Purpose: In response to the rapid spread of COVID-19 in 2020, the government supported facilities and equipment through the 'Urgent Isolation Ward Expansion Project'. Design and remodeling of efficient negative pressure isolation facilities had to be done in a short period of time, and the performance gap between facilities was very large because the types of hospitals and wards of existing medical facilities were diverse. In order to secure the stability of isolation wards between medical facilities and reduce the facility gap, guidelines for planning isolation wards considering the diversity of each hospital should be appropriately presented. In consideration of these points, this study aims to provide basic data for future remodeling guidelines for each plan type of the negative pressure isolation ward first. Methods: We analyzed the plans before and after the change of 13 case hospitals that performed the urgent care bed expansion project for COVID-19 confirmed patients. Before the remodeling, the current status of the facility was analyzed according to the type of corridor, the location of the nursing station, and the location of the elevator. After remodeling, the flow of medical staff and patients, the flow of entry and exit of clean and contaminated items, and the space of negative pressure and non-negative pressure areas. Results: The ward type was divided into three types according to the corridor type and room arrangement: double loaded corridor type with two side wards, race track type with one side ward, and race track type with two side wards. Based on these three types, the standard floor plan type of the isolation ward was proposed in terms of the location of the elevator bank and Nurse station. Implications: When the existing general ward is converted into a negative pressure isolation ward, this study can be a basic data to present customized guidelines for each ward type.
Purpose: This study aims to emphasize interrelation between healthcare policies, design standards and hospital architecture of the United States since 1950s; to examine spatial characteristics of inpatient care facilities through case studies; and to consider the social implication of these spatial changes. Methods: In this study, reviewing the overall healthcare system, design standards and inpatient care facilities of the United States since 1950s, a total of five inpatient care facilities, one for each period, were selected in order to analyze the spatial characteristics. The spatial maps of Space Syntax were employed for analyzing five case studies. Results: The distance between the nursing station, the support service, and inpatient room were getting closer. The spatial structure of inpatient care facilities is transformed from tree structures to annular tree structures. This result shows that the efficiency between patient, staff and support service is higher and the depth of the spaces is getting deeper, which indicates that efficiency for improving healthcare quality affect the spatial structure of inpatient care facilities. Implications: In the future, if Korea's health policy is changed to a demand-oriented health care policy, this conclusion predicts medical planning of hospital will be focused on the efficiency.
Purpose: As the need for a hospital specializing in infectious diseases has increased, construction is being promoted. Hospitals specializing in infectious diseases receive some state subsidies, but in the case of private hospitals, hospital operation efficiency should be considered to prevent cost loss. Therefore, we aim to derive a building plan for a general ward in a hospital specializing in infectious diseases that can be used not only in normal times but also in times of crisis. Methods: In this study, relevant literature review and field interviews were conducted with medical staff working in facilities designated as infectious disease hospitals. Results: The general ward building plan of the hospital specializing in infectious diseases was classified into three categories and presented. 'Spatial composition' for nursing unit and ward zoning, 'Spatial plan' for ward space conversion in normal times and crises, 'Bedroom plan' for effective dimensions and area of the ward. Implications: It can be used as a guideline when designing an infection-facility ward. And it can be a basis for inducing improvements to prevent infection in the ward of existing medical facilities.
Purpose: Regional public hospitals have implemented functional reinforcement projects, and the facilities to accommodate them have increased in size. Nevertheless users in hospital are concerned about space shortage and area imbalances. Therefore I will trace the factors and trends that influence the size, and derive the relationship between these and the uses' critical opinion. Methods: Among the indicators for determining the size of medical facilities, the number of beds and total floor area are the essential indicators that directly affect the composition of space and allocation of area inside the medical facility. The purpose of this study is to investigate the change and the factors of change on the these two indicators in regional public hospitals and analyze the trend of changes. Results: In accordance with support undertaking, regional public hospitals have been increased the number of chronic-based beds and expanded additional facilities such as O.P.D specialized centers, emergency centers and funeral homes for reflecting the needs of the regions and times. However, as a result of analyzing the area, regional public hospitals are growing in size mainly on the ward and O.P.D is only expanded the scope of functional reinforcement division but total area level of O.P.D is lower than the recent level. In addition, the levels of D&T, Supply, mechanical/electrical equipment area related to medical support and control environment quality are very low. This is because the functional reinforcement projects have been done without concerning diagnose the whole facility. Implications: If functional reinforcement projects are conducted, to cope with problems of space shortage and imbalance of area, it is judged that an architectural planning that comprehensively analyzes existing facilities and related departments should be included.
