A Study on the Classification on Nursing Diagnosis through Home Visiting Nursing Care (가정간호사업을 통한 간호진단 분류연구)

  • As the health of people is a integral element of the public well-being, the government adopted and put the home visiting nurse system into practice from Jan. 1990, so that the public might get the necessary health service at the low cost and at home. Thus, it resulted in a rapid increase of the public need for the quality of life. The increasing demand for home care created a necessity for an increased man-power like home care nurse. Since the demand of the publics‘ home care has increased, Seoul Nurses’ Association, one of the branches of Korean Nurses Association has developed for the project team of Home Visiting Nurses Activities. The purpose of this study will analyzed and classified home nursing care activities of home visiting nurse according to the criteria of nursing diagnosis Korean Nurses‘ Association. The subject of the study was 54 patients in 1993 and 148 patients in 1994. All patients received home care at home from visiting nurses were living in Seoul city. The results of the study were as follows; 1) With regard to the nursing diagnosis, 24 items were classified. 2) The patients and families wanted to increase the number of time for home visiting. 3) Main sources of request to visit the patients at home were mainly from the families through nurses or doctors who cared for the patients. 4) In comparison of the characteristics visiting activities between 1993 and 1994, the number of the classification of nursing diagnosis and the number of visiting activities in 1994 were relatively increased than the outcomes in 1993.

Self Disease Diagnosis System Using Enhanced ART2 Algorithm (개선된 ART2 알고리즘을 이용한 자가 질병 진단 시스템)

  • In this paper, we have proposed a self disease diagnosis system for ordinary persons to help the decision of access methods to a specialized medical management, and for medical specialities to discover new diseases and their symptoms easily, using verification of an individual#s health status by a series of processes performed by oneself. In the proposed self disease diagnosis system, illness is decided by 60 kinds of diseases selected using the report called #Diseases that Koreans take seriously# published by Ministry of Health & Welfare and medical contents called #Engel Pharm#, and also using 161 representative symptoms for the 60 kinds of diseases. An individual#s health information is extracted by diagnosis of one#s health status by a clustering of the 60 kinds of diseases using enhanced ART2 algorithm and input vectors from the results of questions for symptoms of each disease.

Knowledge Levels of Turkish Nurses Related to Prevention and Early Diagnosis of Cancer

  • Background: The aim of this study was to examine knowledge about cancer and early diagnosis of cancer among nurses. Materials and Method: This descriptive study was carried out at a University Faculty of Medicine Hospital in Turkey. Study between April and June, 2011, with 325 volunteer nurses. The collection tool consisted of two survey forms. The first was designed for sociodemographic information and the second consisted of 16 questions, prepared in accordance with the literature as open and close-ended, for interviews conducted by researchers. Results: Out of the individuals (n=325), included in the study, 90.8% were female, 63.1% high school-university graduates and 55.1% married, with an average years of service of $6.34{\pm}5.33$ and an average age of $28.1{\pm}5.10$. The mean cancer knowledge point was $70.1{\pm}19.5$. Some 79.1% of nurses had not received cancer related continuing education by specialists. A signified relation was found between the nurse knowledge on cancer and educational level (p<0.05). Conclusions: The nurse, a member of the health staff, is in constant contact with individuals at hospitals, schools, polyclinics, workplaces, and homes. When educating society about cancer, nurses need to have a high level of knowledge regarding early diagnosis and cancer prevention.

MicroRNAs May Serve as Emerging Molecular Biomarkers for Diagnosis and Prognostic Assessment or as Targets for Therapy in Gastric Cancer

  • Gastric cancer (GC) is one of the most common cancers, with high incidences in East Asia countries. Most GC patients have been reported with low early diagnosis rate and show extremely poor prognosis. Therefore, it is necessary to develop novel and more sensitive biomarkers to improve early diagnosis and therapy in order to provide longer survival and better quality of life for gastric cancer patients. MicroRNAs (miRNAs) play crucial roles in GC development and progression. miRNAs have emerged as a novel molecular biomarker for cancer diagnosis, prognosis and therapy with surprising stability in tissues, serum or other body fluids. This review summarizes major advances in our current knowledge about potential miRNA biomarkers for GC that have been reported in the past two years.

