A Primary Neuroendocrine Tumor Mimicking a Thrombus in the Left Atrial Appendage (좌심방이에서 발생한 혈전을 모방한 심장의 일차성 신경 내분비 종양)

  • Most cardiac tumors are metastases, and primary cardiac tumors are rare; even among primary cardiac tumors, primary cardiac neuroendocrine tumors (NETs) are extremely rare. Herein, we report a case of a patient presenting a left atrial mass without past medical history. Because of the location and movement of the mass, as well as the patient's cerebral infarction episode, the mass was initially suspected to be a thrombus. However, the mass was surgically diagnosed as NET.

  • Benign cardiac tumors are almost uniformly curable, but malignant cardiac tumors are almost always fatal despite of modern techniques of diagnosis and surgical treatment Resection of malignant tumors of heart is occasionally feasible and can result in prolonged survival, although cure is unlikely. Survival may be enhanced by postoperative irradiation in selected patients, but chemotherapy does not appear to be beneficial. Fibromyxosarocoma of the left atrium is a primary malignant tumor of the heart which is extremely rare and has a poor prognosis and rapid recurrence. It is important to differentiate the uniformly fatal fibromyxosarcoma from the more common benign atrial myxoma. A 19-year-old woman with a fibromyxosarcoma of the left atrium which was extirpated surgically, and postoperative irradiation was given[totally 4,500 rads]. She is still alive 9 months postoperatively without recurrence.

  • A 70-year-old man with aphasia due to ischemic cerebral events by thromboemboli was admitted. The cause of thromboemboli was investigated, and transesophageal echocardiography and chest MRI revealed an encapsulated cystic mass in the right atrium, not circulating. It was a homogeneous cystic mass suggesting a tumor (Myxoma) rather than thrombus. Right atrial mass was resected together with partial atrial septum under the normgthermic cardiopulmonary bypass. Histologically it was an atrial septal aneurysm, closed on itself, filled with blood. We re-port this rarely seen case with a review of the literatures.

  • A 39-year-old man presented to the department of emergency medicine in Seoul National University Hospital complaining of chest pain, heart palpitation, and headache. Upon arrival, a computed tomography scan showed a 7.0 cm×6.2 cm lesion with the typical features of a paraganglioma. The patient was treated with an alpha-blocker and a beta-blocker prior to surgical intervention. We removed the tumor successfully, and histopathologic findings indicated that the tumor was indeed a paraganglioma. Since intracardiac paraganglioma is a rare disease, we present this case together with a literature review.

  • The most common primary heart tumor in infants is the rhabdomyoma. It is known that tuberous sclerosis is found in half of the patients with cardiac rhabdomyoma. Because the prognosis of this tumor associated with subaortic stenosis is poor, the surgical intervention is indicated. We report one case of left ventricular rhabdomyoma with tuberous sclerosis.

  • Primary tumors of the heart are extremely rare, and about 25 per cent of primary cardiac tumors are malignant. Recently, We experienced a case of primary cardiac liposarcoma occurred on the right ventricular outflow tract and extended into the main pulmonary artery. The patient was 57 year-old man and the chief complaints was severe exertional dyspnea. Emergency operation to relieve the right ventricular outflow tract obstruction was performed under cardiopulmonary bypass. The tumor was 5$\times$4$\times$3 cm in size and infiltrated into the right ventricle and ventricular septum. The tumor in the main pulmonary artery was removed without any difficulty but the on the right ventricular outflow tract was difficult to remove completely. The patient was recovered without any specific problems. After successful surgical resection and postoperative radiotherapy the patient discharged with good condition. Herein, the clinical characteristics of cardiac liposarcoma, the problems in differential diagnosis, the prognosis and current therapy are reviewed.

  • Sj$\"{o}$gren's syndrome is a systemic, autoimmune disorder of the exocrine glands with associated lymphocytic infiltrates of the affected glands and characteristic of dry eye and mouth. We report a case of Sj$\"{o}$gren's syndrome in which the patient presented systemic symptoms without sicca symptoms and had radiologic findings mimicking multiple skeletal metastasis and was finally confirmed with Sj$\"{o}$gren's syndrome with renal tubualr acidosis.

Clinical Experience of Atrial Myxoma in 15 Patients (심방 점액종의 임상적 치험 15례)

  • Fifteen patients[10 female and 5 male from 13 to 73 years of age[mean age 46 years underwent excision of atrial myxomas between 1981 and 1993 at the Chonnam University Hospital. Fourteen patients presented with exertional or resting dyspnea, six with palpitation, five with systemic embolization, and three with syncope. Symptoms were present from 1 week to 14 months before operation. Fourteen tumors originated from the atrial septum of left atrium and one of bilateral atrium. All tumors were pedunculated; tumor sizes were variable from 2x1x1cm to 7x6x5cm. No ventricular tumors identified. The myxomas were successfully removed in all patients, either by shaving them from the atrial septum[n=7 or excising a portion of normal atrial septum with the tumor and ASD patch closure[n=8 . There was no perioperative or late death in our experience. Follow-up is current and complete in all cases[range 6 months to 13 years . Thirteen patients are in New York Heart Association Class I, and the remaining two patients are in Class II. One recurrent left atrial myxoma was identified at 20 months after operation. In this series, excellent results were obtained by simple excision of the tumor, with or without a margin of normal atrial septum. Long-term clinical & echocardiographic follow-up is recommended since late recurrence, although rare, has been reported.

Successful surgical interventions for a giant and complicated myoepithelial carcinoma: a case report

  • Ethmoid myoepithelial carcinoma is a rare tumor, with only 14 cases reported to date. This report discusses the largest tumor of this type ever recorded in the ethmoid region. The tumor caused extensive damage to facial structures, complicating treatment. The patient's age and comorbidities increased the risk of intraoperative bleeding, presenting challenges to the complete removal of the tumor and the reconstruction of the damaged structures. To reduce the risk of intraoperative hemorrhage, shorten the surgery time, and manage potential heart-related complications, arterial embolization was performed using gelatin sponges and coils. Definitive surgery was then carried out using a skin flap and mucosal flap to successfully reconstruct the defect. Postoperative radiotherapy was deemed unnecessary. The patient recovered well, with a satisfactory aesthetic outcome. No recurrence was observed during a 3-year follow-up period.

TUMOR-ASSOCIATED PROTEINS IN RAT SUBMANDIBULAR GLAND INDUCED BY DMBA AND IRRADIATION (DMBA 매식과 방사선 조사로 유발된 백서 악하선 암에 존재하는 단백질에 관한 연구)

  • This study was performed in order to identify changes of the plasma membrane proteins in rat submandibular gland tumors induced by 7,12-dimethylbenz[a]anthracene [DMBA] and X-irradiation. Two kinds of tumor associated membrane proteins (protein A and B) were isolated with 3 M KCl extraction from rat submandibular gland tumors induced by DMBA and X-irradiation. To identify their antigenicities, immunoelectrophoresis and double immunodiffusion was carried out with various proteins extracted from liver, heart, skin and pancreas of adult rats and from embryonic liver, heart and skin. The rabbit antisera against the protein A did not cross-react with any of the proteins extracted from the above mentioned tissues, suggesting that protein A might be tumor specific antigen. However, the rabbit antisera against protein B was precipitated with proteins extracted from the liver of adult and embryonic rats. Polyacrylamide gel electrophoresis of these two proteins (A and B) showed that protein A was a dimer with molecular weights of 69,000 and 35,000 dalton, whereas protein B was a monomer with molecular weight of 50,000 dalton.
