대상자가 인지한 의료서비스 질과 재이용 의도와의 관계 (The Relationships Between Patient's Perceived Quality of Healthcare Service and Intention to Re-visit)

  • Purpose: This study was performed to identify the relationship between patient's perceived quality of healthcare services and intention to re-visit. Method: The study subjects were 242 patients from three local clinics. The data were analyzed using SPSS PC+ 12.0 program for descriptive, Pearson's correlation coefficient, and multiple regression analysis. Result: Patients' perceived quality of healthcare services was measured 5.73$\pm$0.71 using the 7-point scale. The patients' perceived quality was moderately correlated with patient satisfaction (r=0.647, p<0.001) and intention to re-visit (r=0.604, p<0.001). Also, patient satisfaction was highly correlated with intention to re-visit (r=0.788, p<0.001). 3) the reliability factor of patients' perceived quality of healthcare services and patient satisfaction were found significant predictors of intention to re-visit. Conclusion: In conclusion, service quality evaluated by patients influenced on their intention of re-visit and especially, reliability was the most significantly influential variable.

데이터마이닝을 이용한 의료의 질 측정지표 분석 및 의사결정지원시스템 개발 (Analysis of Healthcare Quality Indicators using Data Mining and Development of a Decision Support System)

  • Background : This study presented an analysis of healthcare quality indicators using data mining and a development of decision support system for quality improvement. Method : Specifically, important factors influencing the key quality indicators were identified using a decision tree method for data mining based on 8,405 patients who discharged from a medical center during the period between December 1, 2000 and January 31, 2001. In addition, a decision support system was developed to analyze and monitor trends of these quality indicators using a Visual Basic 6.0. Guidelines and tutorial for quality improvement activities were also included in the system. Result : Among 12 selected quality indicators, decision tree analysis was performed for 3 indicators ; unscheduled readmission due to the same or related condition, unscheduled return to intensive care unit, and inpatient mortality which have a volume bigger than 100 cases during the period. The optimum range of target group in healthcare quality indicators were identified from the gain chart. Important influencing factors for these 3 indicators were: diagnosis, attribute of the disease, and age of the patient in unscheduled returns to ICU group ; and length of stay, diagnosis, and belonging department in inpatient mortality group. Conclusion : We developed a decision support system through analysis of healthcare quality indicators and data mining technique which can be effectively implemented for utilization review and quality management in a healthcare organization. In the future, further number of quality indicators should be developed to effectively support a hospital-wide Continuous Quality Improvement activity. Through these endevours, a decision support system can be developed and the newly developed decision support system should be well integrated with the hospital Order Communication System to support concurrent review, utilization review, quality and risk management.

The Customer Satisfaction Index Model: An Empirical Study of the Private Healthcare Sector in Malaysia

  • The main purpose of this study was to gauge the patient satisfaction index and subsequently discuss the Importance-Performance (IP) matrix analysis of the inpatient services in the context of the private hospital setting. The Malaysian Customer Satisfaction Index Model was employed as the theoretical framework for the above purposes. This study involving 242 patients in Malaysian's private healthcare sector used a Web-based survey as the main method of data collection. Partial least square structural equation modeling (PLS-SEM) was utilized for data analysis. Using Fornell et al. (1996)'s formula, the resulting patient satisfaction index was slightly lower than the "very satisfied" category, the target level required for positioning as one of the world's premier medical tourism players. The IP matrix showed that medical quality is the main competitive advantage of the private hospitals that can propel their growth in the global healthcare marketplace. The results also indicate that outcome quality, patient rights, and privacy, and service quality are the three quality domains that need to be prioritized for further improvement. On the other hand, the servicescape quality domain needs to be strategized as the unique selling proposition as the performance of the private hospitals in this regard is already extremely good.

  • 최근 헬스케어 산업이 사물인터넷(Internet of Thing) 기술과 결합한 스마트 헬스케어 산업이 주목을 받고 있다. 스마트 헬스케어란 애플리케이션, 웨어러블 기기, 플랫폼 등 정보통신 기술은 기반으로 한 건강관리 및 의료 서비스를 말한다. 스마트 헬스케어 산업은 차세대 산업분야인 6T 중 가장 활발한 IT(Information Technology)와 BT(Bio Technology)간의 결합으로 차세대 스마트 헬스케어 기기들이 늘어나고 있다. 다양한 스마트 헬스케어 기기들이 나타남에 따라 스마트 헬스케어 시스템이 다른 유기적 연동 여부와 보안성 품질 여부가 중요한 평가요소가 되고 있다. 본 논문에서는 이를 위해 스마트 헬스케어 시스템의 특성 및 서비스 유형을 조사하고 스마트 헬스케어 시스템의 기술 및 산업 동향을 분석하였다. 이를 바탕으로 스마트 헬스케어 시스템 평가 요구사항을 도출하여 스마트 헬스케어 시스템의 보안성 평가방법과 품질평가기준을 개발하고자 한다. 이는 스마트 헬스케어 시스템의 품질 향상 및 신뢰도 높은 제품 개발을 유도 할 수 있으며, 기술보호 장벽에 대처 할 수 있는 핵심기술로 기대된다.

  • Purpose - The purpose of this study is to shed light on the healthcare management systems in correlation with the advancing technological world, as well as the many diversified systems in different Asian countries. Research design, data, and methodology - This is a comprehensive study on the past researches that have been conducted on the field of healthcare management in different Asian countries. The different types of healthcare coverages were examined by country. Additionally, incorporating the healthcare system with technology is also investigated in this research. Results - The results have shown that there is a rapid incorporation of the technology in today's society with the healthcare systems. This has promising effects for the future, and for the different healthcare systems that exist within other Asian countries around the world. Conclusion - Quality healthcare for people are still elusive, and the healthcare management should improve on ways to better take care of their patients for the near future. By shedding light on the healthcare systems of Asian countries, an idea of the similarities and differences each hold can be assessed and utilized for future goals. By incorporating technology into healthcare systems, this will help in improving the overall care and quality-of-life that patients receive for the future.

