A Study on the Activity and Training Plan of a Field Crew for the Design of Training Scenarios Assuming Chemical Accidents and Terrorism (화학사고·테러를 가정한 훈련 시나리오 설계를 위한 현장 대원의 활동성 분석과 훈련방안에 관한 연구)

  • This article is a study on the activity of rescue workers for designing simulation training scenarios assuming chemical accidents. On the basis of the complexity of the indoor scene in the case of chemical accidents and terrorism, we designed a 12-step simulation training scenario for two teams to analyze the improvement in firefighters' capabilities. On the basis of activity measurement in the simulation scenario, step 2 of training had the most drops in the maximum heart rate, as follow: N1, from 163 bpm to 153 bpm; N2, from 186 bpm to 151 bpm; N3, from 168 bpm to 162 bpm; and N4, from 166 bpm to 152 bpm. In terms of intensity level in the allowable activity time, it was found that in step 2 both N1 and N2 reduced from Level 5 to Level 3, N3 remained at Level 4, N4 reduced from Level 4 to Level 3, and the maximum allowable activity time increased.

The Effect of Moxibustion at Jeonjung($CV_{17}$) on the Heart Rate Variability in Healthy Adults (膻中穴($CV_{17}$) 간접구요법이 정상 성인의 심박변이도(HRV)에 미치는 영향)

  • Objectives : This study was accomplished to investigate the effects of moxibustion at Jeonjung($CV_{17}$) on the heart rate variability(HRV) in healthy adults. And based on that, we tried to find out how moxibustion at Jeonjung($CV_{17}$) effects on the activity and balance of the autonomic nervous system Methods : We investigated on 50 healthy volunteers consisted of 24 subjects in moxibustion group and 24 subjects in control group. On the other hand, two volunteers were ruled out by standard of elimination. The study was established by randomized trial. Moxibustion was applied at Jeonjung($CV_{17}$) and rested during 10 minutes in moxibustion group. No treatment was executed in control group. We measured HRV 3 times : baseline measurement and after treatment in moxibustion group(after rest in control group) twice. The SPSS 12.0 for windows was used to analyze the data by paired t-test, Wilcoxon signed ranks test(in group) and Independent t-test, Mann-Whitney U-test (between the groups). Results : 1. In moxibustion group, mean HRT decreased significantly. Ln(VLF) increased significantly. Ln(TP) decreased significantly at $2^{nd}$ time and increased significantly at $3^{rd}$ time. SDNN, RMSSD, PSI, Ln(LF), Ln(HF), Normalized LF, Normalized HF and LF/HF ratio were not affected. 2. In control group, PSI and normalized HF decreased significantly. SDNN, Ln(TP), Ln(VLF), normalized LF and LF/HF ratio increased significantly. Mean HRT, RMSSD, Ln(LF) and Ln(HF) were not affected. Conclusions : This study suggest that moxibustion at Jeonjung($CV_{17}$) makes no effects on the balance of the autonomic nervous system.

Effects of Topical Anesthetic Cream on Pain at Venipuncture in Children (정맥 천자 시 국소마취크림 도포가 아동의 통증에 미치는 효과)

  • Purpose: This study was done to evaluate the effectiveness of EMLA cream on pain related to venipuncture among children. Methods: In this study, 48 children were evaluated using a sequential measurement for level of pain by Skin Conductance Level (SCL) based on Galvanic Skin Response (GSR), heart rate, and the Visual Analogue Scale (VAS) at four times. Results: The maximum and mean of the SCL were each significantly different between the experimental and control groups and furthermore, the two were also significantly different among observed times. In addition there was a significant interaction between group and time. The children's perceived pain using VAS was not significantly different between the experimental and control groups. There was no significant difference in the heart rate between the experimental and control groups; however, the interaction between group and time was significant. Conclusion: In conclusion, applying topical anesthetic cream to the venipuncture site to reduce pain was effective among the children and therefore it is highly recommended that topical anesthetic cream be applied at the venipuncture site as a nursing intervention to reduce pain when a child has to undergo a venipuncture.

The SPSS 15.0 for Windows was used to analyze the data by paired t-test, Wilcoxon signed ranks test (in group) and One-Way ANOVA, Kruskal-Wallis test (between the groups). Results: 1. In acupuncture group, Mean HRT, PSI, Normalized HF decreased significantly. SDNN, Ln (TP), Ln (LF), Ln (VLF), Normalized LF and LF/HF ratio increased significantly. RMSSD, Ln(HF) were not affected. 2. In sham acupuncture group, all indicators were not affected. 3. In control group, mean HRT, Ln(TP) decreased significantly. SDNN, RMSSD, PSI, Ln(LF), Ln(HF), Ln(VLF), normalized LF, normalized HF and LF/HF ratio were not affected. Conclusions : We suggest that acupuncture at Jeonjung($CV_{17}$) increases the activity of autonomic nervous system and makes effects on the balance of the autonomic nervous system.

