중환자실 시설기준 작성을 위한 기본시설 수준 설정 연구 - 국내·외 시설기준 및 국내 종합병원 병상규모 별 시설 현황분석을 기초로 하여 (A Study to define the range of sample size for setting Intensive Care Unit(ICU) facility guidelines - Focused on analysis of guidelines and present of ICUs facilities by general hospitals size)

  • Purpose : This study is a basic research to establish guidelines for healthcare facility in Korean healthcare market. As a first step, it is a main purpose for making beginning point of ICU guideline to set a basic level of ICU facility size and quality by analysis current ICU facilities and existed domestic guidelines : law, criteria of healthcare insurance review and assessment service, credit of Korea institute for healthcare accreditation. Methods : First of all, the all requirements of existed guidelines are put together and summarized. The summary is compared with that of US, UK, and Australia to know its quality difference with international market. In addition, all hospitals in Korean market are classified based on total bed number and ICU bed number to know the most occupied facility size range in the market. Second of all, by comparing 15 ICUs' current setting of the public general hospital, a general condition of ICU facility would be extracted based on function, floor area per bed, services Results : 72.8% of hospitals in Korea are belonging below 500 beds hospital. Among them 200-299 beds hospitals occupied 35.3% and are shown as the most occupied hospital size. As 15 public general hospitals are analyzed, it is clear that the more bed size and services the more ICU area per bed. As a result it is sure that the 300~500 bed may be a clear line as a general ICU condition whose function and bed number relatively consistent in the range. Implications : to keep the qualified medical environment and contemporary hospital trend, the guideline as a minimum requirement keep naturally out from current healthcare settings and should reflect their limit to reconcile with the new trend in the market.

의료산업에서의 랜섬웨어 대응 방법 (How to Cope with Ransomware in the Healthcare Industry)

  • 의료산업은 세계적으로 빠르게 변화하고 의료서비스가 네트워크와 연결 되면서 다양한 형태의 서비스 제공이 검토되고 있다. 의료정보의 가치는 금융정보의 가치보다 높게 평가되고 있으며, 이로 인해 의료정보의 보호가 매우 중요해지고 있다. 랜섬웨어는 지속적으로 고도화 되고 있으며 정보의 가치가 높은 산업군을 대상으로 하고 있다. 특히, 2017의 랜섬웨어는 성장기를 지나 성숙기로 진입하면서 매우 다양하게 발전하고 고도화 되었다. 의료산업은 대부분 폐쇄망으로 구성되어 있었기 때문에, 악성코드의 위협에 대비가 부족하며, 이로 인해 랜섬웨어의 공격에 매우 취약하다. 단순히 의료산업의 보안표준에 명시되어 있는 기준을 충족하기 위한 보안이 아닌 실제 랜섬웨어의 유입을 효과적으로 막거나, 공격이 성공 했다 하더라도 그 피해를 최소화 하고 복구를 할 수 있는 방안이 추가 되어야 한다. 랜섬웨어는 매우 빠르게 진화하고 고도화 되고 있기 때문에 이에 대한 대비도 매우 빠르게 진행되어야 하고 실무적인 관점에서 접근을 해야 한다. ISO 27799, 27002 표준을 기준으로 진화된 랜섬웨어에 대응할 수 있는 요소를 도출하여 기존의 의료정보보호 시스템을 유지/관리 하면서도 랜섬웨어에 보다 효율적으로 대응할 수 있도록 하였다.

국내 종합병원 진단검사의학과의 면적산정 방법에 대한 연구 - 검사실 사례조사를 중심으로 (A Study on the Space Area Guideline of Clinical Laboratory in Korea - Focused on Laboratory Case Studies)

  • Purpose: Clinical laboratory of hospital has been demanded to extension or relocation regarding with the test number increase and analyzer development. Space area criteria per test numbers, lab functions, hospital bed and lab staffs are needed for draft space programing. So, the purpose of this study is to provide the space area guideline of clinical laboratory for space calculation in planning and design the spatial environment. Methods: Literature review has been used in checking the standards and guidelines. And questionnaire surveys to laboratory supervisors in hospitals have been conducted for the data collection. 60 answers have been analysed statistically by MS Excel program. Results: The result of this study can be summarized into three way calculations. The first one shows that the basic standard workspace and distance is applied in lab design. The second one shows that average space area criteria resulted from case studies is applied by 19㎡ per one staff, 0.9~1.0㎡ per one bed, and lastly linear length calculation of workbenches and analyzers on the bench top and floor mount, is multiplying it by the sum of the counter depth plus aisle width. Implications: In updating the space area calculation guidelines, it is necessary to cooperate with medical staffs and designers.

