Purpose: The aim of this study was to provide information on the extent and variations of elderly residents' nursing care needs, and provision of nursing care across long term care facilities. Methods: A nationwide survey was conducted on nurse managers from 1,041 long term care facilities, by e-mail or fax, from August 16 to September 30 in 2017. A self-reported questionnaire consisting of 5 domains was used to collect data. Results: Facilities with more than 30 residents were more likely to need skilled nursing services and to obtain the nursing staff such as a registered nurse and a nurse's aide. Awareness and satisfaction of hospital-based home nursing care was high in all facilities. In addition, there are some differences in nurse managers' perceptions of the level of healthcare resources and required action by facility size. Nurse managers of senior congregate housings were more likely to have considerable difficulty in dealing with healthcare needs of residents and recognizing the healthcare resource shortage. A majority of nurse managers agreed on the need to employ a registered nurse. Conclusion: This study confirmed that it is essential to increase nurse staffing level and to reform the long term care insurance for enhancing the accessibility of healthcare services, especially for the residents in small long term care facilities. There is also a need to provide diverse education and training opportunities for nursing staff working in long term care facilities.
Purpose. Infant oral health care forms the basis of lifelong health, and the role of child care teachers in contributing to the quality level in the formation of correct habits at this time throughout the infant's health care and education is very important. The purpose of this study was to examine the relationship between the perceived oral health status of nursery teachers and the necessity of oral health education, and to emphasize that the oral health of teachers is closely related to the oral health of infants and young children. Methods. This study was conducted from May to June 2018 by surveying private nursery teachers in some parts of Gyeongsangbuk-do province by using the self-entry method. The research tools consisted of the general characteristics of subjects, subjective oral health status, oral health practice, oral health education awareness, and oral health education needs. We used shceffe as a post hoc test to compare the differences between groups after frequency analysis, independent t-test, and one-way ANOVA analysis. The linear regression analysis was conducted to determine the relationship between perceived oral health status, oral health practice and awareness, and oral health education needs. The statistical level was .05. Results. According to the results of this study, the oral health related practice according to the demographic characteristics is at the final education level, and the oral health related awareness is age, marital status and oral health education needs. The results showed statistically significant differences in final educational attainment and cohabitation. Regarding the relationship between perceived oral health status, oral health related practice and awareness, and oral health education needs There were significant differences in oral health practice(β =.329) and oral health education needs (β =.158). Conclusion. Based on the above results, the child care facility can emphasize the importance of the role of the child care teacher in the oral health care of infants, and the basic data for the establishment of the child care management program in the future by reflecting the role of the teacher in the development of the child care management program of the child care facility. Purpose: The purpose of this study was to develop a child health care protocol for teachers in child day care centers. Methods: The ADDIE model with 10 Kid Keys was applied to develop this child health care protocol. All contents were developed through content validity test by 7 professionals and need assessment and evaluation by child day care center teachers. Results: This protocol consisted of 10 keys, as follows: "Health Examination/Growth & Development", "Practice of Health Life", "Management of Communicable Disease", "Negligent Accident", "Coping with Emergency and Transference", "Child Abuse", "Nutrition/Obesity", "Quality Assurance of Staff", "Parent Education", "Guidance & Supervision of Child Day Care Centers". The contents contained goals, objectives, teaching content, suggested activities for children, writing forms related to each subject, and self-evaluation sheet. Conclusion: This protocol can be practical and effective for child health care in child day care centers and it is hoped that it will be utilized in more child day care centers.
Coronavirus disease 2019 poses an occupational health risk to health-care workers. Several thousand health-care workers have already been infected, mainly in China. Preventing intra-hospital transmission of the communicable disease is therefore a priority. Based on the Systems Engineering Initiative for Patient Safety model, the strategies and measures to protect health-care workers in an acute tertiary hospital are described along the domains of work task, technologies and tools, work environmental factors, and organizational conditions. The principle of zero occupational infection remains an achievable goal that all health-care systems need to strive for in the face of a potential pandemic.
Purpose: This study examined the definition of care coordination, and the activities and roles of nurse care coordinators. The study also proposed suggestions for establishing and expanding the role of nurse care coordinators in community health practice. Methods: This study derived its conclusions by conducting a literature review. Results: The definition of care coordination is a comprehensive concept that includes case management and transitional care, and can be summarized as organizer and integrator of care. According to the literature review, 12 activities and 6 roles of nurse care coordinator were identified such as a collaborator, an individualized care planer, an educator/counselor, a direct nursing care providers, a population care coordinator, and a program evaluator. Training for nurse care coordinators is currently addressed in the Primary Medical Chronic Disease Management Program. Visiting nurses working at the Eupyeong-dong visiting health and welfare service are expected to act as nurse care coordinators, and the role of nurse care coordinators will, in the future, gradually increase in various nursing facilities. Conclusion: In addition to developing competencies to act as a nurse care coordinator, there is need for approaches to health policy that develop both independent role and population focused role as care coordinators.

