  • Objectives: This paper reviews the main principles and values underlying health promotion and reflects upon recent health promotion efforts in Korea. Methods: The essay approaches these issues through the framework of the Ottawa Charter for Health Promotion (WHO, 1986) and other related (WHO) documents. The Ottawa Charter has been an important basis for health promotion worldwide over the last three decades since 1986. Emphasizing the instrumental value of health and the prerequisites for health, it provides the definition of health promotion and elaborates the strategies for health promotion as well as the main health promotion actions. Results: Beyond the values of health as both a fundamental human right and a resource for everyday life, the values and principles related to health promotion shown in WHO documents and other literature include holism, social justice and equity, public and community participation, autonomy, empowerment, socioecological approaches to health, sustainability, intersectoral collaboration, partnership-building, responsibility for health, and so on. Conclusions: Reflecting, subjectively, on health promotion efforts in Korea, some values, including holism in terms of target population, equity, public and community participation, empowerment, and socio-ecological approaches have been realized to some extent, while other values like intersectoral collaboration and partnership have not been considered sincerely in public efforts relating to health promotion. Therefore, future health promotion efforts in Korea should concentrate on incorporating these critical values and principles-based approaches into health promotion activities.

Quaternary prevention is the prevention of unnecessary medical interventions or the prevention of overmedicalization. Methods: Data was collected after a search of the literature focusing on keyword. The curriculum guidelines for family medicine residents were collected from the homepage of the American Academy of Family Physicians. Results: This quaternary prevention is just beginning. The first step to enhance the health promotion services in the community is to identify the barriers pertaining to the delivering of health promotion activities. These barriers are related to the patient, the physician, attitudes, health promotion programs and the healthcare system. The second step is to establish new changes, such as expansion of insurance coverage, improvement of medical payment system, provision of consumer-oriented services, connection and integration of providers, and the service provider education and training. Conclusions: In order to enhance the health promotion services in the community we need to identify the barriers and to establish several changes to overcome them.

  • Health education aims at behavior change rather than just delivering health knowledge to people. In Korea health education activities in public sector began in 1960 and they were included in the primary prevention program in communities. This article reviewed current health education programs in healthy living practice programs provided by local public health centers in Korea and drew implications for the future role of health education in community setting. Health education has been a core function of the National Health Promotion programs in the nation since the enactment of the National Health Promotion Law in 1995. The National Health Promotion programs are funded by the National Health Promotion Fund which are drawn from tobacco tax. The National Health Promotion programs include healthy living practice programs (smoking prevention and cessation programs, moderate alcohol use programs, physical activity promotion programs, and nutrition programs), chronic disease prevention programs, oral health programs and public hygiene programs. Methods of the National Health Promotion programs include health education, health counseling, health class, health information management, survey and research. Smoking prevention and cessation programs include smoking cessation clinic, smoking cessation education, non-smoking environment program, and non-smoking campaign. Moderate alcohol use programs include alcohol use education, moderate alcohol use campaign, alcohol use counseling, and alcohol free environment programs. Physical activity promotion programs include obesity control, targeted exercise program, and exercise civic group programs. Nutrition programs include nutrition management, obesity management, nutrition education, breakfast eating program, and nutrition counseling and treatment programs. The health education programs in community are not efficient today because there are many overlapping contents and short term goals. Community health education programs needs to be more comprehensive. Workforce development is another big issue at the moment because the National credential program will begin in 2009. Variety of community health education programs should be developed and funded by the national health promotion fund.
