WiBro 환경에서의 u-Healthcare 서비스를 위한 무선데이터 품질 측정 방안 연구 (A Study on Wireless Data Quality Measurement Method for u-Healthcare Service in WiBro Environment)

  • 유 무선 통신망의 발전과 스마트폰 등 무선 단말을 이용한 모바일오피스 등 무선서비스 이용이 날로 증가하고, 이를 이용한 u-Healthcare 서비스가 각광 받고 있는데, 기존의 품질 측정 기준은 단순히 속도나 에러율 등만을 고려하였기 때문에 u-Healthcare의 안정성을 보장하기 위해서는 서비스별 품질관리가 필요하다. 본 논문에서는 u-Healthcare 무선서비스 사용자의 품질 보장 및 사용자 요구를 만족시키기 위한 방안으로 무선 환경을 고려한 무선서비스별 품질 측정 기준을 제시하였다. 관련 연구를 통해 3GPP, WiMAX 포럼, GSMA 등 국제 표준화 기관에서 제시한 주요성능 지표와 사업자 사례를 통해 최종 사용자 관점에서의 WiBro 환경에서 u-Healthcare를 제공하기 위한 서비스별 품질기준 체계를 확립하고 각 서비스별로 품질지표 수립의 타당성을 제시하였다.

전문의약품 대중광고에 대한 의약전문인과 일반인간의 인식 비교 연구 (Comparisons of Perception on Direct-to-Consumer Advertisements of Prescription Drugs between Healthcare Providers and Consumers)

  • Objective: The aim of this study was to investigate the difference of perception on direct-to-consumer advertisement (DTCA) of prescription drugs between healthcare providers and consumers. Methods: The online and offline survey was conducted from May 26th to June 5th, 2013. The questionnaire was composed of 15 items about perception on DTCA of prescription drugs. Results: A total of 215 healthcare providers and 202 consumers responded to the questionnaire. Consumers had an overall positive attitude on permitting DTCA of prescription drugs and carried favorable views about the influence of the DTCA of prescription drugs on providing drug information, promoting communications between healthcare providers and consumers, and improving images of healthcare providers. Healthcare providers displayed negative perception for the needs of permitting the DTCA of prescription drugs compared to consumers. They showed somewhat skeptical perception about the influence of the DTCA of prescription drugs on necessities and efficiencies of delivering drug information, promoting communications between healthcare providers and consumers, and improving images of healthcare providers. Both healthcare providers and consumers were concerned about the increase of drug prices following the increase in advertisement expenses of pharmaceutical products. Conclusion: This study identified the perception differences on direct-to-consumer advertisements of prescription drugs between healthcare providers and consumers. This study could be of much help in the process of review on permitting DTCA of prescription drugs in Korea.

Improvement and Backsliding after Chronic-disease Self-management Education in Japan: One-year Cohort Study

  • Purpose: In people who have chronic diseases, disabilities, and rehabilitation needs, self-management education can improve health and health-related behavior, and it can reduce the utilization of healthcare services. The purpose of this research was to assess the long-term effects of chronic-disease self-management education in Japan. Methods: This study included 184 adults living with various chronic medical conditions who participated in the Chronic Disease Self-Management Program (CDSMP) in Japan. Data were collected before the program began, and then collected 3 more times over 1 year. Results: Healthcare-service utilization was low at baseline, and it did not change. Self-evaluated health status, health-related distress, coping with symptoms, communication with doctors, and self-efficacy to manage symptoms all improved after the program. However, there was backsliding in all of the outcomes that had improved. Conclusion: Some benefits of this program can last for at least 1 year, but interventions to prevent attenuation may be needed. For economic evaluations, research should focus on populations with higher baseline levels healthcare-service utilization, including use of rehabilitation services. Also, more attention should be focused on the longer-term decay or persistence of the program's benefits, particularly regarding on preventing and reducing disabilities and with regard to rehabilitation needs.

Positive affect of insect-mediated mental healthcare program to adolescent's emotions

  • Several insects have been shown to exert a strong influence positively on peoples' emotions. Insect-mediated mental healthcare program was designed to help meet the physical, behavioral and developmental needs of adolescents. They were provided with an insectmediated mental healthcare program for a total of 10 sessions, one session per wk, at 1 h per session, accompanied by a pre- and post-test for emotion. Significant differences and correlations were found according to the individual participants' planning, subjective quality of life, self-reported depression and self-worth, respectively. The results indicated that adolescents who participated in the insect-mediated healthcare program group once showed significant improvement in their planning, subjective quality of life and self-worth. However, most of all participants' self-reported depression was not influenced. These findings suggest that pet-insects positively affect adolescent's emotional health via an insect-mediated healthcare program.

우리나라 보건의료 발전을 위한 의료기술평가의 역할 (Roles of Health Technology Assessment for Better Health and Universal Health Coverage in Korea)

  • Health technology assessment (HTA) is defined as multidisciplinary policy analysis to look into the medical, economic, social, and ethical implications of the development, distribution, and use of health technology. Following the recent changes in the social environment, there are increasing needs to improve Korea's healthcare environment by, inter alia, assessing health technologies in an organized, timely manner in accordance with the government's strategies to ensure that citizens' medical expenses are kept at a stable level. Dedicated to HTA and research, the National Evidence-based Healthcare Collaborating Agency (NECA) analyzes and provides grounds on the clinical safety, efficacy, and economic feasibility of health technologies. HTA offers the most suitable grounds for decision making not only by healthcare professionals but also by policy makers and citizens as seen in a case in 2009 where research revealed that glucosamine lacked preventive and treatment effects for osteoarthritis and glucosamine was subsequently excluded from the National Health Insurance's benefit list to stop the insurance scheme from suffering financial losses and citizens from paying unnecessary medical expenses. For the development of HTA in Korea, the NECA will continue exerting itself to accomplish its mission of providing policy support by health technology reassessment, promoting the establishment and use of big data and HTA platforms for public interest, and developing a new value-based HTA system.

