의료보험 통합과 지역의료보험의 재정분석 (The Merge of the National Medical Insurance System and the Financial Analysis of the Medical Insurance Program for the Self-employeds)

  • In Korea, the institutional reform for the national medical insurance system is in process. Eventually, three kinds of the national medical insurance system, i.e., medical insurance program for the industrial workers, the govemment employees and the private school teachers, and the self-employeds, would be merged into an unifed system. In this study, I analyzed the annual trends of the finance in the medical insurance system in Korea, in which I found the financial instability especially in the medical insurance program for the self-employeds. The regression analysis was carried out to forecast the accumulated reserve at the end of this year for the medical insurance program for the self-employeds. I also analyzed the economic effect of the merge of the medical insurance program for the self-employeds by using the case of Japan and Korea. I found that the medical insurance for the self-employeds is expected to have financial deficit at the end of the year 1998 after the merge. In onclusion, it seems to be quite difficult to solve the financial instability in the medical insurance program for the self-employeds after it would be merged. That means that there would be a lot of problems on the way to the merge.

우수한 임상병리사 양성을 위한 교육제도 및 국가면허시험제도에 대한 설문조사 분석 (Survey on the Education System and National Licensing Examination for Fostering Competent Medical Technologists)

  • 본 연구는 대학 교육제도와 국가면허시험 제도에 대한 교수와 병원실무자들의 의견을 조사하여 분석함으로써 우수한 임상병리사 양성을 위한 정책방향을 결정하는데 목표로 하고 있다. 2016년 8월에 온라인 설문지가 255명의 대학교수들과 4,000명의 병원실무자들에게 배포되었다. 회수율은 교수들은 59명(23%), 병원실무자들은 1,099명(27.7%)이 응답하였다. 결과는 기술통계분석과 비교분석을 통해 처리하였다. 대학 교육제도에서는 교수들과 병원실무자들 모두 대학 및 임상실습 교육의 질적 향상과 교육의 표준화가 필요하다고 조사되었다. 국가면허시험제도에서는 교수들과 병원실무자들 모두 특히 실기시험의 개편이 필요하다고 조사되었고, 면허시험제도도 개선이 필요하다고 조사되었다. 위의 결과들은 교수들과 병원실무자들은 우수한 임상병리사 양성을 위해서는 대학 및 임상교육의 질적 향상과 국가면허시험제도의 개선이 필요하다고 생각하고 있음을 보여준다.

셀프메디케이션과 약사직능 (Self-Medication and the Pharmacy Profession)

  • Self-medication is a representative phenomenon in the domain of popular care, originated from perspective of medical pluralism and the pharmacy profession is said to be in the most appropriate position of health care professions to activate self-medication. As any healthcare reform impacts in a country, 2000 institutional separation between medicine and pharmacy implemented in Korea brought a lot of changes in behavior of drug use not only to physicians and pharmacists but also to consumers (patients). In this paper, the reality of self-medication since the institutional separation between medicine and pharmacy was analyzed, based on the empirical data which were collected by the Korea Institute for Health and Social Affairs in 2002. The major finding is that the domain of popular care including self-medication was significantly shrunk, while that of professional care was proportionally expanded. As a result, the following four points were strongly recommended for the purpose of promoting self-medication: i) upgrading the pharmacy education system from 4 year to 6 year level, ii) improvement of continuing education and introduction of GPP (Good Pharmacy Practice), iii) activating 'pharmacal encounter' (i.e., pharmacist-consumer relationship) and iv) promotion of socio-economic research activities and proactive participation in the international self-medication movement of pharmacists in Korea.

근로자의 구강보건에 대한 인식과 실천정도 조사 - 일개 사업장을 중심으로 - (A study on awareness and practice about the oral health of workers)

  • Objectives : The purpose of this study was to examine the oral health awareness and practice of workers. Methods : The subjects in this study were 200 workers in a working place located in Taean-gun, Chung-Nam Province. The data for this study was obtained by means of a self-administered questionnaire. Results : 1. An average of awareness for the oral health of workers was $2.60{\pm}0.44$ points out of 4. They were most aware of the item that toothbrushing should be done after meals($3.06{\pm}0.42$), and the second most widely recognized item was that toothbrushing should be done before bedtime($2.73{\pm}0.60$). 2. An average of practice for the oral health of workers was $2.54{\pm}0.32$ points out of 4. They did the best was brushing their teeth after meals($2.97{\pm}0.33$), followed by cleaning the tongue during toothbrushing($2.91{\pm}0.45$). 3. Awareness of oral health was significantly higher in such groups, including the female, college graduates, unmarried(p<0.05). 4. Practice of oral health was significantly higher in such groups, including the female, college graduates(p<0.05). Conclusions : To reform and improve of the system, incremental oral health care system for workers is needed. and oral health education is needed to increase the motivation of workers to control their basic dental disease.