A Study on the Influence of the Navigator's Personal Characteristics on the Perceived Collision Risk in Close-quarter Situations (선박 근접상황에서 항해사의 인적특성요인이 지각한 충돌위험도에 미치는 영향에 관한 연구)

  • This study focuses on the margin of human error when a navigator is embarrassed by the psychological fear of collision in a close-quarter situation (CQS) and is unable to perform as per the prescribed collision avoidance measures. The purpose of the study is to identify the effects of the navigator's personal characteristics or factors in relation to on-board career (OC), license rating (LR), and age on the perceived collision risk (PCR) in CQSs. In order to obtain quantified data regarding the collision risk perceived by the navigator in four typical CQSs between their own ship and a target ship, this study measured and collated the heart rate variability of 30 navigators on their own ship when two ships approached each other at a speed of 10 knots from 2.5 nautical miles to a collision situation. According to a multiple regression analysis of the measured values, the navigators' OC and LR factors had negative effects on the PCR, while the age factor had no significant effect on PCR. The t-test results showed that the PCR value was significantly higher for navigators with an OC ≤ 4 years than for those with an OC ≥ 5 years, and the LR factor was significantly higher for a class 4~6 group than for a class 2~3. This finding may be applied to the development of collision risk warning systems, particularly for navigators.

Development of Time Varying Kalman Smoother for Extracting Fetal ECG using Independent Component Analysis : Preliminary Study (독립요소분석을 이용한 태아심전도 추출을 위한 시변 칼만 평활기의 개발 : 예비연구)

  • Fetal heart rate monitoring is important information to assess fetal well-being. Non-invasive fetal ECG (electrocardiography) can be derived from maternal abdominal signal. And various promising signal processing methods have been introduced to extract fetal ECG from mother's composite abdominal signal. However, non-invasive fetal ECG monitoring still has not been widely used in clinical practice due to insufficient reliable measurement and difficulty of signal processing. In application of signal processing method to extract fetal ECG, it might be lower signal to noise ratio due to time varying white Gaussian noise. In this paper, time varying Kalman smoother is proposed to remove white noise in fetal ECG and its feasibility is confirmed. Wiener process was set as Kalman system model and covariance matrix was modified according to white Gaussian noise level. Modified error covariance matrix changed Kalman gain and degree of smoothness. Optimal covariance matrix according to various amplitude in Gaussian white noise was extracted by 5 channel fetal ECG model, and feasibility of proposed method could be confirmed.

  • We propose a system that automatically recognizes the tense or relaxed condition of scrolling-shooting game subject that plays. Existing study compares the changed values of source of stimulation to the player by suggesting the source, and thus involves limitation in automatic classification. This study applies SOM of unsupervised learning for automatic classification and recognition of player's condition change. Application of SOM for automatic recognition of tense and relaxed condition is composed of two steps. First, ECG measurement and analysis, is to extract characteristic vector through HRV analysis by measuring ECG after having the player play the game. Secondly, SOM learning and recognition, is to classify and recognize the tense and relaxed conditions of player through SOM learning of the input vectors of heart beat signals that the characteristic extracted. Experiment results are divided into three groups. The first is HRV frequency change and the second the SOM learning results of heart beat signal. The third is the analysis of match rate to identify SOM learning performance. As a result of matching the LF/HF ratio of HRV frequency analysis to the distance of winner neuron of SOM based on 1.5, a match rate of 72% performance in average was shown.

The Effect of Venesection with Cupping Therapy at Jeonjung($CV_{17}$) on the Heart Rate Variability in Healthy Adults (전중혈(膻中穴)($CV_{17}$) 습부항요법이 정상 성인의 심박변이도(HRV)에 미치는 영향)

Methods : We investigated on 60 healthy volunteers consisted of 30 subjects in venesection with cupping therapy group and 30 subjects in control group. The study was established by randomized trial. venesection with cupping therapy was applied at Jeonjung($CV_{17}$) for 10 minutes in venesection with cupping therapy group. No treatment was executed in control group. We measured HRV 3 times : First, baseline measurement and two more times after treatment in venesection with cupping therapy group(after rest in control group). The SPSS 12.0 for Windows was used to analyze the data by paired t-test(in both group), independent-test(between the groups) and Chi-square test(between the sexs). Results : 1. In venesection with cupping therapy group, SDNN, Ln(TP), Ln(LF), Normalized LF, and LF/HF ratio increased significantly. Normalized HF decreased significantly. Mean HRT, RMSSD, PSI, Ln(HF), and Ln(VLF) were not affected. 2. In control group, Ln(HF) decreased significantly. Mean HRT, SDNN, RMSSD, PSI, Ln(TP), Ln(LF), Ln(VLF), Normalized LF, Normalized HF, and LF/HF ratio were not affected. Conclusions : This study suggests that venesection with cupping therapy at Jeonjung($CV_{17}$) increases the activity of the autonomic nervous system and has effects on the balance of the autonomic nervous system.

