도시 영세지역 주민의 건강진단 결과 (Morbidity Pattern of Residents in Urban Poor Area by Health Screening)

  • 보건연구개발 사업을 위한 지역사회 진단의 일환으로 도시 영세지역 주민을 대상으로 시행한 건강진단 조사의 결과를 요약하면 다음과 같다. 전체 대상주민 2,591명중 건강진단에 참여한 영세지역 주민은 437명(16.9%)였으며 남자는 9.9% 여자는 23.9%가 참여하였다. 연령별로는 60세 이상군이 42%로 가장 높은 참여율을 나타냈고 20-29세군이 5.9%로 가장 낮았다. 건강진단에서 이상소견율은 남자가 38.7%, 여자가 45.8%였으며 전체적으로 43.7%였고 연령별로는 60세 이상군이 69.8%로 가장 높고 10-19세군이 10.9%로 가장 낮았다. 국제질병분류(ICD 17 대분류)에 의한 질병분포는 소화기계 질병이 23.7%로 가장 높았고 순환기계 질병 19.7%, 신경계 및 감각기 질병 13.2% 순이었다. Screening test에서 헤마토크릿치가 낮은 경우가 14.6%, 고혈압이 10.1%, 청력손실 5.5%, 간기능이상 4.1%, 당뇨가 2.3%, 단백뇨가 1.4%, chest X-선상 이상소견이 0.9%였다.

종합 건강검진을 원하는 피검진자의 건강특성에 관한 연구 (Characteristics of Individuals Seeking Comprehensive Health Check-ups)

  • This study was carried out to identify basic data for more efficient operation of comprehensive health check-up centers in the national health care system. The researcher reviewed and analyzed the main symptoms of the subjects according to sex, age, occupation and positive rate in the screening tests of a comprehensive health check-up, performed at one comprehensive health check-up center in located in a hospital. The subjects were 512 persons who had a check-up at the comprehensive health check-up, center in K university hospital in Seoul from October 2, 1996 to March 30, 1997. A questionaire developed by the researcher to obtain subject's general characteristics, main symptoms, medical examination, diagnosed disease was used as the tool for the study. The researcher analyzed the data with SPSS PC/sup +/ program. : the distribution of the subject's general characteristics and main symptoms was described with percentages and the difference between main symptoms and screening tests analyzed with X²-test, t-test, ANOVA. The result of this study are as follows : 1. For the distribution of diagnosis in males. liver disease was most frequent and hypertension was second : in females, hypertention and liver disease were not common in that order. 2. The most common complaints were indigestion in the digestive system, sputum in the respiratory system, irregular heart beat in the cardiovascular system, frequent urine in the urinary system vertigo in the hematologic system. 3. The main symptoms according to sex were for females, a statistically siguificant higher rate of complaints in the digestive system, cardiovascular system, urinary system, hematologic system, musculoskeletal, mental & nervous system as compared to males. 4. The main symptoms according to age were that in the the second decade there wara higher rate of complaints in the digestive system, and cardiovascular system than in the older age groups : and in the fifth decade more in the musculoskeletal system. 5. The main symptoms according to occupation were that formers and fishers had a higher rate of complaints in the mental and nervous system as compared to other occupation groups : and office workers, in gynecology. 6. The main symptoms according to diagnosis, were symptoms of the digestive system, frequent in liver disease : symptoms of the cardiovascular system in allergic disease. 7. The screening tests which showed the highest rates were UGI /FGS, PFT : In males a higher rate of abnormality was seen in abdominal ultrasono- graphy and in, females, in mammography. 8. In screening tests according to sex, males showed a higher positive rate for chest X-rays, and LFT, and females, in mammography, pap smear, CBC. 9. In screening tests according to age, higher positive rates of obesity, high blood pressure, chest X-ray, mammography, pap smear, FBS, CBC, urine test were seen in the fifth decade : PFT and abdominal ultrasonography in the third decade. The subjects who wanted comprehensive health check-up were not asymptomatic but had an illness with several symptoms. It is found that the most frequent diagnosed diseases were hypertention and liver disease. So proper observations must be done for them. Comprehensive health check-ups should be changed to a comprehensive examination including treatment beyond examination for those subjects with positive results.

