A Study on Healing Environments and Evidence-Based Design - Focused on Healthcare Environments in the United States - (치유환경 연구문헌 고찰 및 근거중심디자인(Evidence-Based Design) 활성화에 관한 연구 - 미국 치유환경 연구사례를 중심으로 -)

  • This paper assesses the evidence of the built environments' impact on patients' and staff's well-being. It also introduces evidence-based design (EBD) and discusses barriers and solutions for promoting EBD. EBD is an informed approach to design by improving design decisions based on best available knowledge. Designers should provide psychologically and physically supportive environments to promote occupants' well-being. There is increasing evidence that a poorly designed environment elicits negative feelings and stress. Better design will lead to better outcomes especially when it applied to a vulnerable population such as patients, children, and the elderly. Due to high demands of promoting patients' health, healthcare providers and professional designers have been searching for evidence to create healing environments. EBD has been well received in healthcare facilities design and is expanding to other types of environmental design such as educational and commercial design. Design firms should consider EBD as a value-added component of design to meet current and future challenges. It is important for designers to realize that engaging in EBD is not a rejection of creativity, but a means by which to scrutinize their design projects. With critical literature reviews on healing environments, there is sufficient evidence that well-designed environments can promote patients' and medical staff's health outcomes.

  • Recently, U-Healthcare is receiving attentions as a research for reducing the manpower, time in treatment, and etc. Although fundamental technologies, such as sensing, measuring, and etc. are sufficiently investigated. However, Technologies of IT/Medical convergence, which graft IT technologies to medical area, are still in germ. For this, we present a novel healthcare system, which can be applied to the middle sized medical environment, such as private hospital, home, or etc., by means of pre-verified technologies and the expert system. There exist IT element technologies are sufficiently developed in the fields, such as network, database or etc. due to the remarkable developments in IT technologies, and the healthcare is a mission-critical environment. Therefore, it is important not only to investigate novel approaches but also to utilize verified technologies for the U-Healthcare solution. Presented solution provisions automated medical services based on expert system by utilizing the measured data, such as body fat, blood pressure, blood glucose, and etc., in order to provide convenient treatment environment to doctors and nurses. In addition, since people, who do not have medical knowledge, can self-diagnose themselves, it is expected to cut medical costs in various areas. Especially, since each devices communicate with each other through standardized Bluetooth technology, Presented healthcare system is an extensible solution which can easily accept various medical devices. As a result of this, we can safely say that the self measurement and diagnosis services in U-Healthcare are now enhanced by reducing medical cost through our healthcare system.

  • This study aims to propose planning guides and general suggestions in the remodeling of general hospitals. Among the many goals in the hospital remodeling, this study focuses on the enhancing the 'healing environment' to existing building. Accordingly this study analyzes design criteria for 'healing environment' through the case study of 'A' hospital which has been remodeled recently, and compares those criteria with actual execution.

  • In the environment of Internet of Things (IoT), IoT devices are limited by physical components such as power supply and memory, and also limited to their network performance in bandwidth, wireless channel, throughput, payload, etc. Despite these limitations, resources of IoT devices are shared with other IoT devices. Especially, remote management of the information of devices and patients are very important for the IoT healthcare service, moreover, providing the interoperability between the healthcare device and healthcare platform is essential. To meet these requirements, format of the message and the expressions for the data information and data transmission need to comply with suitable international standards for the IoT environment. However, the ISO/IEEE 11073 PHD (Personal Healthcare Device) standards, the existing international standards for the transmission of health informatics, does not consider the IoT environment, and therefore it is difficult to be applied for the IoT healthcare service. For this matter, we have designed and implemented the IoT healthcare system by applying the oneM2M, standards for the Internet of Things, and ISO/IEEE 11073 DIM (Domain Information Model), standards for the transmission of health informatics. For the implementation, the OM2M platform, which is based on the oneM2M standards, has been used. To evaluate the efficiency of transfer syntaxes between the healthcare device and OM2M platform, we have implemented comparative performance evaluation between HTTP and CoAP, and also between XML and JSON by comparing the packet size and number of packets in one transaction.

Critical Analysis of Unmet Healthcare Needs Index for Addressing Regional Healthcare Inequality (지역 의료불평등 해소를 위한 미충족 의료지표 활용의 비판적 분석)

  • Background: Unmet healthcare needs have many advantages for measuring inequalities in healthcare use. However, the existing indicator is difficult to capture the reality of unmet healthcare needs sufficiently and is not quite appropriate in comparing regional inequality. The purpose of this study is to critically analyze the utilization of the unmet healthcare need indicator for regional healthcare inequalities research. Methods: We used the level of healthcare accessibility and healthcare need to categorize the regions that are known to cause differences in healthcare utilization between regions and verified how existing unmet healthcare need indicator is distributed at the regional level. Results: Four types of regions were classified according to the high and low levels of healthcare needs and accessibility. The hypothesis about the regional type expected to have the highest unmet healthcare need was not proved. The hypothesis about the lowest expected regional type was proved, but the difference in the average rate of unmet healthcare needs among regional types was not significant. The standard deviation of the rate of unmet healthcare needs among regions within the same type was also higher than the overall regional variation, which also disproved the whole frame of hypothesis. Conclusion: Failure to prove the hypothesis means the gap between the supposed meaning of the indicator and the reality. In order to understand the current state of healthcare utilization of people in various regions of Korea and to resolve inequality, fundamental research on the in-depth structure and mechanisms of healthcare utilization is needed.

  • Korea's healthcare is in great danger of sustainability. In 2020, the baby boomer will begin to be older, and there is no promise that the total fertility rate of 1.0 or less will rebound, and Korea's economic growth rate is predicted to be less than 2%. Together with these phenomena, Plan for Benefit Expansion in Nation Health Insurance (Moon Jae-in Care) will seriously threaten the sustainability of health insurance finance. In addition, health care in Korea has many problems: excessive medical utilization, rapidly increasing elderly medical costs, concentrating patients into big hospitals, low healthcare personnel but many healthcare facilities and equipment, bad quality of primary and mental care, and fast-growing health expenditure. For sustainability, healthcare of Korea should be reformed. The direction of the reform is people-centered and integrated healthcare in the community which is composed of empowering and engaging people, strengthening governance and accountability, reorienting the model of care, coordinating services, and creating an enabling environment.

  • Peritoneal dialysis (PD)-related peritonitis is a major cause of injury and technique failure in patients undergoing PD. Aeromonas hydrophila is ubiquitous in the environment, and is a Gram-negative rod associated with infections in fish and amphibians in most cases; however, it can also cause opportunistic infections in immunocompromised patients. We report a case of A. hydrophila peritonitis in a 56-year-old male on automated PD. Peritonitis may have been caused by contamination of the Set Plus, a component of the automated peritoneal dialysis device. Although Set Plus is disposable, the patient reused the product by cleansing with tap water. He was successfully treated with intraperitoneally-administered ceftazidime and has been well without recurrence for more than 2 years.

  • Consistently collecting a variety of vital signs is crucial in u-Healthcare. In order to do so, IoT is being considered as a top solution nowadays as an efficient network environment between the sensor and the server. This paper proposes a transmission method and compression algorithm which are appropriate for IoT environment. Results were compared to widely used compression methods, and were compared to other prior researches. The results showed that the compression ratio of our proposed algorithm was 11.7.
