Research Ethics Issues that Frequently Arise in Health Services Researches (보건학 연구에서의 주요 연구윤리 쟁점)

  • As part of efforts to internally strengthen the research ethics capacity of the Korean Academy of Health Policy and Management, we have compiled research ethics issues that are frequently encountered in public health researches. Firstly, when reusing research reports or dissertations as papers, efforts should be made to avoid unfair duplicate publication. Compliance with the institutional review board exemption process is required, and additional efforts should be devoted during the text recycling process to prevent self-plagiarism or unjust duplicate publication.

Study on the Significance of Health Services Research Methods for Studying Traditional Korean Medicine (한의학 연구에 있어 의료서비스연구가 갖는 의의 고찰)

  • Objectives : Many researchers in western societies raised questions on methodological limitations of randomized controlled trial (RCT) for studying Complementary and Alternative Medicine (CAM), and they have tried to find alternatives to RCT. Some of them recently have interest in health service research as one of the alternatives. Discussing the significance of health services research in CAM studies will be helpful to guide the researchers in Korea who are also interested in developing an appropriate research methodology for traditional Korean medicine. Methods : Previous studies on methodological issues of studying CAM and health service researches published in western societies until 2009 were searched mainly from Pubmed. The definition, main research domains, and research methods of health service research and the significance of it for researches on CAM were summarized. Results & Conclusions : Health service researches are important in two aspects. First, health service researches evaluate the actual practice of CAM, including the individualized diagnosis as well as treatment and interaction between practitioners and patients. Second, when evaluating the outcomes, they put an emphasis on subjective evaluation by patients as well as objective evaluation by doctors. Thus, it can reflect the treatment goals of CAM which is improving the overall quality of life and the physical and psychological health. Therefore, health services researches such as descriptive studies, qualitative studies, outcomes studies including economic evaluation, and whole systemic research should be more utilized in order for studying traditional Korean medicine.

The Relationship between Medical Operating Income and Volume of Medical Services Provided at General Hospitals in Korea (종합병원에서 진료량과 의료이익의 관계)

  • Purpose: We examined the relationship between operating income and volume of medical services provided at general hospitals in 2018 according to characteristics of general hospitals and measured as operating income(net income) and volume(adjusted inpatient days) covered or non-covered by National Health Insurance(NHI). Methodology: Finance data from income statement reports in 212 general hospitals and the national health insurance claim data of these hospitals were used. The characteristics of the general hospital were divided into structural, operational, financial, and patient aspects. Operating income and volume were divided into covered and non-covered by NHI. Findings: The results showed high volume hospitals tended to be more profitable than low volume hospitals, especially in non-covered services. Operating income was more likely to be sensitive to non-covered services volume than to covered services volume. Practical Implications: It is necessary to understand the volume of services in non-covered, in order to obtain reliable cost information to be used for the fee schedule. Researches on small size hospitals(<160 beds) are needed, with a large variation in the volume of services and a strong tendency to compensate for the loss in the covered part in non-covered part.

A Study on the Possible New Fusion between Mobile and Healthcare Service (모바일과 의료서비스 간의 새로운 융합 가능성에 관한 연구)

  • As many applications are possible now in mobile environment with the trend of mobile convergence, diverse applications in healthcare industry are also possible in mobile devices. Though lots of researches on mobile and health services are introduced, they are limited to specific area or techniques. This study shows possible directions of fusion between mobile technologies and health services in the future using a data mining technique called association rule analysis. The data used in this study is collected from web pages containing key words related to mobile technologies and health services. The analysis shows that current cases of fusion between monitoring based telemedicine and patients. It also shows another case of fusion between mobile hospital and medical screen charts. These show that fusion between mobile technologies and health services already began in industry. Association rules are found between well-being, city, diet, and sleep. The association rules containing security and privacy, though their associations are not so strong, also show that security and privacy of patient information should be protected in the future. The results show that the fusion of mobile technologies and health services is expected to provide health services to more users and larger areas. It is also expected to create new diverse business models in the future.

Future Directions and Perspectives on Soil Environmental Researches (토양환경분야 연구동향 및 전망)

  • This paper reviews the future directions and perspectives on the soil environmental researches in the 21 century. Previously, the principal emphasis of soil environmental researches had put on the enhancement of food and fiber productions. Beside the basic function of soil, however, the societal needs on soil resources in the 21st century have demands for several environmental and social challenges, occurring regionally or globally. Typical global issues with which soil science should deal include food security with increasing agronomic production to meet the exploding world population growth, adaptation and mitigation of climate change, increase of the carbon sequestration, supply of the biomass and bioenergy, securing the water resource and quality, protection of environmental pollution, enhancing the biodiversity and ecosystem health, and developing the sustainable farming/cropping system that improve the use efficiency of water and agricultural resources. These challenges can be solved through the sustainable crop production intensification (SCPI) or plant welfare concept in which soil plays a key role in solving the abovementioned global issues. Through implementation of either concept, soil science can fulfill the goal of the modern agriculture which is the sustainable production of crops while maintaining or enhancing the ecosystem function, quality and health. Therefore, directions of the future soil environmental researches should lie on valuing soil as an ecosystem services, translating research across both temporal and spatial scales, sharing and using data already available for other purposes, incorporating existing and new technologies from other disciplines, collaborating across discipline, and translating soil research into information for stakeholders and end users. Through the outcomes of these approaches, soil can enhance the productivity from the same confined land, increase profitability, conserve natural resource, reduce the negative impact on environment, enhance human nutrition and health, and enhance natural capital and the flow of ecosystem services. Soil is the central dogma, final frontier and new engine for the era of sustainability development in the $21^{st}$ century and thus soil environmental researches should be carried according to this main theme.

