Evaluation of Health-Related Quality of Life among Patients with Cervical Cancer in Indonesia

  • Background: Evaluation of health-related quality of life (HRQOL) in cervical cancer patients is important in order to design the interventions for improving patient outcomes. Reports of HRQOL among cervical cancer patients in Indonesia are limited. Moreover, measurement using EQ-5D-3L is to our best knowledge has hitherto not been performed. This study aimed to examine the HRQOL of cervical cancer patients in Indonesia using EQ-5D-3L. Materials and Methods: A cross-sectional study was conducted by interviewing cervical cancer patients using the EQ-5D-3L questionnaire. Percentages of patients who reported having problems in each dimension of EQ-5D as well as EQ-5D index score (utility) were calculated. Results: Our findings indicated that the most frequent reported problems were pain/discomfort (67.8%) followed by anxiety/depression (57.5%). The mean of EQ-5D VAS was 75.8 (SD=17.0). The mean (SD) utility scores were 0.85 (0.19), 0.76 (0.20), 0.71 (0.21), and 0.77 (0.13) for cervical cancer patients in stage I, II, III, and IV, respectively. Conclusions: Cervical cancer significantly affects patient HRQOL. Efforts should be made to improve the quality of life of cervical cancer patients especially in terms of pain /discomfort and anxiety/depression reduction.

일부 농촌지역 주민들의 OHIP-14와 EQ-5D에 관한 연구 (A study on OHIP-14 and EQ-5D of residents in some rural areas)

  • Objectives : OHIP-14 and EQ-5D were used, targeting the residents of farming communities to identify the elements that influence oral cavity's health and quality of life due to health and to identify the importance of oral cavity's health in order to increase health of adults' oral cavity and quality of life via improved health. Methods : This research was conducted from July 17th, 2010 to August 16th, 2010 targeting 600 residents in Goryeong-gun, Gyeongsangbuk-do, aging over 40. The data has been analyzed using Mann-Whitney U test, Kruskal-Wallis test and hierarchical multiple regression through SPSS Win Program 18.0 version. Results : 1. OHIP-14 and EQ-5D based on general characteristics showed lower oral health-related quality of life and health-related quality of life on the following cases: women (p=0.004, p<0.001), older (p<0.001, p<0.001), lower scholastic ability (p<0.001, p<0.001), lower average of average spending money (p<0.001, p<0.001), higher number of chronic disease (p<0.001, p<0.001), less drinking (p=0.012, p=0.008), lower perceived oral health and health status (p<0.001, p<0.001) and non smoking showed only EQ-5D (p<0.001). 2. OHIP-14 and EQ-5D based on oral health behavior showed lower oral health-related quality of life and health-related quality of life on the following cases: no periodic oral check-up (p<0.001, p<0.001), less experience of oral health education (p<0.001, p<0.001), horizontal tooth-brushing method(p<0.001, p<0.001) and lower frequency of tooth-brushing showed only OHIP-14 (p=0.042). OHIP-14 and EQ-5D based on oral health status and subjective oral symptom showed lower oral health-related quality of life and health-related quality of life on following cases: number of existing tooth less than 20 (p<0.001, p<0.001), the number of missing teeth more than 9 (p<0.001, p=0.044), DMFT (Decay, Missing, Filling Teeth) index more than 18 (p<0.001, p<0.001), wears denture (p<0.001, p<0.001), edentulous (p<0.001, p=0.002), have xerostomia (p<0.001, p<0.001) and have chewing discomfort (p<0.001, p<0.001). 3. Factors affecting OHIP-14 were gender, age, perceived oral health status, perceived health status, number of existing teeth, dental status, xerostomia and chewing discomfort, and the of reliability (how well it explains) the final model was 48.7%. EQ-5D showed relevance on gender, age, presence of chronic disease, perceived health status, xerostomia, chewing discomfort and oral health-related quality of life, and the reliability of the final model was 42.9%. Conclusions : In order to improve the quality of life of ruralists, oral health needs to be improved or remained by increasing the rate of possession of the existing teeth and preventing the loss of teeth. In order to do so, improvement of accessibility of dental clinic, change of direction from treatment-centered to prevention-centered health care system, development of oral health education program and various oral health care policies which would vitalize continuous oral health care system are considered to be necessary.

