환자안전에 관한 간호사의 경험학습: 낙상 사고를 중심으로 (Nurses' learning experiences from falling accidents on patient safety)

  • Purpose : The aim of this article is to describe the nurses' experiential learning mechanism on patient safety. Methods : To analyze nurses' learning experiences on patient safety cases, a focus-group interview method was used. The Kolb's experiential learning model was used as a reference model. Findings : Without deep reflective reasoning about specific experiences, there is no creative or innovative solutions to experiment actively. Nurses are likely to be reluctant learners when there is no systemic support from formal departments which is in charge of patient safety and quality of care. Conclusion : In order to build patient safety culture in hospital, there should be efforts to make nurses as active learners on patient safety as well as to build learning environments in medical units.

행위별 수가 지불제도 하에서의 사례관리시스템 개발 및 경제성 분석 (Development of Case Management System and Analysis of Economic Feasibility under the Fee-For-Service Reimbursement)

  • Background : Recently, we have experienced various changes in the healthcare environment. Healthcare organizations are facing a financial crisis due to more competitive relationships among themselves as well with low health insurance fees. The purposes of the current study were: (1) to develop a data warehouse-based system for evaluating and monitoring the case management activities, and (2) to measure and analyze its effects. Methods : In order to collect the data for the study, the database on discharged patients was utilized at a university hospital located in Seoul from June 1, 2002 through December 31, 2002. Initially, a data warehouse was built for the case management system. The case management activities were analyzed using structured methodology to establish the case management system. Results : The findings of this study were as follows: (1) A case management system was developed to make it possible to monitor of healthcare quality and resource utilization. The Case management System included monitoring functions regarding utilization reviews, critical pathways, and clinical indicators. (2) Utilizing the case management system, unplanned readmissions were documented among total discharged patients during two months from November 1, 2002 through December 31, 2002. The unplanned readmission rate was 2.3%(276 patients) in total of 11,960 discharged patients. Among them 81 patients(0.7% of total discharges, 29.3% of unplanned readmission) were readmitted to the same physician in charge under the same diagnosis. No significant differences were found in the demographic variables such as gender and age among the patients. (3) After implementing the case management system, 2.9% of average length of stay reduced. Applying cost-benefit analysis, the 2.9% reduction of length of stay represents net profit of ${\backslash}$ 279,592,000 in the year of 2004. In addition, applying value acceleration analysis, cumulative net benefit of ${\backslash}$ 1,481,000,000 was expected by the year of 2007. Also we were able to expect ${\backslash}$ 247,800,000 of cumulative benefit for the prospective 5 years in value linkage analysis. It represents average ${\backslash}$ 787,700,000 of pure net benefit a year. Conclusion : The value of present study would be not only implementing the knowledge management system into the existing case management activities, but also evaluating its effects and estimating its financial benefits. This study suggested that the case management system would be a supportive tool for monitoring and improving the quality of healthcare, and a cost-effective tool for increment of healthcare organization's financial benefit.

포괄적 비즈니스 모델 프레임워크 구축 및 적용 : 헬스케어 비즈니스 모델을 중심으로 (Establishment and Application of a Comprehensive Business Model Framework : Focusing on Healthcare Business Models)

  • A business model (BM) can be developed based on a business model framework (BMF) consisting of key components such as value proposition, customers, and resources. To systematically generate and analyze emerging BMs such as healthcare BM, an integration of diverse BMF components is needed. The present study is to establish a comprehensive BMF and evaluate its applicability to healthcare BMs. Based on a review of eight BMF studies, ten BMF components were identified and classified into five major components (value proposition, resources, organizing model, customers, and revenue model) and five minor components (technology, service platform, delivery, competitive strategy, and growth/exit) by analyses of frequency and functional importance. Lastly, the BMs of three emerging healthcare companies (WellDoc, Inc., CFW Shops, and Aravind Eye Care System) were analyzed and compared in terms of the proposed BMF components. The comprehensive BMF components presented in the study can be of help for developing new BMs and analyzing the strengths and weaknesses of BMs.