Purpose: This study was conducted to survey children's health status and need of customized visiting health care services in one province. Methods: The participants in this study were 237 caregivers of infants and preschoolers. Data were collected at the participant's home or public health center. Results: Many of the children did not receive developmental screening tests or dental examinations. In the beneficiary group, the prenatal checkup rate and children's vaccination rate were lower, and caregivers had more health problems than the other groups. On the assessment of home safety, unsafe conditions were more frequently found in the beneficiary group. The caregivers in the beneficiary group showed lower child rearing confidence than the other groups, and wanted customized visiting health care services most in the areas of developmental screening, regular health check-up and counseling, nutritional supplementation, and oral health care. Conclusion: These results indicate that it is necessary for children and parents in poverty to be provided with professional home visiting interventions for the promotion of child health and prevention of developmental problems and diseases. These findings can be used for developing future customized visiting health care service programs for infants and children in this community area.
The objective of running a long-term care institution is to provide services helpful for maintaining, supporting, and improving elderlies' optimum level of physical, mental, and psychosocial functioning. For the purpose of analyzing the current situations of institutional long term care facilities in Korea, 27 facilities were selected proportionately from each of the cities and provinces, out of the total 152 facilities. About 20% of those who were institutionalized during 25 August through 2 Qctober 1993, the 391 elderlies were chosen on a systematic random basis. The instrument of this study was developed by modifying the tools of CARE, MAI and PCTC. A multivariate approach of discriminant analysis and clustering technique were employed for this study. The Stiudy reveals that there is no clear differentiation of goals and functions among the longterm care institutions in Korea. Staffing patte군 of long-term care facilities shows a shortage of nurses, physical therapists, and dieticians. The linkage between acute care facilities and long-term care is weak, and administration of long-term care faciltiy is carried out by non-professionals. They are responsible for assessing health status before entering the facility, and evaluating elderlies' care. Therefore, it is not surprising to find that most of the facilities have accommodated agede regardless of their real needs and health status. Based upon findings of the analysis, this study has classified long-term care facilities into four types : Type I is to help elderlies maintain independence in daily living activities. Type II facilities have the objective of maintaining and improving the current level of elderlies' function. Type III is to maintain maximum independence of elderlies in activities of daily living. And Type IV is identified for the group of facilities designed to restore or improve functional abilities of elderlies. In conclusion, the following suggestions are made : the need for long-term care should be assessed by multidimensional measurement. Institutional long-term care facilities should be classified and developed in response to type of type of care and service need. Both acute and long-term care facilities should be linked together in order to support the evaluation of service operation and program development.
Purpose: This study aimed to conduct a job analysis of nurse carecoordinators and to identify the frequency, importance and difficulty of each task of their job. Methods: A committee for developing a curriculum (DACUM) was formed and members of the committee defined nurse care coordinators' jobs and enumerated the duties, tasks and task elements by applying the DACUM technique. Then nurse care coordinators enrolled in the pilot project evaluated the frequency, importance and difficulty of each task. Results: From the job descriptions of nurse care coordinators, we identified 12 duties and 42 tasks. Each task comprised 1~5 task elements. Among tasks, 'assess the patient's general health status' was carried out most frequently. Nurse care coordinators perceived that 'check vital signs' and 'strengthen patient competence to promote health behaviors' were more important than all other tasks. The most difficult task was 'develop professionalism as a nurse care coordinator'. Conclusion: The nurse care coordinators' roles developed in this study will serve as the key guidelines for human resource management of care coordinators. Further, job specifications for nurse care coordinators need to be developed, which is necessary for designing education and training programs. We also need to integrate primary health care as an essential component in nursing education.
Purpose: To investigate the health problems and health services in child day care centers. Methods: Data were collected from 115 teachers at 16 child day care centers in Seoul city. A questionnaire was used to collect data, which were analyzed with the SPSS 12.0 program. Results: Most teachers had experienced various child health problems such as colds, hand․foot․mouth diseases, chicken pox, skin injuries, nasal bleeding, vomiting and diarrhea. Furthermore, they even experienced some serious ones including dysentery, measles, asthma and seizures, which demand professional skill. However, there were no registered nurses and most teachers requested that parents take a child home when these health problems happened. Only 31.3% of the child care centers had a teacher with CPR training. Approximately half of the centers kept child health records which included reports on allergic substances, and medical history but only 18.7% of the child care centers offered regular immunizations for the children. Conclusion: Various health problems were found in child day care centers. To maintain the children's health, there is a need to develop and make provisions for health services and programs in child day care centers.
This is an effort to project health care resources in need for the elderly in the future when advanced transportation systems would greatly reduce geographical accessibility to health care services for the rural elderly. Two areas, Kimhae and Chuncheon, were selected for the study. Projection of health professionals and health care institutions for the elderly were made based on the analysis for the morbidity and illness behavior reported to two data sources, National Survey for the Elderly in 1992 by Korea Institute for Health and Social Affairs and beneficiary data by Korea Medical Insurance Cooperation for those living in the study areas in 1992. Projected number of health professionals and health care institutions were estimated for each area under study in years of 2010 and 2030, with those in a Japanese being used as a standard. Policy implications were discussed.
