Familial Otosclerosis Associated with Osteogenesis Imperfecta: A Case Report

  • Otosclerosis, a hereditary disorder characterized by disordered resorption and deposition of bone, results in progressive hearing loss. Osteogenesis imperfecta (OI) is a genetic disorder characterized by recurrent fractures, blue sclera, and varying degrees of hearing impairment; and is a known risk factor for otosclerosis. After adolescence, the risk of fracture decreases, reducing the need for follow-up in OI. However, otosclerosis is a progressive disorder. In this report, we discuss two cases of familial otosclerosis with different clinical features. We hypothesize that the difference in hearing level correlates with the difference in computed tomography findings. The mother, whose case was considered severe, was prescribed hearing aids, while the daughter, who had normal hearing level, was regularly followed up.

턱관절 장애로 인한 청각장애의 치료: 증례보고 (Treatment of hearing loss due to temporomandibular joint disorders: Case Report)

  • Introduction : Patients with temporomandibular disorder may have various ear-related symptoms. If an excessive load is applied to the ear area due to the skeletal shape of the mandibular condyle or malposition of the disc, an auditory problems may occur. Case report : The patient was referred to our clinic due to the suspicion of temporomandibular disorder from the local otorhinolaryngology clinic. A few days ago, his right ear could not be heard. MRI showed that the left TMJ disc was anterior displacement with reduction, the right TMJ disc was anteromedial displacement without reduction. Also Right mandibular condyle showed sclerotic bone change, subchondral cyst and was compressing the frontal wall of the ear on MRI view. Right TMJ arthroplasty was done under the diagnosis of right TMJ osteoarthritis and osteochondroma. Postoperative intermaxillary fixation was done with SAS screw and elastics for 2 weeks. One month after the operation, hearing and TMJ discomfort were recovered without any complications. Conclusions As seen in this case, hearing loss due to benign tumor-like lesions of the temporomandibular joint should be treated surgically to restore the TMJ function and hearing.

구순구개열 환자의 이비인후과적 관리 (Otolaryngologic Management related with Cleft Lip & Palate)

  • Team approach for the management of cleft lip & palate patients is very important. Plastic surgeon, oral-maxillofacial surgeon, orthodontist, otolaryngologist, and speech therapist should be included in the team. Main role of the ENT surgeon may be variable and is up to the team characteristics. Main topics of ENT surgeons' interesting fields are evaluation and management of hearing impairment due to SOM, voice disorder, and velopharyngeal incompetency due to submucous cleft palate & still remained VPI after curative palatoplasty. Basic review of anatomy & physiology related with otolaryngologic aspect of velopharyngeal system was done. Diseases related with hyponasality as well as hypernasality were discussed. Diagnostic and therapeutic methods were discussed. Proper management of hearing impairment and speech disorders are important.

  • Objective : Oriental medical treatment may be possible or effective in patients with facial palsy and hearing disturbance caused by traumatic disorder Methods : The authors observed objectively improvement state of patient that treated by acupuncture, herbal medicine Results : 1. A Facial nerve travel long and narrow bone canal in temporal bone. so slow progressive palsy is caused by nerve swelling and impedimental blood circulation in bone canal, if bruise happens. 2. A patient with traumatic facial palsy, acupuncture, herbal medicine(Igigeopung-tang, Boicyangwitang etc.), electro acupuncture are effective to improving symptoms. 3. Despite acupuncture treatment is taken, hearing disturbance is not improved. 4. In general, everyone consider surgical operation first of all, in the case of having traumatic facial palsy. Though this case, the author are thought that can attempt access of Oriental medical treatment without doing surgical operation.

난청을 동반한 이명 환자의 임상적 특성 (Clinical Characteristics of Patients with Tinnitus Accompanied Hearing Loss)

