Gender Differences in Cardiac Knowledge and Symptoms Recognition in Patients with Acute Coronary Syndromes (일 대학병원에 내원한 급성 관상동맥증후군 환자의 성별에 따른 질병지식 및 증상인지 비교)

  • Purpose: The purpose of this study was to compare the cardiac knowledge and symptoms recognition between men and women with acute coronary syndrome (ACS). Methods: Cross-sectional survey research design and convenience sampling were used in this study. 64 men and 42 women from a university medical center were participated in this study. Data collection were used with self reported questionnaires and medical records. Results: There was no difference in cardiac knowledge by gender. Women marked the higher score in symptoms recognition than men. Significant gender differences were observed in the reports of several symptoms (headache, nausea, palpitation, hand paresthesia, and leg numbness) and with ACS. There was significant correlation between cardiac knowledge and symptom recognition on both men and women. Conclusion: These findings suggest that health care professionals should pay attention to the differences in clinical symptoms between women and men. Furthermore, tailored information about possible symptoms of coronary artery disease according to the patient's gender is needed.

Symptom Clusters in Women with Gynecologic Cancer (부인암 여성의 증상 클러스터(Symptom Cluster))

  • Purpose: Women with gynecologic cancer often experience various physical and psychological symptoms relating to the cancer and its treatment. The purpose of this study was to identify symptom clusters. Method: A survey was conducted on 184 women with diagnoses of cervical, ovarian or endometrial cancer. Fifty symptoms were assessed for prevalence, severity and interference, and symptom clusters were identified. Cluster analysis was done using SPSS version 12.0. Results: Fatigue was identified as the most prevalent symptom (81.52%), lack of vaginal lubrication (2.26) as the most severe symptom, and lack of vaginal lubrication as the most interfering one (2.15). Identified six clusters were: Anorexia-pain cluster (loss of appetite, taste change, weight loss, appearance change, alopecia, weakness, pain), Fatigue cluster (lack of concentration, lack of memory, fatigue, dry mouth), Urinary-bowel distress cluster (urinary difficulty, constipation), Abdominal discomfort cluster (lower abdominal pain, abdominal pain, bloating), Emotional distress (sadness, anxiety-worry, nervousness, restlessness), and Menopausal cluster (sweating, hot flush, fever). Conclusion: The result of this study provides fundamental data to health care professionals in developing interventions for effective symptom management for women with gynecologic cancer by understanding identified 6 symptom clusters.

Adjustment Patterns of Illness Process of People with Hemophilia in Korea (혈우병 환자의 질병과정 적응유형)

  • Purpose: The purpose of this study was to explore adjustment pattern of illness process of people with hemophilia in Korea. Method: 23 people with hemophilia had participated for this study. The data were collected through in-depth interviews and analyzed using Strauss & Corbin's grounded theory method. Results: 'would be free from' was emerged as a core category and it reflects that all participants wanted to be free from the constraints of the disease. The adjustment process was categorized into two stage, the ‘unstable stage’ and the ‘stable stage’. In the process of 'would be free from' four different patterns were identified: hopelessness type; appreciation type; challenge type; and transcendence type. These types were identified based on the degree of pursuing normal life and managing the disease, and social support. The most frequently occurring type was hopelessness type but the participants of this type suffered the most. The transcendence type was the most ideal type, but it occurred the least. Conclusion: The results of this study indicate that people with hemophilia in Korea still suffer from the disease and they need supports. The results would be useful for health care professionals in establishing education and counseling program for the people with hemophilia.

The Association of Ponticulus Posticus & Elongated Styloid Process with Headaches

  • Purpose: The present study was designed to investigate the association of ponticulus posticus (PP) and elongated styloid process (ESP) with headaches. Methods: Analysis of head and neck cone beam computed tomography samples from the archives of the Department of Oral Radiology was done for the presence of partial or complete PP and ESP length, type, thickness, mediolateral angulation, anterioposterior angulation (horizontal & vertical), lateral or medial curvature. This was followed by personal & telephonic questionnaires to the subjects for the evaluation of the presence of headaches & other associated symptoms. Results: Among 134 subjects, 62 subjects (46.3%) presented with headache and 72 subjects (53.7%) did not have any headache. On further analysing the total 62 subjects with headache, it was found out that 31 subjects (50.0%) of them had ESP and PP both, 16 subjects (25.8%) had only ESP, and 15 subjects (24.2%) had only PP. A strong association was present between headache and presence of PP & ESP individually and together. Conclusions: All health care professionals dealing with the head and neck pain disorders should also consider the presence of ESP & PP during diagnosis and treatment.