일본의 노인종합복지관 성공사례의 환경지원성 분석 연구 (Environmental Affordance of a Well Recognized Senior Center of Japan)

  • According to the 2005 Korean census, the 65 and over population now exceeds 9.5% of the total population and is growing rapidly. Meeting elderly environment and care needs of this rapidly growing segment of the population becomes a major challenge for public policies and planners. Since great deal amount of elderly will reside in their houses, aging in place concept becomes important. For the success of aging in place, the quality of individual house unit, community support systems, and quality of senior center of the community are crucial. Since elderly environments and facilities serve not only medical and/or care programs but also social activity program in aging society, senior centers need to promote social activities and other care programs. The purpose of this study is to analyze characteristics of environmental affordance of a well received senior center in Tokyo, Japan. In order to analyze the characteristics, Murtha & Lee user benefit criteria and Lawton's environmental affordance approaches were used. As results, design characteristics and interior elements which provide environmental affordance were enumerated by type of space. Based on needs and user benefit criteria, those features were analyzed. This study shows design characteristics, elements, and attributes which are well received and utilized by elderly users.

2015년 메르스 사태 이후 신축된 종합병원 병동부의 공간구성 특징 및 입원실 세부시설기준에 관한 연구 (A Study of the Spatial Composition and the Facility Criteria of In-patient Rooms in General Hospitals after MERS 2015)

  • Purpose: This study is to address the spacial composition of a standard ward and bedroom size for sake of infection control and efficient medical service. Methods: Spacial composition of a standard ward has been proposed by comparative analysis of 5 big hospitals' wards. Bedroom sizes have been explored on the ground of Health care facility regulations from Korea, USA, Australia and Canada. Of course, Literature and field survey have been conducted in order to draw out various bedroom sizes. Results: 16 basic and some other additional spaces have been proposed for the composition of hospital standard ward. Area of Single bedroom is $11.6m^2$, and that of multi-beded room is $7.4m^2$. Bed to bed Clearance is 1.5m, spacing between bedsize and hard wall is 0.9m in 1~2 beded room, 0.75m in 4-beded room. Space clearance between Foot side of bed and curtain is proposed as 0.3m and additional 0.9m is necessary for the circulation. Implications: The result of this study can be applied to the new cons.

네트워크 분석을 활용한 보건의료 및 간호관련 특허의 특징: 서술적 고찰 (Descriptive Review of Patents in Healthcare and Nursing: Based on Network Analysis)

  • Purpose: The significance of the healthcare industry has grown exponentially in recent years due to the impact of the fourth industrial revolution and the ongoing pandemic. Accordingly, this study aimed to examine domestic healthcare-related patents comprehensively. Big data analysis was used to present the trend and status of patents filed in nursing. Methods: The descriptive review was conducted based on Grant and Booth's descriptive review framework. Patents related to nursing was searched in the Korea Intellectual Property Rights Information Service between January 2016 to December 2020. Data analysis included descriptive statistics, phi-coefficient for correlations, and network analysis using the R program (version 4.2.2). Results: Among 37,824 patents initially searched, 1,574 were selected based on the inclusion criteria. Nursing-related patents did not specify subjects, and many patents (41.4%) were related to treatment in the healthcare delivery phase. Furthermore, most patents (56.1%) were designed to increase effectiveness. The words frequently used in the titles of nursing-related patents were, in order, "artificial intelligence," "health management," and "medical information," and the main terms with high connection centrality were "artificial intelligence" and "therapeutic system." Conclusion: The industrialization of nursing is the best solution for developing the healthcare industry and national health promotion. Collaborations in education, research, and policy will help the nursing industry become a healthcare industry of the future. This will prime the enhancement of the national economy and public health.

