Familial concordance of vascular risk factors is well understood and there are many studies about inter-spouse concordance of the risk factors for coronary heart disease. However, there are no studies for inter-spousal relation of the risk factors in cerebrovascular disease patients. We analysed inter-spouse correlation of cerebrovascular risk factors in patients admitted our hospital due to stroke and their spouses. There was statistically significant inter-spouse concordance in the presence of hypertension(p=0.025) and carotid IMT(r=0.479, p=0.001). However, the further age-adjusted analysis revealed no significant result. Differently from the results of previous studie,, the present study shows no significant spousal concordance. Althought there are tendencies of spousal concordance in some risk factors, the age is major determinant.

Background: We evaluated the efficacy of Cox-maze IV procedure using bipolar irrigated radiofrequency ablation and cryothermy in chronic atrial fibrillation associated with valvular heart disease. Material and Methods: From November 2005 to June 2009, ninety four patients have undergone valvular heart surgery with Cox-maze IV procedure. Preoperative duration of atrial fibrillation was $7.6{\pm}6.5$ years and follow-up duration was $22.7{\pm}12.3$ months. Results: There were two (2.1%) postoperative deaths not related to maze procedure. Two cerebrovascular accidents, five low cardiac output syndromes and two permanent pacemaker implantations have occurred after surgery. Preoperative ejection fraction on echocardiography was $55.3{\pm}8.1%$ and ejection fraction of postoperative six month was $54.7{\pm}6.5%$. Left atrial size of preoperative and postoperative were $61.5{\pm}11.6\;mm$ and $53.1{\pm}8.4\;mm$ at each. Freedom from atrial fibrillation rate at postoperative six-month was 80.7% and the cases of recurrence of atrial fibrillation after six months were three (3.3%). Risk factors for failure or recurrence of maze procedure were old age (p=.010) and preoperative moderate or severe tricuspid regurgitation (p=.033). Conclusion: The Cox-maze IV procedure using RFBP2 and cryothermy is quite safe and freedom from atrial fibrillation at postoperative 6 month was 82.5%. Risk factors for failure or recurrence of atrial fibrillation after Cox-maze IV were old age and preoperative over moderate tricuspid regurgitation.
Background: This study investigated mortality and morbidity in patients requiring postcardiotomy extracorporeal membrane oxygenation (ECMO) support after operations for congenital heart disease (CHD). Methods: CHD patients requiring postoperative ECMO support between May 2011 and May 2021 were retrospectively reviewed. Patients were divided into non-survivors and survivors to hospital discharge. Survival outcomes and associations of various factors with in-hospital death were analyzed. Results: Fifty patients required postoperative ECMO support. Patients' median age and weight at the time of ECMO insertion were 1.85 months (interquartile range [IQR], 0.23-14.5 months) and 3.84 kg (IQR, 3.08-7.88 kg), respectively. Twenty-nine patients (58%) were male. The median duration of ECMO support was 6 days (IQR, 3-12 days). Twenty-nine patients (58%) died on ECMO support or after ECMO weaning, and 21 (42%) survived to hospital discharge. Postoperative complications included renal failure (n=33, 66%), bleeding (n=11, 22%), and sepsis (n=15, 30%). Prolonged ECMO support (p=0.017), renal failure (p=0.005), continuous renal replacement therapy (CRRT) application (p=0.001), sepsis (p=0.012), bleeding (p=0.032), and high serum lactate (p=0.002) and total bilirubin (p=0.017) levels during ECMO support were associated with higher mortality risk in a univariate analysis. A multivariable analysis identified CRRT application (p=0.013) and a high serum total bilirubin level (p=0.001) as independent risk factors for death. Conclusion: Postcardiotomy ECMO should be considered as an important therapeutic modality for patients unresponsive to conventional management. ECMO implementation strategies and management in appropriate patients without severe complications, particularly renal failure and/or liver failure, are crucial for achieving positive outcomes.

A dramatic rise in the number of people dying from heart disease has prompted efforts to find a way to identify it sooner using efficient approaches. A variety of variables contribute to the condition and even hereditary factors. The current estimate approaches use an automated diagnostic system that fails to attain a high level of accuracy because it includes irrelevant dataset information. This paper presents an effective neural network with convolutional layers for classifying clinical data that is highly class-imbalanced. Traditional approaches rely on massive amounts of data rather than precise predictions. Data must be picked carefully in order to achieve an earlier prediction process. It's a setback for analysis if the data obtained is just partially complete. However, feature extraction is a major challenge in classification and prediction since increased data increases the training time of traditional machine learning classifiers. The work integrates the CNN-MDRP classifier (convolutional neural network (CNN)-based efficient multimodal disease risk prediction with TANFIS (tuned adaptive neuro-fuzzy inference system) for earlier accurate prediction. Perform data cleaning by transforming partial data to informative data from the dataset in this project. The recommended TANFIS tuning parameters are then improved using a Laplace Gaussian mutation-based grasshopper and moth flame optimization approach (LGM2G). The proposed approach yields a prediction accuracy of 98.40 percent when compared to current algorithms.

