Institutional Change and Organizational Change: A Multicase Study on the Organizational Adaptation to the Introduction of Pharmacoeconomics (제도 환경 변화와 조직 변화 : 경제성 평가의 도입과 다국적 제약기업의 조직 적응에 대한 다중사례연구)

  • Background: Organizations in the pharmaceutical industry are highly dependent on the institutional environment. The introduction of pharmacoeconomics to the decision-making on the price and reimbursement decisions became strong constraints to pharmaceutical companies in Korea. As little is known about the issue on organization-environment interaction in the healthcare field, this study aimed to figure out how pharmaceutical companies adapted to the environmental changes. Methods: A multicase study method was used, selecting eight cases among multi-national pharmaceutical companies in South Korea. In-depth interviews were conducted with the managers of these organizations, and secondary data were reviewed to complement the interviews. Results: Pharmaceutical companies viewed the new policies as a big threat and sought for actions against them. One of the most distinguishing organizational changes was to construct a Market Access department. Other strategies managing the environment such as co-optation, forecasting, and bargaining were also implemented. These changes were consistent with the predictions of Resource Dependency Theory and Institutional Theory. Conclusions: The interactions between pharmaceutical companies and institutional environments in healthcare were first explored. This study presents a new perspective on how organizations change and the motives for the changes. The findings of this case study will form the basis of further empirical studies.

A Study on the Development Strategies of University Students in Sports Colleges by Evaluating Core Competencies (스포츠계열 대학생들의 핵심역량 평가에 따른 발전방안 연구)

  • The purpose of this study is to analyze the importance and performance of core competencies recognized by Korean sports students and to identify directions and methods for strengthening the competence of sports students in the future direction of future society. The results of analysis of 309 students of sports major students are as follows. First, the importance of core competencies recognized by college students in sports was in the order of self - management capacity, interpersonal capacity, communication capacity, comprehensive thinking ability, resource information technology utilization capability, and global capability. Second, the students' performance of their core competencies evaluated by sports college students was in the order of interpersonal competence, self-management competence, communication competence, comprehensive thinking ability, global competency, and resource information technology utilization competency. Third, the difference between the importance of core competence and the degree of performance was found to be significant in all areas, and the areas of great difference were total thinking, communication ability, resource information technology utilization capacity, self-management capability, Relationship competence. While college students in sports college have relatively high level of interpersonal competence, self-management competence and communication ability, comprehensive thinking ability, resource information technology utilization capacity and global competency are weak. Therefore, It will be able to be reborn as the talent required by society. Therefore, universities need to cultivate competitive sports talent through innovation of curriculum and capacity of faculty, and it should be recognized that this can be linked to the competitiveness of universities.

Reviews of the Shared Medical Appointments: Adopting Innovations in Care Delivery for Patients with Chronic Diseases (공유진찰제: 만성질환 관리를 위한 혁신적 의료서비스 전달방식)

  • Chronic diseases as well as a growing population of older adults are currently the leading cause of ill health and economic burden worldwide. Managing those diseases in one-on-one medical consultations poses substantial challenges due to limited time and resources in the current health care system. Various approaches have been taken to manage these conditions, most with limited success. Shared medical appointments (SMAs) are an innovative care delivery option to make the testing of alternative care modalities a prime concern. SMAs are individual medical consultations carried out in a group of patients with similar diseases by providing education, medication management, and disease monitoring. SMAs, since their initial conceptualization in 1998, have gained much popularity and adopted as one of the standard processes in many countries. Accumulated evidence-based studies show outcomes for increasing access to care, behavioral change facilitated through self-management education, maintained/better outcomes, physician productivity, and enhanced resource management. This review summarizes current evidence regarding the existing status of SMAs abroad. An extensive literature search was conducted on major electronic databases including PubMed and Google Scholar. This study suggests to explore and exploit the SMAs which have unique potential as a healthcare delivery innovation in Korea.

R-peak Detection Algorithm in Wireless Sensor Node for Ubiquitous Healthcare Application (유비쿼터스 헬스케어 시스템을 위한 노드기반의 R피크 검출 알고리즘)

  • The QRS complex in ECG analysis is possible to obtain much information that is helpful for diagnosing different types of cardiovascular disease. This paper presents the preprocessor method to detect R-peak, RR interval, and HRV in wireless sensor node. The derivative of the electrocardiogram is efficiency of preprocessing method for resource hungry wireless sensor node with low computation. We have implemented R-peak and RR interval detection application based on dECG for wireless sensor node. The sensor node only transfers meaning parameter of ECG. Thus, implementation of sensor node can save power, reduce traffic, and eliminate congestion in a WSN.

