A Multi-level Analysis of Factors Affecting Participation in Health Screenings in Korea: A Focus on Household and Regional Factors

  • Objectives: This study divided the factors that affect participation in health screenings into individual, household, and regional levels and conducted a multi-level analysis to identify the factors related to participation in health screenings. Methods: Participants from the 2017 Community Health Survey were classified into 2 groups (under 40 and 40 or older). A multi-level logistic regression analysis was conducted to identify the factors that affected participation in health screenings. Results: The screening rate of the participants was 69.7%, and it was higher among participants aged 40 and older (80.3%) than it was among participants younger than 40 (49.8%). At the individual level, the factors that influenced participation in health screenings included age, economic activity, smoking status, physician-diagnosed hypertension, and a moderate or high physical activity level. At the household level, the odds ratio of participation in health screenings was high for participants who lived in single-person households, lived with a spouse, earned a high monthly household income, and were not beneficiaries of national basic livelihood security. At the regional level, the odds ratio at the 95% confidence interval level of participation in health screenings was high for participants who had trust in the local community and lived in an area with a proportionally high social welfare budget. Conclusions: This study analyzed nationalwide data and confirmed that individual, household, and regional characteristics affected participation in health screenings. Therefore, policies that prioritize the improvement of regional level factors and especially household level factors are likely to be the most effective for improving the screening rate.

The Study of Health Promotion Behavior of Nursing College Student by the Self-Esteem (간호대학생의 자아존중감에 따른 건강증진행위에 미치는 요인)

  • Objectives: This study was carried out to investigate the effects of improvement in health promotion behavior of nursing college students by the difference of self esteem. The students was divided two groups one is low level self esteem the other is high depending on median point of self esteem. The specific objectives were first, to find out the differences of health promotion behavior, self efficacy, health perception, fatigue level, depression, psychosocial wellbeing, second, to establish which factors determine their health promotion behavior between two groups. Methods: Self-administered questionnaires were given to 262 students enrolled in a nursing college. between may and June 2008. The questionnaire items included age, sex, education level, self esteem, self efficacy, health perception, fatigue, depression,, psychosocial well being and health promotion behavior. For statistical analysis, frequency, t-test, regression used for determining the factors effecting health promotion behavior. Results: The influencing factors were self efficacy among low level and self efficacy, fatigue and stress among high level. Self efficacy strong positive impact on health promotion behavior among both groups. Stress and fatigue was only effective among high level group. Conclusion: Based on the study results, improvement of health promotion behavior among nursing students requires the development and application of programs to manage self efficacy and stress as a precondition for depending on self esteem level.

Association between Educational Level of Parents and Adolescent Depression Feeling: Based on 14th Korean Youth Health Behavior Survey (부모의 학력수준과 우울감 사이의 연관성: 제14기(2018년) 청소년건강행태조사 자료를 이용하여)

  • Background: This study aimed to demonstrate the association between the education level of parents and adolescent depression feeling. Methods: We used the chi-square test and the multiple logistic regression model to analyze the data of the "Korean Youth Health Behavior Survey in 2018" (n=43,220) conducted by the Korea Centers for Disease Control and Prevention. Results: As a result of the analysis, a group whose parents' education level is 'above the university graduation (including the graduation of the college)' had statistically significant experience of depression than a group whose parents' education level is 'high school graduation.' Conclusion: When preparing a policy on adolescent depression feeling, we need to consider the association between the education level of parents and adolescent depression feeling. Especially, a higher level of education of parents needs attention and management to foster positive mental health rather than excessive academic pressure, especially in the parent-child relationship of the same gender.

Social Support and Life Satisfaction of Living Alone elderly in Rural Area (농촌 독거노인들의 사회적 지원과 생활만족도)