스마트 헬스케어 환경에서 복잡도를 고려한 R파 검출 및 QRS 패턴을 통한 향상된 부정맥 분류 방법 (R Wave Detection and Advanced Arrhythmia Classification Method through QRS Pattern Considering Complexity in Smart Healthcare Environments)

  • With the increased attention about healthcare and management of heart diseases, smart healthcare services and related devices have been actively developed recently. R wave is the largest representative signal among ECG signals. R wave detection is very important because it detects QRS pattern and classifies arrhythmia. Several R wave detection algorithms have been proposed with different features, but the remaining problem is their implementation in low-cost portable platforms for real-time applications. In this paper, we propose R wave detection based on optimal threshold and arrhythmia classification through QRS pattern considering complexity in smart healthcare environments. For this purpose, we detected R wave from noise-free ECG signal through the preprocessing method. Also, we classify premature ventricular contraction arrhythmia in realtime through QRS pattern. The performance of R wave detection and premature ventricular contraction arrhythmia classification is evaluated by using 9 record of MIT-BIH arrhythmia database that included over 30 premature ventricular contraction. The achieved scores indicate the average of 98.72% in R wave detection and the rate of 94.28% in PVC classification.

출산장려정책 중 고운맘 카드에 대한 산모들의 인식 조사 (The Survey Research on Pregnant Woman's recognition about GO-UN-MAM CARD of Childbirth Promotion Policy)

  • 저출산은 생산인구의 감소와 함께 보험료 부담을 가중시키는 문제 뿐 아니라, 종국에는 국가의 존립을 위협하는 묵과할 수 없는 국가적인 정책과제가 되었다. 이에 본 연구는 현 정부의 출산장려정책 중 경제적 지원인 고운맘 카드의 실효성을 알아보고 산모들의 인식조사를 바탕으로 도출된 문제점에 대한 개선안을 제언하고자 한다. 분석방법은 인식도 조사를 위해서는 빈도분석을 이용했으며, 출산장려정책에 대한 선호도를 파악하기 위해 다중응답분석, 고운맘 카드 사용에 대한 만족도와 재출산 의사정도를 알아보기 위한 카이제곱 테스트를 시행했다. 분석결과, 고운맘 카드 사용으로 인한 산모들의 만족도와 재출산 의사간에는 유의한 관련성을 보였다. 또한, 산모들은 정부기관 및 의료기관과 구전으로 전파된 내용에 의해 해당 정책에 접근하는 것으로 나타났다. 그리고 정부의 지원금액 범위 내에서 병원 청구할인서비스 정도만 활용하는 것으로 나타났다. 그러나 지원금액 및 l일 사용 한도금액과 같은 금전적 요인에서 주로 불만을 표출했다. 그에 따른 상향 조정된 희망 개선안을 요구했다. 위의 결과를 토대로, 정부는 정책시행의 궁극적 목적인 출산률의 상승을 위해 고운맘 카드를 개선 발전시켜나가야 할 것이다.

Cloud-based Healthcare data management Framework

  • Cloud computing services changed the way the data are managed across the healthcare system that can improve patient care. Currently, most healthcare organizations are using cloud-based applications and related services to deliver better healthcare facilities. But architecting a cloud-based healthcare system needs deep knowledge about the working nature of these services and the requirements of the healthcare environment. The success is based on the usage of appropriate cloud services in the architecture to manage the data flow across the healthcare system.Cloud service providers offer a wide variety of services to ingest, store and process healthcare data securely. The top three public cloud providers- Amazon, Google, and Microsoft offers advanced cloud services for the solution that the healthcare industry is looking for. This article proposes a framework that can effectively utilize cloud services to handle the data flow among the various stages of the healthcare infrastructure. The useful cloud services for ingesting, storing and analyzing the healthcare data for the proposed framework, from the top three cloud providers are listed in this work. Finally, a cloud-based healthcare architecture using Amazon Cloud Services is constructed for reference.

데이터마이닝을 이용한 의료의 질 측정지표 분석 및 의사결정지원시스템 개발 (Analysis of Healthcare Quality Indicators using Data Mining and Development of a Decision Support System)

  • Background : This study presented an analysis of healthcare quality indicators using data mining and a development of decision support system for quality improvement. Method : Specifically, important factors influencing the key quality indicators were identified using a decision tree method for data mining based on 8,405 patients who discharged from a medical center during the period between December 1, 2000 and January 31, 2001. In addition, a decision support system was developed to analyze and monitor trends of these quality indicators using a Visual Basic 6.0. Guidelines and tutorial for quality improvement activities were also included in the system. Result : Among 12 selected quality indicators, decision tree analysis was performed for 3 indicators ; unscheduled readmission due to the same or related condition, unscheduled return to intensive care unit, and inpatient mortality which have a volume bigger than 100 cases during the period. The optimum range of target group in healthcare quality indicators were identified from the gain chart. Important influencing factors for these 3 indicators were: diagnosis, attribute of the disease, and age of the patient in unscheduled returns to ICU group ; and length of stay, diagnosis, and belonging department in inpatient mortality group. Conclusion : We developed a decision support system through analysis of healthcare quality indicators and data mining technique which can be effectively implemented for utilization review and quality management in a healthcare organization. In the future, further number of quality indicators should be developed to effectively support a hospital-wide Continuous Quality Improvement activity. Through these endevours, a decision support system can be developed and the newly developed decision support system should be well integrated with the hospital Order Communication System to support concurrent review, utilization review, quality and risk management.

