  • Objectives : This study was conducted to measure the effects of healthcare accreditation (HA) on the changes in infection control (IC). Methods : Questionnaires were e-mailed to 60 hospitals from 23 October to 23 December, 2011. Data were analyzed by SPSS 12.0. Results : Finally 50 hospitals (83.0%) were enrolled in the study: Seoul area (40.0%), tertiary (76.0%), and >500 beds (98.0%). Nine hospitals (18.0%) had a full time infection control nurse[ICN] with 300 beds. Among various factors, ICN (36.0%), hospital facilities (66.0%), instruments (32.0%) and supplies (88.0%) all improved. Hand hygiene increased (53.1% vs 83.2%, p<.001), but it was continued only in 34.1% of hospitals. Healthcare-associated infection (68.4%), multi drug resistant organisms (42.1%) and outbreaks (26.3%) decreased. Reasons for difficulties in satisfying the HA standards were inadequate support which included hospital facilities, instruments, budget, and a shortage of ICNs and healthcare workers (HCWs). Conclusions : HA had effects on the IC, but they were transient. Staffing in ICN and HCW staffing, hospital facilities, instruments, and supplies all need to be improved.

The research results indicate that the design of healthcare facilities with healing effects depend on the considerations of 5 senses of patients. In other words, the sustainable design approach to create a healthy healing environment is very important, and architects must be aware of diverse sensory needs of the patients. In addition, the following factors affection healthcare facilities must be considered to create healing environment for users: - "Emphasis on sustainable Design" for harmony among nature, man, and building. - Creation of user Focused Healing Environment reflecting the suers' senses. - Search for New Hospital Forms through various design experiments to create the most suitable healing environment. - "Perception of Paradigm Shift" form a focus on sickness-oriented to wellness-oriented healthcare facility design. - "Awareness of Creating User-Friendly Environment" covering the entire age group form youth to elderly.

Methods: Survey, Questionaire and statistical analysis are the main research method of this study. In order to collect data related to utilization pattern and favorite functions of the senior people, researchers have visited 4 social welfare facilities located in Southern Gyeonggi Province. 403 questionaires have been gathered from 4 facilities and they have been analyzed by using Excel Program of MS. Results: First, compared to other services, healthcare services have been preferred by many older people in Social welfare Facilities. This means that integrated care centers providing healthcare services for older people rather than services for children or disabilities is desirable. Second, Integrated Care Centers had better be established within the walk distance of elderly people. If it is not easy, the introduction of shuttle bus for older people is desirable. Especially, in case of large Care Center. Implications: This study shows that small facility with community care rather than big facility is desirable for small community in the point of friendliness, convenience, economy, etc.. However it is necessary to combine welfare service and healthcare service even in small centers.

For these factors, it is necessary for the elderly to receive treatment for multidisciplinary diseases, associated with psychological care of sociological concept. It is quite difficult in an acute-care hospital to accommodate the elderly patient because of the fact that an acute-care hospital is required to maintain a high level of medical care and technical standard. That is why specialized medical service is needed for the elderly. In the case of Germany, they are at the stage of the change from large scale facilities to specialized facilities applying the integrated concept. This paper addresses the medical environmental factor and distinction of medical facilities for the elderly through survey and analysis relating to all change in Germany. Therefore, it aims to suggest a fundamental resource for architectural planning and network of medical facilities for the elderly.

  • This study reviewed the background of the advent of and the concept of age-integrated facilities where elderly persons' facilities and children's facilities have been built in the same plot or building or established adjacent to each other. And also it examined the concept, necessity, effects, and kinds of intergenerational interactions. Ten age-integrated facilities in Japan, where it is reported that intergenerational interactions are now being implemented, were selected and visited for surveys. This study implemented interviews with staff members and analyses of the drawings of the facilities to grasp the present state of age-integrated facilities and their plan types, etc. Through analyses of relationships between intergenerational interactions and the spaces in which these take place, this study discusses guidelines for space arrangement and for developing interaction space plans for age-integrated facilities that would facilitate intergenerational interactions.

  • In Germany, community based daycare facilities has been supported by long term care policies since the 1970s. With the legislation of Care Insurance, those policy has been developed further. As the use of daycare facilities decreased and the financial burden for consumers increased, administrators of facilities has been seeking for innovative programs and management methods in order to improve the service. For the same reason, policy makers have been pursuing new regulations of architectural standards of facilities. By looking at legislations (i.e., Law of Care Insurance, Law of Heim, and DIN18025) that stipulate architectural standard of facilities in Germany, this study will identify the development process of architectural change of daycare facilities. In addition, the study aims to contribute to the discussion on the use of day care facilities in Korea in terms of the legislation of Care Insurance soon to be introduced.
