소규모사업장에서의 근로자 금연프로그램 적용 효과 (Effects of the Program for Smoking Cessation of Workers in Small Workplace)

  • Purpose: This study was done to evaluate the program for smoking cessation of workers in small workplace. Method: The study cases were seventy workers who voluntarily participated in the above program and they were from small workplace (under fifty employees) at Yonugdeungpo-gu, Seoul. This program was composed of individual health education and distributing guide books and video. Nurses visited the workplace for the program once two weeks. Result: Nine works (12.9%) succeed and 10 worker(14.3%) failed in smoking cessation. Number of cigarettes smoked a day and level of nicotine dependency of fifty-one(72.9%) workers decreased significantly. Conclusion: The results indicated the program was successful for workers at small workplace. The continual management by nurses' regular visit was the moat important thing for successful program.

부업종사 근로자의 주관적 건강 상태에 영향을 미치는 요인 (A study on Factors Influencing the Subjective Heatlh Status of Dual job Worker)

  • 경제적 사유 및 자기계발 등을 이유로 부업을 택하는 근로자들이 점점 늘어나며 전체 경제활동인구의 1% 이상이 된 상황에서 국가 전체의 근로환경 및 개인의 삶의 질에 밀접한 영향을 줄 수 있는 부업 종사 근로자의 건강과 관련된 연구는 부족한 실정이다. 이에 제6차 근로환경조사를 원시자료로 활용하여 부업 종사자의 주관적 건강 상태에 영향을 미치는 요인들을 노동력 구조, 물리적 환경, 정서적 환경의 세 측면에서 살펴보았다. 그 결과 주업의 종사자 지위에 따라 주관적 건강 상태에 유의미한 차이가 있음을 알 수 있으며, 주업 근무 만족감을 높여주는 것과 더불어물리적으로는 장기간 지속되는 반복 업무를 하지 않고, 정서적으로는 차별 경험을 제공하지 않는 것이 근로자의 주관적 건강 상태에 있어 중요함이 통계적으로 검증되었다.

보건기관(保健機關)의 방문보건사업(訪問保健事業) 담당인력(擔當人力)들의 사업(事業)에 대한 태도(態度) 견해(見解)와 환자관리(患者管理) 양상(樣相) (Attitude and Management Contents of Health Workers Engaged in Visiting Health Service)

  • This study was conducted to use the feedback of health personnel to improve the Visiting Health Service. The data was collected from 471 Home Health Workers serving 24 local health organizations in the Kyung-Pook province. 62.5% of the respondents were men under thirty-nine years of age. 92.8% of the respondents are married. 47.1% had degrees from junior colleges. It was ascertained 52.7% of the workers visited their patients six to twelve times within a six month period. And one to three patients were visited per day by one worker. Workers of older age, higher job position, and more experience were more positive in their feedback about the program. In addition, local health center employees, including nurses, were more positive about the program. Younger workers with a higher level of education, less experience, and lower job position had more insight into the problems of the program. Deeper insight into these problems led to a more negative conception of the program. Older workers with higher ranking jobs were found to be most competent. in their jobs. Workers at the main health center were assessed higher than the workers at the health sub-center or the primary health post. In addison nurses at all centers were found to be slightly more competent than the nurses' aide. The primary health post established the highest degree of patient satisfaction. It was discovered that the more positive the workers felt about the program, the higher their patient satisfaction feedback. There was a positive correlation between management assessment and patient satisfaction. This means that better program management was found to produce higher patient satisfaction. Workers feel being more educated about patient management would lead to better service. However, they take no action to produce these results. Where the problems of the system are most commented upon, the need for further education is greatest. Through multiple regression analyses it is apparent that the assessment of patient management is the greatest variable affecting patient satisfaction of patients is dependent on the management by the visiting health worker. Therefore, the development of the visiting health program is highly dependant on the feedback of those workers with a negative conception of the program. So the development of programs, motivation, education and training must be established. These works would lead to active participation by visiting health workers in the improvement of the Visiting health program.

