Background: Oral cancer is the third most common malignancy in Saudi Arabia, the highest incidence of which is reported from Jazan province. The objective of this study was to evaluate the association of various locally used substances, especially shamma, with oral cancer in the Jazan region of Saudi Arabia. Materials and Methods: A hospital-based case-control study was designed and patient records were scanned for histologically confirmed oral cancer cases. Forty eight patients who were recently diagnosed with oral cancer were selected as cases. Two healthy controls were selected for each observed case and they were matched with age (+/- 5 years) gender and location. Use of different forms of tobacco such as cigarettes, pipe-smoking and shamma (smokeless-tobacco) was assessed. Khat, a commonly used chewing substance in the community was also included. Descriptive analysis was first performed followed by multiple logistic regression (with and without interaction) to derive odds ratios (ORs) and 95% confidence interval (CIs). Results: Mean age of the study sample (56% males and 44% females) was 65.3 years. Multinomial regression analysis revealed that shamma use increased the odds of developing oral cancer by 29 times (OR=29.3; 10.3-83.1). Cigarette (OR=6.74; 2.18-20.8) was also seen to have an effect. With the interaction model the odds ratio increased significantly for shamma users (OR=37.2; 12.3-113.2) and cigarette smokers (OR=10.5; 2.88-3.11). Khat was observed to have negative effect on the disease occurrence when used along with shamma (OR=0.01; 0.00 - 0.65). Conclusions: We conclude that shamma, a moist form of smokeless tobacco is a major threat for oral cancer occurrence in the Jazan region of Saudi Arabia. This study gives a direction to conduct further longitudinal studies in the region with increased sample size representing the population in order to provide more substantial evidence.
Objectives : This study investigated the effect of imbalance of autonomic nerve system on the idiopathic facial palsy by the comparison Heart Rate Variability results of Facial palsy group and healthy control group, and to clarify correlation between House-Brackmann Grade and Heart Rate Variability results. Methods : 119 idiopathic facial palsy patients and 88 health subjects who underwent HRV test were retrospectively reviewed based on medical records. We compared between the HRV results of facial palsy group and that of normal control group, and also compared the HRV results of facial palsy subgroup classified by House-Brackmann Grade. Results 1. All HRV results-Mean Heart Rat(MHRT), Standard Deviation of all the Normal RR intervals(SDNN), Total Power(TP), Very Low Frequency(VLF), Low Frequency(LF), High Frequency(HF), ratio betwween the Low Frequency and High Frequency power(LF/HF ratio) of facial palsy group are decreased compared to that of normal control group, especially SDNN, TP, VLF, LF, LF/HF ratio showed significant difference(p<0.05). 2. HRV results showed no significant correlation in House-Brackmann Grade. Conclusions : This study showed that lower HRV results of facial palsy group than normal control group and suggests that imbalance of autonomic nerve system related with facial palsy. HRV could be a objective tool to reflect condition of idiopathic facial palsy patients.
The purpose of this study was to investigate the effects of the nutrition therapy and drug(simvastatin) treatment on the levels of blood lipids for the hyperlipidemic patients according to genetic polymorphism of apo CIII. Subjects of the study consisted of 43(male: 9, female: 34 )hyperlipidemic patients registered to Kyung Hee Medical Center, Intakes of nutrients for the subjects were determined by 24-hr recall method through a personal interviews. The subjects were instructed to take the hypolipidemic and hypocholesterolemic diets. Compliance was monitored through food records, personal interview, and body-weight measurements. The patients changed their basal diet containing 40. 1g fat(182.7mg cholesterol./day) to a diet containing 30.7g fat(139.2mg cholesterol/day) after 12 weeks of nutrition therapy. The distribution of genotypes for apo CIII by Msp I was 32.5% of TT, 50.0% of TC, and 17.5% of CC. aT the beginning of the study, the level of blood triglyceride was the highest for patients with TT and TC. Following the nutrition therapy, patients with CC exhibited the largest drop of blood triglyceride. Following simvastatin treatment with nutrition therapy, blood LDL-cholesterol decreased in most of the patiens with TT. Meanwhile the distribution of genotypes for apo CII by the Fok I was 10.0% of TT, 47.5% of TC, and 42.5% of CC. The level of blood triglyceride was the highest for patients with CC. Following the nutrition therapy, patients with TC exhibited a significant drop for the blood triglyceride. Following simvastatin treatment with nutrition therapy the level of blood LDL-cholesterol decreased in most of the patients with CC. We concluded that dietary habits and food have been changed by nutrition therapy. And blood lipid levels have been decreased by the restriction of intakes of energy, fat and cholesterol. There was variation in the levels of blood lipids according to apo CII polymorphisms. The level of blood lipids responded to nutrition therapy and drug treatment in different was according to genetic polymorphisms. Accordingly, the choice of individualized therapy based on the patient\\`s genetic polymorphism is very important for effective therapy(Korean J Nutrition 33(8) : 813-823, 2000)