A Study on the Service Network for Mental Health and Welfare in Japan (일본 정신보건복지 서비스네트워크에 관한 연구)

  • Purpose: Economic and social pressures are driving Korea to reform its mental health services. However, it is not easy for the governments to find to the proper method for the mental health service network. This study is to find the mental health service network in Japan. Methods: The survey was conducted by researches and field studies. 1) Researches for mental health service network and facilities. 2) Field study is for Mental Health and Welfare Network in Tokyo. Results: The result of this study can be summarized into three points. The first one, Reform measures are beginning to promote the concept of "normalization" in japanese society. The second one, Facilities of Mental health and welfare system designed by level that can be providing places for people with mental problems. The third one, Facilities consist of barrier-free environment for people with mental problems.

A Study on Performance Perceptions of Hospital Coordinators (병원코디네이터의 직무성과 인식)

  • The goal of this paper is to estimate firm performance of hospital coordinators through a survey on employees of medical institutions placed in Busan and Ulsan. The survey for this paper is constituted by 34 questions of 4 groups related to firm performance, qualification requirement, general fact and a certificate of qualification. The survey was carried out from September 12th, 2008 to September 24th 2008 and 388 question sheets collected finally and used for result analysis. In the result analyses related to hospital coordinator and firm performance, we found some principal outcomes such that 88.6% of respondents approved that hospital image by patients and customers is raised, 87.7% of respondents approved that degree of patient's satisfaction on hospital services is raised, and 81.5% of respondents approved that employees recognized importance of services on customers, by hospital coordinators. In the result analysis on differences in firm performance by presence of a certificate of qualification for hospital coordinator, there were meaningful differences in degree of patient's satisfaction, financial performance and degree of employee's recognition on importance of services. From the all of the above results, we verified that establishment and improvement of educational process for hospital coordinator should be performed through researches on degree of patient's and costumer's satisfaction for hospital coordinator, degree of hospital coordinator's satisfaction for the job and surveys of requirements from related industry.

Analysis on Ubiquitous Home Services in Domestic Future Housing - Comparison with Research Results on Demand of Ubiquitous Home Services - (국내 미래주택관에서 구현하는 유비쿼터스 홈 서비스 현황 분석 - 유비쿼터스 홈 서비스 요구도 관련 연구 결과와 비교를 중심으로 -)

  • With the introduction of the ubiquitous technology, the housing culture centers have demonstrated future housing image or technology. But Ubiquitous Home Services in the future housing were more focused on implementation of environment for the future than exhibition that consumers can experience. The purpose of this study was to identify ubiquitous home services which were realized in domestic future housing and to compare those with results of previous researches on demand for u-services. For this study, field study was conducted with 3 domestic future housings and 14 previous studies on demand of u-services were analyzed by the standard of residents demand. The results of the study showed that u-services for housework and leisure life more than those for security and health, were applied in the future housing. However, residents prefered u-services for security and control of indoor environment to those for housework and leisure life. As a result, it was found thatu-services which residents needed mainly were discordant with those that were displayed frequently in future housing. Also developing u-services reflected the needs of residents, a test bed has to be established in the future housing and then pre-evaluaiton can be conducted.

Demand-supply of Advanced Practice Nurse (APN) and Alternative Benefit Strategies in the National Health Insurance (전문간호사의 수급 현황과 건강보험 급여화 방안)

  • Purpose: The purpose of this paper is to analyze the demand and supply of advanced practice nurses and suggest alternative benefit strategies in the Korean national health insurance. Methods: A revised demand & supply model was used to estimate the excess supply of APNs, and policy making process and key actors in the Korean health insurance were considered to develop a political approach to the APN issue. Results: The social demand for APNs is currently estimated to be less than 50% of its supply and the APN education program fell into difficulties in recruits. No reimbursement mechanism for APN's services in the national health insurance has given no economic incentive to hospital managers who have monopsony power in nursing labor market, which has caused the demand shortage of APNs in hospital industry. Payment for APN's services recognized as one of the most significant strategies to booster the social demand for APN's services should be carefully designed and implemented in the national health insurance. In line with this, key actors in health insurance policy decision-making include government, national assembly, labor unions, NGOs, civic groups, medical associations, and academia. Conclusion: The basic researches for APN's activities and cost-effectiveness analysis in clinical settings are required to support the strategies aforementioned. Constructing a policy network among key actors is able to make the payment strategy feasible, which will increase the socal demand for APNs.

Elderly Care System in the U-Health Environment (U-Health 환경에서의 노인 Care 시스템)

  • As our country is currently facing "aging society," people demands for qualitively and quantitatively better social welfare services. In particular, the abruptly rapid increase in the elderly population requires various welfare services and medical care related to the elderly. It is urgent to resolve these demands. Also, IT technology in Korea is developing rapidly, and U-Health, a part of the Ubiquitous field is under the actively ongoing researches. In this paper, we studied the Elderly care system model suing U-Health technology. We designed the care system of the residential environment designed specific for the elderly. Additionally, based on that design, we studied the elderly care system using the ubiquitous technology. Since this elderly care system combines the existing community-based model and IT technology in conjunction with other mobile devices, we may supply a new care service application, thus solving the problems of the existing care and medical services.