Validity of the Self-efficacy for Interprofessional Experimental Learning Scale in Korea (한국판 전문직 간 학습에 대한 자기 효능감 척도의 타당화)

  • Interprofessional education (IPE) can promote high-quality patient care and good medical outcomes through teamwork among health professionals. However, there are no valid measurements to prove the effectiveness of IPE in Korea. This study aimed to develop and test a Korean version of the Self-efficacy for Interprofessional Experimental Learning Scale (SEIEL). The original SEIEL was translated into Korean by two experienced medical professors, and 368 questionnaires were collected from medical and nursing students (third and fourth year). To analyze the validity of the Korean version of the SEIEL, an exploratory and confirmatory factor analysis was conducted. Cronbach's ${\alpha}$ was used to evaluate reliability. Results from the exploratory factor analysis identified two functions: "interprofessional collaboration" and "interprofessional team evaluation." A significant cross-correlation was found between the two functions (r=0.690, p<0.001), with a Cronbach's ${\alpha}$ value of 0.932. The reliability and validity of the Korean version of the SEIEL was identified in this study. This tool can be helpful in measuring the effectiveness of IPE in Korea.

A Study on Factors for Improving CPR based on Health Care Professionals Treating Cardiac Arrests

  • This study aimed to help build a quality control program to improve cardiac arrest treatment via analysis of medical records in a local tertiary general hospital to evaluate factors that influence clinical outcomes of in-hospital cardiac arrest. At first, the medical records of in-hospital cardiac arrest were analyzed, and targeted surveys about functional and structural factors associated with cardiopulmonary resuscitation (CPR) were conducted amongst the workforce in charge of cardiac arrest treatment. From January 2012 through June 2013, a total of 486 adult cases of in-hospital cardiac arrests, except for those occurring in the emergency room, were enrolled in this study. Among the patients, those of recovery of spontaneous circulation were 57.8%; 13.8% of patients were discharged alive; 8.9% of patients were discharged without significant neurologic sequela. Despite CPR is usually successful when administered as quickly as possible, in this analysis showed that prompt reaction after initial recognition was significantly lower in nurses compared with doctors. Analysis of survey results showed that confidence in performing CPR was significantly associated with the experience of CPR in doctors, while in nurses educational experience showed a correlation. In order to improve quality of in-hospital CPR system maintaining and increasing confidence of CPR performance is the most important factor. Therefore it can be helpful to develop and apply a phased, customized education program using training simulators as well as personalizing them to increase the personnel's confidence in CPR performance.

Development of Self-management Instrument for Pre-dialysis Patients with Chronic Kidney Disease (투석전 만성신장병 환자의 자가관리 도구 개발)

  • This study aimed to develop a valid and reliable self-management instrument for pre-dialysis patients with chronic kidney disease (CKD). A total of 143 patients with CKD recruited from a medical center completed the questionnaire. The dimension and items of self-management were composed by literature review. The items of self-management was evaluated using exploratory factor analysis and measures of reliability. Five factors were extracted and labelled adherence to treatment regimen and partnership, diet adherence, problem solving, health behavior, and pursuit of psycho-social health. The five factors accounted for 51.1% of total variance. Each factors showed acceptable internal reliability with Cronbach's alpha from 0.64-0.79. The developed self-management instrument can be useful in self control of their disease for patients with CKD, and both evaluating patients' self-management and developing intervention program for health care professionals.