Development of a Database System for Efficient Community Health Management - Focus on the Home Visiting Care of Family as a Unit by the Health Centers- (효율적인 지역사회 건강관리를 위한 데이터베이스 시스템 구현- 보건소의 가족단위 방문간호사업을 중심으로-)

  • In recent years, the recipients of the services of the health centers in Korea have been shifted from individual sick persons to families as a unit. As a result, the home visiting care records which are all filled out manually, will be increased. Since there is virtually no increase in the number of community health nurses, the CHNs are required to work more efficiently. One of the ways to make the CHNs' work more efficient is to reduce recording time by using a computer. However, a computer system that can manage the families as a unit has not yet been developed. In response to this need, we developed a database system that can be utilized in home visiting care service. The family assessment data is collected. diagnosed. and evaluated according to the family diagnosis classification. The system for family diagnosis consists of seven areas. Those areas are family structure. maintenance of the family system, interaction and interchange. support. coping and adaptation, health management. and housing environment. The areas of the family diagnosis consists of 99 items in all. We expect the following from this system. First. the CHNs will be able to identify family problems more easily. Second. the community's health level can be confirmed by the statistics the system produces. Thirdly, the CHNs' nursing services will be cost effective via reduced recording time. Finally, the family problems of the sick individuals which have been neglected under the health system oriented on individual persons can be effectively managed.

Regional Difference of Health Care Utilitzation in Korea (의료이용의 지역간 격차 -3차성 내과계 진단군을 중심으로-)

  • This study is conducted to investigate the current status on the utilization of health care and plan for solving this problem. The claims data of the fiscal tear 1995 obtained from the regional health insurance society are used for the study. The main findings of the study are summarized as follows. Indexes(The Extremal Quotient(EQ), coefficients of variance(CV's))which represent the regional difference in the admission rate of the tertiary medical diagnosis group report that there is difference in quantity and quality of utilization of health care. The admission rate is lower in the big city areas, Kyoungkido, Kangwondo and Chunlapukdo. Even after age-sex adjustment, the admission rate is still low in Kangwondo, Chunlapukdo and Kyoungsangpukdo. The big city areas tend to have higher rates in the expenses per claim, hospital days per claim, and daily expenses but the rates are still low in some area in Kangwondo, Chunlanamdo and Kyoungsangpukdo. This result remains as same after age-sex adjustment. There is a large regional difference in average utilization rate for the tertiary hospital of the tertiary medical diagnosis group: 57.2%(SD 11.53). The utilization rates for the tertiary hospital in their large catchment area are 96.34%, 83.19% and 73.22% in each Kyoungin, Kyoungnam and Kyoungpuk areas whereas it is lower in a Chungpuk and Chungnam areas. The regional differences of health care utilization of the tertiary medical diagnosis group gave some relationships with their geographical characteristics such as socio-economic characteristics and supply factors of medical services. It is important that many medical policies should be developed in order to minimize and balance out the regional differences of health care utilization. The service allocation policy should include the reconstruction of manpower policy, developing the resource allocating formula, finding the self-sufficient catchment area and reforcing of public health services. Moreover, in order to achieve the balanced development by region, they should investigate and consider each county's microscopic properties under the consistent macrocopic policy. The further studies to find causes of regional difference are needed.

Current Trend of Mibyeong Health Policy and Service in China through Investigation Activity of Mibyeong Technology -Report of China Technology Research Group- (현지 기술 조사활동을 통한 중국의 미병 정책 및 의료서비스 최신 동향 보고)

  • Objectives : This study aims to report current trend of Mibyeong health policy and service in China from interview of China Technology Research Group. Methods : China Technology Research Group visited Guanganmen hospital, Yanhuang Dongfang Company, Xinjingzhen health center, and Shanghai Shuguang hospital. With an interpreter, We had interviews about Mibyeong health service and policy trend, medical instrument, and research issue. Results :Mibyeong health service was performed based on KY3H system and collaboration with traditional Chinese medicine hospital. Mibyeong health service consists of diagnosis with nine constitutional type, health guide and preventive treatment. Community health center also provide Mibyeong health service, with cooperative treatment of traditional Chinese medicine and western medicine. China's Mibyeong health policy is established by Top-down decision, even though there is not enough evidence for providing health service to consumer. Through constitutional diagnosis, examinations, and treatment, huge data have been stacked; however, assessment and research based on these data are not processed well. Cooperative treatment of traditional Chinese medicine and western medicine is widely provided to patients, and their works are relatively well classified. Conclusions : China plays leading role in Mibyeong service and it seems to be developed more than Korea's. Further study is necessary to establish Mibyeong policy and health service in Korea.

