예방적 경부청소술의 임상적 의의에 관한 연구 (A Clinical Study of the Elective Neck Dissection)

  • Elective neck dissection (END), provide proper information on nodal status and stage which are significant prognosticator in head and neck cancers with clinically $N_{(0)}$ neck. But there are many controversies for the extents, methods of surgery, moreover, whether normal lymph nodes, local defencer, have to be removed or not. The authors performed 47 END in 39 patients of head and neck squamous cell carcinoma from 1984 to 1989 and a retrospective study of the cases was conducted. Eighteen percent of nodal metastasis and five percent of extracapsular spread were found in END specimens. We concluded that END provide significant information for the management and evaluation of prognosis in head and neck cancer.

중사각근 이완자세가 일반성인의 경부 회전에 미치는 영향 (The Effects of Scalenus Medius Muscle Relaxed Posture on Head-Neck Rotation of General Adults)

  • Purpose : This research intended to find out the effects of scalenus medius muscle relaxed posture on the increase of head-neck rotation for general adults. Methods : This research was conducted on 30 subjects whose both sides head neck rotation angles are not the same and that agreed to participate in the experiment. In a posture of sitting erectly on a mat, both sides head neck rotation angles were measured with C-ROM equipment, and while supporting arm in direction opposite the side where head neck rotation limited and leaning over the body at 45 degree and neck at vertical condition against ground, head-neck rotation angles were measured each with C-ROM equipment. Results : The Head-Neck rotation angle on the side of limited head-neck rotation demonstrated more increase in scalenus medius relaxed posture than in erect sitting posture, showing significant difference statistically(p<0.05). The head-neck rotation angle on the side of non-limited Head-Neck rotation demonstrated more decrease in scalenus medius relaxed posture than in erect sitting posture, not showing any significant difference statistically (p>0.05). Conclusions : This posture may be used for preventing limit of head-neck rotation caused of scalenus medius muscle tension and increasing head-neck rotation.

전완유리피판의 두경부재건술 적용결과 (The Results of the Radial Forearm Free Flap for Head and Neck Reconstruction)

  • Background: Free flaps have become a very important role in the ablative head and neck surgery with functional preservation of defect sites. The forearm free flap has many advantage of lack of bulk, ease of dissection, vascularity, and malleability. Patients and Methods: A review of 51 radial forearm free flaps performed between may 1990 and feburary 2001 at the Korea University was undertaken to determine outcome. Patient age ranged 27 to 72 years (mean 56). There were 44 men and 7 women. The most prevalent neoplasm was squamous cell carcinoma. Results: The tans verse cervical artery and the external jugular vein were the most frequent receipient sites for microvascular anastomosis. Total flap loss occurred in 2 cases (4%) and other complications were seen in 42%. Fifteen patients received preoperative irradiation and the complication was higher than non-irradiation patients, but statistically not significant. Conclusion: The radial forearm free flap offers a variety of reconstructive options for head and neck. Its low flap loss and complication rates offer the best choice for reconstruction of defect of head and neck malignancy.

편도암 치료 후 기관절개 부위에서 재발한 1예 (A Case of Tumor Recurrence at the Tracheotomy Site in Tonsil Cancer Patient)

  • The aim of this report was to present a rare case of a tracheotomy site recurrence after operation and post-operative radiotherapy in head and neck squamous cell carcinoma patient. Tracheotomy site recurrence other than subglottic cancer is very rare in head and neck cancer patient. The granulation tissue around tracheotomy was a fertile "soil" for tumor cell implantation. Midline mucosal sparing block which was used to decreased acute toxicity during post-operative radiotherapy could be facilitating the tumor cell implantation. The head and neck surgeon should try every effort to reduce contamination of cancer cells during operation and consider the entire operative field should be included in post-operative radiation portals.

  • Liposarcoma is the second most common soft tissue sarcoma in adult. The incidence of liposarcoma in the head and neck area is very uncommon. Liposarcoma of the neck usually express non-tender palpable mass. The main treatment of cervical liposarcoma is surgical excision and adjuvant radiation therapy. Herein, we report a case of cervical liposarcoma with the review of literature.

고유수용성 신경근 촉진법의 두부·경부 운동 패턴 (Movement Patterns of Head and Neck in Proprioceptive Neuromuscular Facilitation)

  • Objective : The purpose of this study was conducted to find correct head and neck patterns, manual contact. verbal commands with proprioceptive neuromuscular facilitation(PNF). Method : This is a literature study with books, seminar note and book for PNF international course. Result : Keep the information of the biomechanics and neural science in head and neck patterns and emphasize that manual contact, verbal commands and visual stimulus. Manual contacting for movement guide and stability of the $C_0/C_1$ verbal command and visual stimulus for correcting of the $C_0/C_1$ movements. Conclusion : In reminder for PNF learning, begin with head and neck and upper trunk patterns. In that time, Knott and Voss(1968) had not enough information about biomechanic movement components and neural science movement components. But Knott and Voss(1968) emphasized that head and neck patterns relate with trunk, upper extremities and lower extremities directly. Alar ligaments are relaxed with the head in neutral and taut in flexion. Axial rotation of the head and neck tightens both alar ligaments. The right upper and left lower portions of the alar ligament limit left lateral flexion of the head and neck. Therefore, head and neck patterns has to be modify. When head moving, eye and vestibular stimulus will be change. During head and neck patterns, must be consider about stimulus of eye system and vestibular system also.