Purpose: The purpose of this study is to classify space types according to the operating system of health examination centers and compare and analyze their sizes. Methods: Seven examination centers under the K Medical Research Institute with the same operating system and similar examination types and functional spaces are the subject of the investigation. Research is conducted through field investigation, user surveys, and drawing analysis. Results: The operating method of the health examination center can be largely divided into the function dispersed type and the function central type. The function dispersed type was planned as a vertical type, and the function central type was planned as a horizontal type. In the case of the function dispersed type, since the examinees move vertically to use the endoscope center and special examination center, the efficiency of the vertical movement must be considered when planning the function dispersed type of facility. The function dispersed type plans to increase work and manpower efficiency by arranging the areas used at the start and end of the examination. Because the function central type horizontally arranges related functions by area, it should be planned in a structure that makes it easy for examinees to find their way. Implications: Through this study, it is judged that it is possible to suggest architectural planning considerations that vary depending on the operation system of the examination center.
Healthcare technology has been growing and fostering cooperation between industry, university and hospitals as growth engines in korea. So, the medibio research institutes in hospital have been constructed to promote research and industrialization centering on healthcare technology. The purpose of this study is to investigate the cases of research institutes in hospitals, and search the characteristics of building organization of medibio research laboratory facilities. The building function include not only the research lab and business office reflecting on the development platform, and but assembly and meeting room in the ground level. Laboratory floor plans have three types, rectangular, rectangular+linear and linear type, one is traditional and efficient, the others are people and friendly. And building core types are correlated with lab space unit modules, single and double side core are shown in rectangular type. All the laboratories are open lab, composed with laboratory bench and research note writing desk facing the lab service and enclosed lab-support area. And they have communication space looking as warm and cozy common area for the innovation, convergence and collaboration. As the high risk of contamination and high standard for safety and security, equipment and facilities are well managed with biological environment including BSC, fume hood, PCR classification, eye washing and emergency shower.
Purpose: This study presents the results of the analysis on space utilization of kidney dialysis units in regional public hospitals, which plays a key role in local public medical services. The result aims to achieve safety from infection, allow comfort for the dialysis environment, and stability for medical support. The purpose of this study is to present fundamental data for architectural plans for the kidney dialysis unit, as well as to alleviate potential infectious diseases such as COVID-19. Methods: For research purposes, the investigation and analysis of space utilization were based on architectural floor plans, research papers and literature, related legal systems, and public statistics. Of the main 35 regional public hospitals, in regards to data accessibility, 15 facilities were selected to conduct the survey and analysis for the objective. Results: The space composition by area research results of kidney dialysis units in public hospitals are as follows: Firstly, most targets do not have required rooms in the access and support area, except for the hemodialysis beds in the treatment section. Secondly, the access area requires necessary room and space design that took into consideration of convenience and accessibility for patients. Thirdly, in regards to infection prevention and control, proper circulation and room plan is essential for storage and disposal of contaminated products and linen after use. For the treatment area, the arrangement plan needs to establish a visual connection between the isolation room, the nursing station, and the bed area. Additionally, consideration of circulation in the preparation, treatment, observation, examination, and all other rooms in the facility is required. Lastly, for the support area, the room is designed to consider adequate working and meeting spaces for the medical staff, consultation space for patients or guardians, separate storage and disposal of clean and contaminated items, and the storage of various equipment for dialysis. Implications: In preparation for the increase in chronic kidney failure patients and the spread of infectious diseases, such as COVID-19, the researched data demonstrates the basic guidelines for space composition of kidney dialysis units and the significant role of regional public hospitals.
This study was attempted to grasp the push-pull factors of urban to rural migrants in relation to services and infrastructure of rural welfare and culture. Online and offline survey were conducted for urban residents who were willing to return to the rural area and those who were already returning to the rural area. In the fields of health care, social welfare, and education, it was found that the satisfaction level of urban-to-rural migrants was relatively higher than those of latent urban-to-rural migrants. In the field of culture, leisure and sports, the level of satisfaction and concern were similar, so it was found that the expectations for the field were relatively high before returning rural area. As for the agreement degree to expand support, the demand for emergency medical facilities and dementia care program was the highest, and the demand for health care was found to be relatively high. The results of the survey on the top priority items indicate that latent urban-to-rural migrants require support of facility and space item and expansion of program item, and urban-to-rural migrants have high demand for expansion of program item in all field except healthcare field. The results of this study are expected to provide useful information for establishing the direction of the rural area development project in connection with the revitalization of policy of people return to rural area.