베트남 A 병원의 의료서비스 품질이 추천의도에 미치는 영향 : 고객 만족도 매개효과를 중심으로 (Effect of Healthcare Quality on Recommended Intention in Vietnam A Hospital : Focusing on Customer Satisfaction Mediated Effects)

  • This study aims to analyze the quality and satisfaction of healthcare perceived by patients using hospitals and to provide basic data necessary for expanding and settling Vietnamese healthcare services by analyzing the impact on recommendation intentions. The research method selected one hospital in Vietnam and collected data from patients using the hospital and used a total of 286 effective samples as data for hypothesis verification. The research model and hypothesis verification were analyzed with the statistical data from SPSS and AMOS. The findings show that, first, tangibility, accessibility, and reliability all have a positive effect on the quality of healthcare. Second, it has been shown that only accessibility among the quality of healthcare has a positive effect on recommendation intentions. Third, customer satisfaction has been shown to have a positive effect on recommendation intentions. Fourth, when looking at the mediating effect, reliability among the quality of healthcare was shown as a full-mediated effect, but accessibility was shown to have a partial mediating effect and tangibility to have no mediating effect. Contact management is important because customer satisfaction is highly regarded when customers feel positive emotions at the interface from the provision of convenience facilities that support medical services to the reduction of waiting time for patients, employees kindness, treatment, medication, and inspection. It is also confirmed that the demand for convenient and rapid use of hospitals is increasing in Vietnam. In addition, if customer satisfaction is increased through friendly medical staff's response, the intention of recommendation will be even greater.

의료기관인증평가 관련 직무 스트레스가 직군간 이직의도에 미치는 영향 (The Effect of Occupational Stress among Occupational Groups Related to Healthcare Accreditation on Turnover Intention)

  • Purpose: This study aims to investigate the factors influencing health personnel's occupational stress on turnover intention regarding healthcare accreditation. Methods: A survey was conducted from May 17 to May 31, 2021, among participants to examine occupational stress and turnover intention among health personnel working at a 188-bed specialized hospital in Seoul that is preparing for healthcare accreditation. Results: Occupational stress regarding healthcare accreditation was found to have a positive correlation with turnover intention (r=.698, p <.001), and influenced turnover intention the most, which explains the variance of 55.8% (F=29.015, p <.001). There were significant differences between occupational groups in job stress (F=13.292, p<.001) and turnover intention (F=10.930, p <.001) in the healthcare accreditation. Conclusion: Occupational stress regarding healthcare accreditation is higher in nursing than in other occupations, indicating the need to lower the turnover intention of nurses by preparing a national institutional standard for nursing manpower and also put in place an appropriate compensation system for each hospital seeking accreditation.

미충족 의료 경험이 삶의 질에 미치는 영향 (Effect of Unmet Healthcare Needs on Quality of Life)

  • 본 연구는 미충족 의료 경험과 삶의 질 관련 측정지표 간 인과관계를 실증하는 데 목적을 두었다. 이 두 변수간 인과관계 검증을 위해 본 연구는 한국의료패널의 부가조사 원자료를 가공하여 분석을 실시하였다. 삶의 질과 관련된 측정지표로는 운동능력, 자기관리, 일상 활동, 통증/불편, 불안/우울, 주관적 건강상태의 6개 변수를 투입해 선형회귀분석을 실시하였다. 본 연구의 분석 결과 미충족 의료 경험은 본 연구에 투입한 6가지의 삶의 질 하위변수 모두에 통계적으로 유의한 부(-)적 영향을 미치는 것을 확인할 수 있었다. 아울러 선형회귀분석을 통해 도출된 β값을 활용해 상대적 영향력의 크기를 검토한 결과 통증/불편, 주관적 건강상태, 불안/우울, 일상 활동, 운동능력, 자기관리의 순으로 차별적 영향 수준을 나타내는 것을 알 수 있었다. 이러한 분석 결과에 기초해 삶의 질적 수준 향상에 기여하기 위한 미충족 의료에 대한 정책 대응방안으로서 보건과 복지의 실무 연계 강화와 관련한 실무적 시사점을 제언하였다.

의료기관인증이 병원 근로자가 지각하는 환자안전과 질, 직무만족 및 조직몰입에 미치는 효과 -일개 종합병원을 대상으로- (The impact of healthcare accreditation on patient safety and quality, job satisfaction and organizational commitment: about a general hospital)

  • 본 연구는 의료기관인증이 병원 근로자가 지각하는 환자안전과 질, 직무만족 및 조직몰입에 미치는 효과를 평가하기 위한 단일집단 사전 사후 검사 설계연구이다. 대상자는 서울시내 일개 종합병원에서 1년 이상 근무한 405명을 대상으로 하였다. 시험처치는 2012년 4월부터 준비를 시작하여 2012년 11월 의료기관평가인증원으로부터 조사를 받은 의료기관인증이며, 자료 분석은 SPSS(version 18.0) 프로그램을 사용하여 paired t-test와 One-Way ANOVA로 검정하였다. 연구결과, 연구결과, 의료기관인증 후 병원근로자가 지각하는 환자안전과 질(t=7.28, p=.000), 직무만족(t=7.13, p=.000) 및 조직몰입(t=3.44, p=.001)은 통계적으로 유의하게 증가하였다. 따라서 의료기관인증은 환자안전과질, 직무만족 및 조직몰입을 증진시키는 데 효과가 있다고 볼 수 있다.