Achievements, Problems, and Future Direction of the Quality Control Program for Special Periodic Health Examination Agencies in Republic of Korea

  • The ultimate goal of the quality control program for special periodic health examination agencies is to diagnose the health condition of a worker correctly, based on accurate examination and analysis skills, leading to protect the worker's health. The quality control program on three areas, chemical analysis for biological monitoring since 1995, and pneumoconiosis, audiometric testing since 1996, has contributed to improve the reliability of occupational health screenings by improving the issues including standardization of testing methods, tools, diagnostic opinions, and reliability of analysis for biological monitoring. It has contributed to improving the reliability of occupational health monitoring by rectifying the following issues associated with previous monitoring: absence of standardized testing methods, testing tools that are not upgraded, mismatching diagnostic opinions, and unreliable results of biological specimen analysis. Nevertheless, there are issues in need of further improvement such as lack of expertise or the use of inappropriate method for health examination, and passive and unwilling participation in the quality control. We suggested solutions to these problems for each area of quality control program. Above all, it is essential to provide active support for health examiners to develop their expertise, while encouraging all the health screening agencies, employers, and workers to develop the desire to improve the system and to maintain the relevance.

The Trend of Occupational Injuries in Korea from 2001 to 2010

  • Objectives: This study is planned to assess the trend of occupational injuries in Korea from 2001 to 2010. Methods: Ten years of occupational injuries, from 2001 to 2010, were analyzed in order to investigate the changing profiles according to the various characteristics of injuries; economic sectors, age of the injured, and type of injuries. The changing profile of occupational injuries was investigated by comparison with an index-created relative value based on the number of cases of reference category. Results: The fatalities of construction, forest, agriculture, and service show the increasing trend. The nonfatal occupational injuries of the manufacturing sector were higher than those of other sectors in every year but the fatal occupational injuries of construction workers were higher than those of the manufacturing sector. Occupational injuries occurring due to amputation and those of slip and trip increased. The number of occupational injuries for the worker groups of 24 years old and below decreased and 45 years old and above increased. In comparison to the figure of fall from height, the figures of slip and trip or caught in equipment are higher in every calendar year. Conclusion: This study find out construction, forest, agriculture, and service sectors, aged worker with 45 years old and over can be target population for the strategies of occupational safety.

Risk Assessment for Metalworking Fluids and Respiratory Outcomes

  • Background: Metalworking fluids (MWFs) are mixtures with inhalation exposures as mists, dusts, and vapors, and dermal exposure in the dispersed and bulk liquid phase. A quantitative risk assessment was performed for exposure to MWF and respiratory disease. Methods: Risks associated with MWF were derived from published studies and NIOSH Health Hazard Evaluations, and lifetime risks were calculated. The outcomes analyzed included adult onset asthma, hypersensitivity pneumonitis, pulmonary function impairment, and reported symptoms. Incidence rates were compiled or estimated, and annual proportional loss of respiratory capacity was derived from cross-sectional assessments. Results: A strong healthy worker survivor effect was present. New-onset asthma and hypersensitivity pneumonitis, at 0.1 mg/㎥ MWF under continuous outbreak conditions, had a lifetime risk of 45%; if the associated microbiological conditions occur with only 5% prevalence, then the lifetime risk would be about 3%. At 0.1 mg/㎥, the estimate of excess lifetime risk of attributable pulmonary impairment was 0.25%, which may have been underestimated by a factor of 5 or more by a strong healthy worker survivor effect. The symptom prevalence associated with respiratory impairment at 0.1 mg/㎥ MWF was estimated to be 5% (published studies) and 21% (Health Hazard Evaluations). Conclusion: Significant risks of impairment and chronic disease occurred at 0.1 mg/㎥ for MWFs in use mostly before 2000. Evolving MWFs contain new ingredients with uncharacterized long-term hazards.

정신장애인가족의 임파워먼트와 자기옹호 증진을 위한 프로그램의 효과 (The Effectiveness of Program to Promote Empowerment and Self-Advocacy of the Mentally Disabled Persons' Families)

  • Purpose: This study was to verify the effectiveness of group program for promoting empowerment and self-advocacy of the the mentally disabled persons' families. Methods: This program was set up for the mentally disabled persons' families to share their experiences, do consciousness raising, advocate themselves through group process. A pre-post control group design was used for this study. Results: The results of this study verified that group program was effective to promote empowerment and self advocacy of the mentally disabled persons' families. Also group program helped to express and ventilate family burden actively. Conclusion: This study suggests as follows. In the mental health social work practice we need to develop group programs for family to talk about themselves, to ventilate their emotions, to understand their circumstances and to initiative the change of mental health circumstances as well as education and coping skills about mental illness. For this, there is a need for mental health social worker to have a macroscopic viewpoint and work with family. Also there is a need for mental health social worker to promote family self-help groups and to support their construction and growth.