  • This study was conducted to identify the social support to living alone elderly in some rural communities and the level of life satisfaction. For the purpose of this study, face-to-face interviews and questionnaire were performed with 315 old people(male 43 and female 272) aged over 65 living alone in rural communities covered by 14 community health posts within Gyeongsangbuk-do Province randomly selected. The followings are summaries of findings; The average score of support from their children was $4.29{\pm}2.73$ out of 8. Variables that showed a significant difference were religion, level of living, type of medical insurance, frequency of meeting with children, time taken from houses of children by usual means of transportation, and subjective health status. It was found that the score of support from children was high for the elderly who had a religion, a good level of living, benefit from medical insurance, a high frequency of meeting with children, or a good subjective health status, or who resided close to their children's houses. The level of the support from friends and relatives showed a significant difference depending on the subjective health status, of which the average score was $4.13{\pm}2.61$ out of 8. The average score of the level of life satisfaction was $6.83{\pm}4.24$(male $7.60{\pm}4.09$ and female $6.71{\pm}4.26$) out of 17. Male elderly showed the higher level of life satisfaction than female elderly. Variables that showed a statistically significant difference in the level of life satisfaction were religion, level of living, medical insurance, hobby, children, disease, subjective health status, and ADL. That is, the level of satisfaction with life was found to be higher for the elderly who had a religion, a good level of living, benefit from medical insurance, a hobby, children, no diseases, or a good ADL, or who thought that they are healthy. The regression analysis with support from children as a dependent variable showed that the level of support from children was higher for the elderly who had a good level of living, frequency of meeting with children, or a good subjective health status. The regression analysis with the level of support from friends or relatives was higher for old people who had a good level of living. The regression analysis with the level of life satisfaction as a dependent variable showed that the factors which related to the level of satisfaction were sex, religion, level of living, hobby, ADL, and subjective health status. That is, it was found that for male elderly who had a religion, a good level of living or a hobby, or who thought that they were healthy, the level of life satisfaction was higher.

Factors Influencing Perceived Fatigue and Health Promotion Behavior among Hair Dressers (미용사의 지각된 피로도와 건강증진행위에 영향을 미치는 요인)

  • Objective: This study was carried out to examine the factors affecting the fatigue and health promotion behavior of hairdressers and their respective levels. The data were collected from 195 hairdressers working in beauty shops in Daegu and Gyeongsangbuk-do Province using self-administered questionnaires over the period from July 1 to July 31. Methods: A multiple regression model was used to study the factors influencing fatigue and health promotion behavior among hairdressers. Results: This study showed a statistically significant difference in perceived fatigue levels and health promotion behavior according to age, family type, education level, work period, work position, and fatigue level in the past one week. In addition, multiple regression showed that statistically significant factors affecting perceived fatigue were education level, work period, and fatigue level in the past one week. Statistically significant factors affecting health promotion behavior were age and fatigue level in the past one week. The level of perceived fatigue among hairdressers tended to be higher than other workers, and their level of practice of health promotion behavior was generally lower. Conclusions: The improvement of the work environment, such as through the distribution of a proper workload considering workers' characteristics and strengthening of exercise programs to reduce the fatigue level and promote health practices among hairdressers is needed.

A Study on the Difference of Students' Recognition, Attitude, and Knowledge Level of Health Activity by the Health Education (보건교육 수강여부에 따른 보건 인식.태도.지식의 차이에 관한 연구)

  • This study investigated the difference of the university students' recognition, attitude, and knowledge level of health by the health education in Daejeon and Chungnam province. First, in the survey analysis result regarding the health related recognition, the students who had attended lectures of health education were surveyed not only to have higher health related recognition than the students who did not have attended the lecture of health education, but also to appear meaningful in statistics by showing significance level p<0.05 in total score. Second, in the research regarding the health related attitude, the attendance or nonattendance of health education lecture were surveyed not to have high effect on the attitude of daily lives of the university students. Third, in the analysis of health related knowledge, the knowledge level between the students who had attended lectures of health education and who did not have attended the lecture of health education showed that the students who had attended lectures of health education got more excellent correct answer rate. In this survey result, the students who had attended lectures of health education showed higher health related recognition and knowledge than the students who had not attended lectures of health education.

A Study on Health Education Behavior of Parent for Preventive Health Care of Kindergarten Children (유치원 아동의 예방적 건강관리를 위한 부모의 보건교육 형태 연구)

  • The study surveyed 1,127 mothers whose children aged 3 to 5 went to 25 randomly selected kindergartens, which were located in Busan Metropolitan City and the adjacent Gimhae City, and analyzed the preventive health behaviors on their children and the condition of home health education on their children. As the compulsory kindergarten education will e to be implemented within 4 years, this study aimed to draw up the rudimentary material for health education at kindergartens. The study results are summed up as follows: 1. Mothers' awareness of health-related attitude and behavioral level for their children at kindergarten: 1) The general health condition of the children, which mothers are aware of, was that they were healthy on the whole. As their children becomes older, the income level of the parents is lower, and mothers have lower level of educational standard, the health condition of the children was found a little lower. 2) In terms of mothers' recognition of the practice level of preventive health behaviors, the items such as going to bed regularly, washing hands and feet well, and having meals regularly were high in the practice level, while exercise and tooth-brushing were not practiced well. 2. The condition of mothers' health education of their children: 1) It was found that washing hands and feet, restriction of overeating and preference for specific kinds of food, guidance on regular sleep and regular meal were practiced well among items of home health education targeting kindergarten children. 2) Mothers have obtained information on family health from printed matter such as newspaper or magazine rather than the visual media. 3) From the cross analysis of mothers' health education on children and children's health behaviors, as the standard of education of mothers was higher, so was the level of preventive health behaviors of children. 3. From the result of multi regression analysis of the factors influencing the preventive hygiene of kindergarten children, it was found that as the home health education level was higher, so was the health behavior of children. On the other hand, as the mothers were older, the health behavior of children became lower.