Time Pressure, Time Autonomy, and Sickness Absenteeism in Hospital Employees: A Longitudinal Study on Organizational Absenteeism Records

  • Background: Although work absenteeism is in the focus of occupational health, longitudinal studies on organizational absenteeism records in hospital work are lacking. This longitudinal study tests time pressure and lack of time autonomy to be related to higher sickness absenteeism. Methods: Data was collected for 180 employees (45% nurses) of a Swiss hospital at baseline and at follow-up after 1 year. Absent times (hours per month) were received from the human resources department of the hospital. One-year follow-up of organizational absenteeism records were regressed on self-reported job satisfaction, time pressure, and time autonomy (i.e., control) at baseline. Results: A multivariate regression showed significant prediction of absenteeism by time pressure at baseline and time autonomy, indicating that a stress process is involved in some sickness absenteeism behavior. Job satisfaction and the interaction of time pressure and time autonomy did not predict sickness absenteeism. Conclusion: Results confirmed time pressure and time autonomy as limiting factors in healthcare and a key target in work redesign.

  • Amidst the Covid-19 pandemic, human resources play a vital role in the health industry because the staff all has to confront a lot of stresses in serving the country and the people to overcome the severe contagiousness and infection of the virus. It is also the case of Family General Hospital, Da Nang (Vietnam). Therefore, the Hospital identifies talented personnel as a core resource in its sustainable development strategy. Researching on how to retain talented staff to serve the sustainable and long-term development of a private hospital such as Family Hospital is extremely necessary, especially when there are fewer large and modern private health facilities for the healthcare system in Central Vietnam compared to the North and the South. With the analysis of survey data and in-depth interviews from both qualitative and quantitative perspectives (via SPSS 20.0), especially the ANOVA and EFA analyses, and linear multiple regression (Generation 1 methods), this study aims to clarify the aspects that affect the talent retention in the representative Family Hospital. The lessons learned have been a good reference for similar private healthcare models in the process of bringing health-related services to a new level in the competition.

Distribution of Korean safety and health professionals from the perspective of gender equality

  • Background: Gender equality in the workplace remains a challenge. This study aimed to investigate the gender gap of human resources in the field of occupational safety and health (OSH) in Korea. Methods: Several national statistical data that can confirm the gender distribution of personnel in the OSH sector were reviewed. The gender distribution of industrial technical manpower statistics, professional certificate holders in the OSH field, and the status of the appointment of safety and health managers by industry was analyzed. Results: The distribution of professionals in the OSH sector in Korea had a large gender gap with a small number of women. In particular, the proportion of women in the safety field was lower than that in the health field. In the younger age group, while the proportion of women is increasing, the gender gap tends to decrease. Conclusions: Few data are available to understand the gender-related aspects of OSH professions. Nevertheless, our results provide basic information for initiating a discussion on the necessity of establishing gender-sensitive culture and policies in the OSH sector.

Effectiveness of Medical Aid Case Management in Excessive Healthcare User by Interventions based on the Number of Accesss (의료급여 과다이용자의 중재방법별 접근횟수에 따른 사례관리 효과)

  • This study analyzed medical aid case management effects based on the number of access by visits, phone calls, letters, internal investigation, resource links, and requests, in medical aid case management business that is carried out to improve the quality of life of medical care beneficiaries and reduce financial cost. Secondary analysis study using data of 564 high-risk group in the medical aid case management and their healthcare utilization data. Letters had positive correlations with the scores of all case management domains. The higher visits was, the lower the score of self-health care ability was, and the higher the number of phone calls was, the higher the score for reasonable medical use was. While there was no significant difference in medical cost according to aid management by interventions, the higher visits and resource links were, the lower the subjects' total number of payment days was. There is a difference in the various areas of the quality of health -related life and medical use depending on case management by intervention method and its number. It is necessary to carry out the efficient number of access to case management by intervention method.

Stage-wise Combination of Key Factors Affecting Healthcare User Innovation by Using Fuzzy-set Qualitative Comparative Analysis (퍼지집합 질적 비교분석을 통한 의료분야 사용자 혁신 단계별 핵심요인 조합 연구)

  • We examine how combinations of key factors affecting healthcare user innovation vary by innovation stages from idea generation through R&D to commercialization and venturing using a fuzzy-set Qualitative Comparative Analysis (fsQCA) of thirty Korean cases in the healthcare field. Our empirical analysis shows that well-functioning innovation network and easy resource acquisition facilitate ideation of radical user innovation. However, technological capability and governmental support are crucial to make a shift to R&D as well as commercialization stages. Differently, incremental user innovation depends heavily on technological capability of users. Our analysis can provide policy makers as well as corporate innovation mangers with a strategic framework for boosting user innovation along three stages.

  • ATP bioluminescence system and impedance system were evaluated with the objective of reducing the time for microbial analysis of cosmetics formulations from 72 to 24 h. The meaningful correlation (at least $95\%$) was achieved when emulsion were artificially contaminated with low levels of different organisms, including Pseudomonas aeruinosa, Staphylococcus aureus, Escherichia coli and Ralstonia mannitolilytica. The standard agar plate method, ATP bioluminescence and impedance method were used for in this study. Successful evaluation and validation of automated systems has enabled the introduction of ATP bioluminescence and impedance method into routine use within the microbiology laboratory. This has provided a rapid assessment of product quality, resulting in faster throughput and resource maximization.