To evaluate the ischemic heart disease risk factors and analysis on the its relationship between risk factors and ischemic heart disease on EKG findings in a rural area, We conducted cross-sectional health screening test for 1304 persons aged over 30 years. Blood pressure, total cholesterol, HDL cholesterol, fasting blood glucose, BMI and Waist/hip ratio, smoking data and EKG data were collected. Hypertension was classified by the sixth report of the Joint National Committee on Detection, Evaluation, and Treatment of High Blood Pressure and the cutpoints of hyperlipidemia was used National Cholesterol Education Program. The cutpoint of hyperglycemia was used National Diabetes Data Group and those for obese was 30%. The results obtained were as follows; 1. Prevalence of definitive hypertension was 41.2% in males and 41.6% in females. The prevalence of hypertension showed Increasing tendency according to age increase(p<0.05). 2. Prevalence of hyperlipidemia was 20.6% in males and 20.4% in female. In females prevalence of hyperlipidemia showed increasing tendency according to age increase(p<0.001). 3. Prevalence of obese was 23.4% in males and 28.8% in females. Upper body type was predominant in females, but lower body type was predominant in males. 4. Prevalence of hyperglycemia was 11.0% in males and 12.1% in females. The prevalence of hyperglycemia showed increasing tendency according to age increase(p<0.01). 5. The smoking rate was 63.7% in males and 2.6% in females. 6. On the EKG findings, the prevalence of myocardial ischemia and myocardial infarction was 6.7% in males and 7.5% in females. 7. The prevalence of Ischemic heart disease was higher in hypertension than normal in females and higher in obese than normal in males. In males and females the prevalence of ischemic heart disease was higher in hyperglycemia than normal and higher in upper body type than lower body type. But there is no statistical association among them.
Purpose: This study aimed to apply a health partnership program using commercially available mobile health apps to improve cardiovascular risk factors in male employees and verify its effectiveness. Methods: Using a randomized control group pretest-posttest design, male employees with cardiovascular risk factors from five small and medium-sized workplaces were randomly assigned to an experimental group (n = 32) and a control group (n = 31). The experimental group was encouraged to use three mobile health apps for 12 weeks to acquire the necessary cardiovascular disease-related information and practice strengthening training, walking, and diet management appropriate to their level. They also received feedback on their weekly activities and motivational text messages from health partners. Hypotheses were tested using the SPSS WIN 22.0. Results: The experimental group showed a significant difference compared to the control group in terms of their perception of mobile health app (p < .05), self-efficacy for exercise and diet, self-management partnership, and cardiovascular disease prevention health behavior (p < .001). In particular, there were significant decreases in the body mass index, ratio, serum fasting blood sugar, total cholesterol, and triglyceride in the experimental group (p < .001); however, there was no significant difference in high-density lipoprotein-cholesterol. Conclusion: Intervention using mobile apps based on partnership with health managers is effective in improving the objective cardiovascular risk index in male employees; therefore, such intervention should be continuously used as a useful lifestyle modification strategy in the workplace.
Objective : Stroke is a serious disease despite of the recent improvement in medical treatment. Hence, identification of modifiable risk factors for stroke is important. This case-control study was done to demonstrate that relation between risk factors(familial history of stroke, hypertension, diabetes mellitus and heart disease), lifestyle factors(alcohol drinking, psychologic stress, exercise, liking salty and hot foods) and the incidence of stroke. Methods : We interviewed 214 stroke patients(116 in men, 98 in women) as case group and 205 non-stroke patients(118 in men, 87 in women) as control group from January 1, 2001 to July 31, 2001 in oriental hospital of Kyung Hee medical center. We investigated risk factors, lifestyle factors of stroke in all patients and stroke-subtype of case group. Stroke-subtype consists of cerebral infarction and cerebral hemorrhage. Results : The percentage of familial history in the case group was significantly higher than that of control group, especially in the patients of cerebral infarction. The most ordinary preceding disease was hypertension. And the preceding disease of hypertension, diabetes mellitus was associated positively with the risk of stroke. The percentage of exercise was significantly higher than that of control group. The percentage of alcohol drinking, psychologic stress within 1 week and liking salty and hot foods was higher in the case group. However, there was no significant difference between case and control group. Conclusion : According to the above results, we found that familial history of stroke, preceding disease(hypertension, diabetes mellitus) affected to the incidence of stroke and exercise might be predisposing risk factors for stroke.