상급종합병원과 종합병원의 민원처리 담당 직원의 직무 스트레스 비교 분석 (Job stress of customer service representatives: focusing on the tertiary care hospitals and acute general hospitals)

  • Although stress has been implicated to be a risk factor that can threaten physical and mental health, there have been no sufficient studies that analyze the different levels of stress among employees working in the different levels of the hospitals. We aim to identify the general characteristics of hospitals at different levels, to compare the stress levels among customer service representatives working in the tertiary care hospitals as well as acute general hospitals. In addition, we also wanted to analyze the relationship between the types of hospitals and the stress level. The work stress was measured using the Korean Occupational Stress scale. Study subjects' demographic characteristics and lifestyle factors were analyzed using analysis of frequency and multiple regression analysis. Our study revealed that the levels of medical facility were significantly associated with the levels of job stress(P=0.043), and the stress levels of employees working in the acute general hospital's medical facilities were higher than those who were working in tertiary care facilities. We also found that those with higher depression level tended to have higher job stress (P<0.001). Therefore, it is urgent to implement some kind of job stress interventions, especially in the acute general hospital's medical facilities. Moreover, further studies including social and policy research are necessary in order to analyze the overall impact of stress on physical and mental health and to reduce health inequalities among healthcare workers.

Critical Success Factors for the Adoption of Health Management Information Systems in Public Hospitals in Zimbabwe

  • The Zimbabwean healthcare sector faces huge challenges due to increased demands for improved services for a growing number of patients with fewer resources. The use of information and communications technologies, prevalent in many industries, but lacking in Zimbabwean healthcare, could increase productivity and innovation. The adoption of health management information systems (HMISs) can lead to improved patient safety and high-level patient care. These technologies can change delivery methods to be more patient focused by utilising integrated models and allowing for a continuum of care across healthcare providers. However, implementation of these technologies in the health care sector remains low. The purpose of this study is to demonstrate the advantages to be attained by using HMISs in healthcare delivery and to ascertain the factors that influence the uptake of such systems in the public healthcare sector. A conceptual model, extending the technology, organization, and environment framework by means of other adoption models, underpins the study of adoption behavior. A mixed method methodology was used to conduct the study. For the quantitative approach, questionnaires were used to allow for regression analysis. For the qualitative approach, thematic analysis was used to analyse interview data. The results showed that the critical success factors (namely, relative advantage, availability, complexity, compatibility, trialability, observability, management support, information and communication technology expertise, communication processes, government regulation, infrastructure support, organizational readiness, industry and competitive support, external support, perceived ease of use, perceived usefulness, attitude, and intention to use) influenced adoption of HMISs in public hospitals in Zimbabwe.

  • 본 연구는 접근성과 의료서비스 품질, 대외적 이미지를 높임으로써 향후 지방의료원이 지역 내 대표의료기관으로서 경쟁력을 확보하고 경영효율성을 향상시키는데 그 목적이 있다. 조사대상은 604명으로 하였으며, 분석방법은 ANOVA, 공분산 구조방정식을 실시하였다. 결론을 보면, 의료기관 선택요인에 대한 고객들의 만족도나 선호도가 인구통계학적 특성별로 유의한 차이가 있다는 결과는 앞으로 지방의료원이 의료서비스 전문화 전략을 수립할 때 주요 고객집단의 욕구에 대한 면밀한 검토가 이루어져야 할 필요가 있음을 시사하고 있다. 또한 지방의료원이 진료비 수준에 대한 치료효과와 타 의료기관에 비해 저렴한 진료비가 경쟁우위를 확보할 수 있는 중요요인으로 이에 따른 이미지 부각과 의료서비스의 질의 향상을 극대화하여 의료소비자들을 유인하는 전략이 필요하다.