확장현실 기반 융복합 콘텐츠를 활용한 보건의료 분야의 연구 동향 분석 (Analysis of research trends in the healthcare field utilizing extended-reality-based converged contents)

  • 연구목적: 확장현실 기술은 의료 교육의 대화형 프로세스를 통해 자기주도적 학습 향상의기회를 제공한다. 본 연구에서는 확장 현실 기술을 기반으로 한 교육 콘텐츠를 적용한 보건의료 분야의 연구 동향을 분석하고자 하였다. 연구방법: 선정기준에 따른 문헌 검색, 선정, 제외 과정을 통해 확장현실 기술을 활용한 국내 보건의료 연구를 분석하여 최종적으로 관련 문헌 39편을 선정 및 검토하였다. 연구결과: 의학, 치의학, 간호 등 다양한 분야에 걸쳐 확장현실이 적용된 것을 확인하였다. 학업만족도, 몰입도, 흥미 유발 등 긍정적인 효과와 함께 미디어 사용의 어려움 및 사이버 멀미 같은 문제가 관찰되었다. 결론: 향후 연구에서는 보건의료 분야의 다양한 교과 과정에 대한 콘텐츠 개발과 적용을 통한 교육적 접근과 기술의 지속적 발전이 필요하다.

정보통신과 최신 보건의료기술 활용에 관한 간호교육 프로그램 학습성과 평가체계 개발 (The Development of a Program Outcomes Assessment System for Utilization of Information and Communication Technology and the Latest Healthcare Technology in Nursing Education)

  • 본 연구는 간호교육 인증에 필요한 '정보통신과 최신 보건의료기술 활용'의 프로그램 학습성과 평가체계 개발을 위한 방법론적 연구이다. 평가체계 구축의 이론적 타당성 확보를 위해 Kim 등의 평가모형(2009)과 Zimmaro의 루브릭 개발단계(2004)를 적용하였다. 수행준거부터 수행수준, 달성목표, 교육과정, 평가방법, 채점기준 및 자율 개선구조가 포함된 프로그램 학습성과 평가체계가 개발되었다. 개발된 정보통신과 보건의료기술의 학습성과 평가체계는 간호교육에 충분히 활용될 수 있다고 예상된다. 본 연구는 간호학 학사학위과정에서 프로그램 학습성과 평가체계 개발에 관한 구체적인 방향성을 제시한 점에서 의의가 있다고 본다. 추후 본 연구의 평가체계를 적용하여 학습성과 달성도를 분석하는 연구가 진행되기를 기대한다.

정신신경용계 의약품의 치료군 중복처방 평가기준(안) 개발 (Development of Criteria for Therapeutic Duplications of Psycho-Nervous System Drugs)

  • Therapeutic duplication (TD) is frequently reported inappropriate drug use in healthcare settings in Korea. This study was aimed to develop TD criteria for psycho-nervous system drugs (KFDA classification 117 and 119). ATC classification was used to determine therapeutic and/or chemical similarities among the 93 ingredients reviewed. Clinical practice guidelines, textbooks and product labels were referenced for principles and evidences of possible drug combination usage. 16 groups that listed ingredients to be considered as TD were established and the criteria would be helpful to prevent TDs.

국가지정 입원치료 격리병상 시설적격성 평가분석에 관한 연구 (A Study on the Facility Eligibility Inspection of National Inpatient Isolation Units)

  • During the progression of this study, we visited 17 medical institutions. These medical institutions underwent inspection for facility eligibility of national inpatient isolation units to care for patients. Seventeen consultation meetings were held to gather expert opinions, and conferences were held with the medical institutions and local levels of government. Based on these facts, we established a field application model of the criteria of safety-management and conducted a study on this method in order to create more scientific and systematic safety-management criteria including criteria regarding facilities maintenance and methods of equipment inspection. According to an analysis on the 17 medical institutions examined, facility eligibility of 17 national inpatient isolation units was found to be 78.3% on average. Through analysis of the present domestic and foreign conditions of medical institutions, we proposed improvements for when remodeling becomes necessary and established criteria for safety-management in national inpatient isolation units for the care of patients in accordance with the domestic healthcare system.
