  • Dramatic changes are beginning to occur in plastic card technologies today in the fields of banking, health care, id-cards and more. Advanced card technologies such as IC-card are playing an increasingly important role in new and innovation system. The merit of IC-card are more capacity, more secure, more reliable, quick and easy to update, secure offline processing, enabling technology etc than magnetic stripe card. And so many countries began using experiment of IC- card and our country began October 13 in Cheju island. Then there are three types using-experiment (K-cash, VISA-cash, MONDEX) in Korea. There are some problems such as standards, widely using, high cost, double investment etc. I think that important things in IC-card using is an international- standardizations and issuing corporation.

Epidemiology and Histopathological Spectrum of Head and Neck Cancers in Bihar, a State of Eastern India

  • Head and neck cancers are amongst the commonest malignancies, accounting for approximately 20% of the cancer burden in India. The major risk factors are tobacco chewing, smoking and alcohol consumption, which are all preventable. This retrospective study presents data from the histopathology register for a five year period from 2002-2006 at Patna Medical College and Hospital, a tertiary care hospital drawing patients from the entire Bihar state, the 3rd most populous state of India with the majority of the population residing in rural areas. Incidence rates based on sex, age, site of lesion, including age standardized incidence rates for males and females, with mean age of presentation, distribution of histological variants and year wise trend were calculated. Out of 455 head and neck neoplasias, 241 were benign while 214 were malignant. The most common age group for all malignant biopsies was 7th decade for males and the 5th decade for females. Malignant cases were commoner in males than females with the male:female ratio of 3.1:1, which was found to be statistically significant by the chi-square (${\chi}^2$) test. The crude rate and age standardized incidence rate was 0.05 and 0.06 per 100,000 population respectively. Squamous cell carcinoma (SCC) contributed about 96% of all cases, with grade I being the most common. Larynx was the most common site for malignancy, the supraglottic region being its most commonly affected sub-site. This observed incidence patterns in the region are a reminder of widespread unawareness, low healthcare utilization with virtually non-existent cancer programs. It also underlines the need to advocate for reliable cost-effective programs to create awareness, for early detection and plan appropriate management strategies. There is a compelling demand for a cancer registry in this region as well as proper implementation of preventive measures to combat this growing threat of cancer, many of whose risk factors are preventable.

Knowledge, Attitudes, Practices and Barriers Towards HPV Vaccination among Nurses in Turkey: a Longitudinal Study

  • Background: Human papilloma virus (HPV) occurs in women of all age groups, and causes cervical, anal, vaginal, vulvar, penile and oropharyngeal cancers. The aim of the study was to discover what nurses know about HPV infection, testing and vaccination and to determine vaccine practice of their daughters and perceived barriers. Materials and Methods: This cross-sectional and prospective study was carried out nurses who have worked in a hospital between January and June 2014. Pre-test and post-test were used to evaluate the nurses' knowledge about HPV infection, testing and vaccination. This study was performed with nurses who had girls between 9 and 26 years of age for evaluating the behavior of vaccination after three months of education. Results: The mean of pre-test and post-test scores about HPV infection, which included 22 items, were $8.2{\pm}5.6$ and $19.2{\pm}1.5$, respectively. Before education the HPV testing knowledge score was remarkably poor ($1.9{\pm}1.7$ over 5), after education it increased to $4.8{\pm}0.5$. The mean HPV vaccine knowledge score were $3.7{\pm}2.7$ (pre-test) $7.3{\pm}0.8$ (post-test) on a 0-8 scale. The difference between mean total pre-test ($13.9{\pm}9.1$) and post-test ($31.3{\pm}1.9$) scores was statistically significant (p<0.001). After three months of education, only two of the nurses' daughters were vaccinated. The main reason was noted by nurses were not willing to be vaccinated was cost, doubts about safety and efficacy related to the vaccine. About one-third of nurses declared that they would receive the vaccine for their daughter later. Conclusions: Nurses have a crucial role in the prevention, treatment, increasing public awareness and care for population. The education of the nurses about HPV infection, test and vaccination will play an important part decreasing cancer mortality and morbidity.

A Study on Satisfaction Factors with Dental Implant Patients (치과용 임플란트 보철물적용환자의 만족요인)

  • This research has been attempt to examine closely factors that high cost and value to overall satisfaction state for dental implant prosthesis, re-utilization intention, inducement intention of the general hospital dental service user and provide basis data necessary to establish competitive general hospital dentistry management strategy. Collected data using own recording way questionnaire from April 17, 2004 to May 15, 2004 choosing 142 people that agree on this research and question of 361 people that dental implant prosthetic treatment finished from March 1, 1999 to March 1, 2004 for this. Major analyzation consequences are as follows : First, general satisfaction, re-utilization intention, inducement intention for implant prosthesis that highly correlation variables are kindness, explanation, medical treatment level and appreciation of the aesthetic. Second, correlation is high relatively between re-utilization intention, inducement intention in the 3 variables such as general satisfaction re-utilization intention, inducement intention. Third, the result of multiple regression analysis showed that most significant effective factors are satisfaction with explanation of dental implant prosthesis between the 3 dependant variables such as general satisfaction, re-utilization intention, inducement intention. In looking into these consequence, how to improve that raise satisfaction about dental implant prosthetic treatment and to establish patient focused care service system for dental implant prosthesis, it is consider that explain enough about implant prosthesis and raise quality of medical examination and treatment level, including satisfied with fabrication of good esthetic dental implant prosthesis.

