Efficacy of Thalamotomy and Thalamic Deep Brain Stimulation for the Treatment of Head Tremor

  • Objective: Stereotactic thalamic procedure is well known to be a effective treatment for disabling upper limb tremor of essential tremor. However, the effect of this procedure for head tremor, which is midline symptom of that disease entity, has not been sufficiently established. The authors discuss the result of stereotactic thalamic operations for head tremor of their patients who suffered from essential tremor. Methods: We evaluated 4 patients of essential tremor who had head tremor combined with both upper limb tremor. One patient underwent unilateral ventralis intermedius thalamotomy, two patients had unilateral Vim deep brain stimulation(DBS) and one patient had unilateral Vim thalamotomy and contralateral DBS. Postoperative results of tremor were evaluated using our proposed scale. Results: Contralateral upper limb tremors to surgical side were markedly resolved in all patients but there was no meaningful effect for head tremor in 3 patients who underwent unilateral thalamic surgery. In a patient having simultaneously unilateral thalamotomy and contralateral DBS, remarkable improvement of head tremor was observed. Conclusion: Although it is difficult to evaluate the efficacy of thalamic surgery for axial symptom of essential tremor with a few cases, simultaneous unilateral thalamotomy and contralateral DBS would be expected to induce favorable outcomes for head tremor with significant economical advantages.

본태성 진전증의 임상 및 전기생리학적 분석 (Clinical and Electrophysiologic Analysis of Essential tremor)

  • Background: Essential tremor (ET) is a common movement disorder that often causes functional disability. There have been very few investigations about the clinical characteristics of ET in Korea. Therefore, we performed a study showing the clinical features and electrophysiological findings of ET. Methods: We analyzed medical records and accelerometry data of 152 patients (male vs female; 79 vs 73) with ET, who visited the Neurology Clinic of Hospital from 2000 to 2003. Clinical characteristics of ET were summarized including the age of onset, family history, tremor type, body part involved, and associated symptoms. The frequency of tremor was recorded and the spectral analysis of tremor was performed. Results: The age of tremor onset showed bimodal distribution with peaks in the 2nd and 5th decades. Family history was found in 46 patients (30.3%). The patients with the family history presented earlier onset of tremor than patients without the history (mean age of onset, y: 35.2 vs. 49.9, P < 0.001). Tremor appeared most frequently in hands (94%), and followed by head (25%). In head tremor, "no-no" pattern was mainly observed and the head tremor was more frequently observed in female. The frequency of tremor was negatively correlated with age (r=-0.49, P<0.001). Conclusions: The present study indicated some important findings about ET: (1) bimodal distribution for an age of onset, (2) younger age of onset in patients with a family history, (3) decreasing frequency of the tremor according to age, and (4) higher prevalence of head tremor in female patients.

  • Tremor is a rhythmic and involuntary muscular contraction characterized by oscillations of a part of the body. The most common of all involuntary movements, tremor can affect various body parts such as hands, head, facial structures, vocal cords, trunk, and legs; most tremors, however, occur in the hands. Clinically, tremor is classified into postural tremor, resting tremor, action tremor, and other kinds of tremor, and treated according to the causes. The author reports the improvement of the patient, hospitalized at Dong Seo Medical Center, whose tremor was not classified specifically in western medicine but was diagnosed as Ganhyeolbujok (肝血不足) by Korean medicine.

The effect of short-term creatine intake on blood lactic acid and muscle fatigue measured by accelerometer-based tremor response to acute resistance exercise

  • [Purpose] The purpose of this study was to investigate the effects of short-term creatine intake on muscle fatigue induced by resistance exercise in healthy adolescent men, i.e., lactic acid concentration and wrist and head tremor measured by an accelerometer. [Methods] Twelve healthy adolescent men who had no experience with creatine intake were included. The subjects were randomly assigned to the creatine group and the placebo group, followed by 5 days of creatine and placebo intake, and 5 times of 5 sets of leg press, leg extension, bench press, and arm curl exercises at 70% repetition maximum (RM). The lactic acid concentration before and after exercising, rate of perceived exertion (RPE), and accelerometer-based wrist tremor and head tremor during exercise were measured. Subsequently, after 7 days to allow for creatine washout, the same exercise treatment and measurement were performed in each group after switching drug and placebo between the groups. [Results] The level of lactic acid before and after the acute resistance exercise trial was significantly lower in the creatine group than in the placebo group (P <0.05). The mean RPE during the resistance exercise was significantly lower in the creatine group than in the placebo group (P <0.05). There was no difference between the two groups in the mean wrist tremor during resistance exercise, but the mean head tremor values were significantly lower in the creatine group than in the placebo group in the arm curl, the last event of the exercise trials (P <0.05). [Conclusion] Short-term creatine intake reduces the blood fatigue factor increased by resistance exercise, and is thought to suppress fatigue, especially in the latter half of resistance exercise. Therefore, these findings indicate that short-term creatine intake can have an improved effect on anaerobic exercise performance.