성인 비만의 건강특성에 관한 분석적 연구 (An Analytical Study on Health Characteristics of Obesity of Adulthood)

  • This study was carried out to offer the basic data for more qualitative lives of the adulthood with obesity through their health maintenance and control. So, the researcher reviewed and analyzed the health problems of the subjects according to sex, age, and positive rate on the screening tests. The subjects were 91 obese persons in K university hospital in Seoul from December 1, 2000 to January 31, 2001. A Questionnaire developed by researcher to obtain subject's general characteristics, main symptoms, medical examination, diagnosed disease was used as the tool for the study. The results of this study are as follows: 1. For the distribution of diagnosis in subjects, liver disease was most frequent and hypertension was second. 2 The most common complaints were indigestion in the digestive system, throat pain in the respiratory system, dyspnea in cardiovascular system, thirsty in endocrine system, frequent urine in the urinary system, vertigo in the hematologic system. 3. The main symptoms according to sex were for females, a statistically significant higher rate of complaints in the digestive system, musculoskeletal system as compared to males. 4. The main symptoms according to age were that in the fourth decade there were higher rate of complaints in the musculoskeletal system. 5. In screening tests according to sex, males showed a higher positive rate for Alk-Phosphatase and females, in FBS, urine-micro, VDRL. 6. In screening tests according to age, higher positive rates of BP, Chest X-ray, mammography, ESR, CRP were seen in the fourth decades. 7. Obesity was positively related to cardiovascular system, endocrine system, urinary system, musculoskeletal system, mental and nervous system in main symptoms. 8. Obesity was positively related to BP, Sonography, FBS, SGOT, SGPT in screening tests.

Why Screening Rates Vary between Korea and Japan-Differences between Two National Healthcare Systems

  • Both Japan and Korea provide population-based screening programs. However, screening rates are much higher in Korea than in Japan. To clarify the possible factors explaining the differences between these two countries, we analyzed the current status of the cancer screening and background healthcare systems. Population-based cancer screening in Korea is coordinated well with social health insurance under a unified insurer system. In Japan, there are over 3,000 insurers and coordinating a comprehensive strategy for cancer screening promotion has been very difficult. The public healthcare system also has influence over cancer screening. In Korea, public healthcare does not cover a wide range of services. Almost free cancer screening and subsidization for medical cost for cancers detected in population-screening provides high incentive to participation. In Japan, on the other hand, a larger coverage of medical services, low co-payment, and a lenient medical audit enables people to have cancer screening under public health insurance as well as the broad range of cancer screening. The implementation of evidence-based cancer screening programs may be largely dependent on the background healthcare system. It is important to understand the impacts of each healthcare system as a whole and to match the characteristics of a particular health system when designing an efficient cancer screening system.

한일간 암검진 수검율에 영향을 미치는 정책적 요인에 대한 융합연구 (Convergence Study on Factors that Influence Cancer Screening Rate in Korea and Japan)

  • 본 연구에서는 한국과 일본의 암검진 사업의 수검율에 영향을 주는 정책적 요인에 대한 융합연구를 통해 분석하였다. 그 결과는 첫째로 국민건강보험제도의 보험자가 단일보험자일 경우 통합적인 암검진 사업이 가능하기 때문에 검진율에 영향을 미칠 것으로 분석되었다. 둘째로 암과 같은 중증질병에 걸렸을 때 예상되는 의료비 부담이 높으면 경제적 인센티브가 작동하게 되어 검진율에 영향을 미칠 것으로 분석되었다. 일본의 경우에는 건강보험에서 보장하지 않는 비급여항목이 거의 없고 본인부담상한제 등이 잘 정비되어 있어 중증질환이라도 의료비부담이 낮기 때문에 암검진의 수검율이 개선되지 않는 측면이 있다. 일본은 건강보험제도의 보장성 강화가 암검진에 대한 경제적 인센티브를 약화시키는 정책적 요인으로 작용한 것으로 나타났다.

