The Roles and Professional Competencies of Health Education Specialists in Private Health Care Setting (민간 의료기관에서 보건교육사의 활동 영역과 능력 개발)

  • Objectives: In health care setting, patient education and health promotion services are inexpensive and effective initiatives to change health behavior due to use medical service resources and personnel. This study performed to define the responsibilities and competencies of health education specialist in private health care setting. For our suggestion, we reviewed regulatory, recommendation, and programs related to health education and promotion in clinics and hospitals. Results and Conclusion: The health promoting hospital and health services in Europe and innovative hospitals of community health promotion in the U.S. were examples of approaches that supply target groups with health promotion services in health care setting. The National Commission for Health Education Credentialing has suggested the specified responsibilities and competencies of health education specialist in health care setting according to their general duty. Considering the recommendation of the NCHEC, our suggestion included: 1) the three kinds of job scope, 2) the major targets, 3) the specified responsibilities and competencies, and 4) the available health promotion programs in clinic and hospital setting. The suggestion will contribute to the development of job market for health education specialist and to the cooperation with community health resources in health promotion services and comprehensive health care.

The Characteristics and Service Utilization of Home Nursing Care Beneficiaries Under the Korean Long Term Care Insurance (장기요양방문간호 이용자의 특성 및 이용실태)

  • Purpose: This study examined the characteristics and service utilization of home nursing care beneficiaries under the Korean Long-Term Care Insurance (LTCI). Methods: We used assessment data and claim data of National Health Insurance Corporation from July to August 2008. Data were composed of subjects who were the beneficiaries of home nursing care. Results; A total of 634 subjects were analyzed. Of the subjects, 57.1% were 75 years and over. The average score of nursing care need was only 0.71 and the percentage of those whose nursing care need score was zero was 58.0%. More than half of the subjects had partially dependent musculoskeletal conditions, and 75.5% had two or more comorbidities. A third of them usedonly home nursing care, and another third used both home nursing care and general home care at the same time. Those who needed sore care used the largest home nursing care benefits. Conclusion: Home nursing care of LTCI performs community-based healthcare services under LTCI. Throughout the past two years, however, it has not been active. Understanding the characteristics of its users is important in order to develop effective strategies for activating home nursing care.

  • Bluetooth low energy (BLE) beacon is an actively push-to-broadcast electronic signal and can be used for object identification. This paper uses such beacon-based identification and Internet of Things (IoT) technologies for the elder health management service system to simplify the user interfaces and steps for preventive elder care. In the proposed system, an elder's family member, caregiver, or medical worker can conveniently and quickly record daily health management information. Besides, through the statistics and analysis of the data on the back end of the system, it is helpful for the elderly to refer to the data of daily care management and future management trends. Similarly, it is also an essential reference data for system maintenance and the new preventive health care services development.

  • This study is to confirm that SNS information characteristics have an effect on choosing hospital care and satisfaction on hospital. This study is conducted with a total of 301 respondents (150 of document investigation and 151 of mobile SNS research) from 16th January 2014 to 29th January. 168 respondents who have experience to collect SNS health information within 6 months recently was selected subject of analysis. In the results, we could confirm the followings. The reliability among SNS information characteristics has effect on convenience that is source of choosing hospital care. The reliability and agreement have effect on the professionalism among source of choosing hospital care. In the influence on having hospital satisfaction, the more reliable and agreeable, the more increasing of hospital satisfaction. In the influence between hospital choice element and hospital satisfaction, the more convenient and service available, the more increasing of hospital satisfaction. In conclusion, it is expecting to contribute SNS managers or marketer of medical institutions to taking out commitment on medical consumer's SNS.

Experiences of Interaction with Health Care Workers in the Process of Transition of Single Mothers to Motherhood (미혼모의 모성으로의 이행과정에서 보건의료종사자와의 상호작용 경험)

  • Purpose: This study aimed to explore the interactions of single mothers with health care workers during their transition to motherhood. Method: This qualitative study conducted focus group interviews with 16 single mothers who were identified at four facilities for single mothers and provided written informed consent. from Junre to October 2017. Each focus group session was recorded and transcribed verbatim. Result: Four categories emerged from the data: "trapped behind a will of eyes," "oil on water," "standing naked in a public square," and "Support from being called a mother." Conclusion: The study results showed that health care workers should provide health care service to single mothers with thorough consideration and care. The findings suggest that appropriate education is necessary for health care workers and prospective health care workers who provide maternal and neonatal care to improve the medical services that they provide to single mothers.

A Study on Evaluation and Improvement of Long-term Care Hospitals for Changing Long-term Care Hospital Fee System (요양병원형수가제 전환에 대한 요양병원의 평가 및 개선방안 연구)

  • The purpose of this study is to investigate evaluation and improvement of long-term care hospitals for changing long-term care hospitals fee system. Data were collected from 104 CEOs in nationwide long-term care hospitals using structured self-administered questionnaires during August 17 to 31, 2009. Major results of the empirical analysis are as follows; first, to change fixed sum medical fee per day caused to decline the level of geriatric service in 87% of CEOs. Second, 79% of CEOs were dissatisfied with changing fixed sum medical fee per day, and 47% of them were dissatisfied with graded fee for doctor and nurse management. Finally, they suggested that to specialize and to differentiate of long-term care hospitals will drive to improve long-term care hospitals function and to measure workforce based on rate of filled vacancies will increase efficiency and productivity of doctor and nurse management.