미국의 사회보장연금 개혁논쟁에 관한 고찰 (Examining the Debate of Social Security Pension Reform in the United States by 1996)

  • 본고는 1990년대 중반부터 벌어진 미국의 사회보장연금 개혁논쟁의 내용과 쟁점 그리고 그 성격을 규명하는데 목적이 있다. 이를 위해 미국의 사회보장연금이 적립방식으로 시작했다가 부과방식으로 전환한 과정, 논쟁의 기본 안이 된 사회방청 자문위원회의 세 가지 안, 이 세 안을 다소 변형시킨 정치계와 기업계 및 학계의 각종 제안들을 분석했으며, 각 안들의 비교 분석을 통해 주요 쟁점들을 추출하여 비판적으로 논의하였다. 최대 쟁점은 개인계정의 신설 여부와 펀드의 관리운영 주체를 정부로 할 것인가 아니면 민간투자회사로 할 것인가의 여부 및 사회보장연금의 세대간 소득계층간 재분배 효과에 있다는 사실을 규명했다. 동시에 핵심 쟁점을 둘러싼 입장의 차이도 밝혔다. 민영화를 주장하는 입장(IA 및 PSA 지지자)에서는 사회보장연금의 세대내 소득계층간 재분배 기능보다는 개인적 형평성과 저축제고 효과 그리고 민간의 관리운영과 민간 자본시장에의 투자를 선호했고, 부과방식 옹호자들(MB 지지자)은 사회보장연금의 세대간 소득계층간 재분배의 기능의 여전히 유효하고, 부과방식 연금의 기본 틀을 유지하면서 약간의 개혁(사회보장 세율의 인상과 급여 수준의 하향조정)으로 재정불건전 문제를 해결할 수 있다고 보는 것으로 확인되었다.

학생체질검사제도에 대한 인식과 개선방안 연구 (A Study on the Improvement of Physical Examination for Students)

  • Recent domestic findings indicate steep increase of geriatric diseases such as obesity, diabetes, and high blood pressure among children and adolescents. It is, therefore, necessary to establish a new system for constitution inspection that reflects domestic adolescents' health conditions by the introduction of feasible items and systematic methods in the inspect ion. The objective of this study is to grasp the problems and to present proper ways of improving the system. As the method of the research during the period from September 2001 to March 2002 we refer to the domestic and foreign literature, and ask the advice of the specialists in pediatrics, family medicine, endocrine, cardiology. As on-site research we us e survey of nationwide elementary, middle, and high schools, visit representative local schools and have an interview with school nurses. Through the interview we try to find practical problems of school heath care and find proper method of students physic al examination. The abstract of the problems of student examination system is as following. To the question about the extent of the result of physical examination 46. 46.4%(school doctor) and 42.6%(parents and students) of the respondents answered "don't know". It means neglectfulness about examination itself. 62% of the school nurses responded "unsatisfactory" people who take charge of practical affairs have complaint about pre sent examination method. Because of the short time of the examination, and numerous students, the examination was incomplete. Several questions, method of physical examination, content of physical examination and list of physical examination, were raised. Reform measures for physical examination system of students were presented by physician, school nurses, students and parents of student.

방사선학과(放射線學科)의 4년제(年制) 대학(大學) 교육과정(敎育課程)에 대한 연구(硏究) (A Study on Four-year College Curriculum for the Education of Radiological Technology in Korea)

  • The education of radiologic technology began in the regular institute of higher education in Korea in 1963. Up to now from then, our education to bring up the radiologic technologists has developed greatly in quality and quantity, and now departments of radio-technology are founded in the 16 junior colleges in March, 1995. This study was done to verify the necessity and propriety to reform the education system of radiologic technology which was run as two or three year system of college curriculum for 32 years since 1963, and to search for the method to reform in the future. We got the following results from this research. 1. In the survey, on the desirable education year for radiologic technologists, 63.9 % of professors of department of radio-technology and 63.0 % of radiologic technologists chose the 4 year system, 27.9 % of professors and 34.6 % of radiologic technologists chose the 4 year system added to graduate school. 2. In the survey, on the future development of radiologic equipments and technique, 67.2 % of professors of department of radiologic technology and 86.4 % of radiologic technologists have a view of "revolutional development". Also, on the future tasks or roles of radiologic technologists 95.1% of professors and 94.9% of radiologic technologists have a view that "They will increase". 3. On the necessity of extension of education year to 4 years from 3 years, the factor is that development of medical technique and machinery and tools, advance of qualification of radiologic technologists, enlargement and specialization of the business of the radiologic technologists, ballance of education year with other medical sciences, international competitive power and cooperation of radiologic technology, and education continuation of the graduates of department of radiologic technology. 4. They suggested that in the 4 year curriculum of department of radiologic technology, clinical medicine, quality control of radiation and radiologic equipment, related subjects to the radiologic application and computer application should be enforced and clinical practice should be extended more. 5. On the device to found the 4 year college curriculum of radiologic technology, they suggested that first, 4 year curriculum should be founded through the variety of educational year, secondly, department of radiologic technology should be founded in the 4 year health sciences college, thirdly, educational continuation of the radiologic technologists should be systematized on the basis of life-long education.

