외상성 심실중격결손증 1례 보 (Traumatic ventricular septal defect - One case report -)

  • We have experienced a rare case of traumatic ventricular septal defect caused by fall down injury. The patient suffered from congestive heart failure after trauma and represented new developing typical murmur. Cardiac catheterization revealed a large left-to-right shunt at the ventricular level and a 4.4:1 ratio of pulmonary to systemic flow. Left ventricular angiogram also showed ventricular septal defect low in the muscular septum. At operation, the right ventricle and pulmonary artery were found to be moderately dilated. A marked systolic thrill was palpable over the right ventricle near the apex. Through a small left ventriculotomy an oval defect, 2x.3 cm with a firm scarred margin, was found in the lower muscular ventricular septum. This defect was repaired using Dacron patch and ventriculotomy was closed with long strip of Teflon felt. Postoperative course was uneventful and discharged in good condition.

  • Infantile hepatic hemangioma, the most common vascular tumor of the liver in infancy, can occur with acute postnatal liver and congestive heart failure. Nevertheless, its course is often benign, and many children can be diagnosed and treated without surgical intervention. The distinction from malignant diseases is not always easy and it not clear whether invasive procedures for diagnosis and therapy should be performed. Here we report our experiences in our Center for Pediatric Liver Disease and postulate that large studies are needed to avoid unnecessary invasive procedures for these patients in the future.

  • 12년 전 23mm 기계판막과 소심낭편으로 작은 대동맥륜과 심실중격 및 우심실유출로를 확장하는 Konno씨 수술을 받은 28세된 남자가 갑작스런 심부전증으로 전원되었다. 대동맥판막 상부와 하부의 대동맥과 심실 중격을 복원한 소심낭편에 천공들이 있었는데, 그 심낭편은 심하게 석회화 되고 변성되어 있었다. 그 천공들은 석회화 되고 변성되어 딱딱해진 소심낭편과 기계판막 자체의 유동성에서 생긴 균열에 의한 것으로 보였다. PTFE 이식편을 이용하여 Konno씨 수술을 다시 시행하였다.

이차공형 심방중격 결손중 (Secundum Type Atrial Septal Defect)

  • A series of 132 consecutive cases of secundum type atrial septal defect operated from 1963 to 1980 in the department of Thoracic Surgery of Seoul National University Hospital is analysed. The ages of the patients ranged from 12 months to 57 years and the ratio of men to women was 1 to 1.4. Clinical analysis revealed 33 patients [26%] were asymptomatic and 15 patients [12%]had congestive heart failure. Hemodynamic analysis revealed that severe pulmonary hypertension which is the most serious risk factor is ASD developed in 10 patients [9%] and pulmonary artery pressure significantly increased after the age of 20 years. There were 10 patients with sinus venosus defect and 7 patients with partial anomalous pulmonary venous return. The operative mortality was 2.2%.

Loeffler`s endocarditis 에 합병한 승모판 폐쇄부전 치험 (Surgical treatment of Loeffler`s endocarditis associated mitral insufficiency)

  • This is a report of one case of Loffler`s eosinophilic endocarditis associated with mitral insufficiency and LV thrombi treated surgically at the department of Thoracic and Cardiovascular Surgery, Hanyang University Hospital. This patient was a 42 year old female and she has complains of dyspnea, palpitation, orthopnea and generalized edema. Above symptoms has been going for 4 months and NYHA classification was IV. On examination, blood eosinophil was 45 to 50% [WBC-9800 ]. MI and LV thrombi were confirmed by LV ventriculography and echocardiography. Pulmonary congestion and congestive cardiac failure were diagnosis by X-Ray examination, EKG finding and clinical feature and others there were no organic functional disturbance. Mitral valve replacement was performed with Ionescu-Shiley pericardial valve [29mm] replacement. Adjust thumb sized grayish brown colored two thrombi were excluded, lodged in the apex and septal endocardium of LV. Endocardial fibrosis was reliably confirmed under the gross pathology in the heart. The patient had smooth postoperative course and there were no operative complication.

Air pollution and hospital admissions for chronic obstructive pulmonary disease: are their potentially sensitive groups?

  • Recent studies showed that air pollution is a risk factor for hospitalization for chronic obstructive pulmonary disease (COPD). However, there is limited evidence to suggest which subpopulations are at higher risk from air pollution. This study was undertaken to examine the modifying effect of specific secondary diagnosis (including hypertension, diabetes, pneumonia, congestive heart failure) on the relationship between hospital admissions for COPD and ambient air pollutants concentrations. Hospital admissions for COPD and ambient air pollution data for Taipei were obtained for the period from 1999-2009. The relative risk of hospital admissions for COPD was estimated using a case-crossover approach. None of the secondary diagnosis we examined showed much evidence of effect modification.

