위암과 미생물총 (Gastric Cancer and Non-Helicobacter pylori Microbiota)

  • Gastric cancer is the 4th leading cause of death worldwide. The primary cause of gastric cancer is known to be Helicobacter pylori (H. pylori). The advancement of molecular biology has enabled the identification of microbiomes that could not be confirmed through cultivation, and it has been revealed that the microbial communities vary among normal mucosa, atrophic gastritis, intestinal metaplasia, and gastric cancer. It has also been confirmed that the composition of the microbial community differs depending on the presence or absence of H. pylori. Whether changes in the microbiome are causative factors in the carcinogenesis process is not yet clear. Experiments using animal models and in vitro studies on the role of microbes other than H. pylori in the carcinogenic process are underway, but the data is still insufficient.

  • 비피도박테리아 증식인자인 1,4-dihydroxy-2-naphthoic acid 생산 유산균주 LP2가 선행연구에서 자연치즈로부터 분리되었다. LP2 균주는16S rRNA 유전자 염기서열 분석에 의한 균주동정에서 Lactobacillus plantarum(99% 일치율)과 가장 높은 상동성을 나타내어 Lactobacillus plantarum LP2 균주로 명명하였다. LP2 균주의 배양 상등액은 Helicobacter pylori KCTC 12083 균주에 대하여 항균활성을 나타내었으며 이는 생산된 DHNA에 기인하는 것으로 추측되었다.

Inhibitory Activity against Helicobacter pylori of Isolated Compounds from Pinus koraiensis Siebold et Zucc Leaves

  • A phenol substance was extracted from Pinus koraiensis Siebold et Zucc leaf extracts and its biological efficacy was measured. The highest content of the phenol substance contained in Pinus koraiensis Siebold et Zucc leaves was 13.5 mg/g, which was obtained when it was extracted with 80% ethanol. At a concentration of 200 mg/mL, the phenolic substances extracted with 80% ethanol and water showed antimicrobial activities against Helicobacter pylori, producing clear zones of 10 and 12 mm diameter, respectively. Pinus koraiensis Siebold et Zucc. leaf extracts were separated using a Sephadex LH-20 column and 4 fractions were obtained (fractions A-D). Fractions C and D showed the greatest inhibitory activity against Helicobacter pylori producing 10.1 and 12.3 mm clear zones, respectively. These two fractions were purified using a Sephadex LH-20 and MCI-gel column ($H_2O{\rightarrow}100%$ ethanol). Purified compounds A and B were identified as syringic acid and compound C was identified as p-coumaric acid based on $^1H$-nuclear magnetic resonance (NMR), $^{13}C$-NMR, and fast atom bombardment mass spectrometry spectra. When two or more purified compounds were mixed, a synergistic effect of anti-Helicobacter pylori activity was evident. This result indicates that extracts of Pinus koraiensis Siebold et Zucc leaves could be considered a functional food because of their high antimicrobial properties.

  • Helicobacter pylori infection plays an important role in the pathogenesis of chronic gastritis, peptic ulcer disease and gastric malignancy. A diagnosis of infection is thus an important part of a treatment strategy of many gastrointestinal tract diseases. Many diagnostic tests are available but all have some limitations in different clinical situations and laboratory settings. A single gold standard cannot available, but be used for diagnosis of Helicobacter pylori infection in daily clinical practice in all areas, so several techniques have been developed to give reliable results, especially focusing on real time endoscopic features. The narrow band imaging system (NBI) and high resolution endoscopy are imaging techniques for enhanced visualization of infected mucosa and premalignant gastric lesions. The aim of this article is to review the current diagnostic options and possible future developments detection of Helicobacter pylori infection.

  • This study investigated the effect that some polyacetylenes and protopanaxatriol, which were isolated from heated ginseng (family Araliaceae), have on inhibiting Helicobacter pylori (HP) growth. Among the compounds tested, panaxytriol was quite effective in inhibiting HP growth with an MIC of 50 ${\mu}g/ml$. Cinsenoside Rhl and protopanaxatriol weakly inhibited $H^{+}/K^{+}$-ATPase from a rat stomach.

A Convenient Synthesis of an Anti-Helicobacter Pylori Agent, Dehydrodiconiferyl Alcohol

  • Potential anti-Helicobacter pylori agent dehydrodiconiferyl alcohol was synthesized in 44% overall yield, starting from vanillin which could be commercially available. Carbon extension of vanillin followed by the Horner-Wadsworth-Emmons reaction, a biomolecular radical coupling reaction and DIBAL-H reduction gave dehydrodiconiferyl alcohol.