The relationship between socioeconomic status and oral health inequality in the elderly in Korea (한국 노인의 사회경제적 위치와 구강건강 불평등)

  • Objectives: This study aimed to find a way to solve oral health inequality in old age by understanding the effect of the socioeconomic level of the elderly on oral health. Methods: We used data from the 7th Korea National Health and Nutrition Examination Survey. A chi-square test was performed to investigate differences in oral health according to socioeconomic status and demographic and oral health-related factors. Socioeconomic status and oral health inequality were analyzed using multiple logistic regression. Results: The average number of teeth in the elderly was 17.20, which is insufficient for the minimum number of teeth required for mastication. In the analysis of the correlation between socioeconomic status and oral health inequality, education level, income level, and home ownership were factors influencing the oral health of the elderly; education level was found to have the strongest effect. Conclusions: Oral health inequality according to socioeconomic status was confirmed, and it is necessary to measure the level of oral health inequality with active efforts at the government level to resolve the gap in oral health by social class.

Evaluation of the Effectiveness and the Level of Self-Management Support on the Metabolic Syndrome Management Program at Public Health Centers in Seoul Metropolitan City (서울시 보건소 대사증후군관리사업의 효과와 자가관리 지원수준 평가)

  • Background: The purpose of this study was to evaluate the effectiveness and the level of self-management support on the Metabolic Syndrome Management Program at public health centers in Seoul metropolitan city. Methods: The effectiveness on the Metabolic Syndrome Management Program were analyzed using secondary data from 1,312 community residents who were receiving program. The level of self-management support on Metabolic Syndrome Management Program was evaluated using an 'Assessment of Primary Care Resources and Supports for Chronic Disease Self-Management' from four public health centers. Results: The effectiveness on the Metabolic Syndrome Management Program was showed that decreased smoking (p= 0.044) and drinking (p< 0.001), and increased healthy dietary habit (p< 0.001) in health behaviors. It was showed that decreased triglyceride (p= 0.002) and increased high-density lipoprotein cholesterol (p< 0.001) in clinical indicators. The level of self-management support on Metabolic Syndrome Management Program was 98.1 points and it meaned that implementation is done in an organized and consistent manner using a team approach. There was difference in the level of self-management support by public health centers (p= 0.003). Conclusion: The Metabolic Syndrome Management Program in public health centers was effective, and level of self-management support was done as organizational level, but patient input and mental health were insufficient.

Comparison of oral health status of school children utilizing school toothbrushing facility or school dental clinic (양치교실과 학교구강보건실 이용 학생들의 구강보건 실태 비교)

  • Objectives : The purpose of this study is to provide the basic data for the development of oral health education program and to make schoolchildren practice the right oral health care behaviors. Methods : Data were collected from 259 elementary school students including 134 boys and 125 girls in Busan from November 26 to December 20, 2012. The groups were tooth brushing group and dental clinic group. All statistical analyses were analyzed by frequency analysis and chi-square test using SPSS 18.0 program. Results : School dental clinic group showed higher awareness level of fluoride and sealant effect of prevention(p=0.000). Toothbrushing facility group showed higher awareness level of brushing tooth(p=0.011). School dental clinic group showed higher awareness level of fluoride toothpaste(p=0.000). Both school dental clinic group and toothbrushing facility group showed higher awareness level of oral health education(p=0.001). School dental clinic group showed higher awareness level for tooth brushing method after education(p=0.000). Both school dental clinic group and toothbrushing facility group showed higher level of education. Conclusions : School dental clinic group showed higher level for oral health knowledge and toothbushing facility group showed high level for oral health behavior. Both School dental clinic group and toothbrushing facility group showed higher level of oral health education and tooth brushing method after education. Both school dental clinic group and toothbrushing facility group proved to promote oral health. Activation of school toothbrushing facility is very important to change the oral health workforce.