노인의 식생활 형편이 건강관련 삶의 질에 미치는 영향: 2011-2020년 국민건강영양조사 자료를 이용하여 (Effect of Dietary Lifestyle of the Elderly on the Health-Related Quality of Life: Based on the 2011-2020 Korea national health and nutrition examination survey)

  • Purpose : Rapid population aging in South Korea is emerging as serious social problems, and interest in the health-related quality of life of the elderly is also increasing. Therefore, this study attempted to provide basic data to develop measures to improve the health-related quality of life of the elderly by identifying the impact of the elderly's dietary lifestyle on the health-related quality of life. Methods : Utilizing raw data from the Korea national health and nutrition examination survey spanning from 2011 to 2020, this study focused on individuals aged 65 and above. After excluding cases with missing data, 12,731 individuals were included in the final analysis. Various statistical methods including frequency analysis, t-tests, ANOVA, Scheffé test and hierarchical regression analysis were performed using SPSS 25.0 for windows. The statistical significance level (α) was set at .05. Results : The main results of this study were as follows. Firstly, dietary lifestyle exhibited variations depending on gender, age, education level, individual income, basic livelihood type, household type, self-rated health status, and chewing difficulty (p<.001). Secondly, there were disparities in health-related quality of life (as measured by the EQ-5D index) based on gender, age, education level, residence, individual income, basic livelihood security, self-rated health status, and chewing difficulty (p<.001). Thirdly, the EQ-5D index showed that elderly individuals' dietary lifestyle adversely impacted their health-related quality of life (F=353.74, p<.001). Conclusion : Therefore, it is necessary to prepare policies to improve the health-related quality of life by maintaining a proper diet for the elderly, and customized support and management is required taking into account the elderly's gender, age, education level, individual income, basic livelihood security, household type, self-rated health status, chewing difficulty etc.

경기도 지역주민의 사회경제적 특성과 건강습관에 따른 건강관련 삶의 질: 남녀의 차이를 중심으로 (Health-related quality of life assessment according to socio-demographic characteristics and health behavior among Gyeonggi-do citizens: focused on gender difference)

  • Objectives: This study was conducted to investigate the correlation between the health-related quality of life assessment according to socio-demographic characteristics, health behavior, and BMI for Gyeonggi-do community population. Methods: The 2012 Community Health Survey data was used. The data were collected by using a cross-sectional study with face-to-face interview. The variables of smoking, AUDIT, exercise, BMI, and Euroqol EQ-5D instrument were selected for analyses. The Difference on the EQ-5D index among groups were tested with Mann-Whitney U test and Kruskal Wallis test. Results: The EQ-5D index for Gyeonggi-do community was 0.951(${\pm}0.104$). The index were significantly different by sex, age, marital status, education level, and occupation. The health-related quality of life showed lower EQ-5D index in women than men, older than young, and less educated. The index were significantly different by health behavior (smoking, drinking, moderate physical activity and exercise) for both men and women). For BMIs, the index was lower to under-weighted male and over-weighted female. Conclusions: The findings suggest that the future intervention of health promotion programs should be carefully designed and tailored by health behavior and obese groups.

Relationship between residential district and health-related quality of life in Chungnam industrial complex area

  • Objectives This study aimed to evaluate the relationship between residential district of people, such as power plant, steel-mill and petrochemical industries, and health-related quality of life (HRQoL). Methods Using a cross-sectional study design, we randomly recruited participants for our study from industrial areas (thermoelectric power plant, steel-mill, petrochemical industry) and rural areas. Logistic regression analysis was used to identify the relationships between Euro quality of life-5 dimension (EQ-5D) scores and living region, while controlling for sociodemographic characteristics. Results In adjusted model, quality of life decreased with increasing category of age and were lower for females than males. EQ-5D scores of people living in the vicinity of thermoelectric power plant were significant lower than those of people living the vicinity of comparison region (odds ratio, 1.59; 95% confidence interval, 1.00 to 2.53). Conclusions Living region of thermoelectric power plant, was strongly associated with scores on the EQ-5D. More research is needed to elucidate the mechanisms which makes the relationship with the living regions and HRQoL.

