Risk Assessment of Growth Hormones and Antimicrobial Residues in Meat

  • Growth promoters including hormonal substances and antibiotics are used legally and illegally in food producing animals for the growth promotion of livestock animals. Hormonal substances still under debate in terms of their human health impacts are estradiol-$17\beta$, progesterone, testosterone, zeranol, trenbolone, and melengestrol acetate (MGA). Many of the risk assessment results of natural steroid hormones have presented negligible impacts when they are used under good veterinary practices. For synthetic hormonelike substances, ADIs and MRLs have been established for food safety along with the approval of animal treatment. Small amounts of antibiotics added to feedstuff present growth promotion effects via the prevention of infectious diseases at doses lower than therapeutic dose. The induction of antimicrobial resistant bacteria and the disruption of normal human intestinal flora are major concerns in terms of human health impact. Regulatory guidance such as ADIs and MRLs fully reflect the impact on human gastrointestinal microflora. However, before deciding on any risk management options, risk assessments of antimicrobial resistance require large-scale evidence regarding the relationship between antimicrobial use in food-producing animals and the occurrence of antimicrobial resistance in human pathogens. In this article, the risk profiles of hormonal and antibacterial growth promoters are provided based on recent toxicity and human exposure information, and recommendations for risk management to prevent human health impacts by the use of growth promoters are also presented.

Spatial Changes in Work Capacity for Occupations Vulnerable to Heat Stress: Potential Regional Impacts From Global Climate Change

  • Background: As the impact of climate change intensifies, exposure to heat stress will grow, leading to a loss of work capacity for vulnerable occupations and affecting individual labor decisions. This study estimates the future work capacity under the Representative Concentration Pathways 8.5 scenario and discusses its regional impacts on the occupational structure in the Republic of Korea. Methods: The data utilized for this study constitute the local wet bulb globe temperature from the Korea Meteorological Administration and information from the Korean Working Condition Survey from the Occupational Safety and Health Research Institute of Korea. Using these data, we classify the occupations vulnerable to heat stress and estimate future changes in work capacity at the local scale, considering the occupational structure. We then identify the spatial cluster of diminishing work capacity using exploratory spatial data analysis. Results: Our findings indicate that 52 occupations are at risk of heat stress, including machine operators and elementary laborers working in the construction, welding, metal, and mining industries. Moreover, spatial clusters with diminished work capacity appear in southwest Korea. Conclusion: Although previous studies investigated the work capacity associated with heat stress in terms of climatic impact, this study quantifies the local impacts due to the global risk of climate change. The results suggest the need for mainstreaming an adaptation policy related to work capacity in regional development strategies.

여고생의 생활습관, 건강상태 및 건강증진행위가 과민성 장증후군 유발에 미치는 영향 (Impacts of Daily Habits, Health Status, and Health Promoting Behavior on Prevalence of Irritable Bowel Syndrome among Female Adolescents)

  • Purpose: This study was to investigate impacts of daily habits, health status and health promoting behavior on prevalence of irritable bowel syndrome (IBS) among female high school students. Methods: The survey was conducted from 526 female students in high school. Rome III criteria was used to diagnose IBS. Data of daily habits, health status, and health promoting behavior were collected through self-administered structured questionnaires. Results: The prevalence of IBS was 25.7%. Compared to the non-IBS, the IBS group scored significantly lower in exercise time, sleeping time, four dimensions of health status (physical, emotional, social, spiritual health) and management of relationship and stress in health promoting behavior. Risk factors influencing IBS were exercise time, physical health and emotional health status. Conclusion: The results of this study suggest that nursing interventions and education programs are important to be developed, while considering exercise, health status and health promoting behaviors for female adolescents in order to manage their IBS.

민간의료보험이 의료기관 종별 선택에 미치는 영향: 관절염 환자의 외래 이용을 중심으로 (The Impacts of Private Health Insurance on Medical Institution Selection: Evidence from Outpatient Service Utilization among Arthritis Patients)

  • Recently, with the increase in the number of private health insurance subscribers, interest in overuse of the medical service is increasing. This study analyzed the impacts of private health insurance (PHI) on medical institution selection in outpatient service utilization among persons with arthritis. In order to control patients' health status, we extracted outpatient episodes with the same disease (KCD6, M13) from Korea Health Panel. The unit of analysis was an outpatient visit with arthritis in 2014 (n=23,363). In the light of insurance coverage, we redefined three type of private health insurance (ex, indemnity, fixed benefit, and non-insured) as a test variable and two type of medical institution (ex, hospital and physician visit) as a dependent variable. We conducted a probit regression analysis to identify the impacts of PHI on medical institution selection controlling for heteroscedasticity. The results of this study showed that the insured with indemnity were more likely to choose hospital departments than clinics (marginal effect=0.0475, p=0.000). The impact of participation of fixed benefit PHI was not as clear as that of indemnity type (marginal effect=0.0162, p=0.047). In conclusion, this study confirmed that PHI, particularly indemnity type has a significant impact on the selection of medical institutions. Healthcare policy makers should consider that PHI not only affects the overall quantitative increase in healthcare utilization, but also influences the selection of medical institutions.

