Swallowing disorders or dysphagia, which are difficulties with the act of swallowing, can occur at different phases such as oral, pharyngeal, and esophageal phase in the swallowing process. These disorders can be caused by structural, functional abnormalities or psychological problems. Dentists can actively provide patients suffering from dysphagia with comprehensive professional care, especially from the cognitive stage of food to the pharyngeal stage of the swallow. Many cases of dysphagia can be improved with careful managements based on dental professionals' knowledge, including meticulous evaluation of masticatory and swallowing functions, training on eating and swallowing, and fabrication of palatal or lingual augmentation prosthesis. The important thing is that prevention of these disorders through the oral health care instruction and planned follow-up dental visit at periodic intervals in order to manage the problems caused by anatomical, functional, and psychological reason.
A nutrition education program for teachers and caregivers of the preschool children can be most effective when it is based on a comprehensive needs assessment dealing with sociodemographic factors, dietary habits, and nutrition. The purpose of the present study was to investigate current dietary habits and nutrition knowledge of the teachers and caregivers of the day-care centers. Two hundred forty two teachers and caregivers were administered a questionnaire which was designed to ascertain informations on sociodemographic data, dietary habits and nutrition knowledge. Dietary habits of the teachers were found to be significantly different by sociodemographic variables; breakfast skipping/meal irregularities (age, p=0.011); frequency of snacking (education level p=0.031); preference for salty taste(age, p = 0.000, marital status p=0.038); preference for sweet tarte (age p=0.009); preference for vegetables (income level p=0.050); frequency of eating out (age p=0.028, marital status p=0.001); frequency of coffee drinking (age p=0.019). Daycare center teachers' nutrition knowledge level was found to be less than adequate expecially on nutrients that are liable to be deficient in young growing children and their food sources. Proportions of the teachers who answered correctly to the questions on foods rich in vitamin A, iron content of milk, bioavailability of calcium in plant foods were as low as 20.2%-54.5%. The most frequently used sources of nutrition information were mass media such as TV, radio, newspaper and magazines. Only 2.9% of the subjects reported that they obtain nutrition information from health professionals such as nutritionists/dietitians, physicians, and nurses. These findings are applicable at the planning and implementation stages of various nutrition programs for the improvement of dietary habits and nutrition knowledge of the teachers and caregivers of the daycare centers. Further studies are needed to investigate the effects of teachers' dietary of habits and nutrition knowledge on food habits of young growing children.
The purpose of this study is to develop and validate a maternal happiness scale which can be used for mothers who take care of young children, school-aged children, and adolescents. First, the maternal happiness scale was developed based on literature review and the consent of experts. Finally it was consisted of 56 items from 18 elements of happiness through checking the contents validity with professionals. Second, final respondents for the survey were 1300 mothers who had children with and without disabilities in Seoul, Busan, Gyeonggi, and Kyungnam. Third, the 8 factors including the 53 items were extracted through the mothers' appraisal of the items and the review of the adequacy for the factor analysis. The 8 factors were consisted with 'Family relations with family', 'Laid-back life and self-esteem', 'Physical and mental health', 'Independency and development', 'Spirituality', 'Interpersonal relations', 'Appearance', 'Sociocultual environment'. Fourth, the maternal happiness scale demonstrated appropriate levels of construct validity, item internal consistency, distribution of item response, and item discrimination. The results of this study enhance our understanding of the core factors of maternal happiness. In addition, the findings have implications for supporting mothers who take care of young children, school-aged children, and adolescents.

Purpose: The purpose of the study was to understand and describe the overcoming experiences of family members caring for elderly patients with dementia at home. Methods: Data came from autobiographies on the overcoming experiences of caregiving from 31 participants, who had submitted the autobiographies to a public contest held by the Seoul Metropolitan Center for Dementia in 2012. Data were analyzed using qualitative content analysis. Results: Four overcoming stages emerged from the analysis: confronting stage; challenging stage; integrating stage; and transcendental stage, representing transformation of experiences from frustration and suffering to happiness and new hope in life. The confronting stage illustrates severe negative feelings and exhaustion occurring after the diagnosis of dementia. The challenging stage signifies major driving forces in taking good care of their patients. It includes tender loving memories about the patients as well as family and social supports. The integrating stage shows genuine empathy for the patients' situation and the happiness of 'here and now', while the transcendental stage represents new hope in the future. Conclusion: Health professionals need to support caregivers to find true meaning of caring and happiness in everyday life, while providing specific information on dementia care and relieving various negative feelings.