EuroQol 도구로 측정한 성인 공황장애 환자의 건강관련 삶의 질 수준 (Assessment of Health-Related Quality of Life among Patients with Panic Disorder Using EuroQol in Korea)

  • Objectives : The purpose of this study was to assess the health-related quality of life (HRQoL) for patients with panic disorder using EQ-5D and EQ-VAS, and to examine the relationship between health-related quality of life and clinical outcomes following treatment. Methods : 29 patients with panic disorder were recruited from the Seoul Metropolitan area and 20 patients were followed up after two months of outpatient treatment. The Panic Disorder Severity Scale (PDSS) was used to assess the severity of the panic disorder and the changes in symptoms. HRQoL was assessed with EQ-5D and EQ-VAS at baseline and at two months of treatment. Results : All enrolled panic disorder patients showed significantly impairment of HRQoL in view of the subscales of EQ-5D, EQ-VAS index scores. The severity of PDSS was correlated the HRQoL in the panic patients. After treatment, the EQ-5D, EQ-VAS index scores showed significant improvement. Conclusion : Panic disorder patients suffer from lower HRQoL as well as from symptoms of the disorder. They showed clinical improvement and a restored HRQoL with treatment. These outcomes suggest using the EQ-5D, EQ-VAS such as the HRQoL in the assessment of patients with panic disorder is essential.

우리나라 성인의 건강상태 및 건강행태가 건강관련 삶의 질에 미치는 영향 (Effects of Health Status and Health Behaviors on Health-related Quality of Life in Korean Adults)

  • Objectives: The purpose of this study was to investigate the effects of health status and health behaviors on health-related quality of life in Korean adults. Methods: The study was based on the 2017 National Health and Nutrition Survey. Of the total 8,127 people, 5,780 (71.1%) participated: 2,553 (44.2%) were male and 3,227 (55.8%) were female. The variables for this study were demographic factors, disease count, subjective health status, health behaviors, and health-related quality of life (measured by the EQ-5D Index). Results: After analysis, the statistically significant factors associated with health-related quality of life were gender, age, income, education, daytime health status, number of comorbidities (6 or more) and stress. Conclusions: Seven demographic factors have been identified as having a significant impact on health-related quality of life, categorized by model, gender, age and income. Therefore, it is possible that quality of life can be improved through proactive management and by improving subjective health status and health behaviors while in considering of demographic and sociological factors by providing indicators as per gender and age.

Construct Validity of a Korean Version of the WHOQOL-BREF and EQ-5D Questionnaire Applied on Various Cancer Survivors Undergoing Palliative Rehabilitation Programs

  • Background: Although the original version of the health-related quality of life (HRQOL) questionnaires are found to be acceptable, the cross-culturally adapted versions may not be comparable to their original version. Objects: To examine dimensionality and construct validity of two Korean versions of the brief version of the World Health Organization Quality of Life (WHOQOL-BREF) and EuroQOL-5 dimension (EQ-5D) questionnaires. Methods: A total of 77 cancer survivors undergoing palliative rehabilitation programs from two rehabilitation institutes was recruited from April 16, 2018 to June 26, 2019. The WHOQOL-BREF and the EQ-5D were filled out by the various cancer survivors following a particular session of rehabilitation programs. The scores were analyzed with Winsteps Rasch analysis computer program using the rating scale model. Rasch fit statistics were used to determine the dimensionality and the item difficulty calibrations of WHOQOL-BREF and EQ-5D. Results: All items except two, negative feeling, need treatment function and pain prevent activity (mean square [MnSq] = 2.42, 1.82 and 2.51, respectively), were found to be acceptable, while two items of the EQ-5D, anxiety/depression and self-care, were misfit (infit MnSq = 1.65 and 0.38, respectively). Item difficulty calibrations of WHOQOL-BREF match person ability measures (i.e., HRQOL) fairly well. However, the person ability distribution showed obvious ceiling effects for EQ-5D. All items of EQ-5D were appeared to be less challenged in comparison with those of WHOQOL-BREF. Conclusion: Item-level analysis using the Rasch model supports the quality of culturally adapted items used to measure the HRQOL one exception; that is, whether or not to include misfit items as part of the HRQOL measurements. Additionally, cancer survivors undergoing palliative rehabilitation programs appear to have more of a tendency to view the EQ-5D items as being more challenging than the WHOQOL-BREF.