  • The purpose of this study is to define the concept of sustainability, to understand the traditional belief system, to comprehend user-focused healing environment, to understand the current status of Korean Hospitals, to analyze the selected case studies, and to point the way toward enhanced architectural solutions for healing environment in hospitals. The research results indicate that the design of healthcare facilities with healing effects depend on the considerations of 5 senses of patients. In other words, the sustainable design approach to create a healthy healing environment is very important, and architects must be aware of diverse sensory needs of the patients. In addition, the following factors affection healthcare facilities must be considered to create healing environment for users: - "Emphasis on sustainable Design" for harmony among nature, man, and building. - Creation of user Focused Healing Environment reflecting the suers' senses. - Search for New Hospital Forms through various design experiments to create the most suitable healing environment. - "Perception of Paradigm Shift" form a focus on sickness-oriented to wellness-oriented healthcare facility design. - "Awareness of Creating User-Friendly Environment" covering the entire age group form youth to elderly.

  • From the latter half of 20's century, the numbers of dental service workers and dental facilities are noticeably increased and dental patients want the higher-classed and specialized dental service. For the increased facilities and user's needs at dental college hospital, the present condition of dentistry, special features and the field study at dental college hospital are surveyed for this study. The purpose of this study is to provide a architectural planning data on the dental college hospital for keeping up with the development of dentistry.

  • Public health system for more prevention-oriented health promotion rather than hospital-based curative service, focusing population rather than individual, and comprehensive health management in the local community strongly needs to be constructed to solve major issues on efficiencies and equity problems which Korean healthcare system is facing nowadays. Public health promotes and protects the health of people and the communities where they live, learn, work, and play. Medical care tries to cure those who have diseases, but public health tries not to become ill and not to be injured. Debates on how we build or rebuild public health system, which is contrasted with medical care system, are needed in Korea, focusing how needs for healthy community and right to health are fulfilled. Public health specialists for practising population health at local community level should be systematically recruited, the function of public health centers should be strengthened, and new government organization should be established for place-based health management.

Analysis of sedation and general anesthesia in patients with special needs in dentistry using the Korean healthcare big data

  • Background: People with special needs tend to require diverse behavioral management in dentistry. They may feel anxious or uncomfortable or may not respond to any communication with the dentists. Patients with medical, physical, or psychological disorders may not cooperate and therefore require sedation (SED) or general anesthesia (GA) to receive dental treatment. Using the healthcare big data in Korea, this study aimed to analyze the trends of SED and GA in special needs patients undergoing dental treatment. It is believed that these data can be used as reference material for hospitals and for preparation of guidelines and related policy decisions of associations or governments for special needs patients in dentistry. Methods: The study used selected health information data provided by the Korean National Health Insurance Service. Patients with a record of use of one of the eight selected drugs used in dental SED between January 2007 and September 2019, those with International Classification of Diseases-10 codes for attention deficit hyperactivity disorder (ADHD), phobia, brain disease, cerebral palsy, epilepsy, genetic disease, autism, mental disorder, mental retardation, and dementia were selected. The insurance claims data were analyzed for age, sex, sedative use, GA, year, and institution. Results: The number of special needs patients who received dental treatment under SED or GA from January 2007 to September 2019 was 116,623. Number of SED cases was 136,018, performed on 69,265 patients, and the number of GA cases was 56,308, implemented on 47,257 patients. In 2007, 3100 special needs patients received dental treatment under SED while in 2018 the number of cases increased 6 times to 18,528 SED cases. In dentistry, ADHD was the most common disability for SED cases while phobia was the most common cause of disability for GA. The male-to-female ratio with respect to SED cases was higher for males (M : F = 64.36% : 35.64%). Conclusion: The application of the SED method and GA for patients with special needs in dentistry is increasing rapidly; thus, preparing guidelines and reinforcing the education and system are necessary.

노인치위생학 교육과정 개선에 대한 요구도 분석 (Analysis of educational needs for improving the geriatric dental hygiene curriculum)

  • Objectives: This study aimed to analyze the educational needs for geriatric dental hygiene and provide basic data for developing standard curricula for geriatric hygiene. Methods: To investigate the educational needs for geriatric dental hygiene, 212 students from dental hygiene departments and 205 dental hygienists engaged in clinical practice were enrolled. Results: Among the educational needs for geriatric dental hygiene, Geriatric oral health issues category was the highest with 4.06 points for students and 4.05 points for dental hygienists. Students who completed a geriatric dental hygiene course had higher scores for the following sub-domains compared to those who did not: need for geriatric dental hygiene-related theoretical education (p=0.002), needs for practice education with elderly people (p=0.001), and confidence in performing oral healthcare for elderly people after graduation (p<0.001). Meanwhile, clinical dental hygienists who not completed geriatric-related courses had higher scores for, need for geriatric dental hygiene-related theoretical education, need for practice education with elderly people, and confidence in performing oral healthcare for elderly people. but, there was no significant difference (p>0.05). Conclusions: The results of this study showed that students and dental hygienists had high educational needs for geriatric dental hygiene. In particular, there was a high demand for education related to Geriatric oral health issues as well as Geriatric dental hygiene care and skills.