This instrumental case study analysed the play engaged in during play therapy sessions provided for a child suffering from tension, embarrassment, and anxiety. The client, a 4-year-old girl, was referred to play therapy due to high levels of difficulties in expressing her thoughts, and her refusal to engage in group activities at a child care center. Child initiative play therapy and the consultation with the mother focused on building her child rearing skills and helping her create a facilitating environment for her child were employed as the therapeutic intervention. The teacher's weekly report focused on child behaviors at the child care center were continued during the sessions. 12 play therapy sessions were provided and there was a follow-up session after one month. The process of the play therapy itself and the themes thus revealed were analyzed on the basis of Erikson's developmental theory and the findings provide implications for mental health professionals.
Objectives: Our study aimed to determine the attitudes of second- and final-year medical students and doctors (teaching faculty) of modern medicine towards complementary and alternative medicine (CAM) using the Complementary, Alternative, and Integrative Medicine Attitude Questionnaire (CAIMAQ). Methods: We invited 248 second-year medical students, 245 final-year medical students, and 48 faculty members to participate in the study. The CAIMAQ consists of 30 items, divided into five categories assessing various aspects of CAM, and scored using a 7-point Likert scale. The median scores obtained were compared between groups; a p-value < 0.05 was considered statistically significant. Results: A total of 138 medical students and faculty responded and participated in the study, of which, 24 (17.4%) were faculty, 40 (29%) were final-year medical students and 74 (53.6%) were second-year medical students. The overall attitude towards the various CAM concepts and therapies was positive. In general, the faculty were significantly less likely to consider referring patients for CAIM treatments, integrating them with conventional medicine, referring patients to alternative healthcare providers, considering the use of subtle energy fields as an ethical form of treatment, or considering CAIM treatments to be less invasive and harmful compared with conventional medicine. There was no significant difference in the attitudes of second- and final-year students. Conclusion: The attitude of medical students and doctors towards CAM is positive, and although the medical faculty have reservations in recommending specific types of CAM therapies or integrating them with conventional care, building evidence for supporting CAM therapies in specific diseases is likely to increase its uptake among health care professionals.

Background: According to the National Family Health Survey-4, in India, 78.9% of deliveries occur in institutions, although only 42.6% of new mothers initiate breastfeeding within 1 hour of delivery. Purpose: To estimate the proportion of early initiation of breastfeeding (EIBF) among new mothers at discharge from a tertiary care hospital and identify the determinants of delayed initiation of breastfeeding among them. Methods: This was a hospital-based analytical cross-sectional study of 108 new mothers. After obtaining Institutional Review Board approval and informed consent, we interviewed the new mothers on the day of discharge. Multivariate logistic regression was performed using IBM SPSS Statistics ver. 24. Results: The median breastfeeding initiation time was 90 minutes (interquartile range, 30-180 minutes). Overall, 43.5% of the mothers practiced EIBF, 77.4% practiced exclusive breastfeeding, and 43.5% were rooming in at discharge. Reasons for breastfeeding delays included extended recovery time from spinal anesthesia, maternal lassitude, and uncomfortable breastfeeding position due to post-cesarean pain. In the multivariate analysis, a birth weight less than 2,500 g (adjusted odds ratio [aOR], 4.33; 95% confidence interval [CI], 1.12-16.82; P=0.03), cesarean section delivery (aOR, 4.68; 95% CI, 1.57-13.92; P=0.005), and mother's poor knowledge of breastfeeding (aOR, 4.61; 95% CI, 1.44-14.72; P=0.010) were more likely to delay the initiation of breastfeeding. Conclusion: EIBF was practiced by less than half of the new mothers as determined by the cesarean section, baby's birth weight, and mothers' awareness of breastfeeding. Thus, it is vital to improve breastfeeding and nutritional counseling among mothers during the antenatal period and improve healthcare professionals' training to facilitate EIBF, even in circumstances such as cesarean section.