The Effects of Cervi Pantotrichum Cornu Pharmacopuncture and Bovis calculus.Fel Ursi Pharmacopuncture on the Heart Rate Variability (녹용약침과 웅담.우황(BU)약침이 정상인의 심박변이도(HRV)에 미치는 영향)

  • Objectives : This study was to investigate the effects of Cervi Pantotrichum Cornu Pharmacopuncture and Bovis calculus Fel Ursi Pharmacopuncture on autonomic nervous system with Heart Rate Variability(HRV). Purpose of the trial was to observe what influence Cervi Pantotrichum Cornu Pharmacopuncture and Bovis calculus Fel Ursi pharmacopuncture have on the autonomic nervous system. Methods : 40 healthy male volunteers were divided into two experimental groups, such as Cervi Pantotrichum Cornu Pharmacopuncture group and Bovis calculus Fel Ursi Pharmacopuncture group. Study design was a randomized, double-blind clinical trial. 20 subjects in one experimental group were injected Cervi Pantotrichum Cornu Pharmacopuncture, 20 subjects in other experimental group were injected hydrolyzed Bovis calculus Fel Ursi Pharmacopuncture at GB21(Kyonjong). We measured HRV 7 times : baseline measurement and every 5 minutes for 30 minutes after injection. The SPSS 15.0 for Windows was used to analyze the data by the paired t-test(in group) and independent sample t-test(between the groups). Results A. Time Domain Analysis 1. Analysis of Mean HRV, SDNN and Complexity After injection of Cervi Pantotrichum Cornu Pharmacopuncture, Mean HRV was significantly decreased all the times after injection. SDNN was significantly increased at immediate and 25 minute after injection. Complexity was significantly decreased only at 15 minute after injection. After injection of Bovis calculus Fel Ursi Pharmacopuncture, Mean HRV was significantly decreased all the times after injection. Complexity was significantly decreased at 15 minutes after injection. 2. Analysis of HRV index, pNN50 After injection of Cervi Pantotrichum Cornu Pharmacopuncture, HRV index was significantly increased immediately after injection. pNN50 was significantly decreased at immediately, 20 and 25 minutes after injection. After injection of Bovis calculus Fel Ursi Pharmacopuncture, HRV index was significantly increased immediately after injection. pNN50 was significantly decreased at immediate and 25 minutes after injection. B. Frequency Domain Analysis 1. Analysis of Ln(TP), Ln(HF), Ln(LF), Ln(VLF) After injection of Cervi Pantotrichum Cornu Pharmacopuncture, Ln(TP) was significantly increased at immediate and 25 after injection. Ln(HF) was significantly increased all the times after injection. Ln(LF) was significantly increased at 15, 20 and 25 minutes after injection. Ln(VLF) was significantly increased at immediate and 25 minutes after injection. After injection of Bovis calculus Fel Ursi Pharmacopuncture, Ln(TP) was significantly increased at immediately and 25 after injection. Ln(LF) was significantly increased at 20 and 25 minutes after injection. Ln(VLF) was significantly increased at immediate and 25 minutes after injection. Conclusions : We results suggest that Cervi Pantotrichum Cornu Pharmacopuncture and Bovis calculus Fel Ursi Pharmacopuncture in healthy adult men tend to activate the autonomic nervous system and sympathetic nervous system within normal range.

Analytical Evaluation of PPG Blood Glucose Monitoring System - researcher clinical trial (PPG 혈당 모니터링 시스템의 분석적 평가 - 연구자 임상)

  • This study is a performance evaluation of a blood sugar monitoring system that combines a PPG sensor, which is an evaluation device for blood glucose monitoring, and a DNN algorithm when monitoring capillary blood glucose. The study is a researcher-led clinical trial conducted on participants from September 2023 to November 2023. PPG-BGMS compared predicted blood sugar levels for evaluation using 1-minute heart rate and heart rate variability information and the DNN prediction algorithm with capillary blood glucose levels measured with a blood glucose meter of the standard personal blood sugar management system. Of the 100 participants, 50 had type 2 diabetes (T2DM), and the average age was 67 years (range, 28 to 89 years). It was found that 100% of the predicted blood sugar level of PPG-BGMS was distributed in the A+B area of the Clarke error grid and Parker(Consensus) error grid. The MARD value of PPG-BGMS predicted blood glucose is 5.3 ± 4.0%. Consequentially, the non-blood-based PPG-BGMS was found to be non-inferior to the instantaneous blood sugar level of the clinical standard blood-based personal blood glucose measurement system.