Purpose: Long-term survivors of childhood cancer appear to have an increased risk for the metabolic syndrome, subsequent type 2 diabetes and cardiovascular disease in adulthood compared to healthy children. The purpose of this study was to investigate the frequency of the metabolic syndrome and associated factors in childhood cancer survivors at a single center in Korea. Methods: We performed a retrospective review of medical records of 98 childhood cancer survivors who were diagnosed and completed anticancer treatment at Samsung Medical Center, Seoul, Korea between Jan. 1996 and Dec. 2007. Parameters of metabolic syndrome were evaluated between Jan. 2008 and Dec. 2009. Clinical and biochemical findings including body fat percentage were analyzed. Results: A total of 19 (19.4%) patients had the metabolic syndrome. The median body fat percentage was 31.5%. The body mass index and waist circumference were positively correlated with the cranial irradiation dose (r=0.38, P<0.001 and r=0.44, P<0.00, respectively). Sixty-one (62.2%) patients had at least one abnormal lipid value. The triglyceride showed significant positive correlation with the body fat percentage (r=0.26, P=0.03). The high density lipoprotein cholesterol showed significant negative correlation with the percent body fat (r=- 0.26, P=0.03). Conclusion: Childhood cancer survivors should have thorough metabolic evaluation including measurement of body fat percentage even if they are not obese. A better understanding of the determinants of the metabolic syndrome during adolescence might provide preventive interventions for improving health outcomes in adulthood.
Purpose: The purpose of this study was to apply association rule mining to explore the labyrinthine network of cerebral infarction comorbidity and basic data supply to develop cutting-edge physical therapy protocols for cerebral infarction with comorbidity Methods: From clinic records of enrollees of A Hospital in D city, patients over 18 years of age with cerebral infarction and cerebral infarction comorbidity were recruited as a case group. All diagnoses of that hospital were categorized according to the "International Classification of Disease (ICD)" diagnosis system. We extracted code I63 from the "Korea Classification of Disease (KCD)-4". Associated rule mining was done with a priori modeling and Web nodes to examine the strengths of associations among those diagnoses. The support and confidence values of associated rule mining results were examined. Results: The subjects of this study were 2,267 cerebral infarction patients. E11 (Non-insulin-dependent diabetes mellitus), E78 (Disorders of lipoprotein metabolism and other lipidaemias), G81 (Hemiplegia), I10 (Essential hypertension), and K29 (Gastritis and duodenitis) were high frequency diagnoses, being found in 10% or more of total diagnoses of cerebral infarction from frequency analysis results. The highest frequency diagnosis was 1,042 (46.0%) for I10. The second most frequent diagnosis was for E11(21.5%) while the third most frequent diagnosis was E78 (20.2%). Results from a priori modeling and Web nodes indicated that cerebral infarction has a strong association withessential hypertension, non-insulin-dependent diabetes mellitus, disorders of lipoprotein metabolism and other lipidaemias. Conclusion: Cerebral infarction is associated with hypertension, diabetes mellitus, and disorders of lipoprotein metabolism and other lipidaemias. The result of this study will be helpful to clinicians treating patients with cerebral infarction.
The objective of this study was to investigate the environmental effects on duration time of milk flow, peak milk flow, and average milk flow in teats, and to estimate their relationships with milk yields in morning and evening milking, and to provide fimdamental information for the further study on their relationships with clinical mastitis and somatic cell in milk. A total of 6,768 milking records were studied in 72 Holstein cows. The influences of season, parity, lactation month, and milking interval on characteristics of milk flow considering in linear model were significant(p<.05). Duration of milk flow was longest at milking in fall, past first parity and second month of lactation, and with milking interval over than 13.5hrs. Average milk flow rate and peak milk flow rate were highest at milking in summer, past first parity and 8${\sim}$10 months of lactation, and with milking interval over than 13.5hrs. Milk flow rate was positively correlated to milk yield, and negatively correlated to the duration of milk flow. However duration milk flow was positively correlated to the milk yield with high level of correlation coefficient(+.60). For the establishment of optimum selection criteria on these traits, other aspects such as the udder health, disease and respective economic weights of milk flow characteristics in this study must be considered to develop the indices.