Factors Affecting Breast Cancer Screening Behavior in Japan - Assessment using the Health Belief Model and Conjoint Analysis

  • Background: Japanese women in their 40s or older have been encouraged to attend breast cancer screening. However, the breast cancer screening rate in Japan is not as high as in Europe and the United States. The aim of this study was to identify psychological and personal characteristics of women concerning their participation in breast cancer screening using the Health Belief Model (HBM). In addition, the attributes of screening more easily accepted by participants were analyzed by conjoint analysis. Materials and Methods: In this cross sectional study of 3,200 age 20-69 women, data were collected by an anonymous questionnaire. Questions were based on HBM and personal characteristics, and included attitudes on hypothetical screening attributes. Data of women aged 40-69 were analyzed by logistic regression and conjoint analysis to clarify the factors affecting their participation in breast cancer screening. Results: Among responses collected from 1,280 women of age 20-69, the replies of 993 women of age 40-69 were used in the analysis. Regarding the psychological characteristics based on HBM, the odds ratios were significantly higher in "importance of cancer screening" (95%CI: 1.21-2.47) and "benefits of cancer screening" (95%CI: 1.09-2.49), whereas the odds ratio was significantly lower in "barriers to participation before cancer screening" (95%CI: 0.27-0.51). Conjoint analysis revealed that the respondents, overall, preferred screening to be low cost and by female staff members. Furthermore, it was also clarified that attributes of screening dominant in decision-making were influenced by the employment status and the type of medical insurance of the women. Conclusions: In order to increase participation in breast cancer screening, it is necessary to disseminate accurate knowledge on cancer screening and to reduce barriers to participation. In addition, the attributes of screening more easily accepted were inexpensive, provided by female staff, executed in a hospital and finished in a short time.

지역사회 여성 주민을 대상으로 한 대상중심형 위암 조기검진 교육 프로그램의 개발 및 효과평가 (Effectiveness of a Community-based Program Using Tailored Stage-matched Messages to Promote Screening for Stomach Cancer)

  • Objectives: Recent studies have shown that tailored messages for cancer screening to the beliefs and stage of cancer screening behavior of individual women increases the take-up probability. Many studies on cancer screening have used the Transtheoretical Model (TTM) to identify variables associated with cancer screening behavior. This study was carried out to identify the cognitive-behavioral factors associated with stomach cancer screening among women aged 40 years and over, and to develop and evaluate a tailored educational program for stomach cancer screening by stages of change. Methods: Building on the TTM constructs, we conducted a quasi-experimental study(N=283) to test the effectiveness of a tailored educational program for endoscopic stomach cancer screening. We carried out pre and post tests in the experimental group(N=162) and the control group(N=121), and the experimental group was subdivided into an on-line group(N=81) and an off-line group(N=81) by educational methodology using e-mail and the postal service. We used the chi-square test, trend test, and paired t-test to test the effectiveness of the program for stomach cancer using a tailored stage-matched messages. Results: To examine the effectiveness of the program for stomach cancer screening by the tailored stage-matched messages, the stage-matched materials were offered to the experimental group(N=162) four times for 4 weeks. The stage-matched materials consisted of the four types for stomach cancer. The tailored message was effective in changing the cognitive-behavioral factors, such as experience process, behavior process, con opinion for stomach cancer, self-efficacy, and the behavioral stages for stomach cancer screening. The stomach cancer screening adherence was higher for the stage-matched materials using postal mail than for those using e-mail. Conclusion: To improve the stomach cancer screening rate, the use of tailored messages for stomach cancer screening will be generated using an expert system. Therefore the implementation of tailored educational program will be supported a partnership between public and private health organizations and increasing awareness of the necessity of community-based interventions.

강원도 저소득층의 전립선질환 검진사업과 사후관리를 위한 유관기관 협력방안 (Directions for Muti-institutional Cooperation in Follow-up Management of Prostatic Disease Screening for the Low-income Group in Gangwon-do)