Influence of Payer Source on Treatment and Outcomes in Colorectal Cancer Patients in a University Hospital in Thailand

  • The study aimed to compare the 2 main types of insurance used by colorectal cancer (CRC) patients in a university hospital in Thailand: universal coverage (UC) and 'Civil Servant Medical Benefit Scheme' (CSMBS) in terms of hospital expenditure and survival outcomes. CRC cases in stages I-IV who were operated on and had completed their adjuvant therapy in Songklanagarind Hospital from 2004 through 2013 were retrospectively reviewed regarding their hospital expenditure, focusing on surgical and chemotherapy costs. Of 1,013 cases analyzed, 524 (51.7%) were in the UC group while 489 (48.3%) belonged to the CSMBS group. Cases with stage IV disease were significantly more frequent in the UC group. Average total treatment expenditure (TTE) was 143,780 Thai Baht (THB) (1 US$ =~ 30 THB). The TTE increased with tumor stage and the chemotherapy cost contributed the most to the TTE increment. TTE in the CSMBS group was significantly higher than in the UC group for stage II-III CRCs. The majority of cases in the UC group (65.5%) used deGramont or Mayo as their first line regimen, and the proportion of cases who started with a capecitabine-based regimen (XELOX or $Xeloda^{(R)}$) was significantly higher in the CSMBS group (61.0% compared to 24.5% in the UC group, p-value < 0.01). On survival analysis, overall survival (OS) and progress free survival in the CSMBS group were significantly better than in the UC group. The 5-year OS in the CSMBS and UC groups were 84.3% and 74.6%, respectively (p-value < 0.01). In conclusion, the study indicates that in Thailand, the type of insurance influences resource utilization, especially the choice of chemotherapy, in CRC cases. This disparity in treatment, in turn, results in a gap in treatment outcomes.

Prevalence of Types of Cancers in the Elderly Covered by Insurance of the Islamic Republic of Iran Broadcasting Company in 2015 - Comparison with Younger Groups

  • Presently, the world population of the elderly is growing. By improving health hygiene and welfare indicators, mortality and birth rates decrease and life expectancy increases, making the present century the century of elderly. Aging is one of the main risk factors for development of cancer, which itself is the second cause of death in old people. This study was conducted to assess the prevalence of cancer in the elderly covered by the Islamic Republic of Iran Broadcasting (IRIB) insurance program and to obtain suitable programs for cancer screening and early detection, increase patient survival, improve elderly care and to reclaim the cost of treatment in comparison to the national and international statistics. This is a cross-sectional study conducted on all elderly patients diagnosed with malignancy based on their pathology reports. In this study, of the total 75,500 patients covered by IRIB insurance, 17.2% belonged to the elderly group, males accounting for 53.3%. The most common cancers in old men were prostatic cancer (61.3%), colon cancer (10.3%) cancer of the hematologic system, bladder cancer (9.6%), lung cancer (9.1%), thyroid cancer (3.9%) and brain tumors (1.3%). In the elderly women, the most common cancers were breast cancer (80.1%), colon cancer (5.1%), thyroid cancers (4.4%), bladder and hematologic system malignancies (3.6), lung cancer (2.9%) and brain tumors (0.7%). In addition, the prevalence of cancer was almost the same as national and international statistics. With the exception of non-melanoma skin cancer no difference was shown in prevalence of cancer between IRIB elderly patients and the other groups of cancer patients in Iran.

Development of Solid/Liquid Separation Technique for Krill (Eupausia superba) (남극 크릴새우의 고액분리 기술개발)

  • Economic development involves increase in life expectancy as well as human health care. Consequently, demand for fish meal and fish oil is rapidly growing. In particular, Krill (Eupausia superba) oil product is in high demand due to its rich unsaturated-fatty acid, and thus stable supplies are necessary in the krill oil market. It is required for captured krills to be immediately frozen and stored during ship transport, since proteins of the krill are quickly denatured in natural temperature condition. However, the transportation cost has been sharply increased, which encourages researchers to involve in studies for development of efficient oil extraction process. In this study, a solid/liquid separation technique on boat for the krill oil was developed through triple separation tests using only a separator or using either brush or crusher prior to the separator. The separation tests revealed that the efficiency were 46.2, 60.2 and 60.4 % by the separator, combination with brush, and combination with crusher, respectively. In addition, it was found that byproduct, extracted cake, derived from the separation process could be used as a feed stuff. These results suggest that smashing using the brush or crusher prior to the separator is more efficient than using only the separator.