  • It has been suggested that Helicobacter pylori(H. pylori) infections can promote the development and progression of gastric cancer through the modulation of cell cycle regulators such as $p27^{Kip1}$ and Skp2. $p27^{Kip1}$ is a cyclin-dependent kinase (CDK) inhibitor that blocks the G1/S transition necessary for cell cycle progression. Skp2 is a component of the ubiquitin ligase complex called $SCF^{Skp2}$(SKP1-Cullin-F-box), which specifically binds and promotes the degradation of $p27^{Kip1}$. A low level of $p27^{Kip1}$ and a high level of Skp2 have been reported in many types of cancers, including gastric cancer. In addition, a decrease in $p27^{Kip1}$ has been reported in H. pylori-infected specimens. However, data on Skp2 in H. pylori infections are limited. This study examines the changes in the status of Skp2 in H. pylori-infected gastric epithelial AGS cells. For this, we stimulated AGS cells with H. pylori(NCTC 11637) at the ratio of 300:1(bacterium:cell) for 6 hours. The results of an immunoprecipitation analysis, followed by a western blot, indicate that the interaction between Skp2 and 14-3-3 was elevated 3 hours after the H. pylori treatment. In addition, there was an increase in cytoplasmic Skp2 after 3 hours, whereas there was no change in the nuclear level. Since it has been reported that interaction with 14-3-3 and the subsequent cytoplasmic translocation of Skp2 can increase its protein stability, increases in the interaction with 14-3-3 and the cytoplasmic Skp2 after the H. pylori treatment can increase the level of Skp2 in AGS cells. This phenomenon may explain, at least to some extent, the mechanism underlying the relationship between H. pylori infections and gastric carcinogenesis.

  • 무(한국 품종 및 일본 품종)를 사용하여 H. pylori에 의한 위세포독성 및 IL-8생성에 미치는 영향을 살펴보았다. 그 결과 무는 H. pylori에 의한 위세포독성을 농도의존적으로$(2\~10 mg/ml)$ 억제하였으며 또한 위암세포인 KATO III에서 분비되는 IL-8의 생성을 억제시킴$(5\~10mg/ml)$을 알았다. 이러한 사실은 무가 H. pylori감염에 의한 위점막세포의 손상을 방지할 수 있음을 나타낸다.

위암발증 원인균에 대한 생활습관 및 영양섭취에 있어서 농촌지역 주민조사 (Dietary Factors and Life Style Affecting the Causal Infection on the Gastric Cancer of Adults in Rural Korea)

  • 위암발증 원인을 밝히기 위하여 농촌지역 일반 주민들의 Helicobater pylori의 감염율과 생활습관 및 영양섭취 실태와의 상관관계를 검토하기 위하여 강원도 춘천시 인근 농촌 지역 주민들을 조사하였다. 조사 대상자는 40대 이상인 지역 주민 183명으로 남자 81명, 여자 102명에 대하여 설문조사와 식사량 조사, 혈액 생화학적 분석 및 혈청 ELISA법에 의한 Helicobater pylori의 감염 정도를 분석하였다. 그 결과를 요약하면 다음과 같다. 1. 조사 대상자의 학력은 74.3%가 국졸이하였고, 월수입은 80.3%가 100만원 이하였다. 2. 조사 대상자의 직업은 75.3%가 농업에 종사하였으며, 가족형태는 62.8%가 핵가족이었다. 3. 출산 자녀수는 75.9%가 2명에서 5명의 자녀를 출산하였고, 음주자는 46.4%이며 매일 술을 마시는 사람이 12%였다. 흡연자는 21.9%였으며, 비흡연자는 78.1%였다. 4. 평균신장은 157.4cm였고 여자의 평균 신장은 151.9cm, 남자의 평균 신장은 164.4cm였고 남녀간에 유의차가 있었다(p<0.001). 5. Helicobater pylori의 감염율은 총 조사 대상자 중 64.5%였으며, 남자가 65.4%, 여자가 63.7%였다. 감염율은 흡연자, 학력 수준이 낮을수록, 월수입이 적을수록, 출산 자녀수가 많을수록, 50대의 연령에서 많은 것으로 나타났다. 6. Helicobater pylori의 감염 음성자와 양성자간의 영양소 섭취실태를 보면 음성자가 에너지, 총지질(p<0.05), 동물성 지질(p<0.05), 철분 섭취량(p<0.001), 비타민 A의 섭취량(p<0.01), 비타민 C 섭취량(p<0.05)에 있어서 유의적으로 높은 섭취량을 나타내었다. 그러나 칼슘 섭취량은 음성인자가 낮았다.

Updates on the Diagnosis of Helicobacter pylori Infection in Children: What Are the Differences between Adults and Children?

  • Helicobacter pylori infection is acquired mainly during childhood and causes various diseases such as gastritis, peptic ulcer disease, mucosa-associated lymphoid tissue (MALT) lymphoma, and iron deficiency anemia. Although H. pylori infection in children differs from adults in many ways, this is often overlooked in clinical practice. Unlike adults, nodular gastritis may be a pathognomonic endoscopic finding of childhood H. pylori infection. Histopathological findings of gastric tissues are also different in children due to predominance of lymphocytes and plasma cells and the formation of gastric MALT. Although endoscopy is recommended for the initial diagnosis of H. pylori infection, several non-invasive diagnostic tests such as the urea breath test (UBT) and the H. pylori stool antigen test (HpSA) are available and well validated even in children. According to recent data, both the $^{13}C$-UBT and HpSA using enzyme-linked immunosorbent assay are reliable non-invasive tests to determine H. pylori status after eradication therapy, although children younger than 6 years are known to have high false positives. When invasive or noninvasive tests are applied to children to detect H. pylori infection, it should be noted that there are differences between children and adults in diagnosing H. pylori infection.
