Auditory Recognition of Digit-in-Noise under Unaided and Aided Conditions in Moderate and Severe Sensorineural Hearing Loss

  • Background and Objectives: The speech-in-noise test is typically performed using an audiometer. The results of the digit-in-noise recognition (DIN) test may be influenced by the flat frequency response of free-field audiometry and frequency of the hearing aid fit based on fitting rationale. This study aims to investigate the DIN test in unaided and aided conditions. Subjects and Methods: Thirty four adults with moderate and severe sensorineural hearing loss (SNHL) participated in the study. The signal-to-noise ratio (SNR) for 50% of the DIN test was obtained in the following two conditions: 1) the unaided condition, performed using an audiometer in a free field; and 2) aided condition, performed using a hearing aid with an unvented individual earmold that was fitted based on NAL-NL2. Results: There was a statistically significant elevation in the mean SNR for the severe SNHL group in both test conditions when compared with that of the moderate SNHL group. In both groups, the SNR for the aided condition was significantly lower than that of the unaided condition. Conclusions: Speech recognition in hearing-impaired patients can be realized by fitting hearing aids based on evidence-based fitting rationale rather than by measuring it using free-field audiometry measurement that is utilized in a routine clinic setup.

  • Objectives : The purpose of this study is to show the clinical effect of Korean Medical Treatments for Tinnitus with Sudden Sensory Neural Hearing Loss. Methods : The patient was treated by Korean Medical Treatments for 10weeks. The effect of treatments on tinnitus was measured with Visual Analogue Scale(VAS) and Korean Tinnitus Handicap Inventory(K-THI). And the effect of treatments on sudden hearing loss was measured by VAS. Results : VAS of Tinnitus was decreased from 10 to 3 points and K-THI score was decreased from 70 to 26 points. VAS of hearing loss was decreased from 9 to 8 points. Conclusions : Korean Medical Treatments are effective on Tinnitus.

In this paper we describe a case of a non-alcoholic 28-year-old female status post-sleeve gastrectomy who developed SNHL in the setting of thiamine deficiency and WE, with ongoing hearing impairment requiring hearing aids despite thiamine repletion.

Nationwide Analysis of Mortality Rates and Related Surgical Procedures in Hearing Disability Patients in South Korea

  • Background and Objectives: Hearing loss (HL) and its repercussions are major problems in today's society. There are limited data on the relationship between degree of HL and otologic disorders. The aim of this study is to estimate mortality rates, rates of sudden idiopathic HL and related otologic surgical procedures in hearing disability patients in South Korea. Subjects and Methods: Retrospective medical data for 160,205 patients with hearing disability was extracted. Mortality rates, rates of sudden idiopathic HL and related otologic surgical procedures were compared with a normal control group consisting of 865,475 people; approximately 5 times the number of hearing disability patients. Results: According to the Korean National Disability Registry (NDR), 0.458% of the population in South Korea suffered from hearing disability in 2015. Higher rates of mortality and sudden idiopathic HL were reported in hearing disability patients, increasing up to a maximum of 1.594 times and 1,039.695 times, respectively, compared to the normal control group. Mastoidectomy surgery was 2.5 times more frequently performed and pressure equalizing (PE) tube insertion was about 15 times more frequently performed in hearing disability patients. Conclusions: Hearing disability is related to higher risks of mortality, sudden idiopathic HL and otologic surgical procedures, including mastoidectomy and PE tubing.

한방 복합 치료 및 소리 재활 치료로 호전된 노인성 난청 치험 1례 (Therapeutic Effects of Korean Medical Treatment Combined with Threshold Sound Conditioning on Presbycusis: A Case Report)

  • Objectives : The purpose of this study is to report the case of a presbycusis patient whose hearing had improved. Methods : A 81-year-old male diagnosed with presbycusis were treated with Korean medical interventions (acupuncture combined with electromagnetic stimulation and pharmacoacupuncture) and Threshold sound conditioning(TSC) for 3 months. Pure tone audiometry(PTA) was performed for the evaluation. Results : The patient's hearing was improved from moderate to mild level according to the Classification of Hearing Loss(International Organization for Standardization, 1969). The average hearing were improved from 40.5 dB to 32.7dB on the left side, 46.9dB to 34.1dB on the right side. Conclusions : We observed an improvement in a presbycusis patient's hearing and hereby suggest the possibility of Korean medical treatment and TSC's ameliorating effect on sensorineural hearing loss as well as prebyscusis.

  • As the results of investigation, the noise level was between 60 dB and 80 dB in the area of explosion. The residents living within 1 Km would feel uncomfortable. However, hearing loss is not happened by this range. The maximum range that the human can hear is 20,000 Hz and the maximum range that the dog can hear is between 20 Hz and 40,000 Hz. The auditory range for humans to be uncomfortable toward noise is between 1,000 Hz and 2,000 Hz. As the result of this experiment, the auditory range of dog is more wide than of a human. The change of hair cells in the Corti's organ occurred when the dog was exposed to 1,000 Hz at 100 dB for 1 month. Therefore, the structure change of the ear could happen by hearing loss because of noise, but the structure change of hair cells is the worst symptom by hearing loss because of noise.

  • Objective : The purpose of this study is to investigate the protective effect of DADS on the noise induced hearing loss Methods : Twenty healthy guinea pigs (DADS pretreated group and control) were exposed to 100dB a collected environmental noise for 9 hours. An auditory evoked brainstem response (ABR) threshold was measured before and after a noise exposure. For morphological study, cochlea was studied by scanning electron microscope (SEM). Results : The DADS pretreated group showed statistically significant less changes in ABR threshold and SEM findings compared the control group. Conclusion : From these results suggest that the DADS from garlic may have a protective effect in noise induced hearing loss.

  • Objectives : The purpose of this study is to report clinical effects of korean medical treatment based on Bal Mong Ja on a patient with sudden sensory hearing loss. Methods : In this study, we used acupuncture based on Bal Mong Ja on a patient with sudden sensory hearing loss. We also carried out treatments like herbal medication and moxibustion therapy etc. The progress of symptoms were evaluated by THI(Tinnitus Handicap Index), DHI(Dizziness Handicap Inventory), VAS(Visual Analogue Scale) once a week. Results : After 7 weeks, there were significant improvements of auditory acuity and THI, DHI, VAS. Conclusions : Acupuncture based on Bal Mong Ja is effective for relieving symptoms of sudden sensory hearing loss with tinnitus and dizziness.