  • Background : Tinnitus is common disorder with many possible causes and a symptom of many different diseases but has no effective treatment. Most of tinnitus patients experience the disorder to a degree that their quality of life and productivity are impaired. Especially tinnitus accompanied hearing loss causes severe discomfort in patients. Objectives : 1. Through symptoms of tinnitus, we tried to find out the relationship of Heo-sil(虛實) and Zuang fu bian zheng(臟腑辨證) among tinnitus patients accompanied Hearing Loss. 2. By testing THI, we tried to evaluate the quality of life of tinnitus patients group and tried to compare the quality of life between high-narrow tone and low-wide tone tinnitus patients group. 3. In the future, this study was planned to help monitor the prognosis and determine the progress of treatment for tinnitus patients who also have hearing loss. Methods : 43 patients were recruited in this study who was ill with tinnitus accompanied hearing loss. They wrote out the questionnaire about tinnitus and the Tinnitus Handicap Inventory(THI). Results : 1. In tinnitus patients, there were more men than women accompanied hearing loss. The rate of visits was higher as the age increased. 2. Most of the patients who visited the hospital had sensitive hearing loss. 3. In high-narrow tone tinnitus group, phlegm fire congesting-type are more uncomfortable than spleen-stomach weakness-type. 4. In low-wide tone tinnitus group, liver qi depression was the most uncomfortable. Conclusions : The present study suggests that tinnitus pattern may be helpful in differentiating patients with tinnitus, and the effectiveness of treatment can be predicted through differentiation. Zuang fu bian zheng(臟腑辨證) and Distinguishing between Heo(虛) and Sil(實) is expected to have positive effects on tinnitus accompanied hearing loss treatment.

A Case of Idiopathic Sequential Profound Bilateral Sudden Sensorineural Hearing Loss Occurring 37 Days After Unilateral Presentation

  • Sudden sensorineural hearing loss (SSNHL) is a common disorder; however, sequential, bilateral presentation of the disease is rarer than unilateral presentation. Clinical otologists usually focus on treating the side with impaired hearing when patients first present with unilateral SSNHL, and therefore, may not warn patients of the possibility of subsequent hearing impairment in the contralateral ear. Furthermore, it is professionally discouraging when a patient presents with profound, sequential SSNHL after initial treatment. This may adversely impact the doctor-patient relationship, even if the patient is offered the best possible care from their first visit. Herein, we report the case of a patient with profound, idiopathic, bilateral SSNHL with a time interval of 37 days between involvement of both ears. Even though high-dose steroids were administered intraorally and intratympanically, the patient's hearing was not restored, and the patient eventually required bilateral cochlear implant surgery. Our report demonstrates that sequential, profound, bilateral SSNHL may manifest without any specific signs.

청소년의 이어폰 및 음향기기 사용 실태와 문제점에 관한 연구 (Problems of Usage of Earphones, PCPs on Hearing in Adolescents)

  • Purpose: The purpose of this study was to investigate usage of earplones, PCPs, and to give the basic data for adolescent's hearing health education. The study was a descriptive research about three factors affecting hearing loss; the knowledge and attitude about noise, the perception of hearing loss. Method: For research purposes, we conducted a questionnaire survey of 1,480 high school students in Suwon, from 27 October 2007 to 31 2007. Result: Most of the subjets used PCPs for 3-5 years, the average time of using earphones per day was 1-3 hours, awake hearing acuity decrease after using is the hearing acuity became worse some, PCPs type is MP3, use place house, listening contents ballade, disorder condition experience after using if the ear it is sick. The average of concern about hearing scored the highest 3.01${\pm}$0.54. The average of perceived susceptibility scored 3.04${\pm}$0.63 and the average of knowledge about noise scored 2.76${\pm}$0.46. The average of total knowledge and attitude about noise scored 2.93${\pm}$0.52. The average of discomfort of hearing loss(2.20${\pm}$0.63) scored higher than that fear of hearing loss(1.66${\pm}$0.30). The average of total perception of hearing loss scored 2.12${\pm}$0.67. Conclusion: At least in this study are shown in the youth earphone, PCPs used as a spare because of the seriousness of the issue, so as a result of these problems related to hearing about the youth campaign, through advertising or seriousness of the young people need them. In addition, countries also need to create policies and the support for health education, especially during the regular curriculum subjects on subjects that include teenagers in the earphone, PCPs due to problems with spare on the contents of the reservoir. The severity of hearing loss young people, prevention of hearing to protect the youth.

애니메이션을 이용한 순음청력선별검사 도구의 효용성에 대한 연구 (Effectiveness of Computer-Animated Pure Tone Audiometry for Screening)

  • The purpose of this study was to develop a computer-animated pure tone audiometry for screening (CAPTAS) for toddler and to determine its validity and reliability. The CAPTAS utilizes an animated cartoon story producing visual and auditory stimuli. The intensities were 40 dB, 60 dB, 80 dB. The frequencies were 500 Hz, 1000 Hz, 2000 Hz, and 4000 Hz. The subjects were 20 (9 males and 11 females) severely hearing impaired children. As a result, The correlation coefficient between mean hearing threshold of children who were able to perform PTA and average hearing threshold of CAPTAS was performed and it revealed CAPTAS's high validity. And to verify the reliability of the re-test, all children had the CAPTAS and repeated it periodically. The result confirmed the reliability.
