플라톤의 건강관에 대한 고찰 -"국가"를 중심으로 - (A Study on Platonic View of Health in "Politeia")

  • A purpose of this study is to understand Platonic View of Health in $\boxDr$Politeia$\boxUl$. Though Plato was not so much a doctor as a philosopher. he had health care of children at heart. He mapped out an ideal type of nation in $\ulcorner$Politeia$\lrcorner$. and he founded a Akademeia in order to realize his dreams. In his course of education. he put emphasis on the problem of health. He extended poetry education for mental health and physical education for physical health. He placed high value on mental health above physical health. and poetry education corresponds to our reading education of today. He perceived that reading had a considerable influence on mental health promotion. According to his assertion, life style, too. had something to do with health condition. To lead a simple. temperate life makes one' health promote, on the other hand, to lead a complicated, intemperate life makes one' health injure. Morever, he approved of a eugenic marriage and the law of jungle. If one is unable to take care of one' health oneself. he would rather die than live. We cannot accept this proposal by general consent. but we cannot be too careful of our health. We can draw out a philosophy of health from Platonic View of Health. For example. the importance of health education. the preference of mental health. the influence of reading education. and responsibility for self-care, etc. We need to establish a philosophy of health scientifically by lasting study of records.

Do Degree Programs Affect Health Profession Students' Attitudes and Opinions Toward Vaccinations? An Italian Multicenter Study

  • Background: Healthcare workers' attitudes toward vaccination have been widely described in the literature, but a restricted amount of studies assessed healthcare students' knowledge, attitudes, and opinions on this issue. This study aimed to estimate the influence of a degree course on knowledge and immunization behavior among healthcare students and to compare medical students with students from other health profession degree programs to identify possible differences. Methods: A multicenter, cross-sectional study was performed in 2018 in 14 Italian Universities (3,131 students were interviewed). A validated questionnaire was used to assess knowledge, attitudes, and opinions toward vaccinations, with a specific focus on influenza vaccine and attitudes toward mandatory vaccination policies. Statistical software STATA® 14 was used. Results: Significant differences were recorded between medical students and other healthcare students. The intention to get vaccinated against influenza during the next season and having been vaccinated in the previous season was higher in the medical group (p < 0.001). In the group of students of other health professions, we registered a lower probability of identifying themselves as a high-risk group for contracting infectious diseases as a consequence of their profession and health status (aOR 0.49; CI95%: 0.40-0.60) and an increased likelihood of defining their level of knowledge on vaccine-preventable diseases and related vaccinations as "insufficient/sufficient/fair" (aOR 1.31; CI95%: 1.11-1.56). Conclusions: Results show several differences between medical students and students of other health professions when it comes to vaccination knowledge, attitudes, and perceptions, as well as a general low tendency to be vaccinated against influenza.

보건진료원의 업무 및 보건진료소 운영에 관한 고찰 (Duties of Nurse Practitioners in the Community and Management of Primary Health Care Posts)

  • By the rural area health care special law in 1980, Primary health care posts were established in rural areas as fundamental elements of the national health system. Nurses have been deployed to the posts after taking an education course mandated by the special law. However, health care posts have confronted environmental changes over the past 30 years such as an aging and decreasing rural population and advanced traffic systems, which make it necessary to reshape their form and role. Therefore, some guidelines are suggested for future role enlargement of health care posts by analyzing their current management and duties. The guidelines are as follows: 1) enlarging the portion of prevention and management of chronic degenerative diseases, 2) development and practice of diverse health promotion programs, 3) extension of primary health care for the increasing older population, 4) development of health programs for married immigrants, 5) practice of timely maternal child health programs, 6) development of adequate health care posts for low-income people in rapidly urbanizing rural areas and in poor areas in big cities, and 7) revision of laws and institutional arrangements for the role enlargement of health care posts to match social changes and customer needs.

보건대학원 사이버 수업에서의 문제중심학습 (Web-based PBL (Problem Based Learning) in Graduate School of Public Health Courses)

  • Objectives: This paper is aimed to study the satisfaction level and course effects of web-based PBL of the Graduate school of Public Health. Methods: Web-based PBL was implemented from March 1 to June 22 on 19 students and a survey on the satisfaction level of the lessons was taken and analyzed. For the analysis, SAS 9.1 was carried out. Results: The relation of effectiveness of the lessons according to the satisfaction level of each evaluation items, shows a significance according to satisfaction of professors, whether or not the students were satisfied in evaluating themselves (correlation, p<0.05). The satisfaction level of the evaluation on teachers and whether or not the students were satisfied in evaluating themselves shows significant influence on the effectiveness of the lessons (simple regression, p<0.05), more specifically in active class participation and understanding by the students themselves (stepwise multiple regression, p<0.05). Conclusions: In this study the attitudes of professors and students toward the lessons are major influences on the effectiveness of the lessons. Specifically, active class participation and understanding by the students themselves are the most important influences.