물리치료원 독립 개원을 위한 제도적 장치 (The Legal System for the Independent Practice of Physical Therapy)

  • To provide physical therapy service of good quality keeping people healthy and obstain structural reformation coping with the demands of in medical service market to foreign intercourse on 21C, we should make legal system fer the physical therapy practice. Thus we suggest the Ministry of Health and Health and the authorities should, 1. Exclude the provision of physical therapist from the classfication of medical technician on the Medical Technician Law Article 2. and establish the independent Physical Therapist Law 2. Eliminate the provision of physician or dentist's guide the Medical Technician Law Article 1. or reform it to physician or dentist's request so that physical therapists may have a independent practice, or 3. Add the provision of the physical therapy center to the Medical Technician Law, the enforcement ordinances and enforcement regulations, such as the provision of optometrist or dental technician.

문재인정부의 보건의료정책 평가와 차기 정부의 과제 (Moon Jae-in Government Health Policy Evaluation and Next Government Tasks)

  • Moon Jae-in Care can be seen as a 2.0 version of Roh Moo-Hyun Care. Just as Roh Care failed to achieve its coverage rate goal and 30% share of public beds, Moon Care also failed to achieve its expected goal. The reason is that it followed Roh Care's failed strategy. Failure to control non-covered services has led to a long way to achieve a 70% coverage rate and induced the expansion of voluntary indemnity insurance, resulting in increased public burden. The universal coverage of non-covered services caused an immediate backlash from doctors. And Moon government also failed to control the private insurance market. The expansion of publicly owned beds has not become realized and has not obtained public support. Above all, it failed to overcome the resistance of doctors and failed to obtain consent from budget power groups in the cabinet for public investment. It was also insufficient to win the support of civic groups. Communication with interested groups failed and the role of private health care providers was neglected. The next government should also continue to strengthen health care coverage, but it should prioritize preventing medical poor and create a consensus with both medical providers and consumers for the control of non-covered services. Ahead of the super-aged society, the establishment of linkage between medical services and long-term care and visiting health care or welfare services is an important task. All public and private provisions and resources should be utilized in the view of a comprehensive public health perspective, and public investment should be input in sectors where public medical institutions can perform more effective functions. The next government, which will be launched in 2022, should design a new paradigm for health care in the face of a period of transformation, such as the coming super-aged society in 2026 and the Fourth Industrial Revolution, and recognize that the capabilities of the health care system represent the nation's overall capacity.

장애인서비스 전달체계 개편방안에 대한 비판적 고찰 (A Critical Perspective on the Central Governmental Reorganization of the Social Service Delivery System for People with Disabilities)

  • 장애인서비스 전달체계는 욕구평가체계의 결여, 사례관리체계의 미비, 전문인력의 부족 및 지방복지행정기관의 미비라는 문제를 안고 있다. 보건복지가족부는 장애인서비스 전달체계를 개편하기 위해 2007년부터 장애인복지인프라개선방안을 추진하고 있다. 개선방안에서는 장애판정체계의 개편과 장애서비스판정센터의 설립을 제안하고 있으며, 장애판정체계는 기존의 의학적 판정도구 외에 근로활동능력평가도구 및 복지욕구사정도구를 개발하여 장애인의 욕구평가체계를 도입하려고 시도하고 있다. 또한 장애서비스판정센터의 설립 모형으로 독립형, 지자체형, 공단형의 세 가지 모델을 제시하고 2008년 하반기에 시범사업을 실시하고 있다. 본 연구에서는 장애인서비스 전달체계를 개선하기 위한 핵심적인 요소가 보건복지가족부가 통제할 수 있고, 욕구평가와 사례관리를 수행할 수 있는 전문 인력을 갖춘 지방장애인복지행정기관을 120여개 설립하는 것이며, 현실적으로 한국장애인개발원을 정점으로 전국의 장애인복지관을 지방장애인복지행정기관으로 활용할 것을 제안하고 있다.