  • Involuntary hand tremor has been a serious challenge in micromanipulation tasks and thus draws a significant amount of attention from related fields. To minimize the effect of the hand tremor, a variety of mechanically assistive solutions have been proposed. However, approaches increasing human awareness of their own hand tremor have not been extensively studied. In this paper, a head mount display based virtual reality (VR) system to increase human self-awareness of hand tremor is proposed. It shows a user a virtual image of a handheld device with emphasized hand tremor information. Provided with this emphasized tremor information, we hypothesize that subjects will control their hand tremor more effectively. Two methods of emphasizing hand tremor information are demonstrated: (1) direct amplification of tremor and (2) magnification of virtual object, in comparison to the controlled condition without emphasized tremor information. A human-subject study with twelve trials was conducted, with four healthy participants who performed a task of holding a handheld gripper device in a specific direction. The results showed that the proposed methods achieved a reduced level of hand tremor compared with the control condition.

두부진전을 주증상으로 내원한 심인성 진전 환자 치험례 (A Case Report of Psychogenic Tremor Patient with Head Tremor)

  • 가족 간의 갈등으로 인해 발생한 두부진전이 4-5년간 지속된 환자에게 한약물과 한방정신요법을 시행하여 증상의 유발과 악화인자에 대한 통찰을 하고 조절할 수 있는 통제력을 가지게하여 증상을 완화되었고, 수개월이 지난 지금까지 호전상태가 지속되는 것을 경험하였다. 심인성 운동장애는 전체 운동장애 중 2-3% 정도로 드물게 일어나며, 진전이 가장 흔한 형태이기는 하나 상하지에 많이 발생하고, 두부에만 국한적으로 일어나는 경우는 드물다. 이에 상기 증례는 다른 신경학적 증상을 동반하지 않고 두부에만 국한적으로 진전이 발생한 환자에게 한의학적 치료를 하여 유효한 결과를 얻었기에 보고하는 바이다.

  • Tremor is a rhythmic, involuntary muscular contraction characterized by oscillations (to-and-fro movements) of a part of the body. Essential tremor is a 6-8 Hz tremor that is of variable amplitude. It is aggravated by posture and movement and relieved by rest and alcohol. Most affected part are the arms, head and vocal apparatus. The purpose of this study is to show a case of tremor patient with essential tremor improved by oriental medical treatment. The patient was 71-year-old woman who featured essential tremor and at the same time complained general body weakness, nausea, and anorexia. She was treated by oriental medicine with acupuncture, herb medication, and Korean psychotherapy; Eejeong-byunki, Jiun-goron. The features of essential tremor was reduced after taking oriental medical therapy. This result suggested that the oriental medical therapy can be effective for treating essential tremor.

  • Tremor is a rhythmic, involuntary muscular contraction characterized by oscillations(to-and-fro movements) of a part of the body. The most common of all involuntary movements, tremor can affect various body parts such as the hands, head, facial structures, vocal cords, trunk, and legs; most tremors, however, occur in the hands. Tremor often accompanies neurological disorders associated with aging. Although the disorder is not life-threatening, it can be responsible for functional disability and social embarrassment. The clinical study was carried out the 4 patients with tremor who were treated in Dong Seo Medical Center from 1 January 2005 to 31 August. The results were summarized as follows. Tremor has relation to liver according to Jangbu theory. So we performed acupuncture treatment(Ganseonggyeok) and Herbal medication on tremor. After our treatments, tremor and some other symptoms were improved.

Valproic Acid 투여로 인한 약인성 떨림 환자의 한방 치료 치험 1례 (A Case Report of Valproic Acid-Induced Tremor by Korean Traditional Medicine)

  • A 56-year-old male patient with a sudden onset of tremor and involuntary movement of right upper and lower extremities, head, and voice was diagnosed as having a drug-induced tremor, with valproic acid being the culprit drug. The patient had undergone admission treatment at an internal Korean medicine department with herbal medicine according to constitutional diagnosis, acupuncture, moxibustion, cupping, and rehabilitation. The change in the severity of tremor was assessed with the Fahn Tolosa Marine scale. After discharge, the patient took herbal medicine for two more months. After 20 days of admission and 2 months of treatment with herbal medicine, the patient's tremor improved. In this case, the patient with valproic acid-induced tremor showed improvement in symptoms after 20 days of hospitalization and further reduction of tremor and improvement of quality of life were confirmed through follow-up for 2 months.

음성 떨림 측정을 위한 AVTI(Acoustic Voice Tremor index)의 개발과 검증 (Acoustic Voice Tremor index in the measurement of voice tremor: Development and validation)

  • 본 연구에서는 음성 떨림의 음향학적 측정을 위해서 AVTI(Acoustic Voice Tremor index)를 개발하고 검증하는 것을 목표로 한다. 정상 성인 71명, 음성 떨림 환자 41명이 참여하였으며, 모음/아 /를 5초 이상 녹음하였다. 모음 안정구간 3초를 편집하여 Praat 스크립트를 이용하여 음성 떨림 관련 18개의 변수 측정값을 확인하였다. 이 변수들과 청지각적 평가 전반적 중증도(overall severity, OS)를 이용하여 선형 회귀분석을 돌려 AVTI를 구성하였다. 선형 회귀분석 결과, 18개 중 4개의 변수가 유의미하게 확인되고 회귀식이 구성되었다. 내부/외부 타당도 조사에서도 평균 0.8 이상의 높은 연관성을 나타내었다. AVTI는 OS와 0.841의 높은 상관관계를 보였다. AVTI를 통해서 음성 떨림을 예측할 수 있었다. 후속 연구에서는 더욱 많은 음성샘플과 보완된 Praat script를 추가 분석해 볼 필요성이 있을 것으로 생각된다.