We prospectively studied postoperative cardiac arrhythmia after open heart surgery to analyze the types and incidence of cardiac arrhythmia and to predict preoperative risk factors. And also we evaluated the effectiveness of atrial and ventricular epicardial electrodes which were placed during operation Between March 1990 and August 1990, We had operated on in 211 patients and we studied 201 consecutive patients excluding 10 patients. The study group included 99 males and 102 female patients, ages 1 month to 75 years[Mean$\pm$SD=28.0$\pm$21.7 years]. Postoperatively, all patients were regularly seen by the cardiac surgeon and cardiologist, They had continuous electrocardiographic monitoring for the first 3 days, initially in the intensive care unit and were checked routine electrocardiography on the postoperative 7 days, The postoperative cardiac arrhythmia were analyzed and possible associations of this arrhythmia with various pre, intra, and postoperative factors were studied by univariate and multivariate discriminant analysis, The overall incidence of postoperative cardiac arrhythmia except relative sinus bradycardia was 36.8%;[74/201], The incidence of postoperative cardiac arrhythmia in acyanotic congenital heart disease: 19.4%, cyanotic congenital heart disease: 20.8%, cardiac arrhythmia surgery: 33.3%, acquired valvular heart disease: 60.9% and coronary artery occlusive disease: 38.9%. Both univariate and multivariate studies indicated the pre operative symptom duration[p = 0013], the duration of medication[p=0.003], presence of preoperative arrhythmia[p<0.001] and pre-operative left atrial dimension in echocardiography to be the factor promoting postoperative cardiac arrhythmia. Multivariate discriminant analysis showed that the presence of preoperative cardiac arrhythmia, bypass time and the duration of preoperative symptom duration conveyed considerable risk factor on post-operative arrhythmia. The atrial wire electrodes were used diagnostically in 36 and were used therapeutically in 89 among 201 patients. Atrial pacing were used to treat relative sinus bradycardia, accelerated junctional tachycardia or premature atrial or ventricular contractions in 51 patients. Atrioventricular sequential pacing were used in 16 patients and ventricular pacing were used in 20 patients. Hemodynamics were evaluated in 2 patients of relative sinus bradycardia before and after atrial pacing. The atrial pacing increased the amount of cardiac output to 15% more. Because of their great utility in the diagnosis and treatment of arrhythmias, we conclude that routine placement of atrial and ventricular electrodes at the time of operation is indicated regardless of the nature of the open-heart procedure.

The purpose of this study was to investigate factors affecting the heart rate, to analyze the relevance and to use it as a management index of factors of increase in heart rate. A total of 204 adults from November, 2016 to March, 2017 who responded to a personal questionnaire among the adults who underwent coronary computed tomography scans for health screening, were the target of the investigation. In the study, there was a statistically significant difference (p <.05) between the heart rate and the gender, CACS, Stenosis and the forces affecting cardiovascular disease. CACS, Stenosis had a statistically significant difference (p <.05) compared with the other groups in the group with a heart rate of 80 bpm or more, the ability to influence cardiovascular disease was different from that of other groups There was a meaning difference in the heart rate below 60 bpm group compared (p <.05). In conclusion, it is important and desirable to maintain heart rate below 60 bpm in order to prevent having cardiovascular disease history, and below 80 bpm to prevent CACS and stenosis.
Purpose: The purpose of this study was to develop a motivational enhancement therapy (MET) for coronary artery disease (CAD) patients in early stages of health behavior change and evaluate its effects on health motivation, the stages of change, health behaviors, and cardiovascular risk factors. Methods: Using a non-equivalent control pre-post design, the study was conducted on 42 CAD patients who underwent medical treatment or percutaneous coronary interventions in a hospital. The intervention group (n=21) received the MET (MET 1 during admission, MET 2 after discharge via telephone). The control group (n=21) received a standard care. Data were analyzed using descriptive statistics, ${\chi}^2-test$ and t-test with the SPSS 12.0 program. Results: Participants in the intervention group reported significantly increased scores of health motivation (t=-2.093, p=.043), the stages of change (t=-5.682, p<.001), and health behaviors (t=-3.069, p=.004) and significantly decreased scores of cardiovascular risk factors (t=2.131, p=.039) compared to those of the control group. Conclusion: The findings indicate that the MET is an effective intervention in improving health behaviors and decreasing cardiovascular risk factors for CAD patients.