산업장 근로자의 건강증진 프로그램 요구도 (Need Assessment for Worksite Health Promotion Program)

  • This study was conducted to investigate the kinds of health promotion program which workers want, worker's intention for participation, proper method, time, duration and manager. A self-administered questionnaire method was used to collect data from 412 employees of 3 worksites in Chungpook and Kyungkee. This survey was carried out from Aug. 10 to Aug. 20, 1998. The results of this study are as follows: 1. In male, among health promotion programs, favorite ones were physical fitness(32.2%), periodic health check-up(24.6%), and stress management(18.0%). In female, among health promotion programs, favorite ones were periodic health check-up(26.0%), physical fitness(22.0%), and body weight control(19.5%). 2. The more young subjects are, the more they like physical fitness program, and the female like body weight control program regardless of BMI. Manufacturing worker preferred back pain prevention program to clerical worker did. In female, drinker preferred stress management program to non-drinker did. 3. The more old subjects are, the more they like back pain prevention program, and non-smoker preferred body weight control program to smoker did. 4. In health promotion program format which the subjects wanted, learning of self-examination techniques was 41.1%, worksite screening was 3l.0%, availability of pamphlets and audiovisual materials was 20.0%, presentation of worksite educational sessions was 7.9%, and the most wanted manager for the program was medical doctor and then nurse, physical trainer, psychological counselor. The favorite health promotion program duration was less than 30 minutes(49.6%), and the favorite time was before work(49.6%). 5. Among respondents, 48.5% was smokers, 81.8% was drinkers, 39.9% engaged in the regular exercise, 68.2% engaged in regular diet habit. In vaccination, 50.2%, if possible, wanted to be vaccinated and 37.6% never wanted to be vaccinated. 6. Ex-smoker, ex-drinker, the subjects in the regular exercise, and in the regular diet habit responded they were in good health. There was a significant difference between exercise and health status. Need and intention for participation of health promotion program were high in ex-smoker, ex-drinker, the subjects in the regular exercise, and in the regular diet habit. According to results for this study, if the favorite programs, exercise, periodic health check-up, stress management program, are operated at a proper time and with managers they want, this programs can really raise the participation of employees. And as employees want to learn self-examination techniques if worksite educational sessions are performed, health promotion program can effectively be operated.

Concordance Rate Between the Ratings of Clinician and Self Ratings of Worker on a Functional Capacity Evaluation

  • Background: Functional capacity evaluations (FCEs) are designed to systematically assess the capacity to perform work-related tasks and to determine worker's ability to return to the previous job following work-related injuries. These evaluations may be rated either by clinician or worker. There has been a lack of consensus between the two scoring methods. Objects: This study aimed: 1) to confirm if the data are fit to the Rasch rating scale model and 2) to investigate the item-level concordance rate between the ratings of clinician and injured worker of the FCE. Methods: A cross-sectional study was conducted with a sample (n=124) of a rehabilitation program with the Occupational Rehabilitation Data Base for workers with low back pain. The functional capacity evaluation at admission and discharge was administered to clinicians and workers. The data were analyzed using both classical test theory-based Pearson's r and intra-class coefficient followed by item-level analysis with Rasch rating scale model. Results: All items of the FCE, except sitting items rated by clinician at admission and handling items rated by both clinician and worker throughout admission and discharge, were acceptable fit statistics with minor out of ranges for a misfit criterion. This may indicate that the items of the FCE overall fit to the Rasch rating scale model. Few problematic items responding differently to clinician and worker both at admission and discharge were detected with the differential item functioning analysis despite the excellent concordance rate using the two conventional statistics-sitting and handling items at admission and handling item at discharge. Conclusion: The item-level speculations using Rasch analysis of the FCE demonstrate that the ratings of clinician and self ratings of worker were psychometrically acceptable though there was an apparent discrepancy between the raters both at admission and discharge.