Patient satisfaction of medical care in a public health center (보건소 진료서비스 이용자의 만족도에 관한 연구)

  • The aim of this study was to examine differences in the level of satisfaction with medical care at a public health center between patients receiving free-care compared to those paying out-of-pocket. SERVQUAL(Comprehensive Service Quality Measurement Scale), a pyschometrically valid measure for evaluating hospital service quality, was used to assess patient satisfaction. The study sample consisted of 279 patients who received medical care at a public health center. The interview was conducted between April 23 and May 7, 1998. The results was as follow: 1. There were more elderly people among free-care patients than out-of-pocket patients. Education and income levels were lower amongs those receiving free-care. There were, however, no differences in the distribution of gender and religion between the two groups. Patients receiving free-care tended to visit the public health center more frequently compared to those self-paying patients. 2. Overall, free-care patients showed higher satisfaction level than that of self-paying patients. except for the dimension on sympathy and shape.

A Study on the Productivity Trends of Regional Health Care Resource Uses in South Korea (지역 의료자원 이용의 생산성 변화 분석)

  • Objectives : This study purposed to analyze the productivity trends of regional health care resource uses in South Korea. Methods : Data was provided from the regional health care statistics by the National Health Insurance Service(NHIS) and collected from 2011 to 2014 at the 226 administrative regions such as Si(city in Korean), Gun(county in Korean), Gu(district in Korean). Productivity trend was analyzed with Malmquist Productivity Index(MPI). Input variables were the number of medical personnels, facilities, and major medical equipments. Output variables were the number of inpatient and outpatients in model A, and the amount of inpatient and outpatient reimbursements in model B. Results : In model A, the productivity of 62 regions were increased but it was decreased in 164 regions. In model B, the productivity of 123 regions were increased but it was decreased in 123 regions. Conclusions : If these trends were continued, there will be problems with the efficiency of national regional healthcare resource utilization. Health policy makers will require to focus in solving this phenomenon.

Factors Related to Family Caregiver Financial Burden of Out-Of Pocket Expenses for the Nursing home service under Long-term Care Insurance System (노인장기요양보험 시설서비스 본인부담금에 대한 가족수발자의 경제적 부담감 영향요인)

  • The purpose of this study is to investigate the factors affecting family caregiver financial burden of out-of pocket expenses for the nursing home service under Long-term Care Insurance System. We conducted a national cross-sectional descriptive survey from July to September 2010 to collect data based on the long-term care benefits cost specification. Total 1,016 family caregivers completed questionnaires. 185 subjects of total were excluded from the data analysis due to being answered by user(18 cases), or caregivers not to pay for services expenditures(122 cases), having a missing data on family caregivers characteristics(45 cases). Finally, 831 subjects were included in the study. The average financial burden was 3.18(${\pm}0.71$). We divided subjects into two groups by level of burden, high-burden group and low-burden group. In the result of the multiple logistic regression analysis, family caregiver financial burden was significantly higher in family caregivers with ages 40 to 49 compared to less than 40, lower educational level, unsatisfaction for long-term care service, high percentage(more than 50%) of cost-sharing and high total out-of pocket expenses(more than 300,000 won) for long-term care services. Also, Family caregivers who are spouse felt higher financial burden compared to son. This study is meaningful as the first attempt to measure family caregiver financial burden for long-term care service and to identify factors affecting the financial burden. Family caregivers felt financial burden of out-of pocket expenses for the nursing home service. The policy makers, the insurer, and the providers need to pay attention to ease family caregiver financial burden.

Self-care Education Experience, Self-care Performance, and Home Care Service Needs of Patients With Rare and Incurable Diseases Who Use a Home Ventilator (가정형 인공호흡기를 적용하고 있는 희귀, 난치성 질환자의 자가관리 교육 경험과 수행정도, 가정간호 요구도)

  • Purpose: This study aimed to investigate the self-care education experience, self-care performance, and home care service needs of patients with rare and incurable diseases who use a home ventilator. Methods: From2 ventilator rental companies and 5 hospitals, records of 162 subjects were collected to obtain data regarding self-care education experience (23 items) and home care service needs (25 items). Results: Of the subjects, 65.4% were male and 55.6% had a myotrophic lateral sclerosis. The items with the highest and lowest levels of self-care education experience were "using the ventilator"(94.0%) and "community-available resource information"(27.0%), respectively. Meanwhile, self-care was performed well in terms of "personal hygiene"(59.6%) but not performed well in "community-available resource information"(23.7%). "Health status assessment" had the highest need(88.2%) of home care services. Generally, the need of home care services was higher for the patients using invasive ventilators than that for those using non-invasive ventilators. Conclusion: Home care nurses need to not only provide physical care for the patients but also strengthen their roles of enhancing patient access to and utilization of various community resources to provide systematic and individualized training and care.