Incidence of mastitis is highest in dairy cows and it is a leader of all diseases and the economic losses, in dairy farms. The objective of this study is to analyse the incidence of clinical mastitis using the lactation number, lactation month and season of calving. In this study, we used 3,779 lactation data and animal health records collected from 1990 to 2006 at the National Institute of Animal Science. Out of 3,779 cows, a total of 1,721 cows were reported with clinical mastitis, which was 46.3%. The frequency of mastitis increased from 36.9% from first lactation to 56.0% by the fifth lactation. As many as 766 cows (46.9%) showed a recurrence of mastitis after 14 days, apart for two or more and chronic mastitis that were recorded for 657 cows (20.3%). This came to a total of 3,010 cases that had clinical mastitis in the herd for sixteen years. The distribution of incidence of clinical mastitis was highest during the first month of lactation at 24.4%, and it reached to 43.4% by the third month. The incidence of mastitis was even higher for cows in first lactation, showing 28.6% in the first month, and 42.9 percent by third month of lactation. Cows calved during the summer and winter months showed higher rate of incidence with 59.9% and 57.9% within 30 days of lactation, respectively. It is urgently needed to establish a preventative management for heifer cows during their expected first lactation and the clinical specific dry-period management strategies that influence the rate of clinical mastitis during the next lactation. The aim of this study is to present information that might be useful to improve clinical mastitis prevention.

The purpose of this study is to identify the effect of ear acupressure therapy on premenstrual syndrome of the female college students. This research design of our study was a quasi-experimental design. Out of the 27 female college students, 13 were assigned to the experimental group and 14 to the control group. The data was obtained over 3 months from K college located in G city. The use of instrument to assess the premenstrual syndrome is Keele's VAS(Visual Analogue Scale) with the opening records. The subject of the experimental group received the ear acupressure therapy for 3 times for 60 days and the other control group did not get the ear acupressure therapy treatment. We analysed the data and extracted the values of percentage, mean, standard deviation, $x^2$-test, t-test, and Cronbach's ${\alpha}$ using SPSS program. The VAS score of the premenstrual syndromes before the ear acupressure therapy was 7.3(experimental group), 7.46(control group) but after the ear acupressure therapy, the VAS score of the premenstrual syndromes was 3.36(experimental group), 7.17(control group). The result of this study reveals that the ear acupressure therapy was effective in improving the symptoms of the female college students who had the premenstrual syndromes.

Purpose: The purpose of this study was to describe restraint use in care of patients with psychiatric disorders in an attempt to avoid unnecessary restraint use and provide information for developing standards regarding restraint use as a therapeutic maneuver. Methods: For this descriptive study, discharge records from N National Mental Hospital in the year 2009 were reviewed by trained nurses during Dec. 24, 2010 and Mar. 31, 2011. There were 596 restrains applied on 232 of 1,322 discharges. Data collected include general characteristic of patients, the frequency of restraint use, time since admission when restraint was applied, time of the day when restraint was applied, duration of restraint application, place of occurrence, reasons for restraint use, and degree of damage to the patent. Work experience of nurses who applied restraints, number of workforce at the time of restraint, and season of the year was also identified. Descriptive statistics, Chi-square test, t-test, ANOVA, $Scheff{\grave{e}}$ and Jonckheere-Terpstra were applied using SPSS 14.0 to analyze the data. Results: There were 596 restraint uses among 232 patients. Restraints were applied most frequently on males in their 40s, patients diagnosed with schizophrenia, and patients repeating admissions more than 6 times. Restraints were frequently applied within first week following admission, between 16:00 and 20:00, and the average duration of restraint was 5 hours. There were significant differences according to diagnoses of patients in the season restraint occured, time, place of occurrence, reason for restraint, and duration of restraint. Patients with alcoholism received longer restraint application. Conclusion: In order to avoid unnecessary restraint use in patients with psychiatric disorders, nurses and other health care team members need to acknowledge a group of patients such as patients with schizophrenia and alcoholism who relatively frequently restrained or receiving longer restraint. Reasonable and careful decision need to be made when applying restraint in the care of patients with alcohol problem.
Objectives : 'First-night effect' has been a well-known concept since 1960's. It is important because it is one of the major factors to be considered in assessing the reliability of polysomnographic data. However, 'reverse first-night effect' has also been described, resulting in the inconsistency of conceptualization. We attempted to investigate on the first-night effect in adults by having each of them take two nights of polysomnography in a controlled environment. Young healthy adult volunteers were chosen as subjects in order to rule out age- or health-related confounders. Methods : Polysomnography was performed on eight male medical students (mean $age=23.5\;{\pm}\;0.9$) for two nights with Grass model 78 polysomnograph. We scored manually under the standard protocol each epoch of the sleep records. Sleep variables were obtained and compared between the two nights. Results : Sleep period time(SPT) and total sleep time(TST) of the third fraction of night were significantly longer on the first night than on the second night (p<0.05). However, other sleep variables such as percentage of each sleep stage, sleep latency, REM sleep latency, number of waking, and sleep efficiency were not different between the two nights. Conclusion : We could not confirm the existence of first-night effect in this study. In healthy young male adults, it may not happen at all or may happen to a very negligible degree. Young healthy adults may have more adaptability to a new sleep environment. Also, the provision of a reasonably comfortable sleep environment could have helped them with abolition of first-night effect.