우리나라 신체활동 및 운동사업에서의 인구집단 전략 (Population Strategy for Physical Activity in Korea)

  • 현재 건강증진에 대한 다양한 논의가 진행 중으로 학자마다 정의가 다르고 동일한 학자도 문맥에 따라 다르게 사용하고 있는 경우가 많으며, 건강증진 내용의 스펙트럼이 행태변화만을 포함하는 가장 좁은 의미수준부터 지원체계, 질병예방, 건강보호 등을 포괄하는 가장 넓은 의미까지 다양하다. 건강증진의 개념을 어떤 것으로 받아들이든 정책, 개인 기술, 환경변화 등의 측면에서 지역사회(community)가 인간 집단을 포괄하고 있다는 점에서 건강증진의 가장 중요한 장이라는 것은 별로 이론이 없는 것 같다. 건강증진의 개념과 정의가 형성되어가는 시점에서 몇가지 주요한 논란점들이 있는데 그중에서도 건강증진의 접근전략에서 인구집단 전략과 고위험군 접근전략 간의 대립점이 가장 첨예한 논란중의 하나이다. 정부의 정책, 조직의 우선순위, 전문인의 행태 등이 각 개인의 노력과 같이 중요하거나 혹은 더 중요하다는 것이 이러한 입장이며, 건강과 행태에 영호t을 미칠 수 있는 사회체계와 환경에 대해 보다 큰 의사결정권을 가지는 것이 건강증진을 위해 중요하다고 주장한다. 이러한 이유로 건강증진을 논함에 있어 지역사회 전체를 통한 접근이 점점 더 강조하고 있다. 지역사회 건강증진 프로그램은 지역사회자원을 효율적 활용을 통한 보다 강화되고, 통합된, 포괄적, 협력적으로 수행되어져야한다. 이러한 방향은 1986년 오타와 헌장에의 건강증진의 정의에서 살펴볼 수 있으며, 광범위한 건강의 결정요인에 대한 접근이 필수적임을 알 수 있다. 신체활동은 건강의 주요한 결정요인이다. 인구집단 전략은 개인에 대한 신체활동 교육하는 것이 충분하지 않음을 제안한다. 개인 수준의 행태변화도 중요한 것이지만 이에 대한 집단적 변화를 불러줄 환경적 변화를 위한 전략과 균형을 이루어야 한다. 건강증진을 위한 신체활동 인구집단 전략은 사회적, 물리적 지지환경을 강화하고, 국가, 광역 및 기초자치 단체 수준에서 다양한 지역사회자원을 연계, 통합하여야 한다. 지속적인 공공교육 및 사회마케팅이 지역사회 신체활동 조직 및 기관, 산업장, 학교 등을 대상으로 지역사회 모든 주민들이 신체활동에 대한 관심을 증진시킬 수 있도록 협동적으로 이루어져야 한다. 정부와 기관 및 시민들은 지역사회차원의 운동개념으로 주최하고 참여하여 사회적 규범화 작업에 동참하여야한다. 기본적으로 적절하고 충분한 재원의 지원, 인적자원의 개발, 정책 및 입법이 제공되어야한다. 또한 국내외적으로 연구개발과 지식의 공유 및 교환이 요구된다. 한국에서는 전략적 우선순위가 높은 신체활동 프로그램의 범주를 지역 보건소를 기반으로 한 환경적 지원, 생애주기별 접근, 고위험군 및 질환군 접근 등으로 구분하였다. 지역사회에 기반한 신체활동 핵심 프로그램으로 하부구조 구축, 지원적 환경 제공, 지역사회 캠페인, 건강증진 교육 및 홍보, 노인 및 비만인을 위한 신체활동 프로그램, 그리고 운동처방 프로그램 등을 포함하였다. 신체활동증진 및 운동사업은 사회적, 물리적 지원환경 조성 등을 중심으로 전개하며, 보건소를 중심으로 한 운동사업은 지역 공공기관 및 민간기관 등과 연계하여 프로그램을 연동하며, 홍보 및 교육은 지역의 운동단체 및 기관, 각급 학교 등과 협력하여 운동에 대한 관심을 고취하고 신체활동 및 운동을 생활화하는 프로그램을 시행토록 한 전략을 지속, 강화토록 하여야 할 것이다.