Reversal of Doxorubicin-induced Cardiotoxicity by Using Phytotherapy: A Review

  • Doxorubicin as a chemotherapeutic drug is widely used for the treatment of patients with cancer. However, clinical use of this drug is hampered by its cardiotoxicity, which is manifested as electrocardiographic abnormalities, arrhythmias, irreversible degenerative cardiomyopathy and congestive heart failure. The precise mechanisms underlying the cardiotoxicity of doxorubicin are not clear, but impairment of calcium homeostasis, generation of iron complexes, production of oxygen radicals, mitochondrial dysfunction and cell membrane damage have been suggested as potential etiologic factors. Compounds that can neutralize the toxic effect of doxorubicin on cardiac cells without reducing the drug's antitumor activity are needed. In recent years, numerous studies have shown that herbal medicines and bioactive phytochemicals can serve as effective add-on therapies to reduce the cardiotoxic effects of doxorubicin. This review describes different phytochemicals and herbal products that have been shown to counterbalance doxorubicin-induced cardiotoxicity.

심장점액종의 외과적 치료 (Clinical Experience of Atrial Myxoma)

  • Seven patients [six cases of left atrial myxoma, one case of right atrial myxoma] from 24 to 66 years of age [4 male and 3 female, mean age 48 years] underwent excision of atrial myxoma between 1982 and 1989 at Keimyung University Dongsan Medical Center. All patients presented with congestive heart failure, six with cardiac murmur, three with syncope, two with sinus tachycardia, one each with sinus arrhythmia, atrial fibrillation, pleural effusion, peripheral embolization. Symptoms were present from 1 month to 8 years before operation [mean 28 months], All tumors originated from atrial septum and pedunculated. The myxomas were successfully removed in all patients, either shaving them from atrial septum [n=3] or by excising a portion at normal atrial septum with tumor [n=4]. One case was replaced mitral valve with carbomedics-31mm due to severe mitral regurgitation. Follow up is current. No recurrent myxoma has been identified clinically or by echocardiography. In this series, excellent results were obtained by simple excision of the tumor, with or without a margin of normal atrial septum.

Mosquito 겸자를 이용한 Brock 술식의 단기 성적 -중증 폐동맥유출로차단을 동반한 신생아 4예- (Surgical Results of Brock Operation with a Mosquito Clamp in Neonates)

  • Four neonates with critical pulmonary outflow obstruction underwent emergency palliative operation between February 1988 and May 1989 at the department of Thoracic and Cardiovascular Surgery, Kyungpook National University Hospital. There were three boys and one girl, the mean age was 12.25 days [ranging from 3 days to 26 days], and the mean body weight was 3,625 gm [ranging from 3,450 gm to 4,200 gm]. Two patients had severe pulmonic valvular stenosis with intact ventricular septum, a third had pulmonary atresia with intact ventricular septum, and another had pulmonary atresia with ventricular septal defect. All were cyanotic, showed arterial desaturation with metabolic acidosis, and had congestive heart failure. To relieve the pulmonary outflow tract obstruction, we performed transventricular pulmonary valvulotomy [Brock operation] with a mosquito clamp in all cases without extracorporeal circulation. Three patients survived the operation and showed satisfactory postoperative results. The patient who had pulmonary atresia with ventricular septal defect expired 5 days after surgery. We consider transventricular pulmonary valvulotomy [Brock operation] with a mosquito clamp as one of the preferable procedures for critical pulmonary outflow obstruction in the neonatal period.

Sinus Valsalva Rupture 에 대한 외과적 치료 (Surgical Treatment of the Sinus Valsalva Aneurysm Rupture - 5 cases reviews -)

  • We experienced 6 patients with the sinus Valsalva aneurysm rupture during last 12 years [Jan. 1977-Sep. 1989]. Of them. 5 cases were reviewed. They consist of 3 males and 2 females, and the age ranged from 12 years to 40 years with the mean age of 25 years. 4 patients showed congestive heart failure symptoms. The diagnosis was made by 2D-Echo and cine-angiogram. In 4 patients. sinus Valsalva aneurysm ruptured from the Rt. coronary sinus to the Rt. ventricle, and in one from non-coronary sinus to the Rt. atrium. In 2 cases, resection of the aneurysm and simple stitch closure was made. Resection of the aneurysm k patch closure and AVR in one, closure of the fistula, AVR and patch closure of the associated VSD in one, and closure of the fistula, AVR k TVR in one were made in another 3 cases. There was no postoperative mortality case.
