for professional approach'. Conclusion: The results of this study help us to understand patients who attempted suicide through the ICU nurses' experience. It would be useful to develop effective education programs for ICU nurses to preventive strategies for patients who are high risk of recurrence of suicidal crisis situation.

  • Purpose: The purpose of this study was to investigate the emotional response of family members of physically restrained patients in the intensive care units (ICUs). Methods: The study subjects were 200 family members of ICU patients who had been on physical restraints in two university hospitals. Data were collected using the "Instrument of family's emotional response toward physically restrained patients". Results: The mean score of familial emotional response was 2.69 out of a possible 5. The subcategory of acceptance was the highest with 3.56 points followed by depression (3.02), helplessness (2.94), anxiety (2.87), shock (2.74), avoidance (2.64), and grudge (2.08). Multiple stepwise regression analysis indicated that the age of family members, side effects of restraints, and information provision were the variables influencing on negative emotional response of family. Conclusion: Family members showed slightly negative emotional response toward the physical restraints. This finding could be influenced by their limited knowledge of the need for the restraints. Educational programs or fact sheets to be given to family members may be helpful.

  • Purpose: This study was done to assess the personal experiences of the coping process during pregnancy for single mothers. Methods: The participants were 17 single mothers who had stayed in a social welfare facility. Data was collected with an in-depth unstructured interview. Data analysis was done by the grounded theory method. Results: One-hundred twelve concepts and 49 sub-categories were confirmed in the analysis. The sub-categories were grouped into 19 categories; escape from a miserable family, wrong meeting, openness of sex, defenseless state of pregnancy, inevitable result of pregnancy, heartbreak by herself, closure, isolation, difficult situation of being alone, stigma, supporting & protecting, helplessness, seeking, empowering, feeling of loss, conflict, facing issues, assuring a fresh start and becoming-mature. "Being driven over the edge of a cliff" was the key phenomenon which the single mothers experienced during the process of pregnancy. Conclusion: The above results will help nurses assessing single pregnancy mothers' needs and developing a nursing intervention program for supporting them. Therefore, nurses will be able to stop them from "being driven over the edge of cliff". A more vigorous nursing intervention is suggested for the research of the vulnerable classes of medical health care including single pregnant mothers.
