Hearing Rehabilitation with Combined Electroacoustic Stimulation and Ossiculoplasty

  • We report a case of hearing rehabilitation following combined cochlear implantation and ossiculoplasty. A 71-year-old patient visited the clinic for right-sided mixed hearing loss. We targeted neural and conductive components, performing two different operations simultaneously. At two months post-operative, the patient showed satisfactory results with respect to hearing threshold and speech comprehension. Our experience suggests that careful evaluation of patients and consideration of the diverse array of available treatment strategies can be used to provide personalized rehabilitation with maximal effectiveness. To the best of our knowledge, this is the first report to take such an integrated approach to treat hearing impairment and is thus likely to have clinical importance for otologists.

  • In this study, we present an efficient ambulatory speech audiometric system to detect one's hearing problems at an earlier stage as possible without his or her visit to the audiometric testing facility such in a hospital or a clinic. To estimate a person's hearing threshold level in terms of speech sound response in his or her local environment, a digital assistant(PDA) device is used to generate the speech sound with implementing audiometric Graphic User Interface(GUI) system. Furthermore, a supra-aural earphone is used to measure a subject's hearing threshold level in terms of speech sound by the compensating the transducer's gain by adopting speech sound calibration system.

Survey on the Use of MP3 Players of High School Students and the Effect to Their Hearing Thresholds (설문지를 통한 고등학생들의 MP3 사용 실태 및 청력에 미치는 영향)

  • This study surveyed MP3 player usage and evaluated its effect on hearing thresholds in high school students. It is reported an increase in risk factors for hearing loss in adolescents that paralleled the increase in sales of MP3 players. A total of 538 students in two high schools in Gwangju province participated in this study. The students completed questionnaires about their MP3 player use and their pure tone hearing thresholds were tested at several frequencies. The questionnaire results indicated that 61.1%, of the students listened to music on MP3 players for more than one hour per day, 71.6% listened for four or more days per week, and 36.7% had listened for more than 3 years. Listening at a high volume of 60% of the maximum level was reported by 51% and they recognized having ear pains. The pure tone hearing threshold was found to be raised for the adolescents who had a long history of MP3 listening at high volume (p< 0.0001).

  • DSP chip parameters of a digital hearing aid (HA) should be optimally selected or fitted for hearing impaired persons. The more precise parameter fitting guarantees the better compensation of the hearing loss (HL). Digital HAs adopt DSP chips for more precise fitting of various HL threshold curve patterns. A specific DSP chip such as Gennum GB3211 was designed and manufactured in order to match up to about 4.7 billion different possible HL cases with combination of 7 limited parameters. This paper deals with a digital HA fitting program which is developed for optimal fitting of GB3211 DSP chip parameters. The fitting program has completed features from audiogram input to DSP chip interface. The compensation effects of the microphone and the receiver are also included. The paper shows some application examples.

  • Objectives : The purpose of this study was clinical observation about sensorineural hearing loss compare with acupucture treatment and assessments of prognotic factors. Methods : This study was clinical observation about sensorineural hearing loss cases. for diagnosing in these cases, pure tone audiometry was used. The treatment, acupuncture was used. Results : Three patients(37.5%) remarkably improved, one patient(12.5%), slightly improved, four patients(50%), no change. Conclusions : These results suggest that the prognosis by age and initial hearing threshold was significant. The treatment on sensorineural hearing loss considered that bone condution improved.

The analysis of the relation between noise induced hearing loss and noise exposure (소음유발 청력손실과 소음폭로에 대한 연구)

  • In this paper, the analysis of the relation between noise induced hearing loss and noise exposure is described for the A-weighted noise levels and exposure duration. The hearing loss and sensitivity threshold shift is investigated by changing the various parameters such as the effects of aging and noise exposure. Total hearing loss is proportional to a function of exposure level based upon the integral of pressure with time. If the noise exposure term is large so that the presbycusis may be neglected, the hearing loss due to aging and noise exposure becomes the noise induced hearing loss. It is shown that exposure to excessive noise can cause temporary loss of hearing that may become permanent if the exposure is prolonged or intense. An audiogram taken from a person suffering from noise induced hearing loss will usually show the greatest loss of hearing sensitivity in the 4kHz region, which is typically the region most sensitive to damage resulting from many types of industrial noise.

  • Speech reception threshold is a base for word discrimination testing, but it also serves as a check for the reliability of pure tone audiogram. In order to investigate the correlation between SRT and PTA these tests were carried out in patients with conductive hearing loss and normal hearing, using Grason-Stadler 1702 Audiometer. The results were as follows; 1) The difference between the scores of SRT and PTA's was 2.4 dB with a range of -3.3 dB∼+8.3 dB in conductive hearing loss, and was 1.9 dB with a range of -6.7 dB∼+5 dB in normal hearing group. 2) The difference between the scores of SRT and each speech frequency of PTA was 6 dB at 500 Hz, 3 dB at 1,000 Hz and 8.8 dB at 2,000 Hz in conductive hearing loss, and 3 dB at 500Hz, 2 dB at 1,000Hz, and 5dB at 2,000Hz in normal hearing group.

  • The study was conducted from May to September in 1994 to investigate applicability of the Hearing Handicap Inventory for the Elderly-Screening version(HHIE-S) in parallel with the pure-tone audiometer to the initial screening test of noise-induced hearing loss(NIHL) in some noise-exposed workers. Subjects were selected by systemic sampling that took every 10th person from 6, 700 workers taking the annual occupational health examination by the department of Health Maintenance of Dongsan Hospital Keimyung University in Taegu. The authors administered the pure-tone audiometric test and self-reported questionnaire of HHIE-S including items of sociodemographic and job-related variables concurrently. The final subjects analysed were 1,019(488 males and 531 females) excluding fourteen persons who had many missing values in their questionnaires. The reliability coefficients of HHIE-S scale by Cronbach's alpha were 0.84. In the univariate analysis of hearing handicap measured by the HHIE-S, work duration, military service and the hearing threshold loss at 1KHz and 4KHz by the initial audiometer were significant in males while age, work duration and hearing threshold loss at 1KHz and 4KHz were significant in females. In the stepwise linear regression analysis, hearing threshold loss at 1KHz and 4KHz, was the only selected variable explaining the hearing handicap in males and hearing threshold loss at 1KHz and 4KHz, age, and work duration were selected in females. In ROC curves for HHIE-S scores against NIHL as gold standard which was defined by the follow-up audiogram as more than 30dB of the average of 0.5/1/2KHz and 50dB at 4KHz, the optimal cutoff for the parallel HHIE-S appeared to be 8. The results suggest that HHIE-S appeared to have some reliability and validity in this data and might be used in screening NIHL in parallel with pure-tone audiometer in noise-exposed workers.