Historical background and development of applied nutrition and community nutrition in Korea

  • This paper overviews the historical background and development of applied nutrition and community nutrition in Korea. The nutrition studies in the early years focused on animal experiments, human metabolism, and food analysis and therefore were limited to classrooms and research laboratories in universities without spreading into the lives of people. Korean specialists trained through the UN International Course of Applied Nutrition initiated the Applied Nutrition Program (ANP) in Korea in the 1960s. The ANP in Korea was effectively implemented until 1986 with support from UNICEF, FAO, and WHO as a national project to improve the nutrition and health of rural residents. With economic development and urbanization in Korea, the rural-focusing ANP was re-born to a more extended version with the name of "Community Nutrition", targeting the nutrition and health of the entire Korean population. Scholarly associations including the Korean Society of Community Nutrition established in 1995 have significantly contributed to the development of Community Nutrition in Korea and are expected to continue to work for a better connection between nutrition and health promotion.

등산운동의 생리학적 분석 (Physiological analysis of mountain climbing exercise)

  • Physiological analysis of the physical exercise was made on 9 subjects performing mountain climbing. The course between two points (256 and 516 meters altitude) was 1,300 meters in distance and difference of vertical height was 260 meters making the mean grade of 20%. In the field, the heart rates during uphill or downhill walk were recorded by EKG radio-telemetry. In the laboratory, oxygen consumption was obtained by the recorded heart rates, using individual heart rate vs oxygen consumption diagram obtained by treadmill test. the following results were obtained. 1. Uphill walk time was 36.5 minutes, and during this period the mean heart rate was 149.0 heats/min and peak heart rate was 169.2 beats/min. The total heart beats during the uphill walk was 5.433 beats. 2. The ratio of individual mean heart rate during the uphill walk to the maximal heart rate distributed between 66.6% and 98.3%, and the mean of the total group was 83.1%. The ratio of peak heart rate of uphill walk to the maximal heart rate was 94.5% in the group. Thus uphill walk of a 20% grade mountain course was an exhaustive exercise. 3. Oxygen consumption during uphill walk was 2.22 l/min (ranged between 1.79 and 2.70 l/min) and the ratio of this to the resting oxygen consumption was 8.31. The peak value of oxygen consumption during uphill walk was 2.73 l/min and the ratio of this to the resting oxygen consumption was 10.39. 4. Energy expenditure during uphill walk showed a mean of 11.1 kcal/min and the peak expenditure rate was 13.6 kcal/min. The total energy expenditure during 36.5 minutes of uphill walk was 396 kcal. 5. In downhill walk, the time was 31.7 minutes, mean heart rate was 118.4 (ranged between 100.1 and 142.7) beats/min, and the peak heart rate was only 129.4 beats/min. The ratio of mean heart rate to the maximal heart rate was 66.3%. Total heart beats during downhill walk was 3,710 beats. The ratio of downhill oxygen consumption to the resting consumption was 5.70. The rate of energy expenditure was 7.5 kcal/min, and the total onery expenditure during the 31.7 minutes of downhill walk was 228 kcal. 6. The effect of training was manifest in the uphill walk and not in the downhill walk. After training in mountain course walk, i) the uphill time was shortened, ii) mean heart rate increased, iii) time vs heart rate curve became smooth and showed less frequent zig-zag, i.e., the depth of trough on the curve decreased and the magnitude was less than 10 beats. In non-trained subject the depth of trough on the curve was greater than 50 beats and appeared more frequently. 7. Mountain climbing is a good health promotion exercise. For the promotion of health the reasonable amount of uphill mountain walk exercise in a 20% grade course is a walk for 40 or 50 minutes duration once a week.

류마티스 관절염의 예방과 관리 (Prevention and Management of Rheumatoid Arthritis)

  • Rheumatoid arthritis(RA) is the most common inflammatory joint disease and a major cause of disability, morbidity, and mortality. It occurs worldwide, affecting approximately one percent of adults. Inflammation of the synovial membrane surrounding a joint leads to swollen, tender, and stiff joints RA has no known cure and the diagnosis is made based on clinical criteria and many different options exist for treatment. All of these factors magnify the importance of the patient-physician interaction and place a premium on the art rather than the science of medicine. The major goals of therapy for RA are to relive pain, swelling, and fatigue; improve joint function; stop joint daage, and prevent disability and disease-related morbidity. Some combination of nonsteroidal anti-inflammatory drugs(NSAIDs), steroids, and DMARDs is necessary in almost patients. In many combinations of different DMARDs or DMARDs plus biologicals are necessary for optimal control. Additionaly, all patients with RA should be educated about their disease and the therapies that will be used. Patient education is essentially early in the disease course and on going basis Much research is focused on the further development of biological agent for treatment of RA. Elucidation of the trigger or trigers for RA may allow us to begin to think about prevention of RA.