Legal Measures for Handling Internet Health Information (법을 통한 인터넷 건강정보 관리 방안)

  • As people get broader access to health information through the internet, there is a greater need for measure to maximize the social advantages of the internet and to minimize negative side-effects. With this concern, this paper classifies internet health information services sites into : on-line supply of health information, on-line consultation, on-line diagnosis, and on-line sales. As well this paper analyzes domestic laws supporting and/or regulating these services. The efficient provision of internet health services requires comprehensive laws on individual privacy protection, prevention and handling of medical accidents, an electronic prescription form for internet diagnosis, electronic signing, payment for medical expenses, qualifications for internet medical practitioners. Additional laws are required to establish internet pharmacies and internet health goods stores. These new laws can be prepared either separately or through revision of existing laws governing medical practice, pharmacies, and public health promotion. However, as the legal control by the government on cyber processes and entities has a fairly minimal effect, consumers should be encouraged to improve their own capacity for wisely using internet health services and health-service providers should be encouraged to promote voluntary supervision and control of their own services and practices.

Effects of Income Level on the Association Between Hypertension and Depression: 2010-2017 Korea National Health and Nutrition Examination Survey

  • Objectives: This study analyzed the associations of hypertension (HTN) with symptoms and diagnosis of depression by income level among Korean adults. Methods: This study was based on the 2010-2017 Korea National Health and Nutrition Examination Survey data; a total of 29 425 adults (aged 20 years or older) were analyzed. HTN was defined as a systolic blood pressure ≥140 mmHg, diastolic blood pressure ≥90 mmHg, or use of hypertensive medications. Depression symptoms were evaluated based on a questionnaire about depression-related symptoms. A depression diagnosis was defined based on questionnaire responses indicating that a participant had been diagnosed with depression. Household income was divided into higher or lower income ranges based on the median income of the participants. Multiple logistic regression analyses were performed to assess the associations between HTN and depression symptoms/diagnosis in the higher-income and lower-income groups. Results: In the higher-income group, the odds ratio (OR) for the association between HTN and depression symptoms was 1.15 (95% confidence interval [CI], 0.97 to 1.37), and the OR for the association between HTN and depression diagnosis was 1.41 (95% CI, 1.13 to 1.76). In the lower-income group, the OR for the association between HTN and depression symptoms was 1.18 (95% CI, 1.04 to 1.34), whereas the OR for the association between HTN and depression diagnosis was 0.82 (95% CI, 0.70 to 0.97). Conclusions: The associations of HTN with symptoms and diagnosis of depression differed by income level.

Associations of Demographic and Socioeconomic Factors with Stage at Diagnosis of Breast Cancer

  • Background: Stage at diagnosis is one of the most important prognostic factors of breast cancer survival. Because in the breast cancer case this may vary with socioeconomic characteristics, this study was performed to recognize the relationship between demographic and socioeconomic factors with stage at diagnosis in Iran. Materials and Methods: This cross-sectional, descriptive study conducted on 526 patients suffering from breast cancer and registered in Cancer Research Center of Shahid Beheshti University of Medical Sciences from 2008 to 2013. A reliable and valid questionnaire about family levels of socioeconomic status filled in by interviewing the patients via phone. For analyzing the data, Multinomial logistic regression, Kendal tau-b correlation coefficient and Contingency Coefficient tests were executed by SPSS22. Economic status, educational attainment of patient and household head and/or a combination of these were considered as parameters for socioeconomic status. First, the relationship between stage at diagnosis and demographic and socioeconomic status was assessed in univariate analysis then these relationships assessed in two different models of multinomial logistic regression. Results: The mean age of the patients was 48.3 (SD=11.4). According to the results of this study, there were significant relationships between stage at diagnosis of breast cancer with patient education (p=0.011), living place (p=0.044) and combined socioeconomic status (p=0.024). These relationships persisted in multiple multinomial logistic regressions. Other variables, however, had no significant correlation. Conclusions: Patient education, combined socioeconomic status and living place are important variables in stage at diagnosis of breast cancer in Iranian women. Interventions have to be applied with the aim of raising women's accessibility to diagnostic and medical facilities and also awareness in order to reducing delay in referring. In addition, covering breast cancer screening services by insurance is recommended.