Teaching Status and Knowledge of Elementary School Teachers of Children with Attention Deficit Hyperactivity Disorder (주의력결핍과잉행동장애 아동에 대한 초등교사의 지도실태와 지식)

  • Purpose: The purpose of this study was to analyze teaching status and knowledge about children with Attention Deficit Hyperactivity Disorder (ADHD) in elementary school teachers. Methods: The participants of this study were 204 elementary school teachers in Busan. From July to September 2010, the researchers collected data by using self-report questionnaires consisted of questions about their teachers' characteristics and teaching status and KADDS (36 items) developed by Sciutto, Terjesen and Frank. Frequency, percentage, arithmetic mean, standard deviation, t-test and ANOVA were used to analyze the data using SPSS WIN 16.0 program. Results: Less than 50% of the participants had previous education on ADHD. According to this experience, there were significant differences in participants' knowledge about children with ADHD (t=4.703, p<.001). For the 36 items, participants had an average of $19.19{\pm}4.58$ points out of a possible 36. Moreover, lack of knowledge about teaching children with ADHD and communication problems with the parents of these children were considered to be the most disturbing factors. Conclusion: These results suggest that networks among teachers, parents and medical care professionals should be established. The results also indicate that educational programs for elementary school teachers should be developed in order to identify and teach children with ADHD appropriately.

Study of the Drugs Prescribed on Alzheimer's Disease: from the Insurance Claims Data of Korea National Health Insurance Service (우리나라 건강보험 청구자료를 이용한 알츠하이머성 치매 치료제의 사용현황 분석)

  • Objective: The aims of this study are to investigate the total volume of prescribed medicines against Alzheimer's disease (AD) and the trends of usage by analyzing the claims-data from the Korea National Health Insurance Service. Method: The demographic and claims-data were included the major AD treating medicines such as donepezil, galantamine, rivastigmine and memantine, and analyzed during the period of 2010~2012. The assessing criteria were gender, age, habitation, types of medical institution, code of ingredients, outcomes of treatment, volume and amount of claims, and the numbers of patients with dementias. After trimming the data, it were analyzed by the market size, demographic traits, characteristics of medical service, characteristics of each anti-AD medicine, etc. Results: Among the chosen 4 medicines, donepezil had the top prescription volumes. Most prevalent prescribing preparations of donepezil were conventional types. However, among the non-conventional types, oro-dispersible formulation is the fast increasing one in both volume and growth rate. This specialized preparations to improve both toleration and adherence, tend to being prescribed generally at the tertiary medical institutions. While the younger patients with mild-to-moderate AD mostly treated by expensive medicines in resident at the tertiary hospitals, the rest older patients with severe AD have been treated non-expensive one at long-term care facilities. Conclusion: AD is a chronic illness therefore, long-term use of therapeutic medications are highly important. If an anti-AD treatment was applied steadily in the earlier stages, it would be achieved not only improving the quality of life of patient but also reducing the expenses in the medical and nursing cares. As the socioeconomical impacts of AD is expanding, healthcare professionals need to aware the importance of pharmacotherapy and to improve sociopolitical fundamentals.

A review of the qualification criteria for the national examinations for emergency medical technicians (응급구조사 국가시험 응시 자격기준에 관한 고찰)

  • Purpose: Emergency medical technicians (EMTs) have so far been trained as professionals under the same conditions, with no change in the 1995 Act. We aimed to find ways for them to secure expertise in accordance with social needs by strengthening the quality of the EMT education. Methods: This is a descriptive study comparing the operation status of the national emergency medical examination conducted by the Korea Health Personnel Licensing Examination Institute, and the national examinations of EMT paramedic and EMT basic. The scope of the national examinations for EMT was compared by subject and area. Results: The national written exam for EMT paramedic consists of five areas. EMT basic does not include basic medicine in three related subjects, 11 areas, and 18 detail areas. Paramedic care does not include advanced pediatric life support. In addition, nine areas and 20 detail areas are not included. Conclusion: The study suggests the need for institutional supplementation so that those who have completed EMT basic and the subjects prescribed by the ordinance of the Ministry of Health and Welfare at universities, etc., in the Higher Education Act can take the EMT paramedic national exam.