  • The purpose of this study is to examine the impacts of neighborhood sports activity facilities on the exercise time and subjective health level using the National Sport Participation Survey data(2013-2015) in Seoul, Korea. We conducted path analysis including exercise time, subjective health level and various environmental factors. From the survey data, we extracted individual's characteristics including subjective health and exercise time. In addition, we calculated physical environment variables of neighborhood units. The results of this study are as follow. First, most of moderating variables such as gender, age, income and job showed expected relationships with exercise time or subjective health level. Second, the perceived level regarding neighborhood sports facilities such as public sports facilities had a strong positive association with exercise time and subjective health level. Third, the area of public sports facility, neighborhood park and the number of schools had positive effects on the exercise time and subjective health level. Lastly, land use mix and the number of intersections showed a positive effects on exercise time. To sum up, the results indicated that public sports facility, neighborhood park and perceived environment on neighborhood sports facility are critical factors of residents' exercise time and subjective health level.

Identification of multiple sources in a plate structure using pre-filtering process for reduction of interference wave

  • This paper presents novel research into the source localization of multiple impacts. Source localization technology for single impact loads in a plate structure has been used for health monitoring. Most of research on source localization has been focused only on the localization of single impacts. Overlapping of dispersive waves induced by multiple impacts and reflection of those waves from the edge of the plate make it difficult to localize the sources of multiple impacts using traditional source localization technology. The method solving the overlapping problem and the reflection problem is presented in the paper. The suggested method is based on pre-signal processing technology using band pass filter and optimal filter. Results from numerical simulation and from experimentation are presented, and these verify the capability of the proposed method.

Diet and Lifestyle Factors Affecting Obesity: A Korea National Health and Nutrition Survey Analysis

  • This study investigated potential causes of obesity by examining diet and lifestyle factors. The data from the 2008 Korea National Health and Nutrition Survey were statistically analyzed to determine the relative importance of causes of obesity. Because the factors affecting obesity for males and females were significantly different, binary choice logistic models of the male and female subjects were built and estimated separately. Our results show that stress, the irregularity of eating breakfast, and frequency of eating out had the three greatest impacts on male obesity, respectively, and stress, employment status, and age had the greatest impacts on female obesity, in that order.

Impacts of Job Stress and Cognitive Failure on Patient Safety Incidents among Hospital Nurses

  • Background: This study aimed to identify the impacts of job stress and cognitive failure on patient safety incidents among hospital nurses in Korea. Methods: The study included 279 nurses who worked for at least 6 months in five general hospitals in Korea. Data were collected with self-administered questionnaires designed to measure job stress, cognitive failure, and patient safety incidents. Results: This study showed that 27.9% of the participants had experienced patient safety incidents in the past 6 months. Factors affecting incidents were found to be shift work [odds ratio (OR) = 6.85], cognitive failure (OR = 2.92), lacking job autonomy (OR = 0.97), and job instability (OR = 1.02). Conclusion: Patient safety incidents were affected by shift work, cognitive failure, and job stress. Many countermeasures to reduce the incidents caused by shift work, and plans to reduce job stress to reduce the workers' cognitive failure are required. In addition, there is a necessity to reduce job instability and clearly define the scope and authority for duties that are directly related to the patient's safety.

남편이 은퇴한 부부의 가사노동 참여와 공유 활동이 부부관계와 우울감에 미치는 영향 (The Impacts of Household Work Participation and Shared Activities on Marital Relationship and Depression)

  • This study analyzes the impacts of household work participation and shared activities between couples on marital relationship and depression among retired men and women with retired husbands. To investigate this, a survey of 367 married people is conducted on retired men and women with retired husbands. The influencing factors on the marital relationship of retired men are found to be couples' shared activities, gender role attitudes, and subjective health status. The marital relationship of women with retired husbands is influenced by shared activities by couples, the division of domestic labor, and gender role attitudes. The factors affecting retired men's depression are shared activities by couples, perceived health status, type of jobs before retirement, and age. Women's depression is related to household income, shared activities by couples, husbands' jobs before retirement, subjective health status, and division of household labor.

Effects of Natural Alternative Sweeteners on Metabolic Diseases

  • The rising prevalence of obesity and diabetes is a significant health concern both in globally and is now regarded as a worldwide epidemic. Added sugars like sucrose and high-fructose corn syrup (HFCS) are a major concern due to their link with an increased incidence of diet-induced obesity and diabetes. The purpose of this review is to provide insight into the effects of natural sweeteners as alternatives to sucrose and HFCS, which are known to have negative impacts on metabolic diseases and to promote further research on sugar consumption with a focus on improving metabolic health. The collective evidences suggest that natural alternative sweeteners have positive impacts on various markers associated with obesity and diabetes, including body weight gain, hepatic fat accumulation, abnormal blood glucose or lipid homeostasis, and insulin resistance. Taken together, natural alternative sweeteners can be useful substitutes to decrease the risk of obesity and diabetes compared with sucrose and HFCS.