완주군 초등학생들의 건강행위 조사 (A Survey Research on Health Care Behaviors Among Elementary School Students)

  • The objective of the health promotion program was to strengthen the ability and willingness of individuals in the course of each stage of their lives to take action in support of their health and that of their families and communities in the home, the place of work and the school, and during recreation. The purpose of this study was to assess the need of school health promotion through health care behavior survey about diet, accident prevention, cigarette smoking experience, personal hygiene, visual impairment prevention, exercise, and environment protection. The subjects of this study were 914 students composed of Samrye, Bongdong, Samrye-Dong, Iseo, and Songkwang elementary school in Wanju area. The data were collected by questionaire corrected for the purpose of this study which had been developed by WHO, ‘Health behavior in school children’. These were carried out from March 20 to April 20, 1996. The data was analyzed by frequency, percentage, χ²-test with SPSS-PC+ program. The results were as follows. 1. Diet related behavior 1) Girl students eat less breakfast than male students. 2) Girl students eat less 3 times a day than male students. 3) The rate of eating nutritious medicine was high in order of 4th, 6th, and 5th graders. 4) Girl students drink less milk than male students. 5) The intake frequency of eating between meals was high in order of 6th, 5th, and 4th grader. 2. Cigarette smoking behavior 1) The rate of cigarette smoking rarely was high in 5th grader. 2) Male students smoke cigarette more than girl students. 3. Personal hygiene related behavior 1) The rate of washing hands after using toilet room was low in order of 4th, 5th, and 6th graders. 2) The rate of washing hands after using toilet room was low in male students. 3) The rate of washing hands when entering home from outdoor was low in male students. 4) The tooth brushing frequency before going to bed was low in male students. 5) The tooth brushing frequency was low in male students. 6) The rate of bathing was most low in 4th grader. 7) The rate of bathing was low in male students. 4. Visual impairment preventive behavior 1) The distance between eyes and book was near in order of 6th, 5th, and 4th graders. 5. Exercise 1) The exercise time in school was less in 4th grader. 2) The exercise time in school was less in girl students. 3) The exercise time out of school was less in 4th grader. 4) The exercise time out of school was less in girl students. 5) The self evaluation score about exercise ability was low in girl students. 6. Environment related behavior D The rate of collecting waste separately in school was most low in 4th grader.

제2형 당뇨병 환자 식생활습관 및 보건소 영양교육 후 혈당개선효과 (Older Adults with Type 2 Diabetes Improve Glycemic Control after Nutritional Education Program at the Public Health Center)

  • This study was carried out to provide information on the effect of nutrition education program for diabetic patients at the Guri City's Public Health Center. Subjects of this study were 31 persons(male 7, female 24) who attended all courses of "2002 Diabetes Education Class". They were indicated as the 'education group'(EG). Eating and living habits of EG were investigated before the education. EG's weight and blood glucose (post prandial 2 hours, PP2) were examined as well. EG's PP2 reduction was compared with a 'control group'(CG) who didn't join any course in that class. All of the subjects were non-insulin-dependent diabetes mellitus(NIDDM) patients. EG's average age was 62.4$\pm$8.8. Before taking the course, EG's PP2 was 251.5$\pm$29.6mg/dl, and body mass index(BMI) was 26.3$\pm$2.3 on average. Most of them were stressed out from their daily lives and usually had no exercise. Most people of EG ate meals rapidly and liked sweet and fatty foods. After the course of training, EG's weight and BMI before the training were not decreased significantly. However, all of the EG's PP2s, which were measured 4 times(before the meal at the special lunch session, after 2 hours at this meal, after 2 weeks and 4 weeks dietary assembly), were decreased in comparison with the PP2 which was checked prior to joining the training. EG's average PP2 was more reduced than CG's one. In addition, all groups' PP2s were decreased for 8 weeks. After all, this nutritional education at the public health center was effective in glycemic control for diabetes mellitus patients. Especially, when the dietary assembly as practical training was included in the educational process, the patient's dietary intake and PP2 was improved more effectively. Therefore, this study suggests that nutrition work at public health centers is necessary for the Health Promotion Policy.