임신과 출산 경험이 있는 여성의 구강건강 및 치과의료 이용 행태와 건강 관련 삶의 질(EQ-5D)과의 관련성 (The relationship between oral health and dental care usage behavior and health-related quality of life (EQ-5D) in women who have experienced pregnancy and childbirth)

  • 연구목적: 본 연구는 임신 및 출산을 경험한 여성의 구강건강 문제와 치과의료 이용 행태가 건강 관련 삶의 질(EQ-5D)에 미치는 영향을 평가하기 위해 제8차 국민건강영앙조사(2019-2021)의 자료를 분석하였다. 연구방법: 임신 및 출산 경험이 있는 대상자 2,389명 중 1,301명이 최종 분석에 포함되었다. EQ-5D에 대한 영향요인을 파악하기 위해 다중회귀분석을 실시하였다. 연구결과: EQ-5D에 영향을 미치는 요인으로는 일반적 특성(나이, 가계소득, 기초생활보장, 교육수준), 구강건강 문제(저작 문제, 저작 불편함 호소, 말하기 문제) 등이 있었다(p<0.001). 결론: 임신과 출산을 경험하는 여성의 구강건강 문제와 치과의료 이용 행태는 건강관련 삶의 질에 중요한 영향을 미친다. 본 연구결과는 여성의 계속적인 구강관리에 대한 기초자료로 활용될 수 있다.

Sex differences in health-related quality of life among older Korean adults with type 2 diabetes mellitus: a cross-sectional study

  • Objectives: This cross-sectional study examined sex differences in Health-Related Quality of Life (HRQoL) among seniors with Type 2 Diabetes Mellitus (T2DM). Methods: Data from the Korea National Health and Nutrition Examination Survey (2008-2020) were analyzed. The EuroQol-5 Dimensions (EQ-5D), a measure of HRQoL, was used. It comprises five dimensions: mobility, self-care, usual activity, pain/discomfort, and anxiety/depression, each with three levels. Results: Analysis of 3,826 older adults with T2DM showed a significant increasing trend in the EQ-5D Index from the 4th survey phase onwards (P for trend < 0.001 for both men and women). Men consistently reported higher EQ-5D levels than women across all survey years. Women's EQ-5D levels remained lower than men's, maintaining a decade-old disparity (P < 0.05). Men scored significantly higher (P < 0.05) in most EQ-5D domains, except for self-care and anxiety/depression, resulting in a higher total EQ-5D Index (P = 0.001). Increased comorbidities were strongly associated with lower EQ-5D levels in both sexes. Additionally, there was a negative correlation between the EQ-5D Index and refined grain intake for both sexes (P for trend < 0.001), with high-EQ-5D groups consuming fewer refined grains. Women in the high-EQ-5D group consumed more nuts, vegetables, and meat compared to men (P for trend < 0.05). Conclusions: Our study highlights the sex disparities in HRQoL among older adults with T2DM. The findings suggest the need for tailored treatment guidelines aimed at improving the HRQoL of elderly T2DM patients, with a focus on their sex-specific characteristics. Implementing these tailored guidelines could enhance the HRQoL of older women with T2DM and promote more equitable healthcare outcomes. This underscores the importance of considering sex differences to comprehensively improve the well-being of this population.