Purpose: This study aimed to develop a nursing clinical judgment scale (NCJS) and verify its validity and reliability in assessing the clinical judgment of nurses. Methods: A preliminary instrument of the NCJS comprising 38 items was first developed from attributes and indicators derived from a literature review and an in-depth/focus interview with 12 clinical nurses. The preliminary tool was finalized after 7 experts conducted a content validity test based on a data from a preliminary survey of 30 hospital nurses in Korea. Data were collected from 443 ward, intensive care unit, emergency room nurses who voluntarily participated in the survey through offline and online for the verification of the construct validity and reliability of the scale. Results: The final scale comprised 23 items scored on a 5-point Likert scale. Six factors - integrated data analysis, evaluation and reflection on interventions, evidence on interventions, collaboration among health professionals, patient-centered nursing, and collaboration among nurse colleagues - accounted for 64.9% of the total variance. Confirmatory factor analysis supported the fit of the measurement model, comprising six factors (root mean square error of approximation = .07, standardized root mean square residual = .04, comparative fit index = .90). Cronbach's α for all the items was .92. Conclusion: The NCJS is a valid and reliable tool that fully reflects the characteristics of clinical practice, and it can be used effectively to evaluate the clinical judgment of Korean nurses. Future research should reflect the variables influencing clinical judgment and develop an action plan to improve it.

Nutrition is important in the management of diabetes mellitus, however, there are few little education materials specifically designed for older adults. The objective of this study was to develop nutrition education materials for prevention and management of diabetes moll for older adults. Materials developed were a booklet and four leaflets. The contents of materials were based on lesson plans. After several revisions of the draft of materials, illustrations and icons appropriate to the contents were designed using illustrator 9.0 and Photoshop 6.0. The booklet was composed of five chapters and 40 pages. The first chapter began with an introduction about diabetes and diabetes management by diet, exercise and medication. The second chapter dealt with ideal body weight, calculation of adequate caloric intake and food exchange list. The third chapter provided information for meal planning and sample menus. The fourth chapter focused on practical tips on nutritional care of diabetes, by providing tips on reducing sugars, fat and salt, and suggestions on eating for special occasions. The fifth chapter dealt with information in case of low blood sugars, exercise and foot care. The topics of the four leaflets were “Diabetes, what is it and care”, “Food exchange list and meal planning”, “Healthy eating for diabetes”, “Special care for diabetes low blood sugars, exercise and foot care” Each leaflet was composed of six sections and was printed in large paper (B4 size) for older adults. The draft of educational materials were re-viewed by four nutrition professionals and finally pilot-tested with ten adults aged 50 and older. The characteristics of the developed materials are as follows, i) messages are delivered using simple, specific information, ⅱ) messages focused on practical applicable tips, ⅲ) various pictures, illustrations and artwork were created and inserted to enhance understanding and interest, ⅳ) sections including risk factor assessment, calculation of ideal body weight and meal planning were designed to induce the user's participation, ⅴ) sample menus and food pictures were inserted in the booklet, vi) characteristics of older adults and transformed characteristics are diversely used to help the user feel familiarity. These materials are self-explanatory and can be used by older adults. These materials also can be used widely in nutrition education at public health centers or senior centers.

Objectives: In this study, knowledge of environmental disease, experience of environmental health education and necessity of environmental health education of elementary school pupils were compared and analyzed. Methods: The population of this study was from grade 1 to 6 elementary school pupils in Ulsan city, Korea. Five elementary schools were randomly selected from Ulsan City. Seven hundred and twenty-eight responses from the five schools were analyzed (seventy-two were excluded). A self-administered questionnaire measuring sociodemographic variables, Knowledge of environment related disease(atopic dermatitis, asthma etc), as well as educational experience and necessity of environmental health education was analyzed. The survey results were analyzed using SPSS-PC Program 12.0 and the following results by implementing Frequency Analysis, Cross-tabulation Analysis and ANOVA are as follows. Result: Pupils of lower grades more informed than those of higher grades about atopic dermatitis. Among asthma sufferers, many responded that symptoms occurred or improved during the lower grades of elementary school. In terms of the lifestyles of patients with atopic dermatitis or asthma, female pupils, especially those in the lower grades, tended to make changes in their daily habits based on their disease. Also, parents tended to intervene/mediate more in the dietary habits and personal hygiene habits of lower-grade pupils compared to their higher-grade counterparts. Personal hygiene education is currently not provided in schools, and upper-grade pupils have more experience with personal hygiene education compared to lower grade pupils. 430 (59.0%) of pupils replied that education about personal hygiene is necessary in their curriculum. 490 (67.3%) of elementary pupils are aware of the close connection between education in hygiene and human health. When asked if education in hygiene should also be undertaken by teachers and parents, 406 (55.7%) students said that it should be done. Conclusions: Developing a concrete health education program and raising instructors' awareness of the necessity for education in hygiene. Placement of professionals in the personal hygiene education field, is important to provide good health education for elementary school pupils.