  • Objectives: This study aimed to investigate factors related to prostate disease and follow-up procedure on health examination program in Gangwon-do. Methods: The studied population was 16,501 male aged 40 or more, who underwent screening tests for prostatic disease in 2011 health examination program designed for the low income residents in Gangwon-do. The screening tests included prostate specific antigen (PSA), international symptom score (IPSS), and maximal flow rate (Qmax). We conducted chi-square test for trend, two independent samples t-test, and binary logistic regression to find out the relationship between prostate cancer or benign prostatic hyperplasia and risk factors such as age, PSA level, and IPSS. Results: The number of subjects who turned out positive for PSA level (> 3.8ng/ml) was 971 men accounting for 5.9%. The result of post survey on them showed that 511 subjects (52.6%) got the urology check-up: 156 (30.5%) were diagnosed as normal, 286 (56.0%) as benign prostatic hyperplasia, and 69 (13.5%) as prostate cancer. The detection rate for prostate cancer was 8.8% when PSA level was between 3.81 and 9.99g/ml, and was 30.6% when PSA level was more than 10.00ng/ml. The rates of further testing varied depending on the communities from 26.7% to 68.2%, which was mainly affected by the role of local health center. Conclusions: Our findings indicate that enhanced quality control of local health center and empowerment of screening agency would increase the effort of coupled institutions between public and private sectors.

국가 암조기검진사업 참여에 영향을 미치는 인구사회학적 요인 (Participation Rate and Related Socio-demographic Factors in the National Cancer Screening Program)

  • Background : Cancer is the leading cause of death and one of the largest burdens of disease in Korea. In 1996, the Ten year Plan for Cancer Control was formulated and the government then adopted the plan as a national policy. As part of this plan, the National Cancer Screening Program (NCSP) for Medicaid recipients was formulated, and the government adapted this in 1999. For low-income beneficiaries of the National Health Insurance Corporation (NHIC), the screening program has been in place since 2002. In 2002, the target cancers of NCSP were stomach, breast and cervical cancer. This study was conducted to examine the relationships between the participation rate, the abnormal screening rate and the socio-demographic factors associated with participation in the screening program. Methods : To analyze the participation rate and abnormal rate for the NCSP, we used the 2002 NCSP records. The information on the socio-demographic factors was available from the database of the beneficiaries in the NHIC and Medicaid. Results : The participation rate of the Medicaid beneficiaries for the stomach, breast and cervical cancer screening were 9.2%, 15.5% and 15.0%, respectively, and 11.3% and 12.5%, except cervical cancer which wasn't be included in the NCSP, for the beneficiaries of the NHIC. The abnormal rate of stomach, breast and cervical cancer screening were 25.7%, 11.2% and 21.0%, respectively, for the beneficiaries of Medicaid and 42.6% and 19.4% for the beneficiaries of the NHIC. On the multiple logistic regression analysis, gender, age and place of residence were significantly associated with participation rates of the NCSP. For stomach cancer, women participated in the NCSP more than men. The participation rate was higher among people in their fifties and sixties than for those people in their forties and those people over seventy years in age. For the breast and cervical cancer, people in their fifties were more likely to participate in the NCSP than people in their forties and people over sixty. For the place of residence, people in the rural areas participated more than those people in any other places. Conclusions : The above results show that the participation rate and abnormal rate were significantly associated with the socio-demographic factors. To improve the participation rate for the NCSP, more attention should be given to the underserved groups.

일부 지역 치과위생사의 치과 건강보험에 관한 지식 및 심사조정률에 미치는 요인 (Analysis of the knowledge and insurance screening review rates of health insurance claims in the dental hygienist)

  • Objectives: The purpose of the study is to investigate the level of knowledge and screening review rates of dental health insurance claims in dental hygienists. This analysis will provide the educational information to the dental hygienists. Methods: A self-reported questionnaire was completed by dental hygienists in Jeonbuk from December 17, 2012 to January 24, 2013. The questionnaire was distributed by ordinary mail or direct visit. Except incomplete answers, 350 data were collected and analyzed. The study instrument was adapted from the structured questionnaire by Hong and Yoo. The questionnaire consisted of education experience of health insurance management, subjective and objective knowledge, insurance screening review, and need for health insurance education. Data were analyzed by SPSS 12.0 program. Cronbach alpha in the objective knowledge on health insurance rate criteria was 0.836 and this was a reliable figure. Results: The subjective knowledge level of dental insurance was higher in the senior dental hygienists. Subgingival curettage was the lowest percentage of correct answers in the objective knowledge. In recent six months, higher review control rate was shown in the higher claim for health insurance and insurance screening review. Conclusions: The majority of the respondents had lower level of knowledge of health insurance claims. The continuous education of dental health insurance will be necessary in the dental hygienists.