Hospital Management Strategy in Digital Era (터지털 시대의 병원경영전략 수립에 관한 연구 - 병원경영자의 경영개선활동에 관한 인식을 중심으로 -)

  • This study purports to examine the current management and information technology related strategy of Korean hospitals and suggest the effective management strategy in the 21st century when is digital era. Specifically the study tries to analyze the changing trends of strategic orientation and investigate the general management and information technology strategy of Korean hospitals. Self-administered Questionnaires were distributed to 721 hospitals nationwide and finally 98 Questionnaires were analyzed for the study. The results of the study are as follows : 1) Half of the respondent hospitals reported that they have an analyzer orientation in 2000, whereas 19.4% were prospectors, reactors 16.4%, and defenders 14.3%. However, the respondent hospitals intended to have a prospector orientation in the future (2002), while 29.6% planned on being analyzers, 17.3% reactors, and 3.1% defenders. 2) Hospital services for improving patient satisfaction were the most common. strategy for the respondent hospitals, followed by cost containment, organizational restructuring, employee education, purchasing system change, specialization of clinical services, quality improvement of medical care, strengthening the networking with the stakeholders, public relations and marketing strategy, diversification, and installing the information system. However, the strategies of annual salary system, retrenchment of unprofitable services, merit payment based on performance were still not popular for the respondent hospitals. 3) As for the strategies related with information technology, most hospitals have not implemented actively, except for the establishment of home-pages, order communication systems, and insurance claims through electronic data interchange system. 4) There were significant differences in the level of strategy implementation in terms of the ownership, bed size, financial performance, and the top managers I knowledge of information technology. The larger bed size, the higher financial performance, the better knowledge of information technology the top managers have, the more strategies the respondent hospitals implemented. The managerial and political implications for Korean hospitals in digital era were also discussed.

Perceptions and Attitudes of Housewives on Scalp and Hair Management (전업주부들의 두피모발 관리에 대한 인식 및 태도)

  • In this study, "housewives" are defined as married women who take care of housework mostly and do not work outside of home. Changing family types and higher education level of housewives enable them to be more interested in heath, beauty, and appearance. Increased interest and more frequent visits to hair salons cause scalp damage as well as hair damage, which results in hair loss. Moreover, some women suffers from scalp and hair damage due to childbirth, but many of them are not aware of it. Therefore, this study tries to find out the following; housewives' perceptions on hair and scalp management, and how such perceptions are formed. In addition, this study will present the direction for beauty marketing taking into account the characteristics of housewives and provide data necessary for them to keep their hair and scalp healthy. For these purposes, a survey was carried out after modifying the questionnaire through a presurvey. The survey tried to find 1) perceptions on hair and scalp management according to demographic characteristics, and attitude toward hair and scalp management according to demographic characteristics. Data were analyzed using frequency analysis, cross tabulation, t-test, factor analysis, and Analysis of Variance. The results were as follows, most recognize the importance of hair and scalp management but do not receive management because of the high cost, most of them receive information about hair and scalp management from people around them, and the higher the education level and health status, the more active the attitude toward hair and scalp management. These differences were all statistically significant.

Vibration control of small horizontal axis wind turbine blade with shape memory alloy

  • Vibrational problems in the domestic Small Horizontal Axis Wind Turbines (SHAWT) are due to flap wise vibrations caused by varying wind velocities acting perpendicular to its blade surface. It has been reported that monitoring the structural health of the turbine blades requires special attention as they are key elements of a wind power generation, and account for 15-20% of the total turbine cost. If this vibration problem is taken care, the SHAWT can be made as commercial success. In this work, Shape Memory Alloy (SMA) wires made of Nitinol (Ni-Ti) alloys are embedded into the Glass Fibre Reinforced Polymer (GFRP) wind turbine blade in order to reduce the flapwise vibrations. Experimental study of Nitinol (Ni-Ti) wire characteristics has been done and relationship between different parameters like current, displacement, time and temperature has been established. When the wind turbine blades are subjected to varying wind velocity, flapwise vibration occurs which has to be controlled continuously, otherwise the blade will be damaged due to the resonance. Therefore, in order to control these flapwise vibrations actively, a non-linear current controller unit was developed and fabricated, which provides actuation force required for active vibration control in smart blade. Experimental analysis was performed on conventional GFRP and smart blade, depicted a 20% increase in natural frequency and 20% reduction in amplitude of vibration. With addition of active vibration control unit, the smart blade showed 61% reduction in amplitude of vibration.
